NuForm Health TRT reviews show strong service ratings, but we could not verify one all-in annual price. The published $50 monthly kit rate excludes other charges. This is a cash-pay, compounded-medication option to discuss with a clinician—not a shortcut past testing. Near-term fertility plans or an FDA-approved-product requirement point elsewhere. 9 1 7 12

What should you know about Nuform at a glance?
Nuform publishes low kit prices, but its pages do not settle the full bill or the exact medication you would receive. The table separates published claims from the things you still need in writing before paying. 1 2 4
| What you want to know | What we found |
|---|---|
| Injectable TRT price | Published text lists $200 for a 4-month kit ($50/month), supplies included. 1 |
| Other injectable kit listed | The same page text also lists $295 for a 5-month kit ($59/month). We did not confirm that both offers are currently available to order. 1 |
| Doctor fee | $170, called annual on the Why Nuform page and one time on the homepage. 2 3 |
| Labs | $95 Essential Male Panel or $195 Comprehensive Male Panel, through Quest according to Nuform. 5 |
| First-year cost | $960–$1,160 is a two-panel cost model, not a verified all-in quote. It assumes three $200 kits, one $170 fee, and two panels. More testing and other charges can change it. |
| What the medication is | Nuform describes its offer as compounded. We did not identify an FDA-approved testosterone product in the published offer. 1 4 |
| Pharmacy | Nuform says it uses licensed U.S. pharmacies. No dispensing pharmacy was named in the pages we reviewed. 1 4 |
| States served | No state list found in the reviewed pages. Its terms say availability depends on location. 7 |
| Insurance | Not accepted, according to Nuform's terms. 7 |
| Refunds | Medication orders are final at purchase. The physician-fee policy names several refund milestones; clarify which applies before paying. 6 |
| Trustpilot | 4.9 from 1,173 reviews when checked September 23, 2026; this is a company-wide rating, not a TRT-only score. 9 |
Nuform is worth comparing if:
- You pay cash and want a low published kit price, with the full bill confirmed first.
- Your low testosterone has been properly confirmed, or you're willing to get it confirmed.
- You're fine with injections.
- You like texting one dedicated person instead of a help desk.
- Your clinician can explain why a compounded medicine is needed rather than an FDA-approved product. A low price alone does not answer that question. 15
Nuform is not your best starting point if:
- You're trying to have a baby in the near future. Start with a reproductive urologist. 12
- You want an FDA-approved product or want insurance to pay. Start with a clinician who can prescribe the specific approved product and work with your coverage. 15 7
- You'd rather use a cream than injections. Nuform's published men's TRT kits are injectable; another treatment format needs confirmation before payment. 1
- You won't prepay for the multi-month kit offered to you. 1 6
That's the short answer. The longer one matters, because Nuform's own pages describe its doctor fee as both annual and one time. One published kit costs more per month than the other. And one question can make Nuform a costly wrong turn. It isn't about price. It's about whether you want kids soon. We'll get there. 1 2 3 12
First, we don't have an affiliate deal with Nuform. We earn nothing if you choose it. We read Nuform's published care pages and policies, did the math, and checked the medical claims against clinical guidance, so you don't have to open 12 tabs.
TRT Provider Guide is the independent decision resource for testosterone replacement therapy — helping U.S. adults understand how low testosterone is evaluated, compare online and local care models and providers, and choose the next step that fits their health needs, fertility plans, budget, state, and care preferences, with every material claim verified and dated.
The right TRT provider is not the same for every person. It depends on whether low testosterone has been properly evaluated, your symptoms and health history, your fertility plans, your state, your insurance or cash-pay preference, your treatment-format preference, your budget, and whether online or in-person care is the better starting point. Some situations belong with primary care, urology, endocrinology, reproductive urology, or urgent or emergency care when the symptoms warrant it. Because a general answer can't resolve those for you, use TRT Provider Guide's Find My TRT Path tool to map your situation to the right care route and the questions to ask before you pay.
The tool is educational and non-diagnostic. It does not interpret your labs, decide medical eligibility, or guarantee a prescription.

What do NuForm Health TRT reviews actually say?
NuForm Health has a 4.9 rating from 1,173 Trustpilot reviews as of September 23, 2026, with 97% five-star reviews. Recent accounts praise fast replies, named care reps, and low prices. The rating covers all of Nuform's services, not just TRT, and is not a measure of medical results. 9
Those numbers are strong. But a star rating tells you less than it seems. So we read the first 20 review cards displayed under “All reviews” with “Most recent” selected and sorted them.
We sorted 20 recent reviews. Here's what we found.
| Invitation status in the 20-card sample | Count |
|---|---|
| Labeled Invited by Trustpilot | 14 |
| Labeled Unprompted | 6 |
| Total | 20 |
| Treatment mentioned in the same 20 cards | Count |
|---|---|
| Clearly about TRT | 5 |
| About weight loss | 1 |
| About other or general hormone treatment | 2 |
| No treatment named | 10 |
| Off-topic or unclassifiable | 2 |
| Total | 20 |
How we counted: We required an explicit TRT reference rather than guessing from a reviewer's name, symptoms, or a general hormone reference. Invitation status and treatment topic were counted separately. This is a small, dated sample—not a representative patient survey. “No treatment named” does not mean the person did not receive TRT. 9
What this tells you:
- Most reviews in this sample were invited. Invitations are allowed on Trustpilot. Trustpilot says Nuform invites customers whether their experience was good or bad. It just means you're mostly hearing from people Nuform contacted. 9
- Only 5 of 20 clearly mention TRT. Nuform also sells weight-loss drugs, peptides, and women's hormone care. The 4.9 is a whole-company score. 9 3
- People rate the people. Reviewers name their reps, including Romario, Bryson, and Nick. Those accounts help you assess communication, not the quality of a diagnosis or prescription. 9
Other facts from the profile: Nuform claimed its Trustpilot profile in October 2023 and pays for a Trustpilot subscription. It has replied to 57% of its negative reviews, usually within a week. Only 1% of reviews are one-star. 9
What happy TRT patients report
Three recent TRT reviews show the pattern:
- Speed. A reviewer named Bruce (September 2026) said he has "typically received a helpful response within an hour" when he asks about his TRT.
