This page is for you if you are comparing online testosterone care and need a dated shortlist for your state before paying. It covers adult men being evaluated for testosterone deficiency, not gender-affirming hormone care, treatment for women or adolescents, or nonmedical performance use.
This page is not enough for you if you have never had your testosterone measured, you are trying to have a child in the next few years, or you have a history of prostate or breast cancer, a high red blood cell count, untreated severe sleep apnea, or a recent heart attack or stroke. Untested symptoms need a clinical evaluation, which can begin online or in person. Fertility plans and these health concerns need the right clinician, not a state-list shortcut. What has to be true before an online clinic can treat you?
Three separate gates have to be open before an online clinic can treat you: the prescription must meet federal rules, the care must meet your state’s rules, and the clinician must be authorized to treat you where you are. A stricter state rule may add a step; it does not always close the door. Clinical eligibility and pharmacy access still need separate confirmation. 9, 12, 13
Checking only the third gate, and only by reading a marketing page, is the mistake.

Testosterone is a prescription medicine. It is a Schedule III controlled substance in the United States, and lawful patient access requires a valid prescription from an authorized clinician. Any site offering it without one is not a shortcut. It is a problem. 9, 11, 25
What can a state list tell you—and what can’t it?
A state list can help you rule out a program that excludes your location. It cannot confirm your diagnosis, clinician availability, the exact medicine or whether a required local visit can be arranged. 1, 3, 6, 12

A clinic listing your state does not mean a clinician is ready to treat you. It means the company says it offers a service there. Whether you are accepted still depends on your labs, your health history, the exact program you signed up for, and whether the clinic has a clinician available and authorized to treat you there. This page shows you what providers publish and exactly where that information stops. 1, 3, 6, 12
The right TRT provider is not the same for every person — it depends on whether low testosterone has been properly evaluated, your symptoms and health history, your fertility plans, your state, your insurance or cash-pay preference, your treatment-format preference, your budget, and whether online or in-person care is the better starting point. Some situations belong with primary care, urology, endocrinology, reproductive urology, or urgent or emergency care when the symptoms warrant it. Online TRT clinics by state: which programs list your state?
This table compares provider-published coverage across all 50 states and D.C., checked on September 9, 2026. Male Excel lists 39 states, Hone Plus 35, Hone Premium 34 and Whole Health Rx 49; a listing is not a promise of treatment. 1, 3, 6

We tracked three providers that publish state coverage, including both of Hone’s men’s hormone memberships. A fourth, Taurus Meds, is covered separately below because we found no TRT state list on the public pages we checked. Hone publishes two separate state lists for its two men's memberships, and they do not match. Plus lists 35 states. Premium lists 34. Three states fall in the gap:
- Hawaii and South Dakota — listed for Hone Plus, not for Hone Premium.
- New York — listed for Hone Premium, not for Hone Plus.
So a man in Hawaii who reads "Hone is available in my state" and signs up may find he is on the tier he did not want. Both men's tiers do list testosterone cypionate. But the tiers differ in how you get tested: Plus starts with an at-home kit, Premium requires an at-lab draw. Check the membership, not the brand name. 3, 4, 5
Counting it up
The eleven states Male Excel says it does not cover: Alabama, Alaska, Arkansas, Connecticut, Hawaii, Idaho, Louisiana, Minnesota, Mississippi, New Hampshire, Rhode Island. 1
D.C. is not confirmed as available in these lists. Hone lists the District as a separate line and marks its men’s programs unavailable. The Male Excel and Whole Health Rx coverage statements we checked describe “states” without separately confirming the District. 1, 3, 6 If you live in Washington, D.C., do not assume you are included in a phrase like "all states except." Ask, in writing, before you pay.
How fresh are these state lists?
The table records what the providers published on September 9, 2026. It does not establish when a state was added or removed, and it is not a live appointment-availability check. 1, 3, 6
Male Excel. Its current FAQ lists eleven excluded states, including Alabama and Minnesota. We have not verified when either state entered that list. 1
Whole Health Rx. Its current TRT page excludes New York only, and its FAQ says the program covers 49 states. That is a published service-area claim, not proof that every patient in those states can start without an in-person visit. 6

