Nuform Health TRT cost is not just the advertised $50 monthly medication rate. Its page contains $200 four-month and $295 five-month kit offers; adding one published $170 annual fee gives $770 or $1,055 in modeled first-year kit payments and care fees, before labs and other charges. Confirm which offer applies before paying. 1 2

This guide is best for: U.S. adult men paying cash who have had low testosterone properly checked, are fine with injections, and aren't trying for a baby soon.

Start somewhere else if:

  • You want insurance to pay.
  • You need an FDA-approved testosterone product.
  • You're trying to conceive in the near future.
  • Your low testosterone hasn't been confirmed with proper testing yet.

We cover each of those routes below. 6 11 13

How much does Nuform Health TRT cost?

Nuform's TRT page text includes a $200 four-month injectable kit and a $295 five-month kit, with supplies included. Its other pages list a $170 annual physician fee and lab panels at $95 or $195. Those are published price components, not a confirmed all-in quote; the applicable kit, required tests and any other charges still need to be settled. 1 2 4 6

Table 1. Published Nuform TRT price components in one place (checked September 23, 2026)

Cost item What Nuform publishes How you pay What's still open Source
Standard TRT, 4-month kit $200 ($50/month). Injectable. Supplies included. Multi-month kit, not a $50 monthly subscription Whether this is the kit you'll be offered 1
Standard TRT, 5-month kit $295 ($59/month). Supplies included. Multi-month kit Whether it's a current offer or old page content 1
Testosterone + hCG or gonadorelin $120/month. Described as a one-month injectable kit Exact payment period needs confirmation; the same block also says no monthly prescriptions Which additional drug and preparation is offered; how it's billed 1
Physician fee $170. Called an annual physician fee on one page and a one-time fee on another Budget one annual fee unless your written quote says otherwise Whether the fee applies to your plan and when another payment would be due 2 3 19
Male Essential (basic) lab panel $95: $65 for Quest blood work plus $30 for analysis. Nuform says you are not charged at the lab. Per panel Which tests are needed and what repeat orders cost 4
Male Comprehensive lab panel $195: $145 for Quest blood work plus $50 for analysis. Up to 57 markers, per Nuform Per panel Which tests are needed and what repeat orders cost 4
Shipping and handling No amount found on the pages checked. Terms say any charge is shown at purchase. If charged, confirm each order Standard and optional faster-shipping charges 6
Insurance Nuform says it does not accept insurance Cash pay Do not assume your plan will reimburse a charge 6
HSA/FSA Nuform's Our Approach page says funds are accepted Confirm payment and receipt requirements Your benefits administrator's rules for each expense 19

Here's the part that matters: we verified that those statements appear in Nuform's public pages. We did not verify which kit a new patient will actually be offered. Different blocks in retrieved page text can include older or hidden content. The two kit prices are not two options we confirmed you can select.

The sample budgets below are lower than the Male Excel starting-price illustration later in this guide, but neither is a personal quote. But Nuform's own pages don't agree on a few prices. One of those gaps could change what you pay. We'll show you which one, and the exact question that closes it before your money becomes non-refundable.

What changes your number:

  • Which lab panel you get ($95 or $195), and how often your clinician repeats it.
  • Which kit you're quoted. The same Nuform page lists a 4-month kit and a 5-month kit.
  • Whether your clinician recommends another medication, and its separate medical purpose, cost and billing terms.
  • Whether Nuform can provide your specific care and prescription in the state where you will be located. 1 4 18

What the $50 a month includes

Per Nuform's TRT page, the $50 covers the testosterone and the injection supplies. This is the rate for its four-month kit. Nuform lists under-the-skin injections (subcutaneous, or "subQ") and into-the-muscle injections (intramuscular, or "IM"). Nuform says the price is the same either way. Your clinician must choose the preparation and treatment instructions. 1

What the $50 a month doesn't include

  • The separately listed $170 physician fee.
  • Your lab panels, including repeat tests.
  • Any shipping, handling or other charges in your quote. 2 4 6

Those separate items are why $50 a month is not the full bill. In the three-panel, four-month-kit examples below, the listed components add up to roughly $88 to $113 a month before other charges—not a verified monthly payment or complete care budget.

What's the real first-year cost of Nuform TRT?

With three $200 kits and one $170 fee, medication and care fees alone total $770; with three $295 kits, they total $1,055. Adding three panels at the published prices produces the sample subtotals below. Your actual first-year cost also depends on the required diagnostic and follow-up tests, order dates, prescription, shipping and any other charges. 1 2 4 6

We built these numbers ourselves from Nuform's pages. They are reproducible budget examples, not a checkout test or a claim that every patient needs three panels.