- Onboarding. Alan Scruton, posting September 2 about an August 28, 2026 experience, described a prepared, unrushed doctor visit. He got a text about his prescription less than 30 minutes later. He paid extra for faster shipping and had his TRT in under two days.
- Price. Eugene Crowley, posting September 7 about an August 6, 2026 experience, said he compared several clinics and found Nuform the best value, with no upselling. 9
Trustpilot labeled all three reviews Invited. These are individual experiences. They don't prove how fast your replies will be, and they say nothing about medical safety or results.
What about complaints?
The profile includes negative reviews, but its star distribution does not establish what happened in any billing dispute. We did not verify a specific unexpected-fee complaint well enough to present it as evidence here. The price and refund conflicts below come from Nuform's own pages, so you do not have to rely on an anonymous allegation to ask about them. 9 2 3 6
Bottom line on reviews: The recent accounts repeatedly praise Nuform's service. Just don't treat stars as proof that the medicine, the testing, or the price fits you. That's what the rest of this page is for.
How much does Nuform TRT really cost in the first year?
Nuform's published $50 monthly rate is a kit-price equivalent, not a complete care price. The $960–$1,160 model below includes medication, one physician fee, and two lab panels; it does not establish how many tests your care requires or what all additional charges will be. Ask for an itemized quote before treating that model as your budget. 1 2 5
Nuform describes medication sold in prepaid kits, not monthly subscriptions. Here are the published prices we found: 1 4
| Item | Published price | Where Nuform says it |
|---|---|---|
| 4-month injectable kit, supplies included | $200 ($50/month equivalent) | TRT page 1 |
| 5-month injectable kit | $295 ($59/month equivalent) | TRT page 1 |
| Doctor fee | $170 | Why Nuform and homepage; renewal descriptions conflict 2 3 |
| Essential Male Panel, called basic here | $95 ($65 Quest + $30 analysis) | Diagnostic Labs page 5 |
| Comprehensive Male Panel | $195 ($145 Quest + $50 analysis) | Diagnostic Labs page 5 |
| Shipping | No specific amount found | Terms say applicable shipping charges appear at checkout 7 |
These are published-text observations, not two checkout-tested kit offers. A webpage can contain more than one version of an offer. We did not establish which kit a new patient would actually be offered, whether both remain available, or whether the same price applies to a different prescription.
Your year-one cost, built from Nuform's own numbers
The calculations below assume three $200 kits for 12 months, one $170 physician fee, and unchanged prices. Nuform recommends repeat lab work 3 to 4 months into therapy. Follow-up lab prices aren't published separately, so the examples use the entry-panel price for each draw—not a confirmed follow-up quote. 1 2 5
A test that confirms the diagnosis before treatment is different from a monitoring test after treatment starts. The two-panel examples do not budget for two diagnostic draws and a later monitoring draw. The three-panel examples show that difference; they are not a claim that three panels meet every patient's needs. 13
| Scenario | The math | Modeled first-year subtotal | Monthly budgeting equivalent |
|---|---|---|---|
| One basic panel only: incomplete care budget, not a recommended minimum | $95 + $170 + 3 × $200 | $865 | $72.08 |
| Basic panels twice, 4-month kits | $95 + $170 + $600 + $95 | $960 | $80.00 |
| Comprehensive panels twice, 4-month kits | $195 + $170 + $600 + $195 | $1,160 | $96.67 |
| Three basic panels, 4-month kits | 3 × $95 + $170 + $600 | $1,055 | $87.92 |
| Three comprehensive panels, 4-month kits | 3 × $195 + $170 + $600 | $1,355 | $112.92 |
| Three 5-month kits bought in year one, two basic panels | $95 + $170 + 3 × $295 + $95 | $1,245 | $103.75; purchases cover 15 medication-months, not 12 |
Not included: shipping, expedited delivery, applicable taxes, extra diagnostic or monitoring tests, additional services not covered by the quoted physician fee, and medication beyond the listed testosterone kit. Actual test frequency and prescription quantity are clinical decisions—not choices to make by selecting the cheapest row.
Monthly average is not monthly billing. In the four-month model, the medication payments are $200 at the start, around month four, and around month eight. In the five-month model, three $295 payments at the start and around months five and ten buy 15 labeled months. Ask for actual payment and refill dates before comparing those totals.
