What moves your price:
- Your prescription and treatment format. Ask for the price of the exact medication and quantity proposed, not a general hormone-plan starting price. Evolve pricing discussion
- Your state. Evolve's Terms say that in New York, New Jersey, Connecticut and Rhode Island, you pay Labcorp directly for lab work. Terms, section 33(b)
- How much medication you order at once. Evolve permits orders of up to 140 days of most medicines. Its Terms make medication sales final. Terms, sections 33(a) and 33(h)
Evolve may fit you if: your low testosterone has been confirmed, you're comfortable with cash-pay care, you like having a coordinator manage the details, and you're comfortable with an annual care plan once its price and payment schedule are clear.
Look elsewhere if: you need to see the full price before you talk to anyone, you want insurance to pay, you want an FDA-approved product rather than a compounded one, you're trying to have a child soon, or your low testosterone hasn't been confirmed with repeat morning blood tests. What the public documents establish: Evolve publishes a general $75 hormone-plan starting statement, but not a complete TRT price. Its Terms describe an annual care plan, a non-refundable plan after the first physician visit, final medication sales, compounded treatments, and different lab-purchase instructions for NY, NJ, CT and RI. What we could not confirm: your annual care-plan fee, complete first-year total, required initial and repeat-test charges, follow-up lab prices, shipping costs, the exact medicine and pharmacy you'd get, the assigned clinician's credentials and state authority, whether you get your care-plan money back if you cancel before seeing a doctor, and whether Evolve prescribes testosterone in every state it lists.
What we didn't do: book a visit, buy treatment or test Evolve's cancellation. This is a cost guide built from public sources, not a hands-on review.
This guide covers adult men considering care for testosterone deficiency. It does not apply the same testing or treatment framework to women's hormone care, adolescent care, or gender-affirming hormone therapy.
How much does Evolve Telemedicine TRT cost per month?
Evolve's FAQ says a hormone plan starts at around $75 per month. It does not say that this is the medication part, a TRT-specific minimum, or an all-in package. Evolve's other pages give very different ranges, so treat $75 as a general advertising statement rather than a quote. Evolve FAQ

Checked September 24, 2026. These numbers answer different questions. Adding them together would not turn them into an Evolve treatment quote.
The care-plan and lab prices repeated by outside reviews were not confirmed in the current official sources we checked. We have not used them to calculate your budget.
So which number is real? The one Evolve puts in writing for you. Everything else is a clue.
Why the monthly price isn't the whole bill
Evolve's Terms explain how its price is built. Your total includes the fee your licensed provider charges, plus Evolve's platform and administrative fees. Evolve collects both and passes the provider's share along. Terms, section 22
Hone Health publishes a comparable membership split in dollars: $900 a year to its doctors' practice, $78 to the lab and $642 to Hone. Those amounts total $1,620 for membership, not membership plus medication. Evolve describes its fee structure but doesn't show the numbers. That's the gap your written quote needs to close. What will Evolve TRT cost in the first year?
A complete first-year Evolve price cannot be calculated from the public information we verified. The annual care-plan charge, your prescription, required testing and other fees need an itemized quote. The problem is the inputs, not the arithmetic. A general hormone-plan price and a broader assessment estimate do not establish what an Evolve TRT patient buys, how often it is billed, or what is included.

Your first-year budget needs: the care plan, initial and repeat diagnostic labs when required, follow-up labs, medication orders, shipping, separately billed supplies or visits, and any other required charges. Count an included item once, not twice.

What you pay before a doctor reviews your labs

How much you can't get back once care starts
Two things become non-refundable fast:
- The annual care plan, once your first doctor visit happens (Evolve's Terms, section 33(j)).
- Medication purchases, which the Terms make non-refundable and non-returnable (section 33(a)).

Evolve permits up to 140 days of most medicines at once. That does not tell you the price of your order or mean you should buy the maximum. Before paying, ask your clinician and pharmacist how the prescribed quantity, usable life and refill schedule fit together. Terms, sections 33(d) and 33(h)
What year two looks like
Medication and follow-up labs continue if treatment continues. The annual care plan raises a separate renewal question: Evolve's published annual-plan clause does not settle the automatic-renewal process or next year's price. Ask for both in writing. Terms, section 33(j)
Do not assume that all initial testing disappears from later budgets. Your clinician may require more testing, including when you transfer care or your health changes. Keep the next care-plan renewal separate from charges that fall inside your first 12 months. Endocrine Society monitoring guidance
The right TRT provider is not the same for every person — it depends on whether low testosterone has been properly evaluated, your symptoms and health history, your fertility plans, your state, your insurance or cash-pay preference, your treatment-format preference, your budget, and whether online or in-person care is the better starting point. Some situations belong with primary care, urology, endocrinology, reproductive urology, or urgent or emergency care when the symptoms warrant it. Is Evolve's consultation really free?
Evolve advertises a free online consultation form and intake call with its patient care team. Its Terms place the first physician consultation inside the paid annual care plan. Do not assume that requesting the free call includes a free medical evaluation. Evolve contact page · FAQ · Terms

