Make it more exact: enter the actual charge per pharmacy fill and the number of fills—not a dose. Add your share of visits and labs, shipping per order, any separate confirming test, a PSA test if ordered, and other quoted charges. For insurance, enter your expected patient responsibility, not the clinic's full billed charge. A deductible is not another fee to add on top of those same charges.

Who is this calculator for—and who should talk with a clinician first?

You can budget for an evaluation before anyone has diagnosed low testosterone. But a price comparison cannot tell you whether to start treatment, and fertility plans or a complex health history can change the care route you need. 25

Good fit if you:

  • Want a real 12-month number before you pay anyone.
  • Are choosing between your own doctor, an online program, and a local clinic.
  • Got a quote or a checkout screen and want to check it.
  • Are planning HSA or FSA spending.

Resolve the medical question first if you:

  • Haven't had appropriate repeat early-morning testing and a clinician's review of your symptoms or signs. One low number isn't a diagnosis. The calculator can price that first step for you. That does not mean everyone transferring care needs two new tests. 25
  • Want to have a child soon. Testosterone therapy can lower sperm production. The Endocrine Society recommends against starting it in men planning fertility soon. Talk with a urologist or reproductive urologist first. 25
  • Have prostate or breast cancer, a high red-blood-cell count, untreated severe sleep apnea, uncontrolled heart failure, or a heart attack or stroke in the past six months. These are among the Endocrine Society's reasons to recommend against starting testosterone. A past cancer diagnosis also needs specialist review; it is not the same as saying every cancer survivor has the same treatment options. 25
  • Are looking for a dose calculator. This isn't one. Your prescriber sets your dose.

The right TRT provider is not the same for every person — it depends on whether low testosterone has been properly evaluated, your symptoms and health history, your fertility plans, your state, your insurance or cash-pay preference, your treatment-format preference, your budget, and whether online or in-person care is the better starting point. Some situations belong with primary care, urology, endocrinology, reproductive urology, or urgent or emergency care when the symptoms warrant it. How much does TRT cost for the first year?

Published starting-rate subtotals checked September 22, 2026 include $1,043 before follow-up labs at PeterMD and $2,871 before shipping at Male Excel's cream plan. They are not a verified all-in price range: medicine, extra testing, fees, billing dates, and your actual prescription can change the result. Costs a company doesn't publish are shown as unknown, not zero. 1 20 21

Rate equivalent means a price spread evenly over a set time. It is not the cash charged on your billing dates.

These are provider-stated prices, not patient invoices. Each row was checked September 22, 2026. Starting rates can change with your prescription. “Included” describes the company's published bundle, not every test or service a clinician could order. The rows do not establish that the medications or care plans are clinically interchangeable.

How we did the math:

  • Male Excel injections: $99 + ($99 × 12) + ($240 × 6) = $2,727. Cream: $99 + $1,188 + ($264 × 6) = $2,871. These use six every-other-calendar-month medicine charges, not a claim that six 60-day supplies always cover every day of treatment. 1
  • Taurus Meds: $49 + ($149 × 12) = $1,837. With a $17.99 fee each month: $2,052.88. The monthly fee scenario is an assumption, not a confirmed bill. 7 8
  • Hone Plus: $45 + $45 + ($135 × 12) = $1,710 before medicine. The separate lab-schedule page's $50 confirming test makes that $1,715; $80 at-home confirmation makes it $1,745. Hone Premium: $65 + $25 + ($155 × 12) = $1,950 before medicine. If confirmation is included under the general retesting FAQ, the corresponding subtotals would be $1,665 and $1,925.   • Fountain Starter: $199 × 13 four-week periods = $2,587 for 52 weeks. That is 364 days, not 365. A 365-day price equivalent is $199 × 365 ÷ 28 = $2,594.11. A day-zero charge followed by 28-day renewals produces 14 payments, or $2,786, in the first 365-day window. 18
  • TRT Nation: $99.99 × 12 = $1,199.88, plus $129 for each lab panel you buy through it. This is a rate comparison, not a verified order-by-order cash schedule. 19
  • PeterMD: $95 + $948 = $1,043, before follow-up labs. Two $124.50 follow-up panels would make $1,292 before any other charges; two panels is an example, not a verified clinical schedule. 20 21
  • Own doctor, cash: four testosterone tests at a $69 test price plus three CBCs at their $29 regular test price = $363 before physician-service fees. Both product pages separately show a $6 service fee. We did not complete a combined checkout, so we do not assume which fees would be charged together or once per order. 23 24