The testosterone-plus-add-on kit is a separate offer
Nuform also lists a $120 one-month kit combining testosterone with hCG or gonadorelin. Those additions are not testosterone, are not interchangeable, and do not guarantee fertility preservation. 1 14
| Separate combination-kit illustration | The math | Modeled subtotal |
|---|---|---|
| Twelve $120 kits, one $170 fee, two comprehensive panels | 12 × $120 + $170 + 2 × $195 | $2,000, or $166.67 per month averaged over 12 months |
This is arithmetic, not a recommendation for the combination. The exact added drug, its source, required monitoring, and all other charges still need confirmation. The hCG sourcing question appears below.
Year two: On the same four-month-kit assumptions, $695 means $600 in medication plus one $95 panel and no renewed physician fee. If the $170 fee renews, that becomes $865, or $965 with one $195 panel. More monitoring raises the total. None of those examples confirms that the fee disappears or that one annual panel is enough. 1 2 3 5
Three price quirks to ask about
1. One published kit costs more per month. Nuform's 5-month kit works out to $59 a month versus $50 for the 4-month kit. Ask which kit you'll be offered and why. The two page blocks do not prove both are current order options. 1
2. The doctor fee is described two ways. The Why Nuform page calls it one $170 annual fee. The homepage says one time. The refund policy calls it an annual physician fee. Ask exactly what you'll pay today and what renews next year. 2 3 6
3. Kit lengths don't match across pages. The TRT page lists 4- and 5-month kits. Why Nuform says medication is billed in 3- or 4-month kits. None of this is a red flag by itself. It just means the only price that counts is the one in writing. 1 2
The Why Nuform page describes follow-up care as part of the physician fee, while the Our Approach page says displayed prices exclude laboratory and consultation fees. Have the quote state which visits and tests the fee covers. 2 4
How Nuform compares on price
These are provider-stated price structures, not three verified all-in annual bills. Hone now describes a Plus plan in its September 2026 help center, while its TRT marketing page still describes a different Premium offer. 26 25
| Provider and price basis | What the math establishes | What it does not establish |
|---|---|---|
| Nuform: four-month kits | $960–$1,160 using one $170 fee, three $200 kits, and two $95 or $195 panels | Complete testing needs, available kit, shipping, and all other charges |
| Hone Plus: $135/month membership | $1,710 before injection medication: $45 initial testing + $45 at-lab confirmation + 12 × $135 | Annual medication cost; the help center quotes testosterone cypionate from $28 per vial, not a fixed monthly supply 26 |
| Male Excel: current main cost page | $2,727 using $99 consultation + 12 × $99 membership + 12 × $120 injection price equivalent | Shipping, any separate labs, prescription-dependent changes, and exact payments under its 60-day medication billing 29 |
Hone's $80 at-home confirmation option makes that Plus subtotal $1,745 before injection medication. Its separate $65 test + $155/month Premium + $28/month injection marketing example still calculates to $2,261, but it should not be presented as the only current Hone plan or a guaranteed annual bill. 26 25
Nuform's published kit math is lower. The missing charges prevent a verified “save more than $1,000” claim. We don't earn a cent from saying so.
If the published kit price fits your budget: see Nuform's current TRT kit prices, then get the testing, medication, fees, and payment dates in writing before you pay. (Not a partner link.)
Is NuForm Health legit?
Nuform has an operating public website, named medical professionals, and terms identifying the medical practice responsible for treatment. Its co-founding physician is listed by two Michigan health systems as a board-certified neurosurgeon. Those facts support an identifiable business presence—not a blanket finding that every prescriber, pharmacy, or treatment is right for you. 7 10 11
Who runs Nuform
Nuform describes itself as physician-owned and lists this team: 1 9
- Dr. Christopher Elia, DO, co-founder and physician. Corewell Health and Trinity Health Michigan list him as board certified in neurological surgery by the American Osteopathic Board of Surgery. His listed practice includes spine surgery. 10 11
- Romario Yousif, PA-C, co-founder and physician assistant.
- Dr. Shawn Achtman, DO, physical medicine and rehabilitation and sports medicine.
- Megan Brias, RN, and Nicole Choulagh, RN, operations team. 1
The hospital listings independently support Dr. Elia's stated specialty and board certification. The other roles and credentials above are Nuform's published descriptions; we did not independently verify each person's current state license. Dr. Elia's listed specialty is not endocrinology or urology. That's worth knowing if your case is complicated.
How the business is set up
Nuform's terms describe two parts:
- Nuform Health and Wellness, LLC runs the website and platform.
- Christopher Elia, D.O. P.C., doing business as Nuform Health and Wellness, P.C., is the medical practice whose providers treat you. 7
Two differences are worth noticing:
- Addresses. Nuform's current page footers list Troy, Michigan. Its terms and Trustpilot profile list Southfield, Michigan. We did not establish which address should be used for a particular request. 3 7 9
- Dated terms. The terms of service were last revised June 9, 2023. That is the date on the document still linked by the site, not proof that every provision has expired. 7
Who actually answers your medical questions?
Nuform says it assigns a dedicated provider and specialist, with direct phone and text access. Its homepage describes check-ins every four weeks, and the Why Nuform page advertises ongoing access. We did not test response times or confirm who handles urgent messages. 3 2 4
Before you pay, ask which person is your prescriber, who reads your lab results, and who can change your treatment. A friendly coordinator is great. But you want to know who's medically responsible.
Ask for that clinician's full name, credential, and authority to treat patients in the state where you will be during visits. A founder's hospital listing does not verify the clinician assigned to you. 24
Is Nuform's testosterone FDA-approved?