Our assumptions:

  • 12 months of treatment, with one $170 physician fee charged within the budget period.
  • You're approved at your first review. Approval is not guaranteed, and completed testing can still cost money if treatment is declined.
  • Three lab panels are used only to illustrate lab spending: one initial panel and two later panels. This is not a required or sufficient testing schedule. A second early-morning diagnostic test, other baseline tests, or extra monitoring may add cost unless already covered by accepted outside results or your quote.
  • Repeat panels cost the same as the first one in the examples; Nuform has not confirmed that pricing for an individual plan.
  • Whole kits are purchased as needed, at unchanged prices and the quoted duration. A different prescription or payment schedule changes the cash required.
  • Shipping, handling, any applicable tax, extra testing, extra visits and other separately billed items are left out because their amounts were not confirmed. 1 2 4 5 6 11

Table 2. Nuform first-year budget examples—not all-in quotes (published inputs, September 2026)

Scenario Physician fee Labs: 3 illustrative panels Medication payments Sample subtotal Subtotal ÷ 12 First fee + first panel + first kit
Three basic panels, 4-month kits $170 $285 (3 × $95) $600 (3 × $200) $1,055 About $88 $465
Comprehensive first, then two basic panels $170 $385 ($195 + 2 × $95) $600 $1,155 About $96 $565
Three comprehensive panels, 4-month kits $170 $585 (3 × $195) $600 $1,355 About $113 $565
5-month kits, with comprehensive first and two basic panels $170 $385 $885 cash (3 × $295) $1,440 cash $120 $660
Testosterone-plus-add-on price example, with comprehensive first and two basic panels $170 $385 $1,440 (12 × $120) $1,995 About $166 $485
Testosterone-plus-add-on price example, with three comprehensive panels $170 $585 $1,440 $2,195 About $183 $485

Inputs: Nuform's kit descriptions, annual fee and lab-panel page. The last two rows assume the $120 offer is billed one month at a time; that payment schedule is unresolved. They are not recommendations to add a drug or claims that a kit protects fertility. The final column adds three listed charges; it is not a verified amount due at checkout or before shipping. 1 2 4

Why the 5-month kit costs more cash in year one: Two 5-month kits only cover 10 months, so the whole-kit model requires a third. That's $885 in medication payments for 15 months of quoted coverage. Twelve months at the $59 equivalent rate is $708; the remaining $177 is prepaid value allocated beyond the first year. That is an accounting distinction, not a promise about storage life, future treatment or use beyond the pharmacy's instructions. Either way, the quoted medication rate is higher than the four-month kit's rate. 1

What about year two? If the four-month-kit price holds, the $170 fee applies again, and one or two $95–$195 panels are ordered, those components total $865 to $1,160. That is another illustration, not a recommended monitoring schedule or an all-in second-year quote. It does not model five-month-kit carryover. Ask about price changes, the actual fee-renewal date and every other charge before using it as your budget. 1 2 4 6

Want your own number? Use this simple formula with the prices from your written quote:

First-year cash budget = all care fees charged during the year + all diagnostic and monitoring lab charges + all medication-kit payments during the year + shipping, handling and any other charges.

For a repeated charge, multiply its price by the number of times you will pay it. When panels have different prices, add each panel separately. Include another care-fee payment if its renewal falls within your chosen budget dates.

Leave any unknown line blank rather than treating it as $0. A blank means you still have a question to ask. Our TRT cost guide's calculator and paper worksheet separate initial testing, confirmatory testing, medication, care fees and other charges.

If the sample costs fit your budget, the next step is confirming Nuform's actual quote and whether it can treat you where you will be located.

Start Nuform's intake and ask about your state →

Nuform's published process puts the intake form and a specialist call before the physician fee. Completing the form doesn't mean you'll be prescribed anything. 3

The right TRT provider is not the same for every person — it depends on whether low testosterone has been properly evaluated, your symptoms and health history, your fertility plans, your state, your insurance or cash-pay preference, your treatment-format preference, your budget, and whether online or in-person care is the better starting point. Some situations belong with primary care, urology, endocrinology, reproductive urology, or urgent or emergency care when the symptoms warrant it. Because a general answer cannot resolve those for you, use TRT Provider Guide's Find My TRT Path tool to map your situation to the right care route and the questions to ask before you pay.

Find My TRT Path is educational and non-diagnostic. It can't confirm low testosterone, decide whether you qualify for treatment, or promise a prescription. Read our privacy policy.

Can you pay for Nuform TRT monthly?

Not under the standard multi-month kit offers described on Nuform's pages. Nuform says it does not offer monthly prescriptions or subscription plans, and its cancellation guidance says to avoid a future kit purchase by not reordering. That does not refund an order already purchased or settle the terms of a separately charged physician fee. 1 2 5

Here's the one real trade-off in Nuform's pricing.

Nuform does not describe a month-to-month payment option for its standard TRT kits. The four-month offer is a $200 kit, not four separate $50 payments; the five-month offer is $295. Medication orders are final at purchase under its refund policy. 1 5

If paying smaller amounts as you go matters most to you, compare actual dollars due—not just how often a provider bills. Male Excel lists a 60-day medication supply, but its starting injectable order works out to $240, plus a separate $99 monthly membership and the initial consultation. A shorter supply does not automatically mean less cash up front. We compare it below. 9 10

Nuform's published annual care fee is lower than twelve $99 Male Excel membership payments. Nuform's labs are separate, while Male Excel says its membership includes blood testing every six months. Compare the complete plan you will actually receive. 2 4 9

Nuform gives its own reason for kits: fewer gaps from monthly medication drop-offs. Whether that helps you is your call. We did not test its refill process. Its Our Approach page separately says nothing renews automatically without you. 1 19

Can you cancel? Nuform says there's no subscription to cancel. You "simply don't reorder your next kit." The $170 fee is a different story. We cover when it stops being refundable in the refund section below. 2 5

How do refills, follow-ups and medical records work?