Three things this table tells you

  1. The membership is a major bill. At Hone Premium, $1,860 of your first year is membership before you buy a single vial. At Male Excel, membership is $1,188 of it. 1 11
  2. Commitment buys a lower advertised rate at some programs. PeterMD's $1,043 subtotal needs $948 paid up front. Fountain's Pro rate needs $1,799 up front, then another payment at week 48 if you renew on that schedule. 18 20
  3. A partner is not automatically the low-cost answer. PeterMD and TRT Nation have lower published starting-rate subtotals than our partners before missing costs are resolved. We show them anyway, because they're part of the honest answer. A partial subtotal does not prove the lowest final bill. 1 7 10 11 19 20

What we actually verified

  • When: September 22, 2026.
  • Public sources checked: the provider pricing, test, availability, cancellation, refund, and program pages linked below; Quest's test-price pages; current FDA information; Endocrine Society guidance; the federal telemedicine extension and controlled-substance schedule; Medicare/CMS and IRS sources.
  • Verified from those sources: that the quoted prices, terms, and conflicting statements appeared in the identified public documents. The sums and payment-window examples were recalculated independently.
  • Provider-stated, not independently tested: what memberships include, clinical and laboratory processes, support access, refill handling, state coverage, shipping, and how cancellation or guarantees work in practice.
  • What we didn't do: buy medication, open patient accounts, complete a checkout, or run an insurance claim. We also did not contact support or test cancellation. We did not verify an assigned clinician's license or an assigned dispensing pharmacy for an individual patient.
  • What remains unresolved: the fee conflicts flagged in the tables; individual medication and shipping quotes; exact refill dates; any unlisted or extra tests; some dispensing-pharmacy and finished-product details. No “complete total” or cheapest-provider verdict is based on those missing facts.

An honest note about Male Excel's price

One of the higher published subtotals in this table belongs to a company that pays us.

Male Excel does not have the lowest published starting-rate subtotal. Its injection plan comes to about $2,727 plus shipping under the six-payment medicine model. If the lowest yearly total is your top priority, compare itemized quotes from the lower-subtotal care routes rather than assuming this membership will cost less. But a year-long prepayment is not required for Male Excel's advertised membership. Its pricing page says no contract and cancel anytime. That does not mean no money paid ahead: medication is billed in two-month quantities. The membership includes unlimited e-visits with a medical provider and unlimited messaging with the support team, not a promise of a clinician on call. Male Excel says its program includes a blood test every six months. 1 4

And its guarantee has its own written terms page: new members who follow the protocol for 90 straight days, can show they followed it, and don't see improvement can ask for their three months of membership fees back, within 30 days after the 90 days end. At $99 per month, that is $297 in membership fees, not a refund of the consultation, medication, or shipping. 5

The pharmacy restriction matters for your budget. Male Excel's current Terms say its prescriptions are filled through contracted pharmacies and cannot be sent to a different pharmacy. So this is not the care route to choose on the assumption that you can move its prescription to the pharmacy offering the lowest coupon price. 6

A comparison of $2,727 against $2,587 would mix a calendar-month estimate with 364 days of Fountain service. The comparable 365-day rate difference is about $132.89, before either program's missing charges; it still does not compare actual cash payments or establish better value.

The published start is a $99 consultation, followed by a required $99/month membership plus medication and shipping if treatment is prescribed. A prescription isn't guaranteed, and Male Excel doesn't treat patients in 11 states. Its current FAQ provides the list below. 1 3


Why doesn't the advertised TRT price match what you'll pay?

Because the headline can show only one line of the bill: the medicine, the first test, one billing period, or a prepaid rate. A full budget also needs any separate membership, blood tests, shipping, and confirming test, without counting bundled costs twice. Turn every line into a 12-month number before you compare two programs. 1 7 10 18 19 20

You're not bad at math. These prices are built to be read one line at a time. Here's what each headline covers.

To be fair to them: Male Excel prints "Plus cost of monthly Medical Membership" right under its prices, and Hone prints "+ membership" beside its $28. The problem isn't only hiding. It's that separate charges still need to be added together. 1 17