Nuform describes its published TRT offer as compounded; we did not identify a named FDA-approved testosterone product in that offer. Compounded drugs are not FDA-approved, and the FDA does not check them for safety, effectiveness, and quality before sale in the way it reviews approved drugs. A clinic's pharmacy claim does not change that distinction. 1 4 15
Here's the one trade-off that matters most on this page.
Nuform's published TRT offer is compounded, not an FDA-approved product. If an approved product is your priority, a clinician prescribing that specific product is the better care route to investigate. This can be local or online; insurance coverage depends on your plan and prescription. A lower kit price does not establish that a compounded drug is medically appropriate. 15
The FDA says compounded drugs should be used when a patient's medical needs cannot be met by an FDA-approved drug. Ask your prescriber why compounding is needed in your case, not just whether it costs less. 15
If FDA-approved medication is a must, see our guide to online TRT vs. a local doctor or use Find My TRT Path to map that care route.
Two words, explained simply
- FDA-approved means the FDA reviewed a specific product's safety, effectiveness, and manufacturing before it could be sold.
- Compounded means a medicine is prepared by combining or changing ingredients to meet a patient need. It can be legal and useful. It still hasn't gone through FDA approval. 15
Compounded testosterone is not a "generic" of an approved product. FDA approval of another testosterone product does not establish a compounded preparation's quality or equivalence. 15
Nuform's pharmacy claims don't agree with each other
| Where | What Nuform says |
|---|---|
| TRT page | Describes a U.S. compounding pharmacy as 503A and B certified |
| Same TRT page | Says medications come from U.S.-based 503A pharmacies |
| Peptide FAQ text within the retrieved TRT page | Uses the phrase “FDA approved compounding pharmacies” |
| Trustpilot company description | Says medications are compounded in licensed U.S. pharmacies |
| Reviewed provider pages | No dispensing pharmacy named 1 9 |
A 503A pharmacy generally compounds for individual prescriptions under state pharmacy oversight. A 503B outsourcing facility registers with the FDA and must meet federal current good manufacturing practice requirements. Those are different regulatory categories, not two FDA approval certificates. And the FDA doesn't "approve" compounding pharmacies. 15
Ask Nuform to name the pharmacy that will fill your prescription and say which type it is. Its location, license, inspection history, and permission to dispense into your state cannot be checked meaningfully without its identity. We did not independently verify an unnamed pharmacy. 15
The hCG question
Nuform lists a testosterone kit with hCG or gonadorelin. These are different medicines, not two names for the same addition. Neither turns testosterone treatment into a fertility guarantee. 1 14
Since March 23, 2020, FDA has regulated hCG as a biological product. It is not eligible for the ordinary compounding exemptions under sections 503A and 503B. FDA has separate guidance on certain mixing, dilution, and repackaging of licensed biological products; that is not permission to market an unlicensed compounded biologic. 16 15
Ask for the exact hCG product, manufacturer, and FDA-licensed status. We did not identify the hCG Nuform would supply, so this is a sourcing question—not a finding that Nuform has supplied an unlawful product.
Nuform's kit is priced for one set amount
Nuform's kit prices are listed for testosterone "dosed at 200mg weekly." Its page also says dosing is customized, with micro-dosing and different injection options at the same price. That advertised amount is not a dose recommendation for you. Ask what you'll actually be prescribed and whether the kit price changes if your dose does. 1
The quote should identify the exact testosterone preparation, concentration, volume, any added ingredients, and number of vials. Ask the pharmacy about storage and the discard date after a vial is first used. A kit's advertised duration alone does not establish how long each opened vial may be used.
“Is TRT safe?” deserves a better answer than “yes”
Nuform's FAQ answers that question with a flat yes. The real picture has more detail. 1
In February 2025, the FDA requested class-wide labeling changes that removed boxed-warning language about major cardiovascular events after the TRAVERSE trial and added blood-pressure warnings. The trial does not establish that Nuform's compounded product has the same safety profile. 17 18
There is a newer update, too. On June 18, 2026, HHS announced FDA requests to remove the age-related hypogonadism limitation and revise prostate-cancer and enlarged-prostate labeling. Those were requested changes, not confirmation that every product label had already changed. Screening and monitoring remain part of the requested revisions. 19
The Endocrine Society's July 2026 statement still calls for symptoms plus consistently low testosterone, appropriate repeat testing, and clinical evaluation. TRT can be appropriate for men with properly diagnosed hypogonadism. It still carries real risks that need monitoring. 13
What labs does Nuform require, and do they match medical guidelines?
Nuform describes a Quest blood draw before hormone treatment and recommends repeat labs 3 to 4 months after starting. The Endocrine Society calls for symptoms or signs plus consistently low testosterone, confirmed with at least two early-morning fasting measurements on separate mornings. Nuform's public process does not establish whether a separate confirmation test is required or included. 1 5 13
Nuform's two lab panels
| Published panel component | Essential/basic male panel | Comprehensive male panel |
|---|---|---|
| Price | $95 | $195 |
| Testosterone, total and free | Included | Included |
| LH and FSH | Included | Included |
| Estradiol | Included | Included |
| Hemoglobin and hematocrit | Included | Included |
| White blood cell count | Not listed | Listed |
| PSA blood test | Not listed | Listed |
| Thyroid, cholesterol, A1c, liver and kidney markers | Not listed | Listed 5 |
The basic panel lists red-cell measures and platelets, but not a white blood cell count. It should not be called a complete blood count just because hematocrit is included. 5
A few terms in plain English:
- Total testosterone is all the testosterone in your blood. Free testosterone is the small part not attached to proteins.