Here is why this matters more than the table itself. Every "which clinics serve my state" article you find is a photograph of a moving thing. If it has no date on it, it is worthless. If it has a date and the date is six months old, treat it as a starting point and check the provider's own page yourself before you hand over a card number.
That is why the table has a check date. It is not a promise that coverage cannot change tomorrow.
Does every state have a listed option?
There is no state where all three providers’ published lists say no. That means the lists together include all 50 states—not that lawful treatment is available to every reader, or that D.C. is covered. 1, 3, 6
Even in Alabama, Arkansas and Louisiana — excluded by Male Excel and Hone Premium — Whole Health Rx publishes coverage. 1, 3, 6
But read that carefully, because it is exactly the kind of claim this page exists to interrogate. Whole Health Rx's own FAQ says the service is available in 49 states, and in the same FAQ says that certain states have laws preventing clinicians from prescribing without a live consultation or lab work, and tells you to start the questionnaire and enter your state to find out. Those statements answer different questions: where the company advertises service, and what the clinician must do before prescribing. 6
Our read: a broad coverage claim tells you where a company advertises a program. Your state’s own rules, your clinical evaluation and the available clinician network decide what actually happens. 6, 9, 12, 16 Treat "49 states" and "all 50 states" as an invitation to ask a specific question, not as an answer.
If your state shows a green check next to Male Excel
You have a reason to check the program—not a clearance for treatment.
Male Excel publishes an explicit list of the states it does not serve on its own FAQ, so you can check that limit before checkout. On a page about state access, that matters. It lists 39 states—five more than Hone Premium, but ten fewer than Whole Health Rx—and offers a compounded testosterone cream for men who would rather not use needles. Coverage does not settle testing, medication fit or complete cost. 1, 3, 6

What does online TRT cost in year one?
The published prices below support first-year subtotals, not a complete quote for every patient. Male Excel’s $2,727 injection-bundle model includes medication but excludes shipping and unpriced extras; Hone’s totals exclude medication, and Whole Health Rx’s billing schedule needs confirmation. 2, 4, 5, 6, 18
We took each provider's own published pricing pages and did the arithmetic. Assumptions are stated under the table, because a cost figure without assumptions is just a guess with confidence.

What is in those numbers. All prices are provider-stated, checked September 9, 2026. 2, 4, 5, 6, 18
- Male Excel — $2,727 subtotal. That is $99 for the consult, $1,188 for twelve months of required membership, and $1,440 for twelve months of the advertised testosterone-cypionate-and-thyroid bundle at its starting price. It includes medication. It excludes shipping, any price increase tied to the prescription, and additional testing or services if separately charged. The compounded-cream bundle comes to $2,871 on the same assumptions. The model uses twelve advertised monthly equivalents, or six two-month medication fills; confirm actual charge dates because the FAQ also describes 60-day refills. These are advertised bundles, not a recommendation that every TRT patient needs thyroid medicine. 1, 2, 21
- Hone — $1,710–$1,715 for Plus and $1,950 for Premium, before medication. Those are the published test, confirmatory test and twelve-month membership costs only. They exclude medication. The Plus range reflects a real discrepancy: its membership page prices at-lab confirmation at $45, while its lab-schedule page says $50. An $80 at-home confirmation makes that Plus subtotal $1,745. Hone lists testosterone cypionate starting at $28 per vial but does not publish a patient-specific duration at that starting price, so we will not invent an annual figure. Ask them. Scheduled follow-up labs and consults are included in the membership; the plan pages also state free shipping. 4, 5, 18, 19
- Whole Health Rx — the one to ask about. Its FAQ says the first month is $129 as an introductory offer, and after that there is a recurring cost of $229 that includes medication, required lab tests and provider visits. A different answer in the same FAQ says you are charged at the end of the assessment and at every three-month renewal. $129 + (11 × $229) = $2,648 if those prices each buy one month. A monthly equivalent and a quarterly charge can coexist, so do not treat $229 as buying three months—or this model as a verified checkout total. Confirm the billing frequency and each charge amount before you pay. 6
The headline price trick, named: a non-TRT offer
Male Excel’s pricing page advertises treatment “as low as $95 a month.” Two things about that number: 2
- One pricing panel attaches $95 to Triclozene, a compounded clomiphene-and-thyroid preparation; another panel on the same page shows $120. Clomiphene is not testosterone. FDA-approved clomiphene is used off-label in men to stimulate the body’s own testosterone production; that does not make this compounded combination FDA-approved. If you came here for testosterone replacement therapy, this is not it. 2, 24, 30
- It excludes the $99 monthly medical membership, which the same page says is required for treatment.
Do the honest math and the published injection-bundle model at Male Excel averages $227.25 a month in year one, before shipping and other unpriced charges. That is about 2.4 times the $95 entry number; the ongoing starting medication-and-membership sum is $219 a month before those extras. To be fair, Male Excel does disclose the membership requirement in an asterisk on the same page. We are not accusing anyone of hiding it. We are doing the arithmetic. 2
The three providers in this cost table describe cash-pay services, not in-network TRT billing. Male Excel and Hone state that they do not accept insurance; Whole Health Rx states KYZATREX is cash-pay. This does not mean TRT is never covered through another clinician, laboratory or pharmacy. HSA and FSA cards are a different question — Male Excel marks its treatments as HSA/FSA eligible, so ask your plan administrator what applies to you. 1, 2, 6, 22

