AlphaMD TRT cost starts at $98 a month with $1,176 paid yearly, or $129 billed monthly with a three-month minimum. Add the published $49 consultation: the annual base subtotal is $1,225. That fits a cash-pay budget only after you price required labs and extras; it does not establish treatment eligibility or your complete bill.

Best for: adults comparing AlphaMD's cash-pay injection program who can accept its payment commitment. Not for you if: you need insurance-billed membership, a no-commitment plan, or a fertility-focused evaluation before considering testosterone.

AlphaMD plan prices at a glance
| Plan | Price per month | How you pay | Minimum you commit to | 12 months of plan fees | Veteran / military / first responder rate (12-month plan subtotal) |
|---|---|---|---|---|---|
| Annual | $98 | $1,176 once a year | 12 months | $1,176 | $78.40/mo ($940.80) |
| 6-month | $109 | $654 every 6 months | 6 months | $1,308 | $87.20/mo ($1,046.40) |
| Monthly | $129 | $129 each month | 3 months ($387) | $1,548 | $103.20/mo ($1,238.40) |
Plus the published $49 first consultation in our model. Prices as published by AlphaMD, checked September 22, 2026. Twelve-month figures assume the listed rate continues; they exclude extra testing, add-ons and any dose adjustment.
That price is real. On our math, it's the lowest published base subtotal in the four-program comparison below—not a verified lowest complete treatment cost. But three things don't show up in the headline: how you have to pay to get $98, a lab bill AlphaMD doesn't publish, and a federal telehealth deadline that falls within a 12-month plan bought now. We'll walk you through all three — and tell you exactly what to get in writing before you pay a cent.
The quick numbers (AlphaMD-stated, checked September 22, 2026)
- $98 a month = $1,176 charged at once for 12 months
- $129 a month = billed monthly, 3-month minimum
- $49 first consultation
- Follow-up lab tests: extra, priced when your clinician orders them
- 20% off for veterans, active military and first responders
- Cash-pay only — no insurance billing
What changes your number
- Veterans, active military and first responders have published discounted plan rates of $78.40 to $103.20 a month.
- Adding hCG, if prescribed, adds $300 per order at AlphaMD. Your annual bill depends on the number of orders and the product's usable life. AlphaMD hCG pricing.
- Your dose can move the price. AlphaMD's pricing footnote says it may adjust with dosage.
AlphaMD is a strong financial fit to research if you:
- Already have a clinician-confirmed diagnosis based on symptoms or signs and repeat morning blood tests, or you're on TRT now and want a clinician to review a switch
- Are fine with injections
- Want its lowest published plan rate and can pay for a year up front
- Are a veteran, on active duty, or a first responder
Look elsewhere first if you:
- Plan to have a child in the near term — start with a urologist or reproductive urologist (see why below)
- Want insurance to pay — AlphaMD doesn't bill insurance
- Want lab monitoring built into one monthly price — compare the Hone Health Premium lab allowance, including any tests it does not cover
- Want no contract, or would rather use a cream than a needle — compare Male Excel's published terms and costs before choosing
- Haven't been diagnosed yet — start with a clinician-led evaluation, including appropriate repeat early-morning testing. Endocrine Society.

What we actually verified — September 22, 2026
We checked: AlphaMD's pricing table, TRT, hCG, enclomiphene and shop pages; its Terms of Use (last updated September 19, 2026); its new-member, lab-order and switching help pages; its contact and provider pages; and its Trustpilot profile. We checked Taurus Meds, Hone Health and Male Excel pricing and relevant terms the same day. For rules and medicine, we used federal prescribing rules, the federal rule-review register, HHS, the FDA, prescribing information and the Endocrine Society.
Confirmed on AlphaMD's own pages: plan prices and commitments, veteran prices, the $49 consultation offer, stated plan inclusions, hCG and enclomiphene prices, the $50 shop test kit, a named pharmacy partner, and published refund and cancellation terms. These are provider-stated facts, not a tested bill. The arithmetic in our tables was checked separately.
Still not confirmed for your case: required lab panels and prices, your assigned pharmacy and exact product, your state's appointment availability, early-exit charges or refunds on a 6- or 12-month plan, and your renewal price. A named partner does not tell us which pharmacy will fill your own prescription.
What we didn't do: buy a plan, complete a consultation, or contact AlphaMD support. We did not independently verify individual clinician or pharmacy licenses, test checkout, or measure support and shipping times. This is a cost guide built from public records, not a hands-on review. Our process follows How We Review TRT Providers.
TRT Provider Guide is the independent decision resource for testosterone replacement therapy — helping U.S. adults understand how low testosterone is evaluated, compare online and local care models and providers, and choose the next step that fits their health needs, fertility plans, budget, state, and care preferences, with every material claim verified and dated.

How much does AlphaMD TRT cost per month?
AlphaMD charges $98 a month on a 12-month plan billed once a year, $109 a month on a 6-month plan billed twice a year, or $129 a month billed monthly with a 3-month minimum. A $49 consultation comes first. Veterans, active military members and first responders pay 20% less on every plan. AlphaMD pricing; published $49 consultation.
The price per month vs. the bill you actually pay
"$98 a month" is an average, not a monthly bill. On the annual plan, you pay $1,176 in one charge. The 6-month plan is two charges of $654. Only the $129 plan bills you every month — and even that one asks you to stay at least three months, which is $387.