- LH and FSH are signals from the brain that tell the testicles to work. They help show why testosterone is low.
- Hematocrit is the share of your blood made of red blood cells. Testosterone can raise it, so it gets watched.
- PSA is a blood marker used in prostate assessment. It is not a prostate examination or a stand-alone diagnosis. 12
How Nuform's process lines up with the guidelines
| Clinical question | Nuform's published process | What still needs confirmation |
|---|---|---|
| Are low results confirmed on two early-morning fasting tests on separate mornings? | One onboarding lab process is described | Whether another diagnostic draw is required, its timing, and its cost |
| Are symptoms or signs assessed along with results? | A specialist call and clinician consultation are described | What the prescribing clinician evaluates; a service description is not an audit of diagnosis |
| Is the cause of low testosterone assessed? | Both panels list LH and FSH | The clinician's interpretation and any further workup |
| Are blood counts, prostate concerns, sleep apnea, and cardiovascular history assessed? | Both panels list hematocrit; only the comprehensive panel lists PSA | Screening appropriate to age, history, risk, and the actual prescription |
| Is there monitoring after treatment starts? | Repeat labs at 3–4 months and four-week check-ins are described | Required markers, ongoing schedule, result review, charges, and responsibility for follow-up 1 3 5 12 13 |
Nuform's lab page says, “Being told your lab values are normal...to us, means nothing.” That is marketing language, not a reason to disregard diagnostic standards. Ask how the prescriber distinguishes a medical diagnosis from a goal of reaching an “optimal” number. 5 13
A panel's ingredients do not prove that a clinic follows every guideline. Nor does a result below 300 ng/dL, by itself, diagnose testosterone deficiency. Symptoms, repeat results, testing conditions, the assay, and possible causes matter. 13
Our take: ask the prescriber which prostate assessment and other tests fit your age and history. A more expensive panel is not automatically the right panel, and neither price includes a documented second diagnostic draw. And ask Nuform plainly: "Do you require a second morning test before I start?"
See a doctor in person first if you have very low testosterone with headaches or vision changes, a history of prostate or breast cancer, a high red blood cell count, untreated severe sleep apnea, or a heart attack or stroke in the past six months. Those belong with primary care, urology, or endocrinology before any online program. Sudden severe symptoms need urgent or emergency assessment. 12 35
The Endocrine Society guideline calls for further urologic evaluation before treatment when PSA exceeds specified risk-based thresholds; a single cutoff is not a self-clearance rule. The 2026 FDA labeling request also does not remove the need for specialist review of a cancer history. 12 19
Can you use Nuform if you want kids?
If you want a child in the near future, TRT is usually the wrong first step. Testosterone therapy can lower sperm production, and the Endocrine Society recommends against starting it in men planning fertility soon. Nuform's page acknowledges this concern and points toward enclomiphene, but your best first stop is a reproductive urologist—not a medication choice made from a provider page. 12 1
Credit where it's due: Nuform's own TRT page says testosterone therapy suppresses sperm production and steers men who care about fertility toward enclomiphene. 1
Two things to know:
- The testosterone + hCG kit is not a fertility guarantee. The AUA/ASRM male-infertility guideline permits selected non-testosterone medicines, including hCG, for infertile men with low testosterone, based on limited evidence. It does not establish that adding hCG to any TRT kit preserves fertility. No add-on promises your sperm count stays normal. Plus there's the hCG sourcing question above. 14
- Enclomiphene isn't TRT. It works differently and is not an FDA-approved drug. It should not be described as an approved testosterone product used off label. Its evidence, suitability, and costs are separate questions. See our enclomiphene cost guide and our TRT and fertility guide. 20
The effect of testosterone on sperm production varies. This page cannot predict infertility, preserved fertility, or recovery on a set timeline for any reader. 14
If a baby is in your plans: use Find My TRT Path to organize fertility questions and care options, then discuss those questions with a reproductive urologist. The tool does not create a fertility treatment plan.
Does Nuform serve your state?
We did not find a Nuform state list, and its terms say services may not be available in your state. Federal rules currently allow qualifying telemedicine prescribing without a prior in-person examination through December 31, 2026, subject to required conditions. That extension does not prove that Nuform can treat you where you are. 7 21 23
We could not verify a “40+ states” coverage claim from Nuform's own pages, so we won't use it to tell you where care is available. Ask Nuform before you pay anything.
Two details most people miss:
- Where you are during the visit matters. Ask whether your clinician can treat you in the state where you'll physically be during appointments, not just where you live. If you travel a lot or might move, ask how that affects refills. 24
- The December 31, 2026 date. Testosterone is a Schedule III controlled substance and requires a valid prescription. The temporary federal flexibility has conditions, including a legitimate medical purpose, prescribing within usual professional practice, and compliance with other applicable federal and state rules. For testosterone under this flexibility, an online questionnaire alone is not the required real-time audio-video medical encounter. 22 21 23
Without an applicable telemedicine exception, federal law generally requires an in-person evaluation before remote controlled-substance prescribing. The temporary extension is not a permanent nationwide prescribing permission; other federal exceptions and state requirements still matter. 21
A new four- or five-month kit purchased in late September 2026 would extend into 2027. Ask Nuform how it plans follow-up care and future prescriptions if requirements change. The extension's end date is not, by itself, proof that your care will stop or that every patient will need an in-person visit on January 1. 21
Can you cancel Nuform or get a refund?