If cost is your deciding factor
You now have the published components and the gaps, not a false all-in price. Hone’s membership-and-testing subtotal is lower on paper but excludes medication, so it does not establish a cheaper full year. Male Excel’s model includes medication, and its list covers five more states than Hone Premium—not five more than Hone Plus. 1, 2, 3, 4, 5

What happens after you pay: renewals, monitoring and refunds
A monthly price does not tell you when the next charge runs or what happens if care cannot continue. These are provider-stated terms, not a test of the clinics’ actual billing or support.

Scheduled follow-up is not a substitute for diagnostic confirmation. Ask who reviews abnormal results, how to reach a clinician rather than general support, whether extra tests cost more, and how to get your records if you leave. 16, 29
Which clinics publish a required second testosterone test?
Hone publishes a separate confirming-test step, and Whole Health Rx states a two-morning-test diagnostic standard. We did not find a required second-test step on the Male Excel or Taurus Meds pages we checked; that is a disclosure gap, not proof that their clinicians never repeat a test. 1, 4, 5, 6, 20, 28, 41
This is the most useful thing on this page, and it is the reason we would rather you read us than a listicle.
The American Urological Association's guideline on testosterone deficiency is specific about how low testosterone is diagnosed. It treats a total testosterone below 300 ng/dL as a reasonable cut-off, and — this is the strong recommendation — the diagnosis should only be made after two total testosterone measurements taken on separate occasions, both in the early morning. And the numbers alone are never enough. You also need matching symptoms or physical signs. 17, 39
That is not a universal diagnosis from one cutoff. The Endocrine Society also calls for symptoms or signs, consistently low results, accurate tests and a repeat morning fasting blood test. A clinician must interpret the results and assess the cause. 16
Plain English: total testosterone includes the hormone that is bound to blood proteins and the small amount that is not. Testosterone usually varies through the day, so an afternoon result may be lower than an early-morning result. Follow the clinician’s test instructions, especially if you work nights. 16, 17, 39
One test is not a diagnosis. So we went and checked which online programs publish a required second test.

Here is the damaging admission, and we are not going to soften it.
Male Excel is an affiliate partner, and we found no required second confirming testosterone test step on the public pages we checked. Hone does publish one. Whole Health Rx states the two-test standard. If having a separate step written down before you enroll is what would make you comfortable, Hone Premium is the better-documented option for that specific question, provided it lists your state and can meet your care needs. 1, 3, 4, 5, 6, 20
Now the part that matters for most readers. A clinician can order repeat testing whether or not the company advertises it as a numbered step, and Male Excel does require a provider evaluation. Broader coverage does not make a missing confirmatory-testing answer less important. Its published strengths are still real: 39 states—five more than Hone Premium—and an option for compounded testosterone cream rather than injections. But neither benefit proves a better testing process or a predictable all-in price. Resolve the testing question before paying. 1, 2, 3, 16
Ask either one this, in writing, before you pay: "Will I be asked to repeat my testosterone test on a second early morning before anything is prescribed?"

Then you should not be choosing between marketing pages. You should be choosing between documented protocols—and now you know what is published and what still needs an answer.