AlphaMD says the annual plan saves $372 a year compared with paying monthly. The math holds: $31 a month, times 12. AlphaMD pricing.
When does the big charge land? AlphaMD's homepage lays out the order. You book the consultation, get a hormone test kit, meet your clinician again about two weeks later, and then get charged for your medication once you and your clinician agree on a plan. So the $49 comes first. The plan payment comes when your treatment plan is set, before your first shipment. This is AlphaMD's stated sequence, not a checkout flow we completed. AlphaMD: How It Works.
Does your dose change the price?
It can. AlphaMD's pricing footnote says prices may adjust based on your goals and dosage. Older answers on AlphaMD's public Q&A pages describe one standard price that covers most doses, plus a small monthly surcharge above a cutoff. Ask for the price of your prescribed dose before you pick a plan length. Pricing footnote; AlphaMD dose-price answer.
The one catch worth knowing up front
AlphaMD does not offer its $98 price month to month. You pay $1,176 before your first shipment — before you know how your body responds — and even the $129 plan has a 3-month minimum. If paying as you go with no contract matters most to you, Male Excel's published terms are worth comparing. It advertises no contract and cancel anytime.
The tradeoff is clear: AlphaMD's annual base plan costs $372 less over 12 months than its monthly option. That is a saving within AlphaMD's own price table, not proof that the programs below provide the same care or that prepayment explains the company's pricing. AlphaMD prices; Male Excel terms and prices.
What will AlphaMD TRT cost in your first year?
Counting the published $49 consultation, AlphaMD's 12-month base subtotals are $1,225 on the annual plan, $1,357 on the 6-month plan and $1,597 paying monthly. These assume the listed plan rate stays the same and exclude any separately billed diagnostic or follow-up tests, dose changes and add-on medicines. They are the starting point for a budget—not a complete quote. Plan prices; consultation offer; lab process.
We built this table to separate the first payment from the cost of staying for 12 months, then show which charges still need a quote.
| Plan | Consultation + first plan payment | Plan fees for 12 months | 12-month base subtotal, including consultation | Complete budget still needs |
|---|---|---|---|---|
| Annual | $1,225 | $1,176 | $1,225 | Required tests and any other extras |
| 6-month | $703 | $1,308 | $1,357 | Required tests and any other extras |
| Monthly | $178 (3-month base commitment with consultation: $436) | $1,548 | $1,597 | Required tests and any other extras |
| Annual, veteran/military/first responder | $989.80 | $940.80 | $989.80 | Required tests and any other extras |
| 6-month, veteran/military/first responder | $572.20 | $1,046.40 | $1,095.40 | Required tests and any other extras |
| Monthly, veteran/military/first responder | $152.20 | $1,238.40 | $1,287.40 | Required tests and any other extras |
How to read this table:
- "Base subtotal" is our calculation, not a bill. It uses the published plan rate for 12 months plus one $49 consultation. The first-payment column adds that consultation to the first plan payment; it does not mean both are charged on the same day.
- The $50 test kit is not a follow-up lab quote. It is a basic finger-prick testosterone kit in AlphaMD's shop, not a published price for a complete monitoring panel. The lab guide shows test prices after a clinician places an order.
- Unknown isn't zero. Until you have AlphaMD's required tests and their prices in writing, your complete budget is still open.
- Veteran discounts apply to the plan, not the consult, based on AlphaMD's answers about when the discount kicks in. New-member information and linked discount answers.
- Not included above: add-on medicines like hCG, any dose surcharge, and sales tax if charged. AlphaMD says shipping is included. AlphaMD plan inclusions; pricing footnote; payment terms.
Testosterone therapy needs blood work after you start, whether or not a program makes the cost clear. The number and type of tests depend on your clinical plan; we explain the monitoring questions in the lab section below. Endocrine Society.
The right TRT provider is not the same for every person — it depends on whether low testosterone has been properly evaluated, your symptoms and health history, your fertility plans, your state, your insurance or cash-pay preference, your treatment-format preference, your budget, and whether online or in-person care is the better starting point. Some situations belong with primary care, urology, endocrinology, reproductive urology, or urgent or emergency care when the symptoms warrant it. Because a general answer cannot resolve those for you, use TRT Provider Guide's Find My TRT Path tool to map your situation to the right care route and the questions to ask before you pay.
The tool is educational and non-diagnostic. It does not determine eligibility, interpret your test results or guarantee a prescription.
What's included in AlphaMD's price — and what costs extra?
AlphaMD says its plan price covers provider visits, messaging, your prescribed testosterone at standard doses, injection supplies and shipping. The published $49 consult, separately ordered lab tests, hCG and any dose surcharge are outside that base subtotal. Several things that affect your total still need a personal quote. New-member information; lab orders; pricing footnote.
| Included in the plan price (AlphaMD-stated) | Separate or potential charges | Still needs your quote |
|---|---|---|
| Provider visits and follow-ups | $49 first consultation | Required initial, repeat diagnostic and follow-up lab prices |
| Messaging with the care team | Separately ordered lab tests | The exact product you'd receive |
| Testosterone at standard doses | hCG ($300 per 10,000 IU, only with an active TRT plan) | Your assigned pharmacy; the contact page names AnazaoHealth as a partner |
| Syringes, needles and alcohol pads | Higher doses (price may adjust) | What you owe if you leave a 6- or 12-month plan early |
| Shipping | Enclomiphene (a separate program, not TRT) | Your renewal price after the commitment ends |
| Applicable taxes or other disclosed charges | Availability for your state and clinical situation |

How shipping works
AlphaMD says it usually ships about three months of medication at a time, with roughly two extra weeks included as a buffer against shipping delays. For new patients, it says the first shipment can take up to three weeks after your plan is set. For patients switching from another clinic with recent labs or a current prescription, AlphaMD says most get their first shipment within a week of the consultation. New-member shipping guide; switching guide. These are provider estimates, not delivery guarantees.