The distinction comes from Evolve's contact page, FAQ and annual-plan terms. It does not guarantee that a particular caller will meet a specific clinician.
Evolve's homepage blurs the line a little. Its step one talks about meeting a board-certified provider next to complimentary-consultation language. Its FAQ and Terms describe separate intake and paid-care steps. So when you book, ask one simple question: "Is my first appointment with a coordinator or a clinician — and when does payment start?" What does Evolve's annual care plan include, and can you get it back?
Evolve's Terms say the annual care plan covers your first physician consultation, any follow-up visits your physician asks for, and unlimited access to your Patient Care Coordinator. After that first physician visit, the plan can't be refunded under those terms. We did not verify a current published price for that plan. Terms, section 33(j)
What you get
- One initial physician consultation.
- Follow-up visits your physician requests. That's not the same as visits you request.
- Unlimited access to a Patient Care Coordinator, which is not a promise of unlimited physician appointments.
Those are the inclusions Evolve publishes. Ask who handles clinical questions between appointments, how to reach that person, and whether a requested extra visit costs more. Terms, section 33(j)
How many doctor visits is that, in practice? Evolve doesn't give a fixed annual number in that clause. One Trustpilot reviewer described getting two calls a year. What if the doctor says TRT isn't right for you?
Evolve's FAQ says it cannot guarantee treatment. Its Terms say the care plan becomes non-refundable after your first physician visit, without giving a specific refund exception for a decision not to prescribe. How much are Evolve's lab tests, and can you use your own?
The current price of Evolve's complete required lab work was not established in the sources checked. Evolve says outside results often work if they include the markers its clinicians need. In New York, New Jersey, Connecticut and Rhode Island, its Terms direct patients to order and pay Labcorp directly. Evolve FAQ · Terms, section 33(b)
Where your blood is drawn
Evolve says it contracts with Labcorp. For an Evolve-arranged lab order, you print the order from Evolve's patient portal and bring it with you. Its Terms say that if Labcorp tries to collect payment for that order, contact Evolve at (800) 407-4117 rather than paying again. This instruction does not cancel the separate direct-purchase rule for NY, NJ, CT and RI. Terms, section 33(b)
No Labcorp nearby? The Terms say to ask your primary care doctor to order the recommended labs. Ask who pays for that visit and testing; those costs were not established here. Terms
Using labs you already have
Evolve's FAQ says outside results often work if they include the key markers. Ask Evolve how to submit them securely before you buy new labs, and get confirmation of what is accepted and what is still missing. If your clinician orders Comprehensive Testosterone, a separate Total Testosterone, CBC and PSA, their listed prices add up to $326: $159 + $69 + $29 + $69. That is an arithmetic example before discounts, not Evolve's required starter package. Other tests may be needed. Confirm collection timing and the full order before buying.
A few quick definitions. Total testosterone is all the testosterone in your blood. Free testosterone is the small part not attached to proteins. SHBG (sex hormone-binding globulin) is a protein that holds onto testosterone. Hematocrit is the share of your blood made up of red blood cells. TRT can raise it, which is why it gets checked. LH and FSH (luteinizing hormone and follicle-stimulating hormone) are signals from the brain's pituitary gland to the testicles. They help a clinician investigate why testosterone is low. Do not assume that a testosterone test includes them; ask what cause-finding tests your clinician needs and what they cost. Endocrine Society guideline
One blood test isn't a diagnosis
The Endocrine Society's guideline says low testosterone should be diagnosed only when you have symptoms or signs that fit and testosterone is clearly and consistently low. It recommends confirmation with a repeat morning fasting testosterone measurement, using an accurate test. Its July 16, 2026 statement again calls for at least two early-morning, fasting tests and notes a common clinical threshold near 300 ng/dL. Follow-up labs
Evolve describes follow-up labs about 8 to 12 weeks after treatment starts. That is a provider-stated process, not proof of a fixed panel, price or schedule for every patient. We could not confirm the current complete follow-up panel or whether every needed test is included in one charge. Evolve treatment-process article
Ask whether follow-up includes a CBC with hematocrit, which other tests are needed, when the clinician reviews them, and whether that review costs extra. The Endocrine Society recommends checking hematocrit before treatment, at 3 to 6 months and then annually, with further evaluation when clinically needed. What medication does Evolve prescribe, and is it FDA-approved?
Evolve lists testosterone as injections or a topical cream, and its Terms say its treatments are compounded by partner pharmacies. Compounded medicines are not FDA-approved. An FDA-approved product requires a different, explicitly confirmed prescription and dispensing path; that may be through primary care, urology, endocrinology or an appropriate online clinician. Compounded vs FDA-approved, in plain words
An FDA-approved product went through the FDA's review for safety, effectiveness and quality before it was sold. A compounded medicine is prepared by a pharmacy or other authorized compounder. The FDA says compounded drugs are not FDA-approved and do not receive that premarket review. FDA compounding guidance
That doesn't make a compounded product bad. It does mean the two aren't the same thing, and prices aren't apples to apples. Compounding can meet particular medical needs, but quality problems can cause harm; a low price is not evidence of quality or clinical suitability. FDA
Evolve says its partner pharmacies are FDA-inspected and based in the United States. We did not verify which pharmacy would fill your prescription or its inspection and licensing record. Ask for its name, address, state license and the exact medication, ingredients and concentration. You can check the named pharmacy with the relevant state's board of pharmacy. How ordering works
Evolve's Terms describe:
- Order size: up to 140 days of most medicines at once.
- Processing: allow 10 business days after full payment for tracking information. Transit begins after the pharmacy ships; that is not a promise of delivery within 10 business days.
- Multiple boxes: if your order comes from more than one pharmacy, it arrives in separate packages. The Terms mention UPS or FedEx and require a physical delivery address, not a PO box.
- Problems: report missing or damaged items within 48 hours, with photos.
These are provider-stated rules, not delivery times we tested. Confirm the pharmacy, shipping charge and expected arrival date for your actual order. Terms, sections 33(d), 33(e) and 33(h)
The date on your label
Compounded medicines carry a beyond-use or use-by date, and there may also be a shorter limit after opening. Evolve's Terms mention a pharmacy instruction to discard a product after 28 days, then suggest compounded medicine may last beyond its label date. Follow the date and instructions on your pharmacy label. If something doesn't add up, ask the pharmacist rather than relying on website marketing. The same Evolve clause says that quantities extending past a use-by date are held as a refill. Ask how that works for your order and what you pay now versus later. Terms
This matters for cost too. A big order only saves money if it is actually cheaper per usable supply and you can use it as prescribed before the applicable label date.
Refills
Evolve's Terms ask you to contact your coordinator at least two weeks before you run out. One reviewer reported allowing 20 to 25 days in practice. That is an individual experience, not a tested delivery standard or a reason to change the prescribed quantity. Terms, section 33(h) · Ann's May 28, 2026 review
Ask who handles a delayed refill, how to reach a clinician, and how records are transferred if care must move. Do not change your dose or treatment schedule on your own.