Will you need an in-person visit?
A clinic listing your state does not tell you whether an in-person visit is required. The current federal telemedicine rule allows qualifying video-based prescribing without a prior in-person exam through December 31, 2026, but stricter state requirements still apply. A clinic must explain how its care meets both. 9, 12, 13, 14
The federal rule, in plain English
A 2008 law called the Ryan Haight Act generally requires an in-person evaluation before internet prescribing of a controlled substance, with statutory exceptions. Since 2020, temporary federal measures have allowed qualifying telemedicine prescribing without that prior evaluation; they have not suspended every prescribing rule. 9, 10
The current one — the Fourth Temporary Extension, published December 31, 2025 — lets a DEA-registered clinician prescribe Schedule II through V controlled medicines by video telemedicine without ever having examined you in person, as long as the other conditions are met. It runs January 1, 2026 through December 31, 2026. Those conditions include a legitimate medical purpose, usual professional practice and compliance with other applicable federal and state law. 9, 10, 37
DEA has also proposed a permanent "special registration" pathway. As of our verification date, it has not been finalized. 34
When your state adds a step
Federal permission does not override stricter state requirements. Your state can add a step. 9, 13
Alabama is a clear example we verified. The Board of Medical Examiners says telehealth prescribing of a controlled substance requires real-time audio or audio-visual communication, an in-person encounter with the prescriber within the previous 12 months, and a legitimate medical purpose established in that window. But the board also says a real-time video encounter with licensed medical or nursing staff assisting at the patient’s location can satisfy the in-person requirement. That is not the same as an ordinary video call from home. 13, 14
Alabama ended its special-purpose cross-state medical license in 2022. The board describes full Alabama licensure and an Alabama controlled substances certificate for the ordinary physician pathway; any exception or other professional pathway needs its own check. 13
Translation: do not pay for an Alabama online-only start until the clinic explains how it meets the examination requirement. A compliant assisted-video arrangement may be an option, but a national coverage claim does not prove the clinic offers one. 13, 14
New Jersey is a useful contrast. The state’s July 8, 2026 signing announcement describes revisions to telemedicine prescribing requirements for Schedule II medicines. Testosterone is Schedule III, so that Schedule II change is not itself a new testosterone-specific restriction. It does not settle every other New Jersey prescribing requirement. 11, 15
We include it on purpose. "My state passed a telehealth law" does not automatically mean your testosterone access changed. The schedule matters.
What we have not done: we have not read all fifty state rulebooks. We are not going to pretend otherwise, and you should be suspicious of any page that claims a live, definitive legal answer for every state. For states we have not individually verified, use the provider tables only as a starting point, and get your answer from a clinician authorized where you will receive care. What happens to online TRT on January 1, 2027?
The current temporary federal prescribing pathway expires December 31, 2026, and no general 2027 replacement was finalized in the sources checked for this page. It has been extended four times, but another extension is not guaranteed. Get your clinic’s continuity plan in writing rather than treating a forecast as a rule. 9, 10, 34
We are publishing this in September 2026, less than four months from that date. We are not going to predict the outcome. Possible outcomes include another extension, a finalized permanent pathway, or greater reliance on the ordinary in-person and statutory-exception pathways. The expiration would not by itself mean every existing prescription is void or all online TRT must end.
What you can control is your paperwork. Ask your clinic these four questions before you enroll, and keep the answers:
- If the federal telemedicine flexibility lapses on January 1, 2027, what is your continuity plan for existing patients?
- Do you have any clinicians who can see patients in person in my state?
- If you can no longer prescribe to me, will you transfer my records and current prescription details to a local clinician?
- What happens to my membership billing if you cannot prescribe?
A clinic that answers those four clearly has resolved an important part of the decision. A clinic that will not answer them in writing has told you something.
One more reason to keep your records: a local clinician can use your lab history, treatment notes and prescription details when evaluating continued care. Ask for them now if you may need a transfer. That is a paperwork step, not a reason to rush into treatment before a deadline. 29
Is the testosterone FDA-approved or compounded?
FDA-approved testosterone products and compounded testosterone preparations are different categories. Compounded drugs are not FDA-approved, and FDA does not review them for safety, effectiveness or quality before they are marketed. Both categories can be lawful, but a shared ingredient or a clinic’s badge does not establish FDA approval or equivalence. 24
Here is what each provider publishes:

Where the exact product is not disclosed, we are handing you the question:
"Is the testosterone you will ship me an FDA-approved product or a compounded preparation? Which pharmacy fills it, and is that pharmacy licensed to ship into my state?"
Get it in writing before you pay.
If FDA-approved specifically is what you want, Whole Health Rx provides the clearest named-product answer among these listings: KYZATREX. Its 49-state claim still needs the same state and clinician checks as any other program; it is not a clinical recommendation for everyone who wants an oral treatment. Compare TRT treatment formats before treating a product match as a care match. 6, 25
Worth knowing about KYZATREX before you go: its current labeling warns that it can raise blood pressure and cardiovascular risk over time. It also calls for hematocrit monitoring about every three months, while Whole Health Rx’s FAQ lists labs at 90 days, 180 days and then every 180 days. Ask how additional product-specific monitoring is arranged and paid for; the public schedule alone does not answer that. 6, 25
The age-related wording needs care. FDA requested removal of the age-related-hypogonadism limitation from testosterone labels in June 2026. The KYZATREX label record updated August 26, 2026 still says safety and efficacy for that population have not been established. That is not a blanket ban based on age, and the requested label change is not approval for anti-aging treatment based on symptoms alone. The diagnosis and the actual current product label still matter. 16, 25, 26, 33
Which pharmacy would fill your prescription?
A named pharmacy network is not the same as an assigned dispensing pharmacy. Ask for the pharmacy that will fill your prescription and confirm its authority to serve your state.

We did not verify individual clinician or pharmacy licenses for a patient-specific assignment. A company name or certification badge cannot substitute for that check.
Does Taurus Meds publish a TRT state list?
We could not find a published TRT state list on Taurus Meds’ homepage, its Terms of Service, or its public TRT intake landing page. Get state access and any charge due before a clinical decision confirmed in writing before paying. 7, 28, 41
That absence matters more than usual here, because of what its posted terms do say. We read them on September 9, 2026:
- Its Terms of Service say sales of shipped medications are final, with no refund.
- The same terms say blood work tests are non-refundable because orders go to lab partners immediately upon payment.
- Section 28 discloses a recurring healthcare membership fee of $17.99 per billing period, charged automatically. The term does not establish a complete TRT price or prove that this fee is added to every quoted TRT plan.
- Its named management-services partner is OpenLoop Health, Inc. Its named partner pharmacies are RedRock Pharmacy, Health Warehouse, Precision Compounding Pharmacy and Triad Rx. 7
There is also a separate refund policy, and it matters. Updated February 28, 2026, it says the first subscription month is billed when intake is completed, provides refund protection for medical disqualification, and requires cancellation at least 72 hours before the next billing date. It limits itself to subscription fees and says fees for healthcare already provided remain payable. Its sections describe full or remaining-charge refunds, but do not clearly settle separate TRT blood-work charges or a lack of state clinician availability. Ask which rule applies to the exact charge you are about to authorize. 8
Customer-experience signal, not a state-coverage finding: a public Trustpilot review by Carl N., with an experience date of September 5, 2026, reports charges and a medication delay after labs and a consultation, followed by a refused refund request. That report concerns fulfillment and billing—not proof of a state-specific denial. 36
That is one customer’s report. It is not proof of a current policy or a typical experience, and we did not independently verify the events or test the intake or refund process ourselves. The published terms and the separate refund policy—not the review—are the policy evidence.
Here is the mistake to avoid:
A non-refundable test charge is a financial risk if state access has not been confirmed first.
We are not placing a sign-up link for Taurus Meds on this page. Its public intake page displays a LegitScript badge and its terms name pharmacies, but we have not independently verified that certification or a patient-specific pharmacy license. On a page specifically about state availability, we could not verify the one fact this page exists to answer. That is a limit of this comparison, not a finding that the company is illegitimate. 7, 41
To consider them, do this first: email support@taurusmeds.com and ask, "Do you currently have a clinician licensed to prescribe testosterone in [your state], and what exactly will I be charged before that is confirmed?" Keep the reply. Do not email your lab results or medical history to get a general availability answer.
What seven questions should you ask before paying?
Ask about state access, in-person visits, early charges, testing, the exact product, full cost and continuity before paying. These seven questions make those checks concrete.
Copy these. Send them to any clinic, including the ones we link to. Ask for the answers in writing.
- State — "Do you currently have a clinician authorized to prescribe testosterone where I will be during visits? Is that specific to this program, and can you give me the clinician’s name and license details before the appointment?"
- In-person — "Does my state require an in-person visit before a first testosterone prescription, and if so, how do you handle that?"
- Charges before eligibility — "What exactly will I be charged before state access and clinical eligibility are confirmed, which charges are refundable if I cannot be treated, and what is the cancellation deadline?"
- Testing — "Will I be asked to repeat my testosterone test on a second early morning before anything is prescribed?"
- Product — "Is the testosterone FDA-approved or compounded? Which pharmacy dispenses it, and is it licensed to ship to my state?"
- Full cost — "What is my total cost for twelve months, including membership, medication, shipping, repeat and follow-up labs, visits and any local exam? Is billing monthly or quarterly, and what amount runs on each date?"
- Continuity — "What is your plan if the federal telemedicine rule changes on January 1, 2027, or if I move? How do I obtain my records, and what happens to refills and membership billing if you cannot continue care?"
If a company will not put question 1 or question 3 in writing, that is your answer. You do not need to send lab results or a full medical history just to ask a general availability or fee question.
What can you do if an online program cannot serve your state?
A provider excluding your state does not mean TRT is illegal there or that every other care route is closed. Primary care, a specialist or a clinic that can arrange a required local encounter may still be an option. 12, 13, 14
Let us be blunt, because this is where honesty earns more than a click.
If a required in-person or qualifying assisted-video encounter has not been arranged, do not pay for a program that promises an online-only start without explaining how it meets your state’s rules. You risk losing time and money. In Alabama, a compliant assisted-video encounter may meet the board’s requirement; otherwise, start with a clinician who can arrange the necessary local care. 13, 14
Three legitimate care routes to consider:
1. Your primary care doctor. A useful first call, especially if you already have an in-network doctor. Bring your symptoms, ask whether an early morning total testosterone test is appropriate, and ask about repeating it. Your doctor may manage the evaluation or refer you out, which is also fine — that referral is worth having. Check your plan’s network and costs rather than assuming this is always the cheapest choice. 16
2. Urology or endocrinology. A useful specialist route when the cause is unclear, the results need more work, or your history raises testicular, pituitary, prostate or other hormone concerns. Age under 40 alone does not require a specialist. Fertility concerns call for a urologist, reproductive urologist or another fertility-aware clinician. 16, 33
3. A clinic with a physical location in your state. A qualified local clinician may be able to perform the required examination and arrange remote follow-up where permitted. A street address alone does not prove compliance. Ask whether the first visit is in person, who performs it, and which later visits can be remote. 12, 13, 14
Take the seven questions above to any of them. They work just as well in a clinic room as in an email.