Who handles refills or a late shipment?
AlphaMD's new-member guide directs patients to care-team contact details in their profile. It does not give a universal refill-request deadline or guaranteed reply time. Ask who authorizes the next refill, whether labs must be reviewed first, and when to contact the team if tracking has not arrived. New-member guide.
Keep the shipment schedule separate from your treatment instructions. A delivery estimate is not a reason to stretch, stop or change medication without your prescriber's advice.
Are labs included with AlphaMD?
Follow-up labs ordered through AlphaMD's current lab system are paid separately; a free starter kit does not establish that all required testing is included. The guide describes per-test prices through Assist Health or a downloadable order you can take to a lab that accepts it. AlphaMD does not publish a complete monitoring-panel price there, so get your lab quote before you choose a plan length. Current lab-order guide.
How AlphaMD lab orders work now
According to AlphaMD's lab guide (published August 24, 2026; updated September 6, 2026):
- A licensed clinician orders your labs. Sometimes the order is scheduled for a future date, like eight weeks after you start.
- AlphaMD emails you when the order is ready.
- Your lab order page shows each test and its price. Some are marked required.
- You pay for the order. Paying is what sends it to Assist Health — building the order isn't enough.
- Assist Health calls you to book a blood draw near you, then sends results to you and AlphaMD.
Prefer insurance? You can download the lab order instead and take it to any lab that accepts it. From there, scheduling and cost depend on your plan. Confirm the lab is in network and ask your insurer about coverage, deductibles and your share of the bill. AlphaMD's insurance-lab option.
Why older AlphaMD pages tell a different story
If you've read a few AlphaMD pages, you may have seen lab costs described several ways. Here's what each one says:
| AlphaMD page | Date shown | What it says about lab costs |
|---|---|---|
| "How Much Does TRT Cost? Unveiling the Truth" | Updated July 3, 2026 | The $49 consult includes a mailed testosterone test; says there are no hidden fees for repeat testing |
| "Understanding the Cost of TRT" | Updated July 3, 2026 | Additional tests are $49 each; says regular testing isn't required |
| Homepage, "How It Works" | Checked September 22, 2026 | A free hormone test kit comes after you become a patient |
| AlphaMD shop | Checked September 22, 2026 | A basic finger-prick testosterone kit sells for $50 |
| Switching page | Updated September 9, 2026 | Your clinician will recommend follow-up labs 6–12 weeks after starting |
| "How AlphaMD Lab Orders Work" | Updated September 6, 2026 | Tests go through Assist Health, priced per test when ordered, or you download the order to arrange testing yourself |
Our read: These pages do not give one consistent answer about testing costs. A publication date does not prove when a process changed, and an older promise of no repeat-test fees does not settle today's bill. None of it makes today's plan prices wrong. It just means the lab line is one number you need in writing before you pick a plan.
How many follow-up lab rounds should you budget for?
Testosterone can raise your hematocrit — the share of your blood made up of red blood cells. Too high, and your blood thickens. The AndroGel prescribing information, section 5.3, advises clinicians to check hematocrit before treatment, again 3 to 6 months after starting, and then annually. This is a monitoring example from an FDA-approved product's label, not a statement that AlphaMD supplies that product or that every treatment plan has the same schedule.
AlphaMD's own switching page says your clinician will recommend labs 6 to 12 weeks after you start. The follow-up panel may need more than a testosterone result, and dose changes or abnormal results may require extra checks. Ask for the baseline, early follow-up and ongoing testing plan, including who reviews results and the price of each round; do not assume two $50 kits will cover it. AlphaMD switching guide; Endocrine Society monitoring guidance.
Blood pressure also matters. The FDA's testosterone labeling update added blood-pressure warnings; ask how blood pressure and other required safety checks will be handled between visits. FDA labeling update. For the testing steps, see our TRT blood-test guide.
What customers say about switching, labs and support
These are individual experiences, not typical results. AlphaMD had 49 reviews on Trustpilot when we checked on September 22, 2026. We did not verify the reviewers' underlying medical or billing records.
- A smooth switch: Tom Pollock's July 13, 2026 review described a 15-minute call on a Wednesday and an order arriving the next Monday. His summary: "They made switching from my previous provider easy." Original review profile.
- A slow start: Jacob Bartman's review, posted September 4, 2026 for an August 1 experience, described multiple appointments, a 45-minute drive for labs, uploading results himself, and a process that took well over a month. AlphaMD replied publicly with an apology and said its care team would reach out. Review and reply.
- A lab-cost dispute: In a July 1, 2026 reply to the review posted under Clay Rhys, AlphaMD rejected the reviewer's "bait and switch" allegation and said lab options, costs and processes vary with the testing needed. That is a disputed account, not a verified fee schedule. Review and reply.
We can't judge any one person's account. Together, they point to the same move: get your lab plan and prices in writing before you pay for a plan.
