Matrix Hormones Cost in 2026: Plans, Year-One Costs and Exit Fees

Compare Matrix Reformed's seven published plans, year-one base costs, state surcharges, lab exceptions, and cancellation terms before paying.

Matrix Hormones cost starts at $99 a month for injectable testosterone, but only on the annual plan: $1,188 for the year. Monthly billing is $120 with a three-month minimum. This cash-pay option is for men 30+; it is not a no-commitment trial. State, labs and prescription changes can add costs.

Best for comparing a bundled cash-pay price: men 30 or older in a state Matrix serves who can accept its commitment terms. A clinician must still establish whether treatment is appropriate.

Look elsewhere first if you're under 30, you need to be able to stop any month without a fee, you're trying to have a baby soon, or you've never had your testosterone checked and want to "just try it." We'll point you to the right next step for each.

Four things can change your number or rule Matrix out:

  • Your state. HCG plans cost $95 more a month in 10 states. In some states you pay for your own lab work.

How much does Matrix Hormones cost per month and upfront?

$99 is what the annual plan works out to per month. Matrix bills that plan annually, so the injectable plan is $1,188 for the year. Paying on the monthly plan costs $120, and Matrix's refund policy requires a three-month minimum on monthly plans, even though the plan page says "Cancel Anytime."

That's the one thing we'd want a friend to know before he clicks "Get Started."

The plan page shows "Cancel Anytime" near its signup button and "No contracts" in its FAQ. Matrix's Payment, Subscription & Refund Terms (version 3.0, effective February 1, 2026) say this instead:

  • Monthly plans: cancel with at least 14 days' notice. If you cancel before finishing a three-month commitment, an early cancellation fee applies. That fee equals the "full retail price" of any medication and services you received during the commitment.
  • 6-month and 12-month plans: you're committing to the full term. Leaving early doesn't get you a refund. Any leftover balance may become account credit.

Matrix's own FAQ says the same thing in friendlier words: no long-term contract, but a three-month minimum, then month to month with 14 days' notice.

When Matrix's documents disagree, its Terms of Use settle it: the refund policy controls.

So here's the honest trade. Matrix does not give you a true cancel-any-month plan. If walking away after one month without an early-exit fee is your top priority, its monthly plan is not your fit. Male Excel and Hone Health advertise no long-term commitment, but that does not establish a full refund or a fee-free exit in every case; their terms are compared later. Matrix advertises labs, video visits, supplies and shipping within its plan price, subject to state and plan exceptions. Put it in weekly terms and it's easier to feel. The annual plan works out to about $22.85 a week. Paying monthly, it's about $27.69 a week. These are 52-week budget equivalents, not weekly payment plans.

For a full year at unchanged rates, annual is the lowest base price for this Matrix plan. To compare the service without prepaying a year, monthly costs $21 more a month and still asks for three months.

The right TRT provider is not the same for every person — it depends on whether low testosterone has been properly evaluated, your symptoms and health history, your fertility plans, your state, your insurance or cash-pay preference, your treatment-format preference, your budget, and whether online or in-person care is the better starting point. How much does every Matrix Hormones plan cost?

Matrix lists seven plans: testosterone-containing plans have base prices from $99 to $217 a month, including Kyzatrex at $169 before its prescription-dependent addition. Two other plans contain no testosterone, so they are not testosterone replacement therapy (TRT).

We grouped the plans by what's actually in them. A lower price in a different group isn't a cheaper version of the same thing.

Compounded testosterone plans — not FDA-approved products

Prices below are monthly equivalents. The final column assumes 12 months at the same rate. The annual and six-month columns do not mean monthly billing.

The injectable amount above is Matrix's published plan limit, not a recommended dose. An included medicine is not automatically needed by every patient. Labs and other inclusions remain subject to the exceptions below.

Matrix's general plan copy describes treatment as included, but the Kyzatrex card does not itemize capsule quantity or a separate medication charge. Confirm that the quote includes the prescribed capsules and every required lab before treating this as an all-in total.

Compounded enclomiphene is not an FDA-approved product. Pregnyl is an FDA-approved hCG product for specific labeled uses, not approval of every hCG/enclomiphene combination or a promise about fertility.

