Why GoodRx shows you more than one price

If you've checked GoodRx and seen a different number than a friend, you're not imagining it. On the same day, GoodRx showed us several figures for the same 1 mL vial:

  • A headline "as low as" price of $24.00, against an average retail price of $47.67.
  • A package table price of $27.92.

These are different displays, not two verified checkout quotes for the same order. The only price that counts is the one your pharmacy quotes for your exact prescription. Check the product, package, pharmacy, location and any paid-membership requirement before comparing a coupon with a cash price. What pharmacy acquisition data can—and can't—tell you

The federal government surveys pharmacies and publishes the National Average Drug Acquisition Cost (NADAC), a benchmark used in Medicaid reimbursement. Is a 10 mL vial cheaper than 1 mL vials?

On paper, yes — by a lot. A 10 mL generic vial cost about one-sixth to one-third as much per mL as a 1 mL vial in the prices we checked. But the 1 mL products we label-checked are single-dose and the 10 mL products multiple-dose, so real savings depend on the prescribed package, discard rules and usable supply—not just vial volume. The label detail that decides the real savings

Once a multiple-dose vial has been opened, the CDC's injection-safety guidance for healthcare settings says to date it and throw it out within 28 days — unless the manufacturer says something different for that vial. And never past the printed expiration date. CDC injection-safety guidance

That guidance is written for clinics and hospitals. Your own vial's instructions come from its label and your pharmacist. But it matters for your budget. If a 10 mL vial would last you longer than its label allows, some of those savings go in the trash.

Single-dose means one use. Do not count leftover medication in a single-dose vial as a future supply, even for the same person. Ask your pharmacist to price the quantity needed under the actual prescription and discard instructions. CDC injection-safety guidance

Our take: The 10 mL package has a lower listed price per mL in this comparison. Whether it lowers your actual bill needs a usable-supply quote. Just don't assume it. Ask.

Five questions to ask before you pick a package

  1. "What's the cash price for a 1 mL vial and for a 10 mL vial of my prescription?"
  2. "Is the 10 mL vial single-dose or multiple-dose, and how long is it good after I open it?"
  3. "Which manufacturer's product will you give me?"
  4. "Is it in stock right now?"
  5. "Does my prescriber need to change anything for me to get a different package?"

Any package change must be consistent with your prescription and confirmed by your pharmacist and, when needed, your prescriber. These questions just make sure you're comparing real prices.


Why do approved testosterone cypionate packages cost about $2 to $126 per 100 mg?

Package and brand change the listed price sharply: the verified references run from about $1.79 per 100 mg in a 10 mL generic vial to $126.10 in the Azmiro prefilled-syringe package. These are different approved products and price displays, not interchangeable patient quotes or complete care costs.

We built this table by putting the verified consumer package prices into the same unit. It compares package economics, not prescribed doses, clinical suitability or how much medication you can use before discarding a container.

What online programs are really charging for

A medication line and a membership line buy different things. Look at how two published models break down:

That's not automatically a rip-off. Programs pay for clinicians, messaging, lab ordering, shipping and support. But the breakdown tells you what is being charged—not the provider's profit or the quality of care. At Male Excel, the medication-and-supplies bundle is the larger line; at Hone, membership dominates the illustrated total.

For context, KFF Health News reported in 2024 that online programs were charging more than 50 times what Medicare paid per milligram for testosterone cypionate. What does a year of testosterone cypionate really cost?

A year costs more than the vials when visits, testing and other care are billed separately. Peter MD publishes a $948 prepaid plan with lab costs unresolved; Male Excel's published rates produce a $2,727 six-shipment model before shipping, while Hone and Taurus have material quote or fee questions. None of those figures is a universal all-in price.

Three things jump out:

  1. The lowest published prepaid package in this comparison is Peter MD's $948 year. We show it even though it is not one of the three partners disclosed above. The missing lab and product details keep it from being a verified lowest complete bill.
  2. A monthly equivalent is not a billing schedule. Peter MD requires the full year upfront. Taurus's current $49 campaign does not establish the six-month requirement sometimes quoted elsewhere.
  3. Membership is a real cost at Hone and Male Excel. Do not price their medication without adding the required care fee. At Male Excel, the medication-and-supplies bundle is still the larger part of the illustrated subtotal.