Nuform says it assigns a dedicated provider and specialist, offers phone or text contact, and checks in every four weeks. Those are provider-stated service features, not response times or visits we tested. A check-in also does not prove that every clinical consultation, urgent question or extra lab order is included. 2 19

Before a kit runs low, ask who authorizes the next prescription, how far ahead to contact the team, whether labs must be reviewed first, and how an order is approved and paid for. The public pages checked did not provide a complete, patient-specific refill timetable.

Nuform's Our Approach page says your lab results, plan and history can be requested from the team. We did not confirm a formal records-download process, transfer fee or turnaround time. Ask how to obtain both your lab reports and clinical records, and how care would be handed to another clinician. Not reordering a kit and arranging continued medical care are separate tasks. 19

Why do Nuform's own pages list different prices?

Nuform's retrieved TRT page text includes both a $50/month four-month kit and a $59/month five-month kit. Its other pages use different descriptions for kit lengths and the physician fee, while shipping estimates refer to different time periods and milestones. These pages do not settle your personal quote, so ask which offer and terms apply. 1 2 3 19

We compared Nuform's treatment, fee, lab and policy pages. These are the places where the wording leaves a billing or fulfillment question open.

Table 3. Nuform page differences and the question each one creates (checked September 23, 2026)

Topic One published statement Another published statement Question to ask
Kit length and price 4-month kit, $200 ($50/month) 5-month kit, $295 ($59/month), in the same TRT page text "Which kit am I being quoted, and why?" 1
Kit sizes offered 5-month kit on the TRT page 3- or 4-month kits on Why Nuform "What kit lengths can I choose from?" 1 2
Monthly option Testosterone-plus-add-on offer labeled as a one-month injectable kit No monthly prescriptions, printed under the offer "How is this particular kit billed?" 1
Physician fee $170 annual fee on Why Nuform; one-time fee on the homepage Our Approach says no membership fees and no automatic renewal without you "Does the $170 apply to my plan, when is it due again, and what consent is needed?" 2 3 19
Shipping milestones TRT page: about 5–7 days; homepage: 1–10 business days Why Nuform: arrival within 7–10 business days; Our Approach: pharmacy shipment within 2–10 days after approval "Which estimate applies: pharmacy processing, shipping or arrival?" 1 2 3 19
Pharmacy description Wording about 503A and 503B compounding Other blocks refer to U.S. 503A pharmacies "Which pharmacy fills my prescription, and what is its license or registration?" 1 19

These differences do not establish why the pages changed or which block is current for a new patient. The Our Approach page also says prices exclude lab and consultation fees, despite its no-membership wording. They're why a written quote matters more than any headline, including the one at the top of this page.

The shipping estimates also are not all measuring the same thing. A pharmacy's dispatch date and a package's arrival date are different. Nuform's terms say shipping dates are estimates, not guarantees. 6

Does Nuform's price include lab work?

No. Nuform's lab panels cost extra: $95 for the basic male panel or $195 for the comprehensive panel. The blood is drawn at Quest, and you pay Nuform, not the lab. 4 11

Nuform requires bloodwork before hormone treatment. It says it schedules you at a lab near you rather than relying on at-home kits. That does not establish that every at-home test is inaccurate. The basic panel does not list PSA, a blood test used in prostate assessment; your clinician decides which tests you need and how often. 1 2 11

Key terms:

  • Total testosterone is all the testosterone in your blood.
  • Free testosterone is the part not bound to proteins.
  • SHBG (sex hormone-binding globulin) is a protein that holds onto testosterone. It changes how your total level should be read.
  • LH and FSH are signals from the brain that tell the testes to make testosterone and sperm.
  • Hematocrit is the share of your blood made of red blood cells. TRT can raise it.
  • PSA (prostate-specific antigen) is a blood test used in prostate monitoring. 11 12 20

Table 4. What Nuform's panels list—and what those lists don't settle

Test Why it matters or what to ask Basic $95 Comprehensive $195
Total testosterone Diagnosis needs compatible symptoms or signs and consistently low results, confirmed with repeat morning, fasting testing Listed Listed
Free testosterone Used when indicated; the lab method and clinical context matter Listed Listed
LH and FSH Helps find the cause of low testosterone Listed Listed
Hematocrit, in the blood count Checked before and during therapy; an elevated result needs clinical review Listed Listed
PSA Prostate assessment and monitoring depend on age, risk and shared decisions with the clinician Not listed Listed
Estradiol Listed by Nuform; this is not proof every patient needs it at every draw Listed Listed
Cholesterol, A1c, liver and kidney tests General health information; the need for each test is separate from kit pricing Not listed Listed
SHBG Can affect interpretation of testosterone results Not listed Not listed