Eight costs the headline can leave out

  1. Membership. It can be a major line even before medication. At Hone Premium it is $1,860 a year, and at Male Excel it is $1,188. 1 11
  2. Follow-up blood tests. Some scheduled testing is included at Hone, Fountain, and Male Excel. TRT Nation sells its panel separately for $129, and PeterMD lists a follow-up panel at $124.50. “Labs included” does not establish that all additional testing is included. 4 13 14 18 19 21
  3. A second test to confirm low testosterone. Appropriate repeat early-morning testing is part of diagnosis. Hone publishes a confirming-test step, but its pages disagree about the fee. A company not publishing that step does not prove its clinicians omit it. 25 10 13 15
  4. Shipping. Male Excel charges it but doesn't list the amount on its pricing page. 1
  5. Other medicines. Estrogen blockers, hCG, and thyroid medicines are not testosterone. Include a charge only when it is part of your prescribed plan. Male Excel's quoted price already includes thyroid tablets, so ask whether you need them. Ask whether they are optional and what the price would be without them. 1 33
  6. Prescription changes. Male Excel's own FAQ says any change in dosage will change your cost. Hone's per-vial price means your actual quantity matters; it is not permission to change a dose to hit a budget. 1 10
  7. Billing cycles. Four-week billing is not monthly. Every-other-month billing means your card sees different amounts in alternating months. Every-other-calendar-month billing is also not the same as every 60 days. 1 18
  8. Fees in the fine print. Taurus's Terms describe a recurring $17.99 fee, but its offer says no membership fees. Resolve which terms apply before treating the subtotal as your bill. 7 8

Why four-week billing needs a payment calendar

Fountain bills its plans in four-week blocks. Four weeks is 28 days. Thirteen periods cover 364 days, which is not a full 365-day year. 18

Prices from Fountain's TRT plan page, September 22, 2026. The 52-week figures for Standard and Pro spread each plan's rate over 52 weeks. The 365-day rate column uses 365 days rather than 364; neither rate column is an invoice total. The cash example assumes the first charge on day zero, unchanged prices, uninterrupted advance renewals, and a window including day zero but excluding day 365. It excludes any evaluation or unlisted fee. 18

There's a second trap here, and it's about cash, not price. On the Pro plan, your second $1,799 lands at week 48. Part of that payment buys care beyond the first year, but it leaves your bank account during the first year. Same with Standard: if you prepay on day one and renew on schedule, five payments of $499 — $2,495 — can fall inside your first 365 days. Starter's fourteenth payment lands on day 364. That's not a hidden fee. It's just cash leaving earlier than the monthly math suggests.

The same idea applies elsewhere:

  • “Every other month” (Male Excel) means six medicine charges in a 12-calendar-month model. If membership and medicine start together, an injection-plan payment is $339 before shipping, with $99 membership-only payments in alternating months, plus the separate $99 start. The product is also described as a 60-day supply, so confirm actual refill dates rather than treating those phrases as identical. 1
  • “2.5-month supply” (TRT Nation) describes the amount dispensed, not a verified $250 invoice or an exact 75-day renewal. Ask for the charge per order and the refill schedule. 19
  • “Per vial” (Hone) means your medication cost depends on quantity. A vial is not necessarily one pharmacy fill or one month's treatment. Ask how many vials a year you'll use before you compare. 10 11

The calculator separates these questions. A monthly price equivalent can support a planning estimate, but it cannot produce a dated cash schedule until you supply the actual billing amount and rule.

When a company's own pages disagree

These are specific conflicts or gaps in the public documents we checked. We have not assumed why the pages differ. The useful next step is a dated written answer for your purchase.

A credit worth saying out loud: Hone prints plainly on its injection page that the product is compounded and not FDA-approved. That tells you something a bare ingredient name cannot. We could not establish an assigned dispensing pharmacy for a Taurus patient from the current public Terms; ask for the pharmacy's legal name and your exact product before paying for treatment. 17 8


Which blood tests do you need for TRT, and what do they cost?

Diagnosis calls for symptoms or signs plus consistently low testosterone, confirmed with appropriate repeat early-morning testing; the Endocrine Society recommends morning, fasting measurements. Baseline and follow-up safety testing can add to your bill. The example below has $363 in regular test-list prices before service fees, not a universal first-year lab package. 25 23 24

Here's why the second test matters for your wallet, not just your health. In its 2018 guideline announcement, the Endocrine Society noted that about 30% of men whose first testosterone result was low had a normal result on repeat testing. That is a finding cited in that announcement, not a prediction of your next result or a diagnosis from a normal repeat alone. Paying for a proper evaluation is different from paying for treatment before the diagnosis is clear. 26

A few terms, in plain words:

  • Total testosterone: all the testosterone in your blood.
  • Free testosterone: the small share not attached to proteins.
  • Hematocrit: how much of your blood is red cells. Testosterone can raise it, so it's checked before and during treatment.
  • PSA (prostate-specific antigen): a prostate blood test.
  • LH (luteinizing hormone): the pituitary gland's signal telling the testicles to make testosterone. It helps a clinician find the cause of low testosterone. 25 31

Two follow-up draws is our budgeting assumption; your clinician sets your schedule. We used the CBC's displayed regular price rather than treating a temporary discount as a lasting annual price. If each of these seven tests incurred its own $6 fee, the example would be $405; that is a fee scenario, not a verified combined checkout total. 23 24