The fine print that changes your total

  • Male Excel uses both “60-day supply” and “every other month.” The six-shipment model is $99 + (12 × $99) + (6 × $240) = $2,727, before shipping. A strict 60-day schedule starting on day 0 would charge for a seventh shipment on day 360: $2,967 with the same 12 membership payments, before shipping.   • Male Excel's testosterone price includes thyroid tablets. You're quoted for two drugs. Ask whether thyroid is optional, what supports its use, and what a testosterone-only plan would cost.   • Hone's injection price has a unit problem. Its product page says from $28 per month; its current plan pages say from $28 per vial. Neither statement establishes your vial count or annual drug bill.   • Hone says some prescriptions use 28-day supplies. Thirteen such supplies cover 364 days, not 365.   • Taurus's $149 offer and general terms do not fully agree. Section 28 describes $17.99 per billing period unless otherwise stated at purchase.   • Hone's retesting pages also conflict. Paid startup schedules list confirmation fees, but the general retesting FAQ says clinically necessary retesting has no added cost.   • Peter MD's $948 package lists an aromatase inhibitor (an estrogen-lowering drug) along with testosterone. Ask whether there is a separate clinical reason for that medication and whether declining an unneeded add-on changes the price. A bundled extra drug is not proof you need it.   • HSA or FSA funds may help with eligible expenses, not automatically every fee in a program. If a full $2,727 bill were eligible and the only tax difference were 24% federal income tax, its after-tax equivalent would be $2,072.52. One rule: if any line still says "Unknown," your total isn't a total yet. It's a starting number. Call and fill in the blanks before you compare it to anything.

What do online TRT programs charge for testosterone cypionate?

Online programs advertise medication prices, membership fees and bundles that buy different things. A low drug line does not remove the care fee, and a prepaid package does not prove every required test is included. Compare the full quote and ask whether the testosterone is FDA-approved or compounded.

Male Excel does not have a low price. Its published rates produce a $2,727 subtotal before shipping, extra testing or a possible additional shipment inside your budget window. That's $890 above Taurus's $1,837 advertised-fee model, but neither is a complete, equivalent patient quote. If the lowest yearly bill is your top priority, price the pharmacy route first and get Taurus's fee conflict resolved before treating it as a cheaper alternative. But Male Excel skips the six- or twelve-month prepayment. Its plans are no contract, cancel anytime. Its required $99 membership includes unlimited e-visits with a dedicated medical provider and unlimited messaging with its support team. Medication is still paid in advance in 60-day supplies; “no contract” does not mean no advance payment or a refund on every charge. What Male Excel's price includes, per its pricing page:

  • A required $99 online medical consultation, which comes with an at-home hormone test.
  • Testosterone cypionate injections with thyroid tablets, “starting at $120/month.” The page lists $240 for a 60-day supply and describes billing as every other month; the package lists 10 mL of testosterone plus 60 thyroid tablets.
  • 60 insulin-type syringes and needles per 60 days.
  • A required $99/month Medical Membership.
  • Shipping and handling, charged extra.
  • A cream option (testosterone Lipoderm cream) starting at $132/month instead of injections. That is a different compounded treatment format, not the FDA-approved injectable product priced above.

  • Compounded, not FDA-approved. Male Excel's program disclosure describes its testosterone as compounded. Its pricing page alone does not establish the exact product a particular patient will receive. The FAQ names Anazao Health Corporation and WellDyneRx-FL as partner pharmacies; that is not independent verification of which one fills your prescription or its authority in your state.   • No membership fees under the campaign wording, and a refund if a clinician finds you aren't eligible for treatment.

Taurus Meds $49 offer

Worth knowing:

  • The billed term needs confirmation. The live $49 campaign does not establish a six-month prepayment requirement or a separate $199 month-to-month tier. Confirm the offer you are actually buying; do not infer the contract from a monthly equivalent.

A local clinic may charge one bundled fee or bill separately for visits, labs, medication and administration. Prices must be checked for the specific location. One location's offer is not a current quote for another clinic, and a monthly fee does not prove that every required cost is included.

Compounded testosterone can look cheaper, but it isn't FDA-approved, and the FDA does not review each compounded preparation's safety, effectiveness or quality before marketing. Compounded means a pharmacist or clinician combines, mixes or alters ingredients to make a medication for a patient's needs. That can serve a real need, like an allergy to an ingredient. Does insurance cover testosterone cypionate?

A plan can cover FDA-approved testosterone cypionate, but its formulary, clinical criteria, quantity rules and cost-sharing decide what you pay. A cash coupon may beat your insured price, but it cannot be combined with insurance on the same purchase and does not automatically credit your deductible. Compare both for the same prescription. Call the number on your card and ask:

  1. "Is testosterone cypionate on my formulary, and at what tier?" A formulary is your plan's list of covered drugs.
  2. "Does it need prior authorization or have quantity limits?"
  3. "What will I pay at my pharmacy before and after my deductible?"
  4. "Is a 10 mL vial covered the same way as 1 mL vials?"

When a coupon beats your copay — and the catch

GoodRx says you can use its discount instead of your insurance, but not combined with it. That also goes for Medicare and Medicaid. Ask the pharmacist to compare the two payment options before choosing one. GoodRx restrictions · GoodRx and insurance

The catch: a coupon purchase doesn't automatically count toward your deductible. GoodRx suggests asking your insurer whether you can submit the receipt for credit. The math, in two steps

Step 1: Get the pharmacy's quote for the prescribed quantity and its confirmed usable days' supply, after the applicable discard rules.

Step 2: For a rough 30-day medication equivalent, use fill price ÷ confirmed usable days' supply × 30. For your real first-year cash bill, add the full prices of the fills charged in that period; do not prorate a shipment you must pay for upfront.