Nuform says its plans do not automatically renew, so not reordering avoids another kit purchase. That does not undo a charge already made: its medication policy makes orders final at purchase. The physician-fee policy names more than one refund milestone, so get the rule for your case in writing. 4 6
| What you paid for | What the published refund policy says |
|---|---|
| Doctor fee, listed as $170 elsewhere | Refundable until the consultation begins; for hormone patients, refundable until bloodwork is completed; non-refundable once treatment is recommended and accepted. Full physician-fee refund if the provider determines you are not a candidate. |
| Lab work | Refundable before completion; non-refundable after bloodwork is completed. |
| Medication kits | All medication orders are final at purchase. The policy also says it cannot refund an order placed with the pharmacy or accept medication returns after dispensing. 6 |
One wrinkle: “until your consultation begins” and “until blood work is completed” are different milestones. The policy also uses treatment recommendation and acceptance as a milestone. Ask which rule applies to you, in writing, before you pay. A physician-fee refund does not mean completed labs are refunded. 6
The fine print worth 60 seconds
- Arbitration. The terms require individual binding arbitration for covered disputes and include a class-action waiver. They specify Detroit unless otherwise agreed, but also preserve qualifying small-claims actions and provide a document or phone option for some claims of $10,000 or less. 7
- A liability cap. The terms cap Nuform's liability for use of the site at $500 where other limits don't hold. That is the scope of the site's clause, not a conclusion about every possible medical claim. 7
- Access can end. Nuform can revoke your site access "with or without cause." 7
- Your records. Nuform's privacy notice says you may request an electronic or paper copy of medical records, usually within 30 days, with a reasonable cost-based fee. Ask how to get your lab results, visit notes, and prescription details sent to another doctor if you ever leave. We did not test fulfillment. 8
There is also a privacy wording conflict. Nuform's footer makes a broad no-third-party-sharing claim, while its privacy policy describes sharing with service providers and other parties in stated circumstances. Read the policy and ask how to send health information securely; don't treat the footer as a promise that no outside party ever receives data. 8 3
It's worth knowing because you're prepaying for months of a controlled medication.
Does this still sound like your situation? Open Nuform's intake form. Ask for your state availability, written quote, and refund terms before paying. Nothing guarantees a prescription. (Not a partner link.) 7
Nuform vs. Hone vs. Male Excel: which fits you?
Nuform's published kit math is lower, but the available information does not establish a complete-cost winner. Hone and Male Excel publish testosterone cream options, while Nuform's reviewed men's TRT kits are injectable. Their billing and cancellation terms differ, so compare the written offer that matches your treatment format—not just a monthly headline. 1 26 29
| Decision point | Nuform | Hone Plus | Male Excel, main cost page |
|---|---|---|---|
| Published injectable price | $200 per 4-month kit or $295 per 5-month kit; current offer needs confirmation | Testosterone cypionate from $28 per vial, plus membership | From $120/month equivalent, with 60-day medication billing |
| Published cream price | No men's TRT cream offer identified in reviewed pages | Compounded cream from $60/month, plus membership | Compounded cream from $132/month equivalent, with 60-day medication billing |
| Required fee in this model | $170 physician fee; annual vs. one-time conflict | $135/month Plus membership | $99/month membership + $99 consultation |
| Starting labs | $95 or $195 panel; second diagnostic draw and fee not established | $45 initial test + $45 at-lab or $80 at-home confirmation for TRT | Separate required lab costs not settled by the main cost page |
| Follow-up costs | Get included visits and required labs in writing | Membership says follow-up labs and consultations are included | Membership describes ongoing clinician access; confirm the full lab schedule and charges |
| Commitment and exit | Prepaid medication is final at purchase; no automatic renewal stated | Cancel through support, effective at the end of the current billing period | No contract/cancel-anytime wording does not erase prepaid medication charges |
| Injection cost illustration | $960–$1,160 with two panels, excluding unresolved charges | $1,710 or $1,745 before injection medication | $2,727 in 12 monthly price equivalents, plus unresolved charges |
| Cream cost illustration | No verified quote | $2,430 with at-lab confirmation; $2,465 with at-home confirmation | $2,871 in 12 monthly price equivalents, plus unresolved charges 1 5 6 26 27 29 30 |
These figures use each company's published prices checked September 23, 2026. They are not complete clinical or state-availability comparisons. A medication's name alone does not identify the manufacturer or establish whether the exact dispensed product is FDA-approved. The cream offers above are compounded, not FDA-approved products. 28 31 15
Why Hone shows two price structures: Its September 10 help-center page describes $135 Plus, separate initial and confirmation testing, and injection pricing per vial. Its TRT marketing page still describes $155 Premium with a $65 test. The latter's cream arithmetic is $2,645 ($65 + 12 × $155 + 12 × $60), but the Plus illustration is $2,430 or $2,465. Confirm which membership the quote uses rather than mixing the two offers. 26 25
What needs checking at Male Excel: Its main pricing page lists a $99 membership; another live pricing page lists $30. Do not mix the lower fee with the main page's other prices. Male Excel's listed prices also bundle a daily thyroid tablet with testosterone. Ask your provider whether you need it. A bundle does not establish that you need both medicines. 29 30
The Male Excel figures above use 12 advertised monthly equivalents, not a verified calendar of charges. Its page also describes medication in 60-day supplies. Ask when each full order is billed and whether another order falls before your first anniversary. “Cancel anytime” is not the same as a refund on medication already ordered. 29
What about Taurus Meds? It does publish a $49 testing-and-consultation offer followed by $149/month for its testosterone program. Those numbers total $1,837 over 12 monthly payments. Its general terms also describe a $17.99 subscription charge per billing period unless the purchase says otherwise; if added monthly, the same example becomes $2,052.88. Until the order states whether that fee applies and identifies all included care, neither figure is a verified all-in quote. 32 33
Pick the care route that fits your reason
You want a low published kit price and don't mind shots → investigate Nuform's written quote. First resolve the exact medication, why compounding is needed, required testing, state access, and non-refundable charges. Low kit math is useful, but it is not the same as a complete-cost or clinical recommendation.
