What can change your number: add-on charges, the renewal rate, and what your plan actually covers. State authorization, fertility plans, and a need for a covered FDA-approved product can change whether Royal is the right option at all. Billing two months at once would change your first payment, not necessarily the yearly total.

Best for / Royal is a fit to consider if: you're a cash-pay adult man with low testosterone confirmed by proper testing, you want one flat price with labs and visits folded in, and you're comfortable with injections and any required video or in-person examination. Royal still needs to confirm your exact plan and approve care.

Not for you if: you need an insurance-covered FDA-approved prescription (see the insurance route), you're trying to have a child soon (see fertility), or Royal can't identify a prescriber authorized to treat you (see licensing). A licensed clinician must decide whether TRT is appropriate; this comparison does not establish eligibility.

How much does Royal Medical Center TRT cost per month?

Royal Medical Center's TRT plans are advertised at $75, $89, or $195 a month on its current program-options page. All three include testosterone cypionate injections, labs, physicals, and provider consultations, according to Royal. The higher plans add an estrogen blocker ($89) or an estrogen blocker plus gonadorelin or enclomiphene ($195). 1

Here's the plain-English version of each plan.

$75 — testosterone only. Testosterone cypionate is a long-acting injectable form of testosterone. This is the simplest plan and the lowest-priced of Royal's three listed TRT plans. 1 17

$89 — testosterone plus an estrogen blocker. Royal's shop lists this plan as "Testosterone/Anastrozole." Anastrozole is a drug that lowers estrogen. (More on whether you need it below.) 3 18

$195 — the comprehensive plan. Everything in the $89 plan, plus gonadorelin or enclomiphene. Royal says these help the testes keep working while you're on testosterone. The prescription depends on your clinician's assessment, not just which bundle you pay for. That is Royal's stated purpose, not a verified promise to protect fertility. 1 15

Why you may have seen $119, or a different "starting" price

Royal's public pages don't all match. We lined them all up so you don't have to open six tabs:

The pages don't establish why the wording differs or which renewal price applies to your agreement. The mismatch is a reason to ask for written terms, not proof of hidden charges.

But the difference is real money. If someone quotes you $89 for month one and $119 after that, your year costs $1,398, not $1,068. That's $330. So ask one question before you pay: "What will I be charged in month one, and what will I be charged in month two?" Get the answer by email. This is a hypothetical quote calculation, not a verified Royal renewal offer.

Royal's FAQ says hCG is "sometimes prescribed," depending on your labs and goals. If it's added, ask whether it's inside your plan price or on top of it. 8

Royal's price-beat guarantee

Royal's homepage promises to beat a competitor's price by 25% if you find "another clinic that is equal to ours for a better price." We did not find public rules explaining how "equal" is judged or how to claim it. If you plan to use it, ask for the rules in writing first. 6


What does Royal Medical Center TRT cost for a full year?

At Royal's listed prices, twelve monthly fees total $900 on the $75 plan, $1,068 on the $89 plan, and $2,340 on the $195 plan. Those are fee subtotals that assume the monthly price never changes. Add-on medications or a promotional price that rises after month one would change the total; a different billing interval alone changes when you pay. 1

When could the $195 plan cost less than separate add-ons?

This one's worth thinking through. The $195 plan includes gonadorelin or enclomiphene. Bought separately, enclomiphene is listed at $150. 1 5

If that $150 is a monthly price, then $89 + $150 = $239 a month. That's $44 more than the $195 bundle. Over a year, it's $528 more.

So here's the rule: compare the bundle only after your prescriber has explained which medication, if any, is appropriate and Royal confirms the same product, supply period and extra cost. The $75 and $89 plan fees are $1,440 and $1,272 lower per year than $195, respectively, but the plans contain different drugs. Confirm the $150 billing period before you do this math with real money.

How Royal bills: monthly, or two months at once?

We did not find Royal's billing interval in the public pages reviewed. One customer reported two-month billing in a Trustpilot review posted September 1, 2024:

"You will be billed for 2 months at time" — Phil Scott II, Trustpilot, September 1, 2024; experience dated August 31, 2024 (one customer’s report; not verified by us) 13

If that's still how it works, your first charge on the $195 plan could be $390, not $195. Same yearly total, bigger first bite. Ask.

Your money before you're approved

Royal's program-options page says two important things, in its own words: payment "does not guarantee treatment or a prescription," and if you're found medically ineligible, "your program payment will be refunded." 1

That's a meaningful protection, and we give Royal credit for printing it. But it's narrow. It covers being turned down; it does not establish your refund rights if you change your mind later. We did not find the post-approval cancellation terms in the public pages reviewed (more below).