A 30-day equivalent and a calendar-month invoice are different things. Your pharmacist and prescriber—not this formula—determine the right supply and package.

Why the price-per-100 mg shortcut can fail

The package-price table is useful for comparing listed products, but multiplying a per-milligram price by a prescription can understate your bill. Follow-up tests your clinician may order

Testosterone can raise hematocrit — the share of your blood made of red blood cells. The Azmiro label warns this can raise the risk of blood clots and tells patients to finish their lab tests. The Endocrine Society also addresses prostate assessment and monitoring, including PSA (prostate-specific antigen, a prostate blood test) when appropriate. The December 31, 2026 telemedicine deadline

The DEA and HHS extended a temporary pathway for qualifying controlled-substance prescribing without a prior in-person examination through December 31, 2026. For testosterone, the applicable pathway requires real-time audio-video care and compliance with other federal and state requirements; a questionnaire alone is not the exception. If the temporary pathway expires without a replacement, an online program may need a different lawful prescribing arrangement, which could include an in-person examination. It is not a prediction that every online patient will need a new visit on January 1. What happens to records and refills if you leave?

Ask for your lab results, consultation notes, medication list and itemized receipts before closing the account. Is there a testosterone cypionate shortage in 2026?

Supply is still uneven. The ASHP bulletin updated September 3, 2026 lists Pfizer's Depo-Testosterone vials on back order with no estimated release date, while several generic products are listed as available. If your pharmacy can't fill your prescription, ask which manufacturer it orders from and whether another maker's product is in stock.

Is it legal to buy cheap testosterone cypionate online?

Patient use of testosterone cypionate requires a valid prescription, and online dispensing must be through an appropriately licensed pharmacy. Testosterone is a Schedule III controlled substance in the United States; a “research use only” label is not a patient-prescribing loophole. How online prescribing is legal right now

Some online prescribing uses the temporary federal pathway extended through December 31, 2026; other patients may have a prior in-person evaluation or another lawful basis. On December 10, 2025, the FDA held an expert panel on testosterone therapy. A panel discussion isn't a rule change. Red flags that a "cheap" source isn't legitimate

  • No prescription required
  • "For research use only"
  • No pharmacy license you can verify with the relevant state board
  • Pressure to use hard-to-reverse payment methods instead of verifiable pharmacy billing
  • Bodybuilding-style dosing advice

To check a pharmacy, look it up with your state board of pharmacy. The FDA provides a state-by-state license lookup. Also check for a U.S. address and phone number, a prescription requirement and a pharmacist who can answer questions.


What should you know about fertility and safety before you pay?

Testosterone therapy can lower sperm production, so current or future plans for children belong in the clinical discussion before a treatment commitment. Blood pressure, blood counts, prostate history and other health risks can also change the right care route. Fertility

  • Before starting: tell the clinician about current or future plans for children. The Endocrine Society recommends against starting testosterone in men planning fertility in the near term. Endocrine Society clinical guideline
  • Current or future plans for children: the joint AUA/ASRM infertility guideline's published Part II advises against testosterone monotherapy in men interested in current or future fertility. The guideline discusses different treatments for selected clinical situations; that is not a reason to add a drug to TRT without a specialist's assessment.   • Recovery isn't guaranteed. A clinical study of men with testosterone-associated infertility found that recovery after stopping testosterone and receiving fertility treatment varied with age and prior treatment duration. What changed on the label

On February 28, 2025, the FDA told makers of testosterone products to update their labels. The changes included:

  • Added: results from the TRAVERSE trial.
  • Removed: boxed-warning language about increased risk of cardiovascular problems.
  • Added: warnings about blood pressure increases.
  • Retained at that time: the limitation stating that safety and effectiveness for age-related hypogonadism had not been established.

There was a further development on June 18, 2026. HHS announced that FDA was requesting removal of that age-related limitation and revisions to prostate-cancer and enlarged-prostate labeling. The Hikma and Pfizer prescribing information retrieved on September 17, 2026 still carried the older age-related limitation and prostate-cancer contraindication. A clinician must check the current labeling of the exact product alongside the patient's history and applicable guidance—not treat a policy announcement as personal clearance for treatment. TRAVERSE was not a testosterone cypionate trial. It studied testosterone gel in 5,246 men ages 45–80 with symptoms, two low fasting testosterone results, and established or elevated cardiovascular risk. For major cardiovascular events, testosterone gel was no worse than placebo within the trial's preset limit. Atrial fibrillation (an irregular heartbeat), acute kidney injury and pulmonary embolism (a blood clot in the lungs) were more frequent in the testosterone group. A cost point from the guidelines

For men whose low testosterone is tied to excess weight (a BMI over 27) with no other cause found, the Endocrine Society says weight loss is typically the first-line treatment. Ask your clinician whether that applies to you before you sign up for a year of anything. It depends on your prescription and on how long the vial is good after opening. Does insurance cover testosterone cypionate?