Two quick definitions help here. FDA-approved means the FDA reviewed that exact product for safety and effectiveness before it was sold. Compounded means a drug is mixed, combined or changed to meet a patient's needs. The FDA says compounded drugs are not FDA-approved and aren't reviewed before sale for safety, effectiveness or quality. That doesn't make them useless. The "Natural T Support" and enclomiphene plans deserve a plain label. They don't contain testosterone. They are intended to stimulate the body's own testosterone production, not replace testosterone directly. FDA scientific review. They are not "natural TRT," and they're not a budget version of testosterone therapy. Whether one makes sense for you is a clinical question, not a price question.


What does the Matrix Hormones price include, and what costs extra?

Matrix advertises the basic injectable plan as covering the prescription, lab work, provider visits, injection supplies and shipping, with self-pay lab exceptions. Extra costs show up in specific situations: self-pay lab states, the HCG surcharge states, higher oral doses, a provider "upgrade" to a pricier plan, and refills ordered outside a plan. What labs does Matrix order before starting? Its FAQ lists total and free testosterone, estradiol, a complete blood count (CBC), a comprehensive metabolic panel (CMP), a lipid panel, and prostate-specific antigen (PSA) for men over 40 or as needed. Quick translations:

  • Total testosterone is all the testosterone in your blood. Free testosterone is the small part that isn't bound to proteins.
  • A CBC includes hematocrit, the share of your blood made of red blood cells. Testosterone can raise it, so it gets watched.
  • PSA is a protein made by the prostate. It's used to help monitor prostate health.

Matrix sends a lab order you take to a Quest or Labcorp location. That's worth knowing if you live somewhere without one nearby. Matrix's lab process.

What remains unanswered: the public documents do not settle whether a confirming second early-morning testosterone test is included, or the exact number of first-year blood draws and follow-up visits for your plan. Ask for the initial panel, confirmation test and monitoring schedule separately. The Endocrine Society guideline requires appropriate repeat morning testing for diagnosis; an advertised lab bundle is not proof of that process.

About anastrozole: it's FDA-approved to treat certain breast cancers in women after menopause. Prescribing it to men is off-label. If it's added to your plan, ask why it's needed for you. Anastrozole labeling.


Does Matrix Hormones cost more in some states?

Matrix adds $95 a month to its hCG plans in 10 listed states and does not offer Kyzatrex in six listed states. Other notices cover self-pay labs and unavailable plans, but we could not establish complete current lists for those groups. Check the state selector and get the terms for your chosen plan in writing.

This is the part worth checking before you compare headline prices, because it depends on where you live. Here are the published restrictions and the gaps:

HCG surcharge and Kyzatrex states: Matrix plan page, September 28, 2026. We have not published a complete lab-self-pay or basic-TRT availability list because we could not confirm one from Matrix's current public materials. Being absent from an exclusion list does not establish clinical or legal eligibility.

A self-pay lab warning worth doing the math on. The discount is smaller than it looks for annual plans. If Matrix confirms the typical $40 annual discount, that is $40 for the whole year while you cover your own labs. A single direct-pay total testosterone test at Labcorp OnDemand was $69 when we checked on September 28, 2026, and Matrix's starting panel includes several more tests than that. This is a price example, not a quote for Matrix's full panel or a substitute for diagnostic testing. In a self-pay state, get a cash quote for the full panel before you choose a billing term.

Massachusetts needs a separate lab check. It's on Matrix's hCG surcharge list. If Matrix also quotes self-pay labs for your plan there, add both costs; we did not independently confirm that second charge for every Massachusetts patient.


What happens to your money if you cancel or aren't approved?

If Matrix's provider decides you are not a clinical candidate, its refund policy says the refund is the amount paid minus $120; with one $120 payment, that leaves $0 back. Outside that limited exception, fees are nonrefundable once clinical services begin. Possible account credit is not cash and expires 12 months after issuance. Refund policy.

This section is the one most people skip. Please don't.

"But I never got any medicine." That may not matter. The policy says services start once any of these happen: intake review, lab ordering, provider evaluation, appointment scheduling, or medication management. After that, fees count as earned. So "I hadn't started treatment yet" isn't the same as "I hadn't started services yet." The limited non-candidate exception above still matters; it is not a general refund for changing your mind. The policy also says voluntary discontinuation does not entitle you to a refund or credit, so do not count on a credit. Read the full terms.

