TRT and fertility

External testosterone can suppress sperm production. Anyone who may want biological children now or later should discuss that goal with a qualified clinician before starting TRT.

Why this matters

The Endocrine Society recommends against starting testosterone therapy in men planning fertility in the near term. The AUA guideline also addresses fertility evaluation and counseling in testosterone-deficient men interested in fertility. Recovery of sperm production after stopping testosterone is variable; no website can predict an individual's timeline or outcome.

Questions for a fertility-focused conversation

  • Could the symptoms and low results have another cause that should be evaluated?
  • What is the reproductive timeline, and is fertility evaluation or preservation appropriate before treatment?
  • What are the evidence, regulatory status, risks, and monitoring needs of any alternative approach proposed?
  • Who coordinates care if reproductive endocrinology or urology input is needed?

Some clinicians may discuss approaches other than exogenous testosterone in selected situations. That does not make any alternative suitable, proven for every symptom, or appropriate without individualized evaluation. Do not substitute internet guidance for reproductive or endocrine care.