Panel contents are provider-stated on Nuform's Diagnostic Labs page. "Not listed" means not named in that published panel; it does not rule out a separately ordered test. The clinical context comes from the Endocrine Society, not from Nuform's panel names. 4 11 12

What that means for your budget: If your clinician recommends PSA or another test missing from the basic panel, ask about the added test's price and whether a broader panel is needed. Do not assume age alone means you must buy three comprehensive panels. The $300 difference between the $1,055 and $1,355 examples is simply three $100 panel-price differences. 4 11

The Endocrine Society calls for a discussion of prostate monitoring in men ages 55–69 being considered for TRT who have a life expectancy over ten years, and in men ages 40–69 at increased prostate-cancer risk. For those who choose monitoring, assessment begins before treatment and is repeated 3–12 months afterward. Your clinician should apply that guidance to your situation. 11 12

How many lab tests will you need?

Nuform says it recommends repeat labs 3–4 months into therapy. The Endocrine Society recommends checking testosterone and hematocrit after treatment begins, with follow-up timing based on the test and the patient; its monitoring guidance includes an initial 3–6-month check and later ongoing review. Neither source establishes that every Nuform patient needs exactly three full panels in the first year. 1 12

Your clinician sets the real number, not this page. Ask separately about tests to confirm the diagnosis, tests before treatment, tests during treatment, and any repeat needed after an unexpected result or a treatment change. Get the charge for each order, not just the initial panel.

One test can't diagnose low testosterone

The Endocrine Society recommends diagnosing low testosterone (hypogonadism) only when a man has compatible symptoms or signs and consistently low levels. That means a morning, fasting test, confirmed by repeating it. A single low result is not enough to confirm a new diagnosis under that guidance. No single cutoff, including 300 ng/dL, is a diagnosis by itself. 11

Ask Nuform whether it will accept recent results from your own doctor so you don't pay twice. We did not confirm its outside-lab acceptance rules, timing window, assay requirements or charge for a second diagnostic draw. Our blood tests for TRT guide explains the difference between diagnosis and monitoring.

What else can show up on your Nuform bill?

Nuform's terms say it doesn't accept insurance and that you pay any shipping and handling shown at purchase. We did not find a fixed shipping amount on the pages checked, and the terms also allow applicable taxes. Ask for every charge in writing before you pay the $170, including any optional faster shipping. 6

Here's what real customers say about fees. These are individual reports from Nuform's Trustpilot page, checked September 23, 2026. They describe single experiences and don't prove Nuform's current policy either way.

"Pricing is also top-tier. Transparent, competitive, and no hidden fees or upselling."

— Eugene Crowley, Trustpilot review published September 7, 2026; experience dated August 6, 2026. Marked Invited. Read Nuform's Trustpilot reviews.

"I paid a small additional fee for expedited shipping"

— Alan Scruton, Trustpilot review published September 2, 2026; experience dated August 28, 2026. Marked Invited. Read Nuform's Trustpilot reviews.

These reviewers describe TRT service and billing experiences. The invited-review labels are visible; the source does not establish a further financial relationship. Neither quote proves a standard shipping charge, typical experience or medical result. 8

The practical question is still the same: "Please list every fee I'll be charged, including shipping, per kit."

Does Nuform take insurance, HSA or FSA?

Nuform's terms say it "currently does not accept insurance." Its Our Approach page does say HSA/FSA funds are accepted. Ask Nuform what receipt or payment process it offers, and check the expense with your benefits administrator; acceptance is not a guarantee of reimbursement or eligibility for every charge. 6 19

If insurance coverage matters to you, start by asking an in-network primary care doctor or urologist and your plan for a covered-care estimate. Coverage depends on the visits, labs and exact prescription, not just the clinic's name. We cover that care route in the comparison below and in our TRT cost guide.

What are Nuform's refund rules if you change your mind?

Nuform's refund policy uses several physician-fee cutoffs: consultation start, completed bloodwork for hormone patients, and acceptance of recommended treatment. It also promises a full physician-fee refund if the provider decides you're not a candidate. Completed labs and purchased medication have separate nonrefund rules, so a physician-fee refund is not a refund of everything you spent. 5

Table 5. When your money can become non-refundable (Nuform refund policy, checked September 23, 2026)

Step or decision Charge at issue What the posted policy says
Fill out intake form No charge is listed for the form itself in the process checked Confirm the next charge before authorizing it; intake is not clinical approval
Pay the physician fee $170 Refundable until consultation begins; for hormone patients, the policy also says until bloodwork is completed. Confirm which milestone governs your case.
A provider says you're not a candidate Physician fee Refunded in full
Bloodwork is completed Lab charge, such as the listed $95 or $195 panel Lab fee is not refundable once completed
You accept recommended treatment Physician fee Becomes non-refundable
Medication is purchased or ordered with the pharmacy Kit charge All medication orders are final at purchase; no refund once ordered with the pharmacy, and no return once dispensed

The policy describes different services and milestones. This table is not a guarantee that every patient reaches them in this order. 3 5

Two things to notice:

  1. The best time to ask questions is before you pay anything. Get the applicable physician-fee cutoff in writing rather than assuming a future blood draw is the only deadline.
  2. "Not a candidate" refunds only the physician fee under that clause. If you've already had labs done, that lab money stays spent. 5

One wrinkle: the policy says the fee is refundable "until your consultation begins" and also "until blood work is completed" for hormone patients. Those can happen in either order. Ask Nuform which cutoff applies to you. 5

Is Nuform's testosterone FDA-approved or compounded?