A few more terms worth knowing before you pay:

  • Chargebacks: Matrix asks you to contact support first and allow 5 business days. It says chargebacks filed against its policy may be contested.
  • Arbitration: The Terms send disputes to individual arbitration and waive class actions. You can opt out within 30 days of first agreeing, by sending a notice to Matrix Reformed, LLC, 7901 4th St N #25584, St. Petersburg, FL 33702, attention "Arbitration Opt-Out." Include your name, address and a clear statement that you wish to opt out. Read the exact opt-out clause.
  • Support hours: Matrix lists Monday to Friday, 8:30 AM to 5:30 PM Eastern (written as EST), by phone at (813) 333-2226 or email at support@matrixreformed.com. These are published hours, not a tested response-time promise. Support and policy details.

Our take: budget for the $120 non-candidate deduction even if you already have recent testosterone results in hand. A prior result does not guarantee candidacy, and the public policy does not reduce that fee for you. And if there's a real chance you'll stop within three months, the monthly plan isn't the escape hatch the "Cancel Anytime" badge suggests. Refund policy · Diagnostic guidance

If you've read all that and still want to compare a Matrix quote, the monthly plan requires less cash up front than prepaying six or twelve months. It does not remove the three-month commitment or establish that you should start treatment.

Matrix's sales pages, FAQ, Terms of Use and refund policy need to be read together to understand commitment, labs, refunds and pharmacy claims. Some differences are plan-specific exceptions; others conflict, and Matrix's Terms say the refund policy controls. The table below lines up the published statements.

Is Matrix's testosterone FDA-approved or compounded?

Matrix describes its injectable and cream testosterone as compounded preparations, which are not FDA-approved products. Kyzatrex is an FDA-approved testosterone product; Pregnyl is an FDA-approved hCG product that Matrix says some patients receive. Matrix doesn't dispense medicine itself. It says prescriptions are filled by licensed U.S. compounding pharmacies, naming Empower, Strive and Revive Rx. Its FAQ names Strive Compounding Pharmacy in Gilbert, Arizona, as its "recommended pharmacy of record." Refills can come in 30-, 60- or 90-day supplies depending on your plan. These are Matrix's disclosures and FAQ statements, not verification of the pharmacy that will fill your prescription or its current license in your state. Ask for the exact product, manufacturer or compounder, dispensing pharmacy and refill lead time before paying.

Matrix's Terms also say you may choose to fill select prescriptions at an outside pharmacy, with your insurance if applicable. You'd pay any extra costs yourself, so ask how that changes your subscription before doing it. Matrix Terms.

Testosterone is a Schedule III controlled substance in the U.S. That means federal law treats it as a drug with abuse potential. It needs a valid prescription and ongoing medical oversight. The Kyzatrex price has a built-in step up

Kyzatrex is $169 a month through Matrix, billed monthly. Matrix adds $140 a month if your dose goes over 400 mg a day. That makes it $309 a month, or $3,708 if the higher tier applies for all 12 months. Matrix's price tier.

Here's why that line matters more than it looks. Kyzatrex's labeled starting amount totals 400 mg a day, which is the boundary of Matrix's lower price tier. If a clinician prescribes more than that, Matrix lists a $140 monthly addition. That's a clinical decision, not something to steer around. Just know the price moves with it. For comparison, Drugs.com lists a pharmacy cash price of about $399 for a 120-capsule supply of Kyzatrex 200 mg, before any visits or labs. That is a quantity-based pharmacy price, not a monthly care plan, so it does not establish a like-for-like saving against Matrix's $169. Matrix's card does not itemize the capsule quantity; confirm medication inclusion and supply in the written quote.

The Kyzatrex label also requires two things worth knowing about cost. First, low testosterone must be confirmed with morning blood tests on at least two separate days before starting. Second, blood pressure and red blood cell levels need monitoring. The current label lists removal of the blood-pressure boxed warning in July 2025, but blood pressure increases remain in its warnings. It calls for hematocrit checks before treatment and about every three months during care; ask whether those tests and blood-pressure follow-up are covered. HCG and enclomiphene aren't fertility guarantees

Testosterone therapy can lower sperm production. Sperm production is called spermatogenesis, and outside testosterone can signal the testes to slow it down. That doesn't mean every man becomes infertile. It also doesn't mean fertility returns on a schedule after stopping. Kyzatrex fertility warning · Endocrine Society guidance

Pregnyl's FDA-approved labeling includes selected cases of hypogonadotropic hypogonadism in males—a condition in which hormone signals to the testes are too low. It is not an FDA-approved "fertility add-on" for every man on TRT. The Endocrine Society's guideline recommends against starting testosterone in men who are planning a pregnancy in the near term. Is Matrix Hormones cheaper than other online TRT programs?