Use this before you call. Fill in only what Royal confirms in writing. Leave anything unconfirmed marked "unknown" — don't count it as free.

If an upfront payment covers the first two months, count those months once, not again as an extra fee. Your complete first-year cost is the sum of all confirmed required charges, less confirmed credits; it remains unknown while required charges are unresolved.


What does Royal's monthly fee include — and what isn't written down?

Royal says every TRT plan includes testosterone, provider consultations, labs, physicals, and medication; its FAQ also describes supplies, and its $195 TRT page advertises free shipping. The public pages we checked do not define the full lab panel, later testing cadence, how an out-of-state patient completes the physical, or post-approval cancellation terms. Those are the gaps to close before your first charge. 1 7 8

What "labs" should mean

This matters because "labs included" can mean one test or ten. The Endocrine Society calls for compatible symptoms or signs plus consistently low testosterone, confirmed with repeat early-morning fasting testing using accurate assays. Its July 2026 statement specifies at least two tests and describes a common threshold near 300 ng/dL — a guide, not a universal diagnosis. One low number isn't a diagnosis. 14 15

Once treatment starts, monitoring usually includes your testosterone level and your hematocrit (the share of your blood made of red blood cells, which testosterone can push up). PSA (prostate-specific antigen, a prostate blood test) and prostate monitoring depend on age, risk and a discussion with your clinician. Blood pressure also matters: FDA-required testosterone labeling warns about increases. 15 25

So ask Royal by name: "Which tests are in my baseline panel, which are in the 90-day follow-up, and how often after that?"

A 90-day treatment check is not a substitute for confirming the diagnosis before treatment. For the steps to discuss, see TRT blood tests.

A low price should not bypass a medical risk review. A history of prostate or breast cancer, high hematocrit, untreated severe sleep apnea, or a recent heart attack or stroke needs clinician assessment before a treatment decision. Primary care, urology or endocrinology may be the right starting point. 15

The right TRT provider is not the same for every person — it depends on whether low testosterone has been properly evaluated, your symptoms and health history, your fertility plans, your state, your insurance or cash-pay preference, your treatment-format preference, your budget, and whether online or in-person care is the better starting point. Some situations belong with primary care, urology, endocrinology, reproductive urology, or urgent or emergency care when the symptoms warrant it. Who prescribes at Royal Medical Center, and are they licensed in your state?

Royal names one physician on its About page: Dagoberto Rodriguez, MD, whose Florida medical license is clear and active. Your clinician must be licensed or otherwise authorized under the rules of the state where you're located, and we did not verify a complete Royal clinician-by-state roster. Ask Royal who will prescribe for you, then check that person's authorization with the appropriate state board. 9 10 26

This is also part of the answer to "Is Royal Medical Center legit?" Royal publishes a clinic address at 1000 E Hillsboro Blvd, Suite 102, in Deerfield Beach, Florida, a named physician and a named pharmacy group. Its customer reviews are mostly positive. None of that proves that a particular prescription is appropriate or lawful in your state. And its named physician has a public licensing record you should know about. 9 10 13

What the public record shows

We read these records directly from the Florida Department of Health and the Oregon Medical Board on September 23, 2026.

What this means for you

Let's keep this in proportion. The Oregon order is from 2021, and the other-state entries are dated 2022; the profile also contains the later Florida status noted above. His Florida license is active today. These records do not establish how Royal treats patients now, or the present license status of every clinician who works there. 10 11 12

What the record does tell you is that state licensing at an online clinic isn't a formality. Testosterone is a Schedule III controlled substance — a drug the federal government regulates more tightly than an ordinary prescription because it can be misused. A valid prescription and the required state and federal prescribing authority are still necessary. 26 27 28

We did not verify which Royal clinicians cover which states. Customer reviews mention other staff and clinicians, but a review is not a license record. A national ordering website also does not prove that a clinician can treat you in every state. 13 26

So the fix is simple, and it's in your hands.

How to check your prescriber's license

  1. Ask Royal for the full name of the clinician who will prescribe for you.
  2. Search that name on the appropriate state licensing board’s website. For a physician, use the state medical board directory or DocInfo, run by the Federation of State Medical Boards. For a nurse practitioner, check the nursing board. Ask the relevant board about any separate telehealth authorization. 26 40
  3. Confirm current authorization in your state, and read any listed restrictions or board actions. An active license and a disciplinary history can appear together.

If Royal can't give you a name, that's your answer.

Need the prescriber's name before you pay?