Nuform says its testosterone comes from U.S. compounding pharmacies. The FDA states that compounded drugs are not FDA-approved, which means it doesn't review their safety, effectiveness or quality before they're sold. The TRT page text checked does not identify the exact testosterone preparation or dispensing pharmacy for your prescription, so ask for both before you pay. 1 13

Here's what that means in plain terms.

FDA-approved testosterone products went through FDA review and come with official labeling. Compounded testosterone is a preparation made by a compounder rather than an FDA-approved finished drug product. Compounding can meet a real medical need, but it does not make the resulting drug FDA-approved or equivalent to an approved product. Nuform's page calls its product "bio-identical." That's Nuform's marketing term, not an FDA approval category. 1 13 14

What Nuform's TRT page doesn't tell you:

  • Which testosterone. The page says testosterone without identifying the exact preparation you would receive. Ask for its name, strength, ingredients and labeling.
  • Which pharmacy. Nuform describes the pharmacy two different ways (see Table 3) and doesn't identify the one assigned to your prescription in the content checked. FDA's 503A and 503B categories are not product approvals. A compounding claim also does not verify a pharmacy's license.
  • How your prescription affects price. A kit's advertised duration is not a promise that every prescription lasts the same length of time. Your clinician decides your dose. Ask how the prescription changes the kit price and the number of orders you would pay for. 1 14

If you want an FDA-approved product, ask a prescriber who offers that option to name the exact product and dispensing pharmacy. Primary care, urology or endocrinology can be a starting point, with insurance checked when relevant; FDA-approved care is not limited to local clinics. Male Excel, discussed below, also describes compounded testosterone, so switching to its advertised program does not resolve this requirement. Our TRT treatment options guide separates approved products from compounded preparations. 9 13 23

One legal fact that applies everywhere in the United States: Testosterone is a Schedule III controlled substance under federal law. You need a valid prescription from an authorized clinician. Ongoing clinical monitoring is part of appropriate treatment; a paid kit or online questionnaire does not replace that care. 11 15 16

What safety checks matter before choosing the lowest price?

A low fee is not a reason to skip a medical review. Fertility plans, an elevated hematocrit, untreated severe sleep apnea, prostate or breast cancer, and a recent heart attack or stroke are among the issues that can change whether testosterone is appropriate and which clinician should evaluate you. Discuss these with primary care or the relevant specialist before choosing a treatment program. 11

Testosterone can also raise blood pressure. In 2025, FDA required blood-pressure warnings while removing the older boxed-warning language about increased adverse cardiovascular outcomes. That change was not a finding that testosterone is risk-free or that compounded preparations had been approved. Ask how your blood pressure and other treatment risks will be monitored. 13 23

How much does Nuform TRT cost when fertility matters?

Nuform lists testosterone plus hCG or gonadorelin at $120 a month, but the kit's billing terms and exact additional drug still need confirmation. The Endocrine Society recommends against starting testosterone in men planning fertility soon. If a baby is in your near-term plans, talk with a urologist or fertility specialist first. 1 11 20

The cost: The listed $120 medication rate is $70 a month more than the four-month kit's $50 equivalent rate. Over 12 months, that's $840 more in medication-rate terms. With the same illustrative fee and lab assumptions as Table 2, the listed components total $1,995 or $2,195. Neither figure is a complete fertility-care budget or a verified one-month payment plan. 1 2 4

The medical side: Testosterone from outside the body tells your brain to slow the signals that drive sperm production (spermatogenesis). No add-on guarantees your fertility will be protected. Nuform's lab panels don't include a semen analysis, which is the test that actually measures sperm. 4 20

hCG and gonadorelin are not interchangeable. Human chorionic gonadotropin, or hCG, has FDA-approved products with specific uses; Pregnyl's label includes selected male cases of low testosterone caused by inadequate pituitary signals. That does not establish that Nuform supplies Pregnyl, that its unnamed preparation is approved, or that adding hCG to TRT guarantees fertility. Nuform does not identify a specific gonadorelin product in the offer, so its exact product status and clinical purpose must also be clarified. Neither additional drug is testosterone replacement by itself. 1 20 21

Talk with a specialist first if you:

  • Want a pregnancy in the next year or two.
  • Already have trouble conceiving.
  • Have been told you have very low LH or FSH, or a known pituitary or testicle problem.