Does Evolve Telemedicine take insurance, HSA or FSA?
Evolve says it doesn't accept or bill any insurance company. It says you can pay with an HSA or FSA card, but card acceptance does not establish that every fee is an eligible medical expense. What an HSA saves you
An HSA is a health savings account; an FSA is a flexible spending arrangement. Both have tax rules, and they are not interchangeable. Qualified medical expenses can receive favorable tax treatment when the account's requirements are met. IRS Publication 969
For a simple illustration, $1,000 of eligible expenses × a 22% federal income-tax rate = $220 in potential federal income-tax savings from using eligible pre-tax funds rather than taxed earnings. This is not an Evolve price or a discount on its bill. Your tax situation, contribution rules, reimbursements and account eligibility decide the actual result.
Ask your account administrator about the specific care-plan, testing and prescription charges. Do not assume a platform or membership fee qualifies just because a payment card works. IRS guidance
Can you get reimbursed by insurance yourself?
We did not find a clear superbill policy in the Evolve pages checked. A superbill is an itemized receipt used to request insurance reimbursement. Ask Evolve whether it provides one, then ask your insurer about the exact clinician, service and medication; a receipt is not a coverage guarantee. Evolve insurance statement
If insurance matters most
Start with your insurer's network, benefits and medication coverage rules. Ask which visits, tests and exact FDA-approved products it covers, and what approval steps apply. Do not assume that Evolve's cash-pay services or compounded prescription will be reimbursed.