We would rather lose the click than watch you pay for something that cannot be delivered.
What if I move, or travel, while on TRT?
Moving or traveling can change whether your clinician is authorized to provide a telehealth visit and prescription where you are. It does not automatically cancel an existing prescription; state practice rules, prescribing authority and pharmacy dispensing rules need separate checks. Tell your clinic before you move, and get your records either way. 9, 12, 29
Practical steps:
- Tell your clinic before the move, not after your refill fails. Ask directly whether the clinician can lawfully continue your care where you are going.
- Get your records. Your lab history and your current prescription details help a new clinician evaluate continued care. HIPAA generally gives you a right to obtain records from covered providers, with limited exceptions. Request them in writing and ask about the process and any permitted copy fee. 29
- If your refill is disrupted, talk to your current prescriber first. Do not change your dose or stretch your supply on your own. Ask the prescriber and a licensed pharmacy about lawful options, including a local clinician when needed. Testosterone is a controlled substance, and stopping or altering it is a clinical decision, not a logistics one. 9, 11, 25
- Do not use an out-of-state address to stay eligible. Tell the clinic where you will physically be during the visit. A different billing address does not establish the clinician’s authority there, and misreporting your location can undermine the clinical record and lawful prescribing checks. 12
What changes if you plan to have children?
Testosterone therapy can suppress sperm production, so fertility plans belong in the conversation before you start, not after. Clomiphene, enclomiphene and hCG are different medicines with different regulatory status; none is testosterone replacement therapy, and none guarantees fertility. 16, 25, 30, 31, 32
This is short and it is serious.
Taking testosterone from outside the body signals your own system to make less of it, and that can reduce sperm production. We are not going to tell you that you will become infertile, and we are not going to promise that anything preserves or restores fertility. Both of those claims get made online and neither is safe to rely on. 16, 25
You will see clomiphene and enclomiphene marketed alongside TRT — Male Excel markets its compounded clomiphene-containing Triclozene preparation, and Hone lists compounded clomiphene and enclomiphene on both men’s tiers. They aim to stimulate the body’s own testosterone production rather than replace testosterone. They are not TRT and should never be described as "natural TRT." 2, 4, 5, 30, 31
Non-testosterone alternatives: three different approval situations