Matrix's annual injectable base is lower than several other published starting-price models, but it is not proven to be the cheapest complete first year. PeterMD starts below it before follow-up labs, while Hone's medication quantity and Male Excel's shipping and billing dates need a quote. Compare the required care, treatment format and exit terms—not just the smallest number.

Three differences change the comparison. PeterMD's $1,043 plus one $124.50 follow-up panel equals $1,167.50; with two, it is $1,292. Neither is a personal quote until the required testing count is known. That alone rules out a blanket claim that Matrix is always cheapest.

Hone's sales page advertises injections from $28 a month, but its Premium help page says $28 per vial and adds a $25 confirmation test for TRT candidates. We use the more itemized help-page model: $65 + $25 + (12 × $155) = $1,950 before medication. Do not assume one vial lasts one month. Hone sales page · Premium help page

Male Excel's $2,727 illustration is $99 + (12 × $99) + (6 × $240). Six 60-day supplies cover 360 days, not a full 365-day year. If medication renewals are exactly 60 days apart starting on day one, a seventh $240 charge can fall within the first year, making that model $2,967 plus shipping. Its page also says every other month, so ask for actual charge dates rather than treating either model as a final bill. FDA-approved oral testosterone: compare Kyzatrex separately

For Kyzatrex, Matrix lists $169 a month before its prescription-dependent $140 addition; Henry Meds lists $179 a month. Those are $2,028 and $2,148 in unchanged 12-month base prices, not proof of identical capsule quantities or covered services. Which one fits you?

  • You're 30+, in a state Matrix serves, and can accept a year of prepayment. Compare Matrix's $1,188 annual injectable base with the complete written quote, including your testing and exit terms.
  • You want to assess the service before prepaying a year. Matrix monthly reduces the upfront payment, as long as you're fine with three months minimum and the stated early-exit fee.
  • You need no long-term commitment. Compare Male Excel's and Hone Health's written cancellation terms. Their marketing does not promise a refund of medication or all prior charges.
  • You're under 30. Matrix's testosterone plans aren't open to you. Start with proper evaluation, then check any other provider's current age and state requirements.
  • Matrix does not offer your chosen plan in your state. Confirm another provider's exact product, assigned clinician and availability before paying, or compare local care.
  • You want a membership with scheduled check-ins. Male Excel publishes 60-day assessments and blood testing every six months. Ask what extra monitoring your own situation requires. Published schedule.
  • You are comparing FDA-approved oral testosterone. Matrix's lower Kyzatrex base price is worth checking, but the prescribed amount, medication inclusion, labs and state restrictions decide the actual bill.
  • You want insurance to pay. Matrix is cash only. Start with your own doctor and your plan's rules.
  • You're trying to conceive soon. Start with reproductive urology.
  • You've never had your testosterone checked. Start with primary care or our blood-test guide, not a subscription.

If Matrix's written quote fits your budget and care needs, you have a reason to keep it on your shortlist. You've already seen the fine print; do not pay until the remaining costs and clinical questions are answered.

Does Matrix Hormones take insurance, HSA or FSA?

Matrix's Terms and FAQ say it does not accept public or private health insurance or cost-sharing plans; its services are cash pay. HSA or FSA card acceptance was not established from Matrix's own pages, and tax eligibility is a separate question. We did not find HSA or FSA card acceptance in the Matrix pages checked. Ask support before you count on it, check with your account administrator and keep itemized receipts.

Two places insurance can still help:

  1. Labs in self-pay states. Matrix says you can use your own insurance for the lab work. Coverage, network rules and your share of the bill still come from your insurer. Plan disclosure.
  2. Filling at your own pharmacy. Matrix's Terms allow select prescriptions to be filled outside its network, with insurance if it applies. Check that it doesn't leave you paying twice. Outside-pharmacy terms.

Is there a Matrix Hormones discount code?

We found no public coupon or discount code on the Matrix pages checked. Published price reductions include choosing a longer billing term—$21 a month less for the annual injectable rate than monthly billing—and a typical onboarding discount where labs are self-pay. Confirm any discount and its conditions in writing. Published prices and discounts.