Quest's direct-to-consumer testosterone page gives a 3–5-business-day result estimate, accepts HSA/FSA payment, and does not bill insurance for that direct purchase. It lists an additional $79 for an in-home collection where available. That is separate from insurance billing for tests your own clinician orders through a laboratory. Follow the preparation instructions on your actual order. 23

The Endocrine Society's July 16, 2026 statement adds two things worth knowing:

  • Ask for a standardized test. It says many testosterone tests vary from lab to lab, and it urges tests certified by the CDC's Hormone Standardization (HoST) program.
  • A threshold near 300 ng/dL is common, but it isn't a diagnosis by itself. Diagnosis needs symptoms or signs plus consistently low, accurately measured levels. 27

“Not listed” means we couldn't find it on the company's checked public pages. It doesn't prove a clinician will never order it. It means you should ask. These are provider descriptions, not an endorsement that each initial panel completes the needed evaluation.

TRT Nation's FAQ itemizes total testosterone, a CBC, a comprehensive metabolic panel (CMP), sensitive estradiol, and PSA for men over 40. It describes accepting qualifying outside blood work, not home-test kits. These are its program rules, not a statement that one panel completes the diagnostic process. 19

For the two Hone plans, the lab schedules list testing at months 3, 6, 9, and 12 in the first year. They are not identical panels: the Plus schedule includes hematocrit at months 6 and 12, not at every listed draw; Premium lists CBC testing in its scheduled follow-up panels. Confirm any extra tests your clinician orders and who pays for them. 13 14

A published testing sequence can help you compare—not guarantee the right medical care. Hone provides detailed plan-specific lab schedules, including a confirming-test step, but its own confirmation-fee pages disagree. Its priced injection product is compounded, and $28 per vial is not a complete medicine budget. Should you talk to a doctor before budgeting for online TRT?

You can budget now, but talk with a clinician before choosing treatment if low testosterone hasn't been confirmed, you want a child soon, or you have certain health conditions. In those cases, a cheap plan cannot settle the medical question. A calculator can't tell you whether testosterone is safe or right for you — a clinician who can examine you can. 25

The Endocrine Society's guideline recommends against starting testosterone therapy in men who:

  • Are planning fertility in the near term.
  • Have breast or prostate cancer, or specified prostate findings or PSA elevations that require further urologic evaluation.
  • Have an elevated hematocrit.
  • Have untreated severe obstructive sleep apnea.
  • Have severe urinary symptoms or uncontrolled heart failure.
  • Had a heart attack or stroke in the past six months.
  • Have thrombophilia, a condition that raises the risk of blood clots. 25

Where to go instead:

  • Never had the needed repeat early-morning tests: primary care or urology can assess symptoms, records, testing, and possible causes. 25
  • Want a child soon: a urologist or reproductive urologist. Testosterone can lower sperm production. That's a medical decision, not a budget add-on—hCG is not testosterone and is not a guarantee of preserved fertility. 25 33
  • Unusual results with no clear reason, or possible pituitary or testicle problems: endocrinology or urology. 25
  • Overweight (BMI over 27), with low testosterone and no other cause found: the Endocrine Society's 2026 statement says weight loss is typically the first-line treatment. A clinician should assess whether that applies, rather than an online price tool deciding it. 27
  • Chest pain, trouble breathing, or stroke signs: seek emergency care now. A new painful, swollen leg also needs urgent medical assessment, especially with breathing problems or chest pain. 31

How much does TRT cost with insurance or Medicare?

Your cost depends on your plan's coverage, network, deductible, testing rules, and exact prescription. Add your expected share of visits and labs to your pharmacy cost; do not assume an advertised cash-program price will be reimbursed. Male Excel and Taurus describe cash-pay care, and Male Excel's Terms specifically prohibit submitting those program charges to an insurer. 6 7 38

Insurance doesn't have one price. Your plan's rules make the number. Get these three answers, then put the patient-responsibility amounts in the calculator:

  1. How much of your deductible is left this year, and when it resets.
  2. What you pay per visit and per lab draw, including any amount due before the deductible is met.
  3. Whether the exact testosterone product is on your formulary—your plan's covered-drug list—which tier it uses, and whether prior authorization is required.

For a 12-month budget that crosses January 1, ask about both benefit years. Do not add the remaining deductible as a second charge after already entering patient shares that include it. You also cannot predict your total deductible use from TRT alone when other health spending shares that deductible.

What the medicine costs if you pay cash at a pharmacy: ask for a quote for the exact product, strength, package size, quantity, and refill schedule your clinician prescribes. A one-vial coupon price is not a monthly or annual prescription budget. Confirm FDA approval for the finished product, not just the ingredient name; an approved testosterone cypionate product and a compounded preparation are different categories. 28 31 See our testosterone cypionate cost guide →

What a visit costs if you pay cash: it varies. Ask the office for a written estimate before you go. A Medicare payment rate is not the same thing as that office's price to a self-pay patient.