| hCG | Certain products, including Pregnyl, are FDA-approved for selected cases of male hypogonadotropic hypogonadism—a problem with the hormone signals to the testes. That is not approval of every fertility-preservation regimen and does not guarantee preserved sperm production. 32 |
If having a child in the next few years is on your list, start with a urologist, reproductive urologist, or a fertility-aware clinician, not a general online program that has not addressed fertility. What else should you know about online TRT by state?
A state listing answers only the service-area question. These answers separate that claim from legal prescribing, medical eligibility, testing and what you may have to pay.
Can you get TRT online in every state? The three providers with published lists here collectively list every state, but none of these lists includes all 50 states and D.C. That does not establish a lawful online-only start for every patient in every state. Being listed also is not the same as being accepted; that still depends on your labs, your history, prescribing requirements and clinician availability. 1, 3, 6, 9, 12
Which states don't allow online testosterone prescriptions? A provider’s exclusion list is not a list of state bans. Some states add examination requirements: Alabama requires a recent in-person encounter for this pathway, but its board allows a qualifying assisted-video encounter to satisfy that requirement. We have not established a complete list of every state’s restrictions here. 13, 14
Do I need an in-person visit to start TRT? Not always under federal law: through December 31, 2026, the temporary pathway allows qualifying video prescribing without a prior in-person evaluation. But state law or the clinician’s assessment may require a visit. Ask your clinic directly about your state and the exact program. 9, 12, 13
Does my online TRT doctor have to be licensed in my state? The clinician needs the required authority where you are physically located during the visit. A full state license is one pathway; some states also have lawful registrations, temporary permissions or exceptions. A company’s coverage map does not independently verify the assigned clinician’s authority. 12
What happens to online TRT after December 31, 2026? The current federal flexibility expires that day. It has been extended four times and DEA’s general special-registration pathway was not final in the sources checked for this page. We are not predicting the outcome. Ask your clinic for its continuity plan in writing before you enroll. 9, 10, 34
Can I use an address in another state to qualify? No. A billing address in another state does not establish lawful access where you are physically located during the appointment. Report your actual location so the clinician can check the applicable rules. 12
Why do two Hone memberships have different state lists? Hone publishes separate availability for its men's Plus and Premium memberships. Three states differ: Hawaii and South Dakota appear on Plus but not Premium, and New York appears on Premium but not Plus. Check the exact membership, not just the brand. 3
Is the District of Columbia included when a clinic says "all states except"? Do not assume so. Hone lists D.C. separately and marks its men’s programs unavailable there. The Male Excel and Whole Health Rx state statements checked here do not establish D.C. coverage. Ask before you pay. 1, 3, 6
Does a clinic excluding my state mean TRT is illegal there? No. A company’s service area is not a ruling on state law. Another provider or a local clinic may be able to evaluate you, subject to the relevant rules and your health needs. 12, 13, 14
Is online TRT cheaper than a local clinic? Not automatically. The figures here are not comparable all-in quotes: Hone’s models exclude medication, Male Excel’s exclude shipping and other unpriced charges, and Whole Health Rx’s annual model is conditional on the billing period. Compare a full written quote with your in-network local options before deciding which is cheaper. 1, 2, 4, 5, 6, 22
Does insurance cover online TRT? Male Excel and Hone say they do not accept insurance, and Whole Health Rx describes this KYZATREX program as cash-pay. That is not a ruling on whether your plan covers evaluation, testing or a prescription through another provider. HSA and FSA eligibility is separate from insurance coverage. 1, 2, 6, 22
Is compounded testosterone the same as FDA-approved testosterone? No. FDA-approved medicines have been reviewed by the FDA for safety, effectiveness and manufacturing quality. Compounded preparations are not FDA-approved; FDA does not review them for safety, effectiveness or quality before marketing. Both can be lawful. Ask which exact product you are being sent. 24
Can I get my blood work done locally? Hone Premium names Quest and Whole Health Rx names LabCorp. Hone Plus and Male Excel start with at-home kits, but that does not mean every later test is at home; Hone Plus also lists an at-lab confirming option. Confirm the draw site, collection timing and repeat-test process before you pay. 4, 5, 6, 18, 20
What is the next step once you find a state match?
If a program lists your state, confirm two facts with the clinic in writing: that a clinician can lawfully evaluate you there, and what you will be charged before access and clinical eligibility are confirmed. Then decide whether to pay for an evaluation—not whether to start testosterone.
If the program cannot meet your state’s requirements, the useful move is a phone call to a local clinician rather than another hour of searching.