Can Evolve's price go up after you start?
Yes, Evolve's Terms say its medication list is updated and ingredient supply and demand change with market conditions. They do not promise that your starting price is locked. A different prescription or order quantity can also change the bill, so ask for the next order's price before authorizing it. Terms, section 33(i)
Two things from Trustpilot are worth knowing — as individual reports, not proof of a policy:
- June 16, 2026: a reviewer using the name L A reported a sharp prescription-price increase over three months and compared the price with another company. Evolve replied on August 4, saying its pricing reflects physician oversight, care coordination, support and compliance, and that medication-only comparisons omit those services.
- August 6, 2026: a reviewer named Shauna, discussing women's hormone care rather than TRT, reported a price increase without notice. The review gives January 12, 2026 as the experience date; it is not evidence of an August price change for TRT patients.
These reports are on Evolve's Trustpilot profile. They do not establish how often increases occur, whether the compared prescriptions were identical, or what your bill will be.
How to protect yourself:
- Get your price per order in writing.
- Ask how much notice you'll get before a price change.
- Confirm the quantity, label dates and cancellation point before authorizing a non-refundable order.
Where can Evolve's refunds, refills and fine print affect your cost?
The annual care plan is non-refundable after your first physician visit, and medication sales are final under Evolve's Terms. Stopping future charges is also a different thing from getting past charges back. A few lines in those Terms are worth reading before you sign. Evolve Terms

Stopping future charges isn't the same as getting money back
When you cancel, ask two separate questions. "What will you stop charging me?" and "What, if anything, will you refund?" Get both answers in writing, plus a confirmation that billing has ended. And ask how to get a copy of your medical records if you switch providers.
Keep the cancellation request, response and billing confirmation. Ask whether cancellation affects the care plan, an unprocessed medication order, future refills, or all three. Evolve's annual-plan clause does not supply a complete cancellation-and-renewal workflow. Terms

We can't say whether Male Excel beats Evolve on price without Evolve's quote. Male Excel's advertised injection-program components produce a $2,727 subtotal: $99 initially, plus 12 × ($120 medication + $99 membership). That excludes shipping and is not a verified all-in first-year bill. Its medication price is a monthly equivalent for 60-day orders, so the dates and amounts actually charged need checking. Is Evolve Telemedicine legit?
Evolve has an active public website, contact process and Terms naming E-Volve Company, Inc. as the platform operator. That establishes what the business publicly represents, not the quality of an individual clinician or prescription. How Evolve is set up
Evolve's Terms say it's a management platform. It handles scheduling, billing, technology and care coordination. It doesn't practice medicine or run a pharmacy. The clinicians are independent professionals who make their own medical decisions, and the pharmacies are separate businesses. Terms
It does mean your fee is split between the provider and the platform — which is why the itemized quote matters. Terms, section 22

These are not four complete, comparable first-year quotes. For Male Excel, 60-day orders can put a charge inside the first year for medication used later. For Hone, a vial price cannot be turned into a monthly medication cost without the prescribed quantity and usable supply. For Taurus, clarify the advertised rate, any separate fee and the refund scope before using its subtotal. Evolve still needs the itemized quote. Which one fits you?
These are decision rules, not a cheapest-provider ranking:
- You want Evolve's coordinator-led care and can accept an annual plan: keep Evolve in consideration after you get the quote and refund answers in writing.
- You want to inspect a published testing and membership breakdown: Hone provides one, but medication still needs its own price and quantity.
- You want to compare a published medication-plus-membership model without an annual care-plan commitment: look at Male Excel's fee structure and actual cancellation timing; final medication sales still matter.
- You are considering Taurus's $49 entry offer: resolve the separate-fee and refund questions before paying. The entry price is not a complete diagnosis or a prescription guarantee.
- You want insurance or an FDA-approved product: start with a clinician and pharmacy that can meet those requirements, often through your existing health system.
- You're planning to have a child soon: a urologist or reproductive urologist first. More on that below.