Compounded products are a different category. They aren't FDA-approved. Don't assume a plan will pay for a compounded cream or injection. An FDA-approved product is not automatically covered either. Check the exact product with the insurer. 28 38

If you're on Medicare

Medicare Part D coverage depends on the drug plan and its rules. In 2026, the Part D annual out-of-pocket threshold is $2,100; CMS lists $2,400 for 2027. These figures apply to covered Part D drug spending across the benefit year—not the entire cost of TRT, office visits, laboratory tests, premiums, or noncovered cash purchases. They also do not promise that your particular testosterone prescription is covered. 38 39

Male Excel says it is not enrolled in Medicare or Medicaid and tells people covered by a government plan to consult a participating provider. If you're on Medicare, ask any cash program whether it will treat you before you pay. Also ask how cash payment affects coverage and reimbursement. 6

If your plan says no

Get the written denial and the plan's exact reason. A clinician can address missing diagnostic records, the chosen product, or other plan requirements; a calculator cannot determine whether an appeal will succeed.

On June 18, 2026, HHS announced that FDA was requesting testosterone-label changes, including removal of the limitation of use concerning age-related hypogonadism. FDA's current information page describes that request. A requested label change is not a guarantee that your product's label has changed, that your plan covers it, or that age alone establishes a diagnosis. Requested label changes can take time to show up. Ask your prescriber whether that's worth raising in an appeal, using the current product label and the plan's actual denial. 29 30 25

When cash can beat insurance

Compare the actual patient totals for the same services and product. Cash might be lower in a particular quote, but it is not automatically cheaper with a high deductible, and a coupon purchase may be treated differently by your plan. Coupon purchases may not count toward your deductible, so ask your plan. Also ask how it treats the purchase toward any out-of-pocket limit.

Does the type of testosterone change the cost?

Yes: the exact product, quantity, insurance coverage, and care fees change the price. Compare FDA-approved products separately from compounded preparations, and keep medicines that are not testosterone out of the TRT medication line. Pick the format with your clinician first, then price it. 28 29 31

Two terms, used carefully:

  • FDA-approved means the finished product was reviewed by the FDA for safety, effectiveness, and manufacturing quality.
  • Compounded means ingredients are combined, mixed, or altered to make a medication. The FDA says compounded drugs are not FDA-approved, it doesn't verify their safety, effectiveness, or quality before they're sold, and they aren't the same as generic drugs. A pharmacy being registered with FDA does not make each product it compounds FDA-approved. 28

Some programs describe their products as “bioidentical.” It doesn't make a compounded product FDA-approved, the same as an approved product, or safer. 28

Non-testosterone medicines stay separate. Male Excel's Triclozene is clomiphene-based, not TRT. Clomiphene's approved indication concerns ovulation in women; use in men is off-label. That does not make a compounded clomiphene preparation FDA-approved. Certain hCG products have selected male indications, but hCG is not testosterone and is not a blanket fertility-preservation guarantee. Do not add either simply because a program offers it. 1 32 33

Taurus Meds: a lower offer-based subtotal, with unresolved terms

Bottom line: $1,837 under its advertised $49 start and $149 monthly price—or $2,052.88 only if the $17.99 fee in its Terms applies monthly. Neither figure is a confirmed personal quote. 7 8

$49 is advertised for a blood test at Labcorp or Quest and a clinician consultation. If you live more than 50 miles from one, Taurus says it sends an at-home kit at no extra charge. Its offer page lists treatment at $149 a month and says a clinician determines eligibility. Taurus says its hormones are compounded, and it charges cash rather than billing insurance. 7

The limitation that's easiest to miss is paperwork. Its sales page says "no membership fees," and its Terms describe one. The Terms allow a different offer to be stated at purchase; ask whether that applies to you. Its sales page advertises a 90-day guarantee, and its refund policy limits cancellation refunds to medical disqualification. Its process also describes a medication payment before the consultation. We did not verify that the $149 offer requires a six-month prepayment; ask what commitment and advance payment apply to your order. 7 8 9

Male Excel: higher published subtotal, no annual contract

Bottom line: $2,727 plus shipping for injections, $2,871 plus shipping for cream under the six-medicine-payment model. Its pricing page says no contract. Its current FAQ excludes Alabama, Alaska, Arkansas, Connecticut, Hawaii, Idaho, Louisiana, Minnesota, Mississippi, New Hampshire, and Rhode Island. 1 3