You'd rather use a cream than inject → compare Hone or Male Excel's published cream programs. Nuform's reviewed men's TRT offers are injectable. Both alternatives list compounded creams, so neither is an automatic answer to a request for an FDA-approved gel. 1 28 31
- Compare Male Excel's testosterone cream program and its current cost page.
- See Hone's testosterone cream information and the Plus membership prices.
You don't want a multi-month membership commitment → check Hone's monthly terms. Its help center says cancellation takes effect at the end of the current billing period. Ask separately about medication quantities, payment timing, and refunds; a cancelable membership does not answer all three. 27
You want FDA-approved medication or insurance → start with an appropriate prescribing clinician and your coverage rules. A local or in-network practice is a useful starting point; online care is not automatically excluded. See online TRT vs. a local doctor.
You're trying to conceive → a reproductive urologist. Not an online TRT purchase. 12
You haven't had low testosterone confirmed → start with evaluation and testing. Primary care or a urologist can assess symptoms, arrange appropriate repeat morning tests, and investigate the cause. 13

What should you ask Nuform before you pay?
These ten questions cover the main unresolved costs, clinical responsibilities, and exit terms. Ask them on your first call and get the answers in writing, by text or email, before you order a kit.
Copy these and bring them to your call:
- Which pharmacy will fill my prescription, and is it a 503A pharmacy or a 503B outsourcing facility? What is the exact medication, and why is compounding needed rather than an FDA-approved product?
- Do you require a second early-morning testosterone test before I start treatment? Which diagnostic and monitoring tests are included, and which cost extra?
- Which prostate assessment is appropriate for my age and history? Does my panel include any PSA test you require, and what would an added test cost?
- What exactly do I pay today? Is the $170 fee one time or yearly, which consultations does it cover, and is anything credited toward medication?
- What does shipping cost, and what do follow-up labs cost? What other charges could apply during the first year?
- Which kit will I get, and does its price or duration change with my prescription? What are the exact payment dates, refill-request deadline, and pharmacy processing times?
- If my plan includes hCG, what is the exact product and manufacturer, and is it an FDA-licensed product? If it includes gonadorelin instead, what is the reason and evidence for that choice?
- Can you treat me in my state, including where I'll physically be during visits, and what is your continuity plan beyond December 31, 2026?
- Who is my prescriber, who reviews my results, and who handles urgent treatment questions? How do I obtain my records or send them to another clinician?
- Which physician-fee refund milestone applies to me? When will each charge become non-refundable, and how do I prevent another medication order?
Write “confirmed in writing,” “needs follow-up,” or “not answered” next to each one. One unanswered question about state access, medication, required testing, or a non-refundable charge is enough to pause payment.
Copyable first-year quote worksheet
Use this with the questions above. Enter unknown, not zero, for a charge the clinic has not settled. Keep symptoms and laboratory values out of this budgeting worksheet.
| Budget line | What to record from the written quote |
|---|---|
| Initial clinical evaluation | Charge, what it includes, payment date, refund cutoff |
| Initial diagnostic labs | Exact panel or tests, amount, payment date |
| Separate confirmation testing | Whether required, exact test, amount |
| Physician renewal or membership | Amount, frequency, start date, included visits |
| Medication orders in the first 12 months | Exact product, quantity, price of each order, billing dates |
| Follow-up and monitoring | Clinician's schedule, included tests, separately billed tests or visits |
| Shipping and other charges | Standard or expedited shipping, applicable tax, supplies, other fees |
| First-year cash total | Add all payments due during your first 12 months. If a required amount is unknown, the total is incomplete. |
| Monthly budgeting equivalent | Divide the complete first-year cash total by 12. This does not change how the clinic bills you. |
Keep the quote date and expiration date with the worksheet. For a multi-month kit, record both the amount paid and the supply period so prepaid medication beyond year one does not disappear from the comparison.
How did we check Nuform?
We read Nuform's public care, pricing, lab, refund, privacy, terms, and intake information on September 23, 2026. We compared medical and regulatory claims with Endocrine Society, AUA/ASRM male-infertility, FDA, DEA, and HHS sources and classified 20 displayed Trustpilot reviews. We did not become a patient, buy a kit, or talk to Nuform staff.
What we actually verified
Date checked: September 23, 2026.
Sources checked: Nuform's TRT page, Why Nuform page, homepage, Our Approach page, intake destination, Diagnostic Labs page, refund policy, privacy policy, and terms and conditions. Nuform's Trustpilot profile. Corewell Health and Trinity Health Michigan provider directories. Endocrine Society clinical guidance and its July 2026 statement, AUA/ASRM male-infertility guidance, FDA compounding and labeling notices, the DEA extension, and HHS state-licensing guidance. Hone's official pricing and membership pages, Male Excel's two pricing pages, and Taurus Meds' offer and terms.