A named medical director is not the same as knowing your assigned prescriber. We did not verify that Male Excel identifies each patient's prescriber before payment. Ask either clinic for that name and state authorization; a local practice where you book a named clinician is another care route.

Male Excel also costs more at its published starting rates: the consultation, membership and medication arithmetic reaches $2,727 for injections or $2,871 for cream, before shipping and any other unconfirmed charges. These are advertised-rate subtotals, not complete first-year quotes. Its cream is compounded, and its conditional refund guarantee covers membership fees only. A higher price does not settle the prescriber question. 31 32 33

Is Royal Medical Center's testosterone compounded or FDA-approved?

Royal doesn't identify the exact testosterone product a given patient will receive. Its website says a provider may prescribe a compounded drug as an alternative to an FDA-approved product, and it names Belmar Pharma Solutions as its dispensing pharmacy group. Compounded drugs are not FDA-approved; ask for the exact product before enrolling if FDA approval is a requirement. 1 16

FDA-approved means the FDA reviewed the product's safety, effectiveness, and manufacturing before it could be sold. Compounded means a pharmacy prepared the drug, often for an individual prescription, without that FDA approval of the finished preparation. Compounding is permitted when applicable legal requirements are met. It just isn't the same thing, and the two should never be treated as identical. 16

Credit where it's due: Royal prints a compounding disclosure on the product pages we checked and names its pharmacy group. That is useful disclosure, but it does not identify your exact prescription or filling location. 1 2 3 4

Which Belmar pharmacy fills your prescription?

Belmar runs two kinds of facilities. Its own website says its 503B outsourcing facilities make batches for office use, while its 503A pharmacies fill individual prescriptions. Ask Royal which exact Belmar location fills yours and verify that pharmacy’s authorization to dispense to your state. The group name alone does not settle that question. 24

FDA-approved testosterone and compounded testosterone are different products

Non-testosterone medications: what else comes in the package

Keep these separate in your head. Each is a different drug with a different regulatory status. None of the medications in the next table is testosterone replacement therapy.

For anastrozole, clomiphene or hCG, approval of a named manufactured product does not make a compounded preparation FDA-approved. Ask what will actually be dispensed. 16

Enclomiphene’s compounding status: an FDA advisory committee voted in 2022 against adding enclomiphene citrate to the 503A Bulks List. FDA’s list updated May 14, 2026 still places it in Category 1, substances under evaluation. The advisory vote is not a finished-drug approval or, by itself, a final nationwide compounding ban. 21 22

Do you have to take the estrogen blocker?

An estrogen blocker is listed in the $89 and $195 plans. Whether you need it is a medical question for your prescriber, not a pricing question. In a Trustpilot review updated February 16, 2026, Matthew Beachy reported a disagreement over an estrogen-blocker recommendation; we did not verify his labs or the clinical reasoning. 1 13

Ask before you choose a plan: "What lab result or clinical reason would make you add an estrogen blocker, and is it a separate medicine or part of a compounded preparation?" If there's no clear answer, ask the prescriber to explain the options, including whether the $75 plan fits your actual care needs. Do not choose or change a medication just to fit a price tier.


Will Royal Medical Center TRT affect your fertility?

It can. Testosterone from outside the body can lower or stop sperm production (spermatogenesis), and no program can promise to protect or restore fertility. Royal adds gonadorelin or enclomiphene to its $195 plan and says they help maintain natural function, but anyone planning children should discuss fertility with a qualified clinician before treatment. 1 15 39

The Endocrine Society recommends against starting testosterone therapy in men planning fertility in the near term. Plans for children later still belong in the conversation; neither infertility nor recovery on a set schedule can be predicted for an individual from a program description. 15 39

Royal's marketing says its testicular stimulant "preserves natural testosterone production," and one of its pages says clomiphene can "increase sperm volume." Those are Royal's claims. Clomiphene, enclomiphene, and hCG are not TRT, and they aren't interchangeable with it. Enclomiphene has never been FDA-approved. Paying $195 instead of $75 is not a fertility plan. 4 7 19 20 23 39

Does Royal Medical Center take insurance, HSA, or FSA?

Royal advertises flat cash prices, but we did not verify insurer billing or HSA/FSA paperwork for these plans. We couldn't find whether Royal provides itemized receipts or a letter of medical necessity for an HSA or FSA claim. To use insurance, check an in-network clinician and a covered FDA-approved prescription first; that clinician may be local or online.