The $99 consultation includes an at-home blood-spot test for testosterone, estradiol, DHEA-S, thyroid (Free T3), and PSA. The displayed medication packages include thyroid tablets, so you're looking at a bundle of two medicines. Ask whether the thyroid tablets are optional and what result supports them. Its kit doesn't list a CBC or LH, and the checked pages do not establish a separate confirming-test price, so ask how the clinician handles those needs. 1 2

Its pricing page doesn't say whether the injectable is FDA-approved or compounded; the cream is compounded. Confirm the exact product before paying for treatment. Male Excel's FAQ names AnazaoHealth and WellDyne pharmacies, but naming pharmacies is not independent confirmation of which one will fill your prescription or of that exact product's approval. Hone Health: detailed lab schedules, membership-heavy pricing

Bottom line: using the separately listed confirming-test prices, the examples are $1,710 for Plus or $1,950 for Premium before medicine. Confirmation-fee conflicts and the medicine quantity prevent either from being a complete price. 10 11 13 15

Plus is offered in 35 states and uses at-home kits. Premium is offered in 34 states, including New York, and uses lab draws. Hawaii and South Dakota get Plus only; New York gets Premium only. No men's hormone plan is offered in Alabama, Alaska, Arkansas, Connecticut, Delaware, Kansas, Louisiana, Minnesota, New Hampshire, New Jersey, North Dakota, Pennsylvania, Rhode Island, South Carolina, or D.C. PeterMD: a low prepaid starting subtotal, with separate labs

Bottom line: $948 up front plus a $95 first panel. Its current TRT page also displays $654 for six months, but the year-one model here uses the 12-month plan. Follow-up panels are listed at $124.50 each; ask how many are needed and whether other tests cost extra. 20 21

The current TRT page excludes Alabama and Idaho, while its laboratory page lists additional limits, including New York and New Jersey. Program availability and access to a particular lab service are separate questions. Its refund policy says treatment sales are final once medication is received, and cancellation of an auto-renewal requires at least 48 hours' notice before processing. Capture the checkout price and the terms before prepaying for a year. 20 21 22

PeterMD's site also markets hCG alongside TRT with the claim that “nothing shuts down” when they are started together. Do not rely on that as a guarantee of preserved sperm production or fertility. TRT Nation: low advertised monthly rate, labs sold separately

Bottom line: $1,199.88 as a 12-month price equivalent, plus $129 for each lab panel bought through the program. Its FAQ requires labs after the first 10 weeks, again six months later, then yearly, and says it treats patients 24 and older. It accepts qualifying outside lab work, but that does not establish that a separate diagnostic confirming test is included. 19

Its page says labs, video visits, and support are included, and "pay nothing if not qualified." We did not establish one evaluation charge or a complete patient checkout from that page. Confirm your state's availability, cancellation terms, medication, and any costs outside the bundle before paying. 18

Your own doctor, urologist, or endocrinologist

Bottom line: this is a care route to price when insurance, a named FDA-approved product, fertility, or a complex evaluation is central. Its total depends on the clinician, pharmacy, tests, network, and insurance benefits; it is not automatically slower or cheaper than online care. 25 29 38

It can change when a prepaid or introductory rate ends, when the prescription changes, or when a bill follows weeks rather than calendar months. Ask for the month-13 price and actual payment dates in writing. “Cancel anytime” does not mean every advance payment is refundable. 1 6 9 16 18 22

“Cancel anytime” isn't the same as “refundable.” Ask which future charges cancellation stops, which subscriptions must be cancelled separately, and whether any commitment or notice period remains. Ask separately what happens to money already paid, especially after a pharmacy has processed or shipped an order. 6 9 16 22

Ask this one question, word for word: “What will I be charged at month 13, and on what date?” If you can't get that in writing, you don't yet have a confirmed renewal price.

Also ask how to obtain your laboratory results, visit notes, and medication history if you transfer care, and whether there is a charge. A refund policy is not a medical-record transfer policy. Do not assume the next clinician will accept every test or continue the same prescription.


What should you ask a TRT provider before you pay?

Ask for an itemized 12-month quote, your month-13 price, what happens to your money if you're not a candidate, and whether the medicine is FDA-approved or compounded. A quote is not complete until you know which charges it includes and when they are due. Save the answers with the date.

Copy these into an email or the provider's chat.