For these situations, start with a reproductive urologist or endocrinologist rather than any online TRT kit, Nuform's included. Bring up longer-term fertility plans too, not only an immediate pregnancy goal. Our TRT and fertility guide explains the care questions. 11 20

A note on enclomiphene: Nuform also lists enclomiphene, a pill that works differently from testosterone. It isn't TRT, and its costs don't belong in this budget. A compounded enclomiphene preparation is not FDA-approved; it should not be confused with an approved clomiphene product simply used for a different indication. FDA's enclomiphene review materials distinguish the two, and the AUA/ASRM guideline describes clomiphene use in men as not FDA-approved for that use. See our enclomiphene cost guide. 1 13 20 22

Can you get Nuform TRT in your state?

We could not confirm a current, complete Nuform TRT state list from the public pages checked. Federal rules currently allow certain controlled-substance prescriptions by telemedicine without a prior in-person evaluation through December 31, 2026, when their conditions are met. That is not proof that Nuform or an assigned clinician can treat you in every state. 16 17 18

What we know:

  • Nuform has an active official intake destination, but we did not test it as a state-availability checker. Ask the team about the state where you will physically be during the appointment and where medication would be dispensed or delivered.
  • Federal telehealth flexibility for controlled substances, including testosterone, runs through December 31, 2026, under the fourth temporary extension from the DEA and HHS. For testosterone under this pathway, a questionnaire or audio-only call is not a substitute for the required real-time audio-video encounter.
  • The prescriber must have the applicable prescribing authority, and federal and state requirements still apply. State rules, your clinical needs or the provider may require an in-person visit. 1 16 17 18

Ask for the assigned clinician's name, professional license type and state authorization. The provider's statement that it uses licensed clinicians is not an independent license check for the clinician who will treat you. Check again before moving or attending an appointment from a different state. 19 18

Why this matters for your budget: A 12-month plan that starts now runs past December 31, 2026. We're not predicting remote care will end. But it's smart to ask Nuform what happens to your refills if federal rules change, before you prepay months of medication. 16 17

Is Nuform cheaper than Male Excel or a local doctor?

Nuform's sample kit-and-lab budgets are lower than Male Excel's published starting-price illustration, but that is not a complete like-for-like patient quote. Male Excel's $99 consultation, $99 monthly membership and injections from $120/month add to $2,727 across twelve monthly equivalents, before shipping or other charges. Local care through insurance needs its own estimate and can provide an FDA-approved-product path. 1 2 4 9 10 23

Table 6. Nuform vs Male Excel vs local care (published inputs checked September 23, 2026)

Item Nuform Health Male Excel Local doctor or urologist
First-year budgeting $770 or $1,055 for modeled kit cash payments plus one care fee, before all labs and other charges. Three-panel examples are shown in Table 2. 1 2 4 $2,727 starting-price illustration: $99 consultation + 12 × $99 membership + 12 × $120 medication equivalent. Actual payment dates can change year-one cash. 9 10 No universal total. Obtain visit, lab and pharmacy estimates for your plan and prescription.
Medication billing Published 4- or 5-month kits; applicability needs confirmation. 1 A 60-day supply, described as billed every other month; starting injection order works out to $240. Membership is separate. 10 Depends on the prescription, pharmacy, payer and quantity dispensed.
Ongoing care fee $170 annually on Why Nuform; confirm how that applies to your plan. 2 19 $99 per month. 9 Office charges or your plan's cost-sharing; ask about follow-up visits.
Labs Extra: published panels at $95 or $195, with Quest blood work. Repeat requirements and prices need a quote. 4 Provider says comprehensive blood testing every 6 months is included in membership; its initial process uses an at-home finger-prick kit. Confirm the tests needed before prescribing and any extra charges. 9 Ask which tests are ordered, which lab is in network and what you will owe.
Treatment formats listed Injectable subQ or IM options. 1 Injectable or cream. Cream starts at $132/month before membership and other charges. 10 Ask which approved injections, gels or other specific products the clinician can prescribe. 23
Compounded or FDA-approved Compounded supply, per Nuform. Exact preparation and pharmacy need confirmation. 1 19 Describes compounded testosterone. Confirm the exact preparation and pharmacy. 9 An FDA-approved-product path is possible; the actual prescription matters. 23
Refund promise Service-specific refund policy; no symptom-based refund guarantee found there. 5 Advertised refund is three months of membership fees if its 90-day conditions are met, not a refund of all treatment spending. 9 Ask the practice and pharmacy for their own policies.
States Complete current state list not confirmed in the pages checked Provider says all states except Alabama, Alaska, Arkansas, Connecticut, Hawaii, Idaho, Louisiana, Minnesota, Mississippi, New Hampshire and Rhode Island. 9 Depends on the clinician's authorization and actual appointment availability. 18
Insurance and benefit funds No insurance accepted; HSA/FSA acceptance is provider-stated. 6 19 No insurance accepted. Says FSA, HSA or HRA reimbursement may be possible. 9 Ask your plan about the practice, tests, exact drug and any prior authorization.

These are different care models, not a ranking of clinical quality. Neither provider's published price settles your diagnosis, prescription, pharmacy, state eligibility or complete testing plan.