Would you rather have lab monitoring built into one monthly price? Hone Health's Premium plan advertises regular lab testing and physician consults for $155 a month, plus medication. Using its $65 starter test and testosterone from $28 a month gives a $2,261 twelve-month advertised base model. That is not a guarantee that every extra test is covered: ask which panels and repeat checks are included. Hone pricing and plan details.

Can you cancel AlphaMD or get a refund?
You can stop future renewals by canceling before the cutoff AlphaMD shows at checkout, but most money you've already paid isn't refundable. Its terms allow a full membership-fee refund within 72 hours only if no services were requested or rendered; clinical fees and medication have separate limits and exceptions. The public terms do not explain a prorated refund or release from a 6- or 12-month commitment if you leave early. AlphaMD Terms of Use, sections 11–12.
Here's what AlphaMD's Terms of Use (last updated September 19, 2026) say, and what it means for your wallet:
| Question | What the terms say | What it means for you |
|---|---|---|
| Does it renew on its own? | Yes. Charges repeat until you cancel. | Put the renewal date in your calendar the day you join. |
| When do I have to cancel? | Before the cutoff shown at checkout. The terms give 48 hours before renewal as an example. | Screenshot the cutoff when you check out. |
| Is the consult refundable? | Generally not, once the consult happens or your intake is reviewed. | Ask what is refundable before clinical work starts. |
| Is the membership refundable? | In full if canceled within 72 hours and no services were requested or rendered. | The time limit alone does not qualify you for a refund. |
| Is medication refundable? | Prescription medicine can't be returned. Replacement or refund is possible for wrong, damaged or undelivered medicine. | Budget each shipment as spent. |
| What if I'm told I'm not eligible? | Refunds follow the policy shown at checkout and may exclude clinical review time. | Ask what you'd get back before you pay. |
| What if I leave a 6- or 12-month plan early? | No specific prorated-refund or early-release rule in the public terms reviewed. | Ask in writing. |
| How are disputes handled? | Individual arbitration, Delaware law, claims within one year. A chargeback can lead to account suspension. | Keep every billing question in writing. |
| Can I finance it? | Klarna or Afterpay may be offered. | Financing doesn't change what's refundable. |
"Cancel anytime" and "refundable" aren't the same thing
Canceling in time can stop the next renewal. It doesn't automatically send back the last payment or erase an existing commitment. That matters most on the annual plan, where one charge covers 12 months.
How to request cancellation or records
AlphaMD's terms say the billing cutoff will be shown at checkout or in your account. Save it, then request written confirmation of the cancellation date, any final charge and any refund. For a refund review, the terms list contact@alphamd.net and (206) 801-0205. We did not test the cancellation workflow. Terms, sections 11–12.
Before transferring care, ask the medical practice for your lab results, prescription history and treatment notes, plus the process and any fee for sending them to your next clinician. AlphaMD's privacy policy describes requests for certain personal information, but that is not a published promise of an instant, complete medical-chart download.
Canceling a plan is a billing decision, not a medical one
If you're on testosterone and want to leave any program, talk with a clinician about your medication before you stop or change it. Your billing and your treatment are two separate decisions.
Is a contract the dealbreaker for you? Male Excel says there's no contract and you can cancel anytime. It offers testosterone cream as well as injections, and it lists its plans as FSA/HSA eligible. Its published starting injection model is $99 a month for required membership plus medication from $120 a month, with a $99 consultation: $2,727 across twelve monthly equivalents before shipping, taxes and any testing not included. Medication is billed every 60 days, so get the dated payment schedule rather than treating that model as a calendar-year invoice. Male Excel pricing.
Its prices also include a daily thyroid medication — ask whether that's optional for you. A thyroid drug is not testosterone and needs its own medical reason. The FDA says animal-derived thyroid drugs are not FDA-approved; confirm the exact thyroid product and its status rather than assuming every medicine in a bundle is approved. FDA thyroid-drug information.

Is AlphaMD's testosterone FDA-approved or compounded?
AlphaMD's pages refer to compounded medication, but they do not identify the exact finished product every patient receives. Its contact page names AnazaoHealth as a partnered pharmacy, while its terms allow different pharmacies depending on the program and location. Ask whether your own prescription is FDA-approved or compounded and which pharmacy will dispense it. Contact page; fulfillment terms.
What AlphaMD says
- Most men get testosterone cypionate or testosterone enanthate injections, according to AlphaMD's TRT page. AlphaMD TRT page.
- AlphaMD's switching page says delays can come from its partner compounding pharmacies. Switching guide.
- A public AlphaMD answer says it works with compounding pharmacies rather than local pharmacies because they tend to cost less. AlphaMD pharmacy answer.
- The new-member page says the pharmacy begins production of your medication once your plan is set. New-member guide.
| What the public record shows | What that does not establish |
|---|---|
| The contact page names AnazaoHealth as a partner. | That AnazaoHealth will dispense your prescription, or which location would fill it. |
| An undated public Q&A also names Wells Pharmacy and Empower Pharmacy alongside AnazaoHealth. | That all named partners remain available for your program and state. |
| The program refers to compounding pharmacies. | The approval status of the exact finished product you will receive. |
| The terms allow pharmacy selection or transfer for several reasons. | A fixed pharmacy assignment throughout your membership. |
This distinction comes from AlphaMD's contact page, pharmacy Q&A and terms. A public partner name is useful; it is not a product-level or license audit.