  1. What will I be charged, in total, over my first 12 months? Please itemize every charge and when it's due.
  2. What will I pay at month 13, and on what date?
  3. What will you charge before the clinical decision, and what do I get back if your clinician says TRT isn't right for me?
  4. Is my testosterone FDA-approved or compounded, and which pharmacy fills it? Can the prescription be sent to my own pharmacy?
  5. Which follow-up blood tests are included, how often, and what do extra ones cost?
  6. How will you confirm the diagnosis with appropriate repeat early-morning testing, and is any separate test included?
  7. What does shipping cost per order?
  8. Does my price change if my dose or format changes?
  9. How do I cancel? Do I cancel the membership and the medicine separately? What notice period and refund limits apply?
  10. Do you treat patients in my state, who will provide my care, and will I need an in-person visit?
  11. How are fertility plans assessed before prescribing? What is the referral route if TRT is not appropriate?
  12. How do refills, clinician questions, lab-result access, and medical-record transfers work, and are there separate charges?

Start with #1 and #3. They establish the proposed bill and the amount you could pay before the treatment decision. Clear answers to those two do not replace checking the rest.

Using insurance? Ask your plan instead: your cost per visit and lab draw, whether the exact testosterone product is covered and at what tier, whether it needs prior authorization, and when your deductible resets.

Can you pay for TRT with an HSA or FSA?

Eligible prescribed medical care, medicines, and tests may qualify, but a provider accepting an HSA/FSA card does not prove that every part of its membership qualifies. Confirm eligibility with your plan administrator and keep itemized receipts. The tax examples below apply only to an eligible expense funded with qualifying pre-tax contributions. 40 41

Male Excel and Quest advertise HSA/FSA payment options. Male Excel's FAQ also says it does not provide letters of medical necessity for HSA/FSA reimbursement. That matters if your administrator requires one for a bundled membership. 1 3 23

We show possible federal income-tax savings separately. They don't lower the provider's bill; they illustrate the effect of funding an eligible expense with pre-tax money.

Example: $2,727 × 22% = $599.94. These are arithmetic examples, not provider discounts or guaranteed tax savings. They exclude payroll and state tax effects, contribution limits, and your individual tax circumstances. A new purchase from money already in an HSA does not create a second tax deduction for the same dollars. 41

HSA contribution eligibility generally requires qualifying health coverage and other conditions; Medicare enrollment affects the right to contribute. You can still use existing HSA funds for qualified expenses when you are no longer eligible to contribute. FSA use-it-or-lose-it rules may have plan-specific grace-period or carryover provisions. Confirm the account rules rather than treating the tax percentage as a reduction in the clinic's invoice. 41


Is TRT a lifelong cost?

For some men with confirmed testosterone deficiency, treatment and monitoring are ongoing rather than a short course. Duration depends on the cause and the clinician's assessment of benefits, risks, and response, so a three-year model is a budget scenario—not a prediction that you need lifelong TRT. 25 31

Don't stop or change treatment on your own — talk with your prescriber. And settle fertility plans before you start.

That makes year two and year three as important as year one.

Years two and three omit the modeled initial consultation and diagnosis charges, not all future evaluation. Prices can change at any time; this shows how today's structures add up. Extra testing, a change in care, or a different prescription can change the total. Fountain's $7,761 figure for three 52-week periods covers 1,092 days, not the 1,095-day example above. Leap years and actual billing dates need their own cash calendar.

Fertility changes the plan, and the price. Testosterone can lower sperm production. If children are in your plans, see a urologist or reproductive urologist before you start. The right path may not be standard TRT at all, and it may cost differently. No test, add-on medicine, or calculator here promises preserved fertility or recovery on a set timeline. 25 33


Will the December 31, 2026 telehealth deadline change your cost?

Possibly, but an added visit cost is not certain. The current federal temporary extension permits qualifying telemedicine prescribing of controlled medicines, including testosterone, without a prior in-person visit under its conditions through December 31, 2026. A 12-month budget made in September 2026 crosses that date, so ask what your provider would require if the rules change. 36 37

This comes from the DEA and HHS's fourth temporary extension, published in the Federal Register on December 31, 2025. For testosterone, the temporary pathway includes qualifying audio-video care and other legal requirements; it is not blanket permission to prescribe from a questionnaire. It does not override state rules or promise service in every state. Male Excel already tells patients that, based on DEA and state laws, a testosterone plan "may require an in-person medical exam." 36 37 3

This isn't a reason to rush. It's a reason to ask your provider: “If the federal rules change on January 1, 2027, what will you require of me, and what will it cost?” Add any quoted in-person visit to the calculator. Do not add an invented visit fee or assume what the next federal rule will be.


How does this TRT cost calculator work?

We read the identified public price and policy pages, calculated the stated charges, and kept missing or conflicting costs unresolved. The calculator applies transparent planning and payment-date rules to your entries; it does not establish coverage, medical eligibility, or a complete quote from a starting price. Sources and rates were last checked September 22, 2026.

What we count: start fees, confirming tests, membership, medicine, follow-up visits and labs, supplies, shipping, and other fees. A charge marked included is not added again, and a blank or disputed charge is not silently set to zero.