Check the payment dates: Male Excel's page describes both a 60-day supply and billing every other month. Six 60-day supplies cover 360 days, not a full 365-day year. If a seventh starting-price $240 order is charged inside your first year, the same consultation and twelve membership charges would total $2,967, before shipping and other costs. Get the actual order dates; do not treat either $2,727 or $2,967 as a guaranteed cash total. 9 10

Male Excel's listed injectable package pairs testosterone with a daily thyroid tablet. Its cost page says prices vary by dose, and shipping and handling is extra. Check its current checkout before you pay. Ask why any additional medicine is being proposed for you; a bundled thyroid tablet is not proof you need thyroid treatment. 10

A testing question to resolve before enrolling: Male Excel's homepage says a clinician may prescribe before lab results arrive when considered appropriate. The wording does not establish whether suitable prior results are already available for a given patient. For a new low-testosterone diagnosis, the Endocrine Society still calls for compatible symptoms or signs and consistently low results confirmed with repeat morning fasting testing. Ask how the clinician will meet that standard before testosterone is prescribed. 9 11

Who should skip Nuform, and where to go instead

  • You prefer a cream, or want to compare a shorter medication billing period → Male Excel is an option to research, not automatically a cheaper or better clinical fit. Its cream starts at $132/month and its injection at $120/month, both before the $99 monthly membership and other charges. It says blood tests every six months are included, but you must confirm the initial diagnostic workup and additional testing. Its testosterone is compounded too. 9 10

The refund limit matters: Three $99 membership fees equal $297. That is the advertised amount at issue after the guarantee's conditions—not a promise that treatment works or that medication, testing, consultation and shipping costs are all returned. It doesn't serve the 11 states listed above, so check yours. 9

  • You want insurance to pay, or you need an FDA-approved product → Start with your primary care doctor or a urologist and confirm the exact covered prescription. Our TRT cost guide explains which charges to check.

  • You're trying to conceive soon → Start with a reproductive urologist. 11 20

What should you ask Nuform before you pay?

Seven questions help turn Nuform's published prices into a usable written quote. Ask them during your first specialist call, before authorizing a payment. Confirm refund terms then, rather than relying on the blood draw as a guaranteed deadline.

Copy this message and send it, or read it from your phone during the call:

"Before I pay anything, can you send me a written, itemized quote? I'd like to know:

  1. Which TRT kit am I being quoted: the 4-month $200 kit, the 5-month $295 kit, or a different offer? What is due now, and what would I pay on each later order during the first year?
  2. What's the shipping charge per kit, and are there any other fees, taxes or optional charges?
  3. What is the exact testosterone preparation, strength and labeling, and which pharmacy fills it? Is the finished product FDA-approved or compounded?
  4. Which tests do I need to confirm the diagnosis before treatment, which panel do I need during treatment, how often will I repeat it, and what does each order cost?
  5. Will you accept recent blood work from my own doctor?
  6. Does the $170 fee apply to my plan, when would it be due again, and what do I authorize? Please state the refund cutoff for that fee and for labs and medication.
  7. Can the assigned clinician treat me in the state where I will be during visits, and can the pharmacy deliver there? Could an in-person visit be required, and who would arrange and pay for it?"

Add these if they apply to you:

  • If another medicine is proposed, which one is it, why is it needed, what is its approval or off-label status, and how is it billed?
  • If my prescription changes, how does that change my kit price and the number of paid orders?
  • How do I request refills, and how do I get copies of my lab results and records?
  • Who handles clinical questions between follow-ups, and which contacts or extra visits would cost more?

Asking for a quote doesn't mean you qualify or that you'll be prescribed anything. A clinician makes that call.

Once you have your answers, drop the real numbers into the formula in the first-year section. Then you'll have a budget based on your actual quote, with any unresolved or changeable charges kept separate. You do not have to commit to treatment to ask what it costs.

Is Nuform worth the money?

For a cash-pay adult man whose low testosterone has been properly confirmed, who's fine with injections and isn't planning a baby soon, Nuform's published prices warrant a closer look. The trade-offs are compounded medication, multi-month non-refundable kit purchases, and a few prices and terms you'll need to confirm in writing. A low sample subtotal is not proof of the best clinical care or the lowest complete quote. 1 2 5 11 13

Keep Nuform on your shortlist if:

  • Total yearly cost matters more to you than monthly billing.
  • You're comfortable with injections.
  • You prefer a blood draw at a lab rather than an at-home collection kit.
  • You like the idea of texting or calling the same care team. Nuform says it assigns a dedicated provider and specialist and checks in every four weeks. 1 2 19

Pick something else if:

  • You want insurance coverage or an FDA-approved product.
  • You're trying to conceive soon.
  • You want a cream or other non-injection format.
  • You can't prepay for the multi-month kit in your quote. 1 6 11 13

What we couldn't confirm:

  • The shipping fee and other patient-specific extra charges.
  • The exact testosterone preparation and pharmacy for your prescription.
  • A complete current list of states where Nuform can provide TRT.
  • How the published $170 annual fee applies to your specific treatment plan and payment dates.
  • Your required initial and repeat tests, their prices, and acceptance of outside results.