The same Q&A uses the phrase “FDA approved pharmacies.” That does not establish FDA approval of a compounded medicine. FDA inspection or registration is not a substitute for approval of the exact finished drug. FDA compounding explanation.
What "compounded" means
A compounded drug is mixed by a pharmacy for a prescription. The FDA is plain about this: compounded drugs are not FDA-approved, which means the FDA doesn't check their safety, effectiveness or quality before they're sold (FDA). The FDA also says compounded drugs aren't the same as generic drugs. A generic is FDA-approved; a compounded drug is not.
That does not establish the safety or quality of any individual preparation. The FDA warns that poor compounding can cause contamination or too much or too little active ingredient, and says compounded drugs should serve patients whose needs cannot be met by an approved drug. Ask why compounding is needed for your prescription—not just whether it costs less. FDA compounding guidance.
Testosterone is a controlled substance
Testosterone is a Schedule III controlled substance. That's a federal category for drugs with accepted medical uses and a risk of misuse. You need a valid prescription from an appropriately authorized clinician, and there's no legitimate shortcut around that. The clinician must be allowed to treat a patient in the state where you are located and meet applicable controlled-substance rules. DEA scheduling; HHS prescribing rules; HHS state licensing.
If you want an FDA-approved, manufactured product
Ask AlphaMD whether it can prescribe one, and what that would cost. A public AlphaMD answer says a clinician can send a prescription to a local pharmacy in some cases, but the plan already includes medication, so ask whether a local fill adds a separate pharmacy bill or changes your plan price. AlphaMD local-pharmacy answer. Another care route is your own doctor or a urologist, with an FDA-approved prescription filled at a pharmacy that works with your insurance. Find My TRT Path can help you map that route; our online-vs-local care comparison explains the tradeoffs. Approval belongs to the exact medicine, not the clinic or payment model. FDA explanation.
Is AlphaMD legit?
AlphaMD has a live registration process, published terms, contact details and named clinicians. Those facts show a public operating presence; they do not independently verify a clinician's license, pharmacy compliance or the quality of your care. Whether it's right for you comes down to the price and fit details on this page. Registration; terms; contact; provider list.
- How it's set up: AlphaMD says it provides the technology and admin. Medical care comes from independent licensed clinicians through affiliated medical practices. Terms, section 3.
- Certification: We did not independently confirm an active third-party certification for this audit. A displayed badge would not replace checking the actual prescriber, pharmacy and prescription.
- Clinicians: AlphaMD's provider page lists clinicians by name, including Saba Haq, MD, and Trace Owens, ARNP. Ask who would treat you, then verify that person with the relevant state medical or nursing board. This audit did not complete those individual license checks. Provider list; HHS licensing guidance.
- Reviews: Mostly 5 stars on Trustpilot, but only 49 reviews when we checked. That is a small customer-experience sample, not proof of clinical safety or typical service. Trustpilot.
- Company and website: The alphamd.org homepage redirected to alphamd.net when checked. The current terms identify AlphaMD, LLC, consistent with the LLC name in the footer. Ask which medical practice and pharmacy will appear on your records and bills. Homepage; current terms.
Does AlphaMD serve your state?
AlphaMD's switching guide says it serves most U.S. states and directs readers to registration to check availability. It does not provide a complete excluded-state list there. We did not test a clinical appointment in any state. Switching guide.
Before paying, confirm your state, the clinician assigned to you, appointment access and any in-person evaluation required for your situation. Where you are physically located for the visit matters; an online clinic's website being accessible nationwide does not establish prescribing access. HHS licensing guidance.
How does AlphaMD's cost compare with Taurus Meds, Hone Health and Male Excel?
AlphaMD's annual plan has the lowest published base subtotal in this four-program model: $1,225 including its advertised consultation, before separately billed tests and extras. Taurus Meds, Hone Health and Male Excel use different testing, membership and billing arrangements, so these figures do not establish the lowest complete treatment bill. Our math: Taurus = $49 + 12 × $149 = $1,837. Hone = $65 + 12 × ($155 + $28) = $2,261. Male Excel = $99 + 12 × ($99 + $120) = $2,727. These assume the listed prices continue and the stated starter offer applies.
Taurus's unresolved fee matters. Section 28 of its terms states a recurring $17.99 membership fee per billing period unless the purchase says otherwise. The $149 offer does not clearly reconcile that fee. If it is additional and charged monthly, it adds $215.88 over 12 periods, taking the modeled subtotal to $2,052.88 before other exclusions. Do not assume it is waived, included or monthly until Taurus confirms your checkout terms. Its general refund policy also does not mean every cancellation qualifies for the offer's guarantee.
Male Excel's billing calendar matters. It bills medication every 60 days, and its injection and cream prices include a daily thyroid medication. Six 60-day medication periods cover 360 days, not a full 365-day year; another charge may fall inside your first year depending on billing dates. The $2,727 figure is a twelve-month-equivalent price comparison, not a promised year-one cash total. Your dose may cost more.
The honest bottom line: AlphaMD's $1,225 annual base model is $612 below Taurus's $1,837 advertised model, $1,036 below Hone's and $1,502 below Male Excel's. Those are differences between the stated models, not verified savings on equivalent care. A higher-priced program may still fit a reader who needs a different payment commitment, treatment format or included lab allowance. For a wider budget comparison, see our TRT cost guide.