If you have insurance, ask about coverage for the clinician, the exact FDA-approved testosterone product and the required lab work. Check any prior-authorization requirement, copays and deductible with your insurer. Then compare your confirmed out-of-pocket total with Royal's written quote, not just its $900–$2,340 fee subtotals.

To use an HSA or FSA, ask Royal for an itemized receipt showing medical services and medication separately, and ask your account administrator which expenses qualify and what documentation is needed. Request a letter of medical necessity if the administrator requires one. A bundled membership payment is not automatically a qualified expense just because a provider accepts the card. 30


Can you cancel Royal Medical Center, and what if you're not approved?

Royal's program-options page says your program payment will be refunded if you're found medically ineligible. We did not find post-approval cancellation or refund terms in the public pages reviewed. Customer reports raise questions about two-month billing and missed-appointment fees, so get the applicable rules in writing before your first charge. 1 13

Here's what we found, both sides included:

  • If you're turned down: Royal says your program payment is refunded. Ask which payments count, whether anything is deducted, and how long it takes. 1
  • Missed appointments: A July 23, 2024 Trustpilot reviewer reported a $40 charge for a missed visit. In its September 11, 2024 public reply, Royal said patients are charged only after multiple missed appointments. We did not verify a current fee schedule. 13
  • Early cancellation: We did not verify an early-cancellation fee. Ask for the terms that apply to your agreement.

Before your first charge, get these five answers in writing

  1. Is there a minimum commitment?
  2. How do I cancel — phone, email, or portal — and how much notice do you need?
  3. Is my final charge prorated, or is a full two-month cycle billed?
  4. What happens to medication that's already been shipped?
  5. How do I get copies of my labs and prescriptions if I leave?

There is a published records-access route. Royal's privacy policy says patients may request access to their health records and lists its Privacy Officer at 844-965-0305. Ask for the request process, any applicable fee and how records can be sent to your next clinician. The policy is an access statement, not proof that we tested its response time. 29

One more thing. Cancelling a billing plan isn't a medical plan. If you're already on testosterone, talk with your prescriber — or your next one — about continuity before you stop. Keep your lab results so a new clinician isn't starting from zero.

These are calculations from published prices, not equivalent packages or guaranteed total bills. Male Excel labels its medication prices as monthly equivalents for 60-day supplies billed every other month; six 60-day fills cover 360 days, not a full 365-day year. Confirm the actual charge dates and whether another refill falls inside your first year. Its advertised injection/cream bundles also include thyroid medication; that is a separate clinical decision, not a standard requirement for TRT. 31

If injections rule Royal out for you, Male Excel advertises a compounded testosterone cream. It is not an FDA-approved alternative, and the advertised fees are higher. Royal had a 4.5-star rating from 2,033 Trustpilot reviews when checked on September 23, 2026, with 10 reviews in the past 12 months. The score therefore mostly reflects older experiences. Positive examples praise the flat price, shipping and staff; negative examples raise billing, appointment-fee or prescribing-communication concerns. 13

A few things worth knowing about that 4.5:

  7. Is my testosterone an FDA-approved product or compounded? What is its exact name, supply duration, and filling Belmar pharmacy location?
  8. On the $195 plan, will I get gonadorelin, enclomiphene or the clomiphene mentioned on your other page, and why?
  9. What lab result or clinical reason would make you add an estrogen blocker? Is it separate or part of a compounded preparation?
  10. How do my current or future plans for children change the evaluation and the clinician I should see?
  11. How do I cancel, how much notice do you need, and is anything charged if I cancel early?
  12. Is there a missed-appointment fee, and how does your 25% price-beat guarantee work?

Also request the first-shipment estimate, the refill-request deadline, how to reach a clinician between visits, and the process for getting your records. A low monthly fee is not much help if you cannot get those basic service details.

Yes, when the prescription and care meet federal and state requirements. Testosterone is a Schedule III controlled substance, and the fourth temporary federal extension permits qualifying telemedicine prescribing without a prior in-person examination through December 31, 2026. It does not remove state authorization, clinical-assessment or other prescribing requirements. 26 27 28

The U.S. Drug Enforcement Administration (DEA) and the Department of Health and Human Services (HHS) extended these telehealth flexibilities for a fourth time, covering January 1 through December 31, 2026. That matters to every online TRT buyer, not just Royal's. December 31 is this extension's end date, not a promise that the same rules will apply afterward. 28

Two things the federal rule does not do: it doesn't override state practice rules, and it doesn't guarantee any clinic can treat you. Ask Royal which examination and visit format your clinician requires, including any in-person visit. There's no legal shortcut around a valid prescription, and any website offering testosterone without one is one to avoid. 26 28