The questions in the section above cover all five.

What customers say: The Trustpilot profile checked displayed 4.9 out of 5 from 1,173 reviews. Those reviews cover several Nuform services, not just TRT. Ratings reflect service experience, not medical results or safety. 8

How did we check Nuform's TRT costs?

We read Nuform's treatment, fee, lab, refund, terms, FAQ and Our Approach pages on September 23, 2026. We added up the published components into clearly labeled budget examples and flagged anything not confirmed or described two different ways. We did not buy treatment, complete checkout or contact Nuform's support team.

This page follows our method, How We Review TRT Providers. That method keeps four things separate: facts we verified, facts a provider states, customer-experience reports, and our own editorial conclusions. It also calculates complete cost instead of repeating a teaser price. When a charge is unknown, we say so rather than calling it free. We don't give providers numeric scores.

What we actually verified

What else should you know about Nuform Health TRT cost?

The main questions are whether your quoted kit is current, which tests and care fees apply, and when each charge becomes non-refundable. These short answers separate published prices from the details only your written quote and clinical evaluation can settle.

Is Nuform TRT really $50 a month?

The medication works out to $50 a month on the listed four-month kit, but the kit price is $200. That does not include the separately listed care fee and lab charges. The TRT page text also contains a five-month $295 offer, so confirm your applicable offer rather than assuming $50 is your quoted rate. 1 2 4

Is the $170 physician fee charged every year?

Nuform's Why Nuform page calls it an annual fee that covers a year of care. Its homepage calls it a one-time fee, while Our Approach says no membership fees and no automatic renewal without you. Ask whether the $170 applies, when another payment would be due and what your authorization covers. 2 3 19

How much are Nuform's lab tests?

The male basic panel is $95 and the comprehensive panel is $195. Both include Quest blood work and Nuform's analysis fee; Nuform says you aren't charged at the lab. Which panels and repeat tests you need, and their actual repeat-order prices, still need confirmation. 4

Does Nuform require blood work for TRT?

Yes. Nuform says bloodwork is required for hormone treatments, including TRT. It recommends repeating labs 3–4 months after starting. That does not establish its price or process for the separate repeat morning test used to confirm a new low-testosterone diagnosis. 1 2 11

Does Nuform take insurance?

No. Nuform's terms say it "currently does not accept insurance." Its Our Approach page says HSA/FSA funds are accepted; check the specific expense and documentation with your benefits administrator. 6 19

What does Nuform charge for shipping?

We did not find a fixed shipping price on the pages checked. Its terms say you pay any shipping and handling shown at purchase. One verified Trustpilot review reports paying extra for faster shipping, but that report does not establish a standard charge. 6 8

Can I get a refund from Nuform?

The physician-fee policy has clauses about consultation start, completed hormone bloodwork and accepted treatment, plus a full fee refund if the provider says you're not a candidate. Confirm the applicable cutoff before paying. Completed labs and purchased medication are subject to separate nonrefund rules. 5

Can I cancel Nuform?

Nuform says there is no subscription to cancel: you don't reorder the next kit, and Our Approach says nothing renews automatically without you. That does not reverse a medication purchase or guarantee a refund of the physician fee. Confirm future payment authorization and arrange any needed care transfer with your clinician. 2 5 19

Is Nuform's testosterone FDA-approved?

Nuform says its testosterone comes from compounding pharmacies. The FDA states compounded drugs are not FDA-approved. Ask Nuform which testosterone and pharmacy you'll get. 1 13 14

Does subQ cost more than IM at Nuform?

Nuform says its price does not vary between the subcutaneous and intramuscular options it lists. Your actual preparation, prescription and kit quote still control the cost. Your clinician decides the appropriate treatment instructions. 1

How much is Nuform TRT with hCG?

Nuform lists testosterone plus hCG or gonadorelin at $120 a month. The page does not settle which additional drug, preparation or payment period applies. No add-on guarantees fertility protection, and trying to conceive calls for a specialist discussion rather than choosing a kit by price. 1 11 20 21

Is there a Nuform discount code?

We did not verify a current TRT-specific discount code or a discounted checkout. Ask whether a promotion applies to your exact kit, what it excludes, and whether it affects only the first order. Do not subtract an unconfirmed discount from your budget. 1 7

Do I need an in-person visit for Nuform TRT?

It depends on the clinical situation, the prescriber and applicable law. The federal extension allows qualifying telemedicine prescribing without a prior in-person evaluation through December 31, 2026, under its conditions, including the required encounter and prescribing authority. It does not guarantee that Nuform can provide your care remotely or that no in-person visit will be needed. 16 17 18

How is Nuform's enclomiphene different from TRT?

Enclomiphene is a pill that works differently from testosterone. It isn't TRT, and it's priced separately. Compounded enclomiphene is not FDA-approved; do not confuse it with an approved clomiphene product used off-label. See our enclomiphene cost guide. 13 20 22