What AlphaMD's lower base subtotal requires
It isn't a monthly $98 bill. AlphaMD gets paid for a year in advance at that rate, separately prices ordered labs, and describes injections as its main TRT format. Those are the tradeoffs you can verify—not proof of why the company set its prices. Each matters to your choice: prepaying before you know how you'll respond, a lab bill you can't see yet, and whether the treatment format fits your clinical plan.
Which option fits your situation?
AlphaMD's annual plan may fit a cash-pay reader with a clinician-confirmed diagnosis who wants to compare injections and can accept prepayment. Compare another program or local care if you need a different lab allowance, no fixed commitment, another treatment format or insurance billing. And if having a child is a near-term goal, see a urologist or reproductive urologist before starting any testosterone program. Fertility guidance.

When online TRT isn't the right first step
Diagnosis comes first. The Endocrine Society's July 2026 statement calls for a clinician's assessment of symptoms or signs plus consistently low, accurately measured testosterone, with at least two early-morning, fasting tests. Total testosterone means the overall amount of testosterone in your blood. A common threshold near 300 ng/dL is not a stand-alone diagnosis; the test method, reference range and clinical situation matter. One mailed finger-prick kit, on its own, doesn't meet that bar. Ask how your diagnosis will be confirmed and whether a repeat diagnostic test is included in the price. Endocrine Society statement; clinical guideline.
Some situations need specialist review before routine online TRT care. The Endocrine Society guideline recommends against testosterone therapy, or requires further evaluation where specified, for men with these conditions:
- Plans to have a child in the near term
- Breast or prostate cancer
- A prostate lump, or a PSA (a prostate blood test) above 4 ng/mL, or above 3 ng/mL with high prostate-cancer risk, without a urology workup
- A high hematocrit
- Untreated severe sleep apnea
- Severe urinary symptoms
- Uncontrolled heart failure
- A heart attack or stroke in the last 6 months
- Thrombophilia, a condition that increases the tendency to form blood clots
Those belong with a urologist, endocrinologist or your primary doctor. A past cancer diagnosis also needs an individual review; it is not something a price page or questionnaire can clear. Whether an in-person examination is needed depends on the clinical situation and applicable rules.
Fertility deserves its own conversation. Testosterone therapy can lower sperm production. AlphaMD says this plainly on its own TRT page. If a child is part of your plans, talk with a urologist or reproductive urologist before starting anything. No add-on medicine can guarantee your fertility. Endocrine Society; TRT and fertility.
Emergencies can't wait. If you're on testosterone and get chest pain, sudden shortness of breath, or a painful, swollen leg, get emergency care. Testosterone prescribing information, blood-clot warning.
How much do hCG and enclomiphene cost at AlphaMD?
AlphaMD lists hCG at $300 per 10,000 IU and limits its shop offer to people with an active TRT plan. Enclomiphene is a separate program at $129 to $159 a month—more than the TRT base rate at each listed plan length. Neither is the same as TRT, and neither price establishes what a clinician would prescribe for you. hCG; shop restriction; enclomiphene.
hCG: the add-on that can change your bill
hCG (human chorionic gonadotropin) is a hormone that signals the testes to produce testosterone. It is not testosterone replacement itself. At AlphaMD, the shop listing is only for members with an active TRT plan, billed when ordered and reordered. AlphaMD shop; Pregnyl prescribing information.
AlphaMD lists hCG at $300 per 10,000 IU and says a supply typically lasts 6 to 20 weeks. Multiplying $300 × 52 ÷ 20 and $300 × 52 ÷ 6 gives an annualized medicine-use equivalent of $780 to $2,600. That is not an actual first-year payment range: you pay for whole orders, and the product may reach its use-by limit before its contents are used. AlphaMD's price and supply estimate.
For example, three $300 orders cost $900; nine cost $2,700. Neither is a confirmed order count for your care. Ask for the exact product, the expected paid orders and the pharmacy's use-by dates rather than budgeting fractional vials. Your clinician decides whether you need hCG at all.
Approval depends on the product and use. FDA-approved hCG products exist for specific conditions. Pregnyl's label includes selected cases of low testosterone caused by deficient pituitary signaling; it does not list adding hCG to TRT to guarantee fertility preservation. Using an approved drug for an unapproved purpose is called off-label use. AlphaMD's ingredient name and price alone do not identify the finished product you would receive. Pregnyl label, Indications and Usage.
Enclomiphene costs more than TRT at AlphaMD
Enclomiphene is a pill that nudges your body to make more of its own testosterone. It isn't testosterone, and AlphaMD notes it isn't FDA-approved as a standalone medication. FDA staff's review of its drug application describes the lack of established clinical benefit that prevented approval of that application; availability through compounding is not FDA approval. AlphaMD disclosure; FDA staff presentation, page 39.
| Plan length | Enclomiphene monthly equivalent | TRT monthly equivalent |
|---|---|---|
| 12 months | $129/mo | $98/mo |
| 6 months | $139/mo | $109/mo |
| 3 months | $159/mo | $129/mo |
If you're weighing enclomiphene because it's a pill, know that it costs $30 to $31 more per month than TRT on every plan at AlphaMD. AlphaMD enclomiphene pricing; TRT pricing. These are separate treatment decisions, not medicines to swap based on price.
If fertility is why you're asking
Neither hCG nor enclomiphene is interchangeable with TRT, and neither guarantees you'll stay fertile. The Endocrine Society recommends against starting testosterone therapy in men planning fertility in the near term. If that's you, a urologist or reproductive urologist is the right first stop. Endocrine Society guideline.