How we count a planning year: monthly rates use 12; every-other-calendar-month rates use six; fixed-day rates use 365 divided by the number of days in the billing period. A per-vial price needs a quantity. A per-fill price needs the number of fills. Neither one establishes a prescription or a payment calendar by itself.

How we count actual cash: use an actual charge amount, its first payment date, and its renewal rule. Count payments on or after the window's start and strictly before its end. A monthly charge anchored to January 31 advances to the last day of February and then March 31, without drifting permanently to the 28th. A service period can extend beyond the date window even though its full prepayment is inside it.

How we label unknowns: quoted, provider-stated, included, confirmed $0, not applicable, conflicting, or unknown. A known subtotal remains partial when a needed cost or count is missing. Dates can be incomplete even when a planning amount is available. Comparing two partial subtotals does not produce a lowest-cost provider.

What we don't do: buy medicine, open accounts, or run insurance claims. Company statements about what's included are their claims, labeled as such. The published examples do not independently verify an individual clinician, pharmacy assignment, treatment outcome, or refund experience.

What else should you know about TRT costs?

Use these answers to check the gaps that a monthly advertisement cannot settle. Your own quote and billing dates matter more than a low starting rate.

How much does TRT cost per month?

There isn't one complete monthly price. For example, Male Excel's injection model is $2,727 before shipping, or $227.25 per month averaged over 12 months. That includes the modeled start fee. Your actual bills may be bigger or smaller in a given month. Hone Premium's paid-confirmation example is $162.50 per month before medicine. These are partial models, not a market-wide price range. 1 11

How much does TRT cost per year?

The examples start at $1,043 before follow-up labs for PeterMD's prepaid year. Male Excel's injection model is $2,727 plus shipping, and its cream model is $2,871 plus shipping. Missing costs and different billing periods mean those numbers are not a complete low-to-high range. 1 20 21

How much does TRT cost without insurance?

Add the actual cash prices for care, labs, medicine, and fees. Through your own doctor, the illustrative lab model here has $363 in test-list prices before service fees and additional tests, plus visits and pharmacy fills. Online bundles can be compared only after you identify what they leave out. 23 24

Does insurance cover TRT?

Coverage depends on the plan, medical documentation, exact prescription, network, and authorization rules. Get the insurer's answer for your product and clinician; FDA approval is not a promise of coverage. Male Excel's program is cash-pay and its Terms prohibit submitting program charges to insurance. 29 38 6

Does Medicare cover testosterone therapy?

Your plan may cover an eligible prescription under its rules. The $2,100 in 2026 and $2,400 in 2027 Part D thresholds concern covered Part D drug spending, not all treatment costs or a guarantee of testosterone coverage. 38 39

What's the cheapest way to get TRT?

Compare written quotes for the care and medication you actually need. Start with your covered clinician and pharmacy costs when using insurance; compare all-in cash and prepaid offers separately. No row with unknown medicine, lab, shipping, or fee amounts proves the cheapest complete treatment.

Why do online TRT programs charge a membership?

Providers describe memberships as paying for selected clinical visits, testing, support, and other services. What is included differs. At Male Excel the published membership is $1,188 a year; at Hone Premium it is $1,860 before medicine. Compare the membership line, not just the medicine line. Check which services are included. 1 11

How much do TRT blood tests cost?

Quest lists its testosterone test at $69 plus a $6 service fee. Its CBC page shows a $29 regular price, a $26.10 promotional price when checked, and a separate $6 fee. A full budget depends on the ordered tests, repeat testing, fee treatment, and monitoring schedule—not one panel price. 23 24 25

Why did my TRT price go up after I started?

Check whether a starting rate ended, the prescription or included services changed, or your bill follows weeks rather than months. Then compare the actual invoice with the agreed renewal terms. Ask for your month-13 price and its charge date in writing. 1 6 18

Can I use my HSA or FSA for TRT?

Qualified medical expenses may be eligible. Check with your plan administrator, especially for bundled memberships; a provider accepting the card is not a tax ruling. Keep receipts and avoid claiming the same expense twice. 40 41

Is compounded testosterone cheaper than FDA-approved testosterone?

Not necessarily; compare quotes for the exact product and care services. Compounded drugs are not FDA-approved and are not the same as FDA-approved generic drugs. A bundled price does not establish equivalent products or equivalent care. 28

Is this a TRT dose calculator?

No. Your prescriber sets your dose. This tool only prices care.

Do I need to enter medical information?

No. The calculator asks for costs, billing details, and optional care preferences—not symptoms, lab values, doses, or insurance ID numbers. The care-route tool is separate; read the privacy policy before using the website's tools.