Be careful with coupon sites that promise a Matrix code. Do not assume a code changes the three-month minimum or the refund rules; a discount needs its own written terms. If someone gives you a code, ask Matrix to confirm the final price in writing.


How does Matrix work from checkout to first shipment?

Matrix describes a process of checkout, intake, a lab order for Quest or Labcorp and a video visit before prescribing. It says an appointment can typically be scheduled within 10 days of the blood draw—not that the visit and shipment are guaranteed within 10 days. A blood test alone doesn't diagnose low testosterone. The Endocrine Society says diagnosis needs symptoms or signs of low testosterone, plus consistently low levels on accurate tests, including repeat early-morning, fasting blood draws. No single cutoff or one test establishes the diagnosis. Hypogonadism is the medical name for impaired testicular function; low testosterone may reflect a problem in the testes or the brain signals that control them. Endocrine Society diagnostic guidance.

Ask Matrix whether a confirming second morning test is part of its process, and whether it's included. Ask for the answer before paying; the name of a lab panel alone does not answer it.

About online prescribing rules: Federal telemedicine flexibilities for controlled medications currently run through December 31, 2026. They allow qualifying DEA-registered practitioners to prescribe without a prior in-person examination only when the required conditions are met; state laws and prescriber licensing still apply. Is Matrix Hormones legit?

Matrix's legal pages identify Matrix Reformed, LLC, and its provider bio names Dr. Peter Dashko, D.O., as medical director. Florida's official record lists Peter M. Dashko's osteopathic-physician license OS8677 as Clear/Active, expiring March 31, 2028. That is a specific credential check, not proof of the quality of your future care or the license of your assigned clinician. Provider bio · Florida license record

What we could check:

  • The company. Matrix's Terms and refund policy name Matrix Reformed, LLC, governed by Florida law. The same plans appear at matrixhormones.health and matrixreformed.com.
  • The medical director. Matrix names Dr. Peter Dashko, D.O., as medical director. The Florida Department of Health record lists Peter M. Dashko, osteopathic physician, license OS8677, Clear/Active, with a March 31, 2028 expiration. Matrix says providers are assigned by state licensing, so you may see someone else.
  • Online pharmacy review. matrixreformed.com displays a LegitScript seal. We did not independently establish its current certification status, and a displayed seal does not establish FDA approval or the license of your dispensing pharmacy.
  • Scope. Matrix's FAQ says it won't prescribe testosterone for bodybuilding or performance.

What we couldn't check: the licenses of every clinician who might treat you, the pharmacies' current inspection records, or how the care actually feels. We didn't enroll. Before your visit, ask for your provider's name and license state. Then look them up with your state medical board.

How do you build your own Matrix number before you pay?

Your real Matrix cost is your plan payments, plus any required costs your plan doesn't cover, minus any credits confirmed in writing. An unknown line isn't a free line. Fill this in from Matrix's page and a written answer from support, then compare it with any other quote.

Use the number of charges due within your first 12 months, not just a monthly equivalent. Count a charge or credit only once. Mark a line included, not applicable or unknown rather than silently treating a blank as zero.

Keep the reply, accepted terms, receipts and dated screenshots. They can help resolve a disputed charge. Matrix's FAQ says prescriptions can be accessed in its patient portal, but we did not establish a complete process for downloading lab reports and visit notes after cancellation.

Does Matrix Hormones include blood work?

Matrix advertises included lab work with self-pay state exceptions. What if Matrix says I'm not a candidate?

The policy says the refund is what you paid minus a $120 onboarding fee. With one undiscounted $120 monthly payment, that leaves $0 back; with one $1,188 annual payment, it leaves $1,068. This is a limited refund rule, not a separate $120 charge to add to every approved patient's budget. Refund policy.

How much is Matrix's TRT + HCG plan?

$140 a month annual, $150 on a 6-month term, or $160 monthly. In Alabama, Arkansas, California, Iowa, Kentucky, Louisiana, Massachusetts, Nevada, Ohio and South Carolina, Matrix adds $95 a month. State surcharge list.

How much is Kyzatrex through Matrix?

$169 a month, billed monthly. Matrix adds $140 a month when the prescribed amount exceeds 400 mg a day, making it $309.