Does AlphaMD take insurance, HSA or FSA?
AlphaMD's terms say its services are cash-pay and not billed to insurance, but that does not mean HSA or FSA use is categorically barred. AlphaMD does not promise reimbursement, and an outside lab may bill insurance only if your plan and the lab allow it. Klarna or Afterpay may be offered for payment. Payment terms; outside-lab option.
Three separate questions, not one
- The plan fee: cash-pay rather than billed to insurance.
- Your lab tests: possibly covered at a lab that accepts the order and works with your insurance. Your plan decides.
- HSA or FSA: check with your plan administrator before you count on it. Ask AlphaMD for an itemized receipt either way.
When insurance might be cheaper
If you have insurance and your doctor diagnoses low testosterone, ask your plan whether it covers the visits, required tests and exact FDA-approved testosterone product. Ask about prior authorization, deductibles, copays and the dispensing pharmacy before comparing that written estimate with a cash-pay program. AlphaMD's cash-pay terms do not establish what your insurer will cover, and paying cash does not remove the need for a proper evaluation.

Are there AlphaMD discounts or promo codes?
AlphaMD publishes discounted plan rates for veterans, active military and first responders, along with switching, referral and email offers. The switching page lists two different codes, so confirm which one works before you count on it. None of these offers was checkout-tested for this audit. Published plan discounts; switching offer; new-member offers.
- Veterans, active military and first responders: 20% off the listed plan rates ($78.40, $87.20 or $103.20 a month). AlphaMD says the discount is applied after the consult and requires proof of eligibility. Ask which documents it accepts and how to submit them securely; do not send identification to this website. Discount information.
- Switching from another clinic: a $5 consultation (normally $49) and $50 off your first month. It's for people currently on TRT elsewhere, with a prescription from the last year, or with lab results less than a year old. The page shows code SWITCH2026 in the offer box but SWITCH2025 in its step-by-step section. Ask which one applies. AlphaMD switching offer.
- Referrals: the person you refer gets $30 off their first month, and you get your next month free. AlphaMD referral terms on its homepage.
- Email signup: AlphaMD offers $30 off the first month. AlphaMD new-member page.
We did not find a clear public rule on combining these offers. We left every promotion except the published veteran/military/first-responder plan rates out of our tables, because a budget shouldn't depend on a code that might not work. Confirm whether a first-month credit applies to your chosen prepaid plan and whether the discount continues at renewal.
Does a 12-month TRT plan run past the current telehealth deadline?
A 12-month plan bought in September 2026 runs beyond December 31, 2026, the end date of the current federal temporary prescribing extension. A final special-registration rule is under federal review, but the reviewed record does not establish the final requirements for your care. Ask AlphaMD what happens to visits, costs and prepaid months if requirements change. Current extension; pending rule review.
What's happening
- The current rule: The DEA and HHS extended telehealth prescribing flexibilities for controlled substances a fourth time, effective January 1 through December 31, 2026 (Federal Register).
- The pending rule: The DEA submitted its final "special registration" rule for telehealth prescribing for federal review on August 25, 2026. The official agenda lists November 2026 as a target for final action, not a guaranteed publication or effective date. Official pending-review record; regulatory agenda.
The temporary extension does not waive medical standards or state law. It allows qualifying DEA-registered clinicians to prescribe controlled medicines under stated conditions, including the relevant telemedicine visit requirements; it does not guarantee that AlphaMD can serve every patient without an in-person examination. HHS prescribing guidance; HHS state licensing.
What it could mean for you
The December date alone does not show that online prescribing will stop afterward or that you will need an in-person visit. What matters for this purchase is the provider's written plan for any required change in care—and its treatment of unused prepaid fees. The table below shows starting payments and commitments, not a prediction of money you would lose.
| Plan | First base-plan payment | Minimum base-plan commitment |
|---|---|---|
| Annual | $1,176 | $1,176 across 12 months |
| 6-month | $654 | $654 across 6 months |
| Monthly | $129 | $387 across 3 months |
Our take: The annual plan saves $372 a year over monthly and $132 over the 6-month plan at the published base rates. The 6-month option reduces the first plan payment by $522, or about 44%, compared with annual prepayment; it does not guarantee a refund if care changes. Ask AlphaMD who would arrange and pay for any required in-person visit and what happens to unused prepaid fees before choosing either commitment. AlphaMD pricing; refund terms.
What should you ask AlphaMD before you pay?
Seven written answers turn the advertised price into a budget you can check: your charges and renewal date, your lab prices, your exact medication and pharmacy, your exit terms, your dose price, your discount, and AlphaMD's plan for 2027. If you only ask two, ask about lab prices and what happens if you leave early.
Copy and send this
Hi AlphaMD — before I choose a plan, could you send me the following in writing?
- What I'll be charged today, when my plan payment will be charged, how much, and my renewal date and renewal price. Please include all mandatory membership, shipping, supply or other charges, and confirm availability in my state.
- Which initial, repeat diagnostic and follow-up lab tests I'll need in my first year, when, and what each costs through Assist Health. Is the starter kit included, does it replace any ordered test, and who reviews results and clears refills?
- The exact testosterone product I'd receive, whether it's compounded or an FDA-approved manufactured product, and which pharmacy fills it. Who is my prescriber, and how can I confirm that person's authority to treat me in my state?
- What I'd owe or get back if I leave a 6- or 12-month plan early, or if I'm found not eligible after paying. How do I cancel, and how do I get my lab results, prescriptions and treatment records sent to another clinician?