Published facts we confirmed: The kit and lab price text, $170 fee descriptions, refund and arbitration wording, named business entities, no-insurance statement, and public medication and care descriptions. These establish what Nuform publishes—not what a patient will be charged or how every encounter is performed.
Independently sourced facts: Dr. Elia's hospital-directory specialty and board-certification listings; the federal extension's dates and conditions; the compounded-versus-approved distinction; and the clinical guidance described on this page. We did not equate a hospital listing with a current license check for an assigned prescriber.
Provider-stated, not independently tested: Dedicated care teams, response times, four-week check-ins, Quest arrangements, shipping estimates, lack of automatic renewal, pharmacy licensing, and fulfillment of records requests.
Customer-experience signals: The displayed Trustpilot rating, invitation labels, treatment-topic sample, and the three attributed service accounts. We did not verify their medical outcomes or invoices.
What we could not confirm: Which states Nuform currently serves; which clinician would treat a specific reader; that clinician's current state authority; the exact pharmacy and dispensed product; shipping and follow-up lab prices; the actual kit available to a new patient; the physician-fee renewal terms for that offer; whether separate diagnostic confirmation is required and included; whether the proposed hCG is an FDA-licensed product; or the complete first-year bill. We did not complete checkout, test the entire intake flow, or verify the visible offer on each device layout.
Our editorial conclusions: The fit calls on this page are our judgment, based on the facts above. They are not medical advice and not a finding that you qualify for treatment.
How to check the 20-review sample
The display order was: Mis Silas; Cameron Messick; Rafael Moura; D Buchanan; Annegrete Kaarup; Tristan; Christian; M. Bunting; Eugene Crowley; Donna Shaba; Corey Kauffman; Bruce; Jonathan Ruthven; Kimberly Bailey; Julie; Sarah Mac; Alan Scruton; Nate; Tyler W; Alec Klott. The five explicit TRT cards were Rafael Moura, Eugene Crowley, Bruce, Alan Scruton, and Alec Klott. The six Unprompted cards were Mis Silas, Annegrete Kaarup, Donna Shaba, Corey Kauffman, Tyler W, and Alec Klott. 9
This records a dated display, not a claim that the same cards will stay first or that the sample represents all patients. We kept off-topic cards in the denominator instead of silently discarding them.
Our full method is explained in How We Review TRT Providers. It separates verified facts, provider claims, customer reports, and our own conclusions. It accounts for identified costs and makes missing charges visible rather than turning a teaser price into an all-in total. We don't give providers numeric scores. Learn more about TRT Provider Guide, read our editorial standards and privacy policy, or report a correction.
What else should you know? NuForm Health TRT FAQ
Nuform's public materials answer some practical questions, but they do not settle every new patient's prescription, price, or access. These answers distinguish a published policy from a detail you still need the care team to confirm.
Does Nuform take insurance?
No. Nuform's terms say it doesn't accept insurance. You pay cash for the doctor fee, labs, and medication. Ask your HSA or FSA administrator whether your specific charges qualify. 7
Is Nuform the same as NuForm or NUFORM?
Yes. The company writes its name as “Nuform” and “NUFORM.” People often search for “NuForm.” It's the same Michigan company at nuformhealth.com. 3 7
How fast does Nuform ship?
Nuform gives different estimates on different pages: 1–10 business days, about 5–7 days, and 7–10 business days. Our Approach separately describes shipment from the pharmacy within 2–10 days after approval. One reviewer paid extra for expedited shipping and got medication in under two days. 9 Dispatch and delivery are not the same event, and the terms do not guarantee an arrival date. Ask which estimate applies to your order and whether extra shipping fees apply. 3 1 2 4 7
Does Nuform offer testosterone cream or gel?
The men's TRT kits identified in the reviewed pages are injectable. We could not confirm whether a cream or gel can be prescribed through another arrangement. Ask before paying; don't infer that a general hormone-service page includes the treatment format you want. If you prefer a cream, see the comparison above. 1
Does paying the $170 fee guarantee a prescription?
No. Nuform's terms say it doesn't guarantee any prescription will be written. If the provider decides you're not a candidate, the doctor fee is refunded. That does not also promise a refund for completed labs. 7 6
Can I use bloodwork from my own doctor?
We did not find a published outside-lab acceptance rule in the reviewed materials. Ask which tests it accepts, how recent they must be, and whether they must be drawn in the early morning. Also ask what separate confirmation testing is required. Having prior results does not by itself establish eligibility.
Does Nuform treat women?
Nuform offers separate women's hormone services. This page covers men's testosterone replacement therapy only. It is scoped to adults being evaluated for testosterone deficiency, not adolescents or gender-affirming care. 3
Can I switch to Nuform from another TRT clinic?
Some customer reviews describe changing providers, but a review is not a transfer policy. Before switching, ask both clinics about transferring your records and lab results, and don't stop or change your current treatment without talking to a clinician. Get the receiving clinician's requirements and refill plan before relying on uninterrupted care. 34 8
Is TRT Provider Guide part of Nuform?
No. TRT Provider Guide is the independent decision resource for testosterone replacement therapy care. We're not a clinic, pharmacy, lab, or medical practice, and we earn nothing from Nuform.