Who should choose Evolve, and who should start somewhere else?
Evolve may fit a man whose low testosterone has been confirmed, who wants a coordinator-led cash-pay program, and who's comfortable with its quoted annual-plan terms. It is not an insurance-billing option or a confirmed FDA-approved-product path. First, make sure it's really low testosterone
Tiredness, low sex drive and weight gain have many causes. That's why guidelines ask for symptoms plus consistently low levels on repeat early-morning tests. If you want children soon
Testosterone therapy can lower sperm production. The effect varies, and nobody can promise that fertility will be preserved or recover on a particular schedule. The Endocrine Society advises against starting testosterone in men planning fertility in the near term. If a child is in your near-term plans, talk to a urologist or reproductive urologist before starting any testosterone program, including Evolve. Other treatments exist for that situation. When specialist or in-person care should come first
A history of prostate or breast cancer, elevated hematocrit, untreated severe sleep apnea, a recent heart attack or stroke, or known pituitary or testicle problems needs a clinician's focused assessment. The Endocrine Society's guideline identifies several of these as reasons not to start testosterone without resolving the relevant risk or evaluation. What changed on testosterone labels in 2025 and 2026
In February 2025, the FDA announced removal of boxed-warning language about increased adverse cardiovascular outcomes from testosterone product labels. That followed the TRAVERSE trial, which did not find an increase in major cardiovascular events in the population studied. The FDA also required blood-pressure warning changes. There is a newer update. In June 2026, the FDA requested removal of the labeling limitation about unestablished safety and effectiveness in age-related hypogonadism, along with revisions to prostate-cancer and enlarged-prostate information. That was a request to update prescribing information, not proof that every product label had already changed. Check the current label for the exact FDA-approved product. These actions do not approve compounded products or mean TRT is risk-free. In its July 2026 statement, the Endocrine Society reiterated the need for a sound diagnosis, risk assessment and ongoing monitoring, and noted remaining long-term safety gaps. Your year-one cash cost = care-plan payments + initial and repeat diagnostic labs + follow-up labs + medication orders + shipping + separately billed supplies or visits + other required charges actually paid during the first 12 months.
If each order has the same price, medication cost is price per order × number of orders charged during that period. Do the same for shipping. If quantities or prices differ, add the individual orders instead. Do not count an included charge twice.
Use actual proposed billing dates. Do not turn a 60-day or 140-day supply into a monthly invoice by assuming every month has 30 days. A charge near the end of year one may buy medicine used in year two; show that rather than hiding the cash outlay.
Keep three numbers separate: cash due before the first physician visit, cash paid during the first year, and the next renewal commitment. Divide the first-year total by 12 for a monthly budget — just remember that isn't how Evolve bills you.
Check the quote, not an invented price target. A lower number is not a better deal if it omits testing, medicine or visits that the other quote includes. There is no verified Evolve-specific threshold here for a strong, typical or overpriced quote.

Will you need an in-person visit for online TRT in 2027?
The current temporary federal rule runs through December 31, 2026; it does not establish a universal answer for 2027. During the extension, qualifying clinicians may prescribe controlled medicines through telemedicine without a prior in-person examination when all legal conditions are met. Here's the background. The Ryan Haight Act generally requires an in-person medical evaluation for an internet prescription of a controlled substance unless an exception applies. The fourth temporary extension continues specified telemedicine flexibilities from January 1 through December 31, 2026. It is not a permanent waiver of prescribing rules. DEA and HHS final rule
What this means for you:
- Through December 31, 2026, the temporary federal framework can permit testosterone prescribing after a real-time audio-video evaluation without an earlier in-person exam, when its conditions are satisfied.
- State law still applies. The federal extension does not grant a clinician authority to treat or prescribe everywhere.
- For care after 2026, ask about the applicable rule then, any in-person evaluation or referral needed, and who would pay for it. This extension alone does not settle those costs.
The prescription must have a legitimate medical purpose in the usual course of professional practice and meet the clinician's registration requirements. Current federal regulation, section 12.1
If you're starting close to the end of the year, ask Evolve how it plans to handle patients if an in-person exam becomes required. Get the records-transfer and referral process along with the price; do not assume that a year of paid care guarantees unchanged access rules.
How did we research Evolve's TRT cost?
We read Evolve's public pages and Terms on September 24, 2026, checked its Labcorp partner catalog, and compared the published price components of three other providers. We didn't sign up as patients. Each comparison distinguishes published provider claims, customer reports, our arithmetic and facts that remain unconfirmed.
We followed How We Review TRT Providers, our published method. It keeps four things separate:
- Verified facts — what a named source actually published on the date checked, and calculations that can be repeated from disclosed inputs.
- Provider-stated facts — claims about care, availability or fulfillment that we can attribute but have not independently tested.
- Customer-experience signals — individual reviews, which can raise questions but cannot establish a policy or a medical result.
- Our conclusions — judgments such as asking for a complete quote before a non-refundable commitment, grounded in the evidence shown.
We calculate complete cost when the inputs support it rather than repeating a starting price. For Evolve, those inputs are incomplete, so we supply the quote-gap tables and worksheet instead of an invented total. We don't give providers numeric scores, and being listed here isn't an endorsement.