- The price of my prescribed dose, plus any add-on medicine you'd expect to recommend. For an add-on, please give the exact product, expected paid orders and any use-by limit that changes how often I must reorder.
- Which discount applies to me, and whether it combines with other offers.
- If an in-person visit or care transfer becomes required during my plan, who arranges it, who pays for it, and what happens to my prepaid months, including after December 31, 2026.
Please mark anything that can't be known until after my consultation. Thank you.

How did we check AlphaMD's prices?
We read AlphaMD's current pricing, treatment, terms, contact, help, shop and review pages, checked Taurus Meds, Hone Health and Male Excel the same day, and did the math ourselves. Every price is provider-stated and dated; every estimate shows its assumptions. We didn't buy a plan or complete a consultation.
- Our method: We follow How We Review TRT Providers. It separates verified facts, provider-stated facts, customer-experience signals and our own conclusions. It also tracks the full cost instead of repeating a teaser price—and labels the total incomplete when a required cost is unknown.
- How to tell them apart on this page: Prices and policies are labeled as AlphaMD-stated or as the other provider's own claim. Year-one subtotals and scenario calculations are our math, with the assumptions shown. Review summaries are labeled as individual experiences. "Our read" and "Our take" are our editorial judgment.
- What we don't do: We don't score providers, and being listed here isn't an endorsement.
- Scope: This page covers cost for adult men considering testosterone therapy for low testosterone. It isn't a guide to gender-affirming care, treatment for teens, or non-medical testosterone use.
- What can change: Prices, starter offers, lab allowances, assigned pharmacies, state access, cancellation terms and federal prescribing rules. The “Last verified” date records the check made for this page; it is not a promise that a quote or rule will stay the same.
Our publication standards and correction process are available in Editorial Standards and Corrections. Review our Privacy Policy before sending information; you do not need to submit health records to use the cost worksheet.
What else should you know about AlphaMD TRT cost?
AlphaMD's advertised rate, first payment and complete care budget are different numbers. These answers cover the remaining price and access questions without treating a promotion or one test as a guaranteed care plan.
How much is AlphaMD TRT per month?
AlphaMD publishes $98 a month on a 12-month plan billed yearly, $109 a month on a 6-month plan, or $129 a month billed monthly with a 3-month minimum. Our cost model adds its published $49 consultation and leaves separately billed tests and extras outside the subtotal. Prices; consultation offer.
Is AlphaMD really $98 a month?
Yes, if you prepay $1,176 for 12 months. Paying month to month costs $129. AlphaMD plan prices.
What does the $49 consultation cover?
A visit with a licensed clinician. AlphaMD's July 2026 cost article says it also includes a mailed testosterone test. Its newer lab guide routes testing through a paid order, so ask whether your first labs are included. Consultation offer; current lab guide.
Is the $50 test kit in AlphaMD's shop the same thing?
It's a separate listing: a basic finger-prick testosterone kit for $50. Ask whether that kit is already included in your offer and whether your clinician requires a different or repeat diagnostic test; do not assume one kit completes the evaluation. Shop; Endocrine Society testing guidance.
Does AlphaMD include blood work?
The current lab guide describes separately paid test orders through Assist Health or a downloaded order for a lab that accepts it. A starter-kit offer does not establish that all diagnostic and follow-up blood work is included, and insurance coverage is not guaranteed. AlphaMD lab guide.
How long until my first shipment?
AlphaMD says up to three weeks after your plan is set for new patients. For people switching with recent labs or a current prescription, it says most receive medication within a week of the consultation. New-member information; switching guide. These are estimates, not guarantees.
Can I use lab results I already have?
AlphaMD's switching offer lists recent lab results or a prescription from the last year among its stated entry routes. Your clinician still decides whether those records are sufficient and whether new tests are needed. Switching guide.
Does AlphaMD serve every state?
AlphaMD says most U.S. states, and that you can confirm availability during registration. We did not find a complete excluded-state list in the pages reviewed; confirm access for your location and assigned clinician before paying. Switching guide.
Does AlphaMD have a military or veteran discount?
Yes — 20% off every plan for veterans, active military and first responders, with ID. It's applied after the consult. AlphaMD prices; discount information.
Can I cancel AlphaMD anytime?
Cancel before the disclosed cutoff to avoid future renewals, but that does not by itself erase a minimum commitment or refund a previous payment. Membership fees qualify for a full refund within 72 hours only if no services were requested or rendered; clinical fees and medication have their own limits and exceptions. The public terms reviewed do not spell out early-exit refunds for 6- and 12-month plans. AlphaMD terms.
Is AlphaMD the cheapest online TRT?
AlphaMD has the lowest published base subtotal in the four-program model on this page. We cannot confirm the lowest complete treatment bill because required labs, add-ons and some competing-program fees remain unresolved. We didn't compare every provider. Your own doctor plus insurance may cost less if your plan covers treatment.
Is alphamd.org the same as alphamd.net?
The alphamd.org homepage redirected to alphamd.net when we checked. Use the current official site and verify the full web address before entering payment or health information. AlphaMD homepage.
How much is AlphaMD female TRT?
AlphaMD lists it from $119 a month. Its women's program page says higher doses and additional medicines may cost extra. This page covers men's TRT; the men's diagnostic framework and cost model should not be applied to women. AlphaMD women's program pricing.
