Ulta Lab Tests Testosterone Reviews: Which Test to Order, What It Really Costs, and What It Won't Tell You

Compare Ulta's exact Quest test codes and assay methods, separate collection fees, state restrictions and monitoring-panel gaps before choosing cash-pay bloodwork or clinician-led evaluation.

Ulta Lab Tests sells Quest testosterone testing without a prior visit to your own doctor. It can fit cash-pay testing when you know the exact test and have someone to review it. Match the test code, confirm your collection site and total, and plan clinical follow-up. Ulta lists four excluded states. Buying a test is not a diagnosis.

That's the short version. This Ulta Lab Tests testosterone reviews guide goes further, because Ulta sells at least five different testosterone tests with confusingly similar names, and the cheapest one isn't always the smart buy. There's also one number on your Quest report that can quietly mislead you. We'll get to it.

Ulta is a good fit if you: pay cash, know what test your clinician needs, can use a collection site that accepts your order, and have someone to review the result.

Start with a clinician instead if you: want a diagnosis or treatment advice, need insurance billing, or need a fertility evaluation. Ulta lists NY, NJ, RI and HI as unavailable for its direct testing. 1

Is Ulta Lab Tests legit for a testosterone test?

Ulta Lab Tests, LLC is a real test-ordering service with published patient terms and listings tied to Quest Diagnostics assays. You pay Ulta, use a collection site that accepts your order, and read your results in your Ulta account. Ulta lists New York, New Jersey, Rhode Island and Hawaii as unavailable for its direct testing. 1, 4, 5

Here's how the pieces fit together. Knowing who does what helps you judge the reviews.

Ulta Lab Tests testosterone assays, costs and clinical follow-up table 2: Is Ulta Lab Tests legit for a testosterone test?
Who What they do for your testosterone test
Ulta Lab Tests Sells the order, provides the lab requisition (the order form), holds your results online. Ulta says a physician authorization is included with every purchase, so you don't need your own doctor's order. 45
Quest Diagnostics Performs the laboratory test. Confirm which collection site accepts your requisition before you visit.
Your treating clinician Reads the result alongside your symptoms and health history. Confirm who will provide that evaluation and whether a visit costs extra.

"No doctor's visit" doesn't mean "no doctor." Ulta says physician authorization is included. That authorization is not the same as a visit to assess your symptoms or discuss TRT. 2, 45

Why Hawaii is on the list

On January 6, 2026, Hawaii's Department of Health issued a Notice of Violation and Order against Ulta's CEO. The state said Ulta had provided direct-to-consumer lab testing to Hawaii residents since May 2022, which Hawaii law doesn't allow, and that Ulta "is not a clinical laboratory certified under federal law." The notices ordered the end of affiliations with three local collection businesses and assessed each business a $15,000 fine. They allowed a hearing request within 20 days of receipt. We didn't verify a later ruling on the case. 16

The notice concerns Ulta's Hawaii ordering and collection arrangements. It does not assess the accuracy of an individual Quest result. Ordering access and laboratory measurement are separate questions. 16

Ulta's own materials now list four states where its direct testing isn't available: Hawaii, New York, New Jersey and Rhode Island. 1

Which Ulta testosterone test should you order?

For a clinician-selected total-testosterone measurement when low levels are suspected, Ulta's mass-spectrometry option, Quest code 15983, is worth comparing. Quest directs that use toward its MS assay; the Endocrine Society calls for accurate testing rather than one universal method. Ask whether free testosterone is needed for a borderline total result or a condition that changes SHBG. 7, 14

This is where the names can trip you up. Ulta's catalog lists several testosterone tests with almost identical names. The prices differ, and so do the methods. We matched each Ulta listing to its Quest test code, then checked what Quest says each test measures and how.

Price snapshot, not a current quote: these figures came from indexed copies of Ulta's own listings retrieved October 23, 2026; the available catalog copies were about four months old. Current product prices, promotions and checkout totals were not verified. The illustration adds the $12.95 fee stated in Ulta's retrieved FAQ; your selected site may charge more. Methods are from Quest's test directory. 2, 6–11

Watch out for "Compare" versions of the same test

Ulta sometimes lists the same Quest test under more than one name. We found at least two "Compare" listings, "Compare - Total Testosterone Test - Male (EW)" and "Compare - Male Testosterone Test (LGC)," that run the same immunoassay as test 873. The labels use competitor-style comparison names. Both identify Quest code 873. 19

The name alone doesn't tell you the method. Shop by Quest code, not by product name. These two listings identify the same underlying Quest assay; their prices, bundled services and order conditions can still differ. 19

What "total," "free" and "SHBG" mean

  • Total testosterone is all the testosterone in your blood, both the part attached to proteins and the part that isn't.
  • Free testosterone is the small share that isn't attached to anything. Quest puts it at about 1% to 4% of the total. 12
  • SHBG (sex hormone-binding globulin) is a protein that grabs onto testosterone. When SHBG is high or low, total testosterone alone may not give the clinician the full picture. 14

The Endocrine Society's guideline starts with total testosterone. It adds free testosterone, measured by equilibrium dialysis or estimated with an accurate formula, when total is near the low end or when a condition changes SHBG. Use the units and reference interval on the actual report; free-testosterone results from different methods are not interchangeable. 14

Why the method matters when low testosterone is suspected

Test 873 is an immunoassay. Quest describes it as useful for tracking stable testosterone levels in adults when a diagnosis is not being made. Quest's own clinical notes say immunoassay "lacks sensitivity, specificity, and precision at lower concentrations." Its 873 entry points to an MS assay when low levels are expected. 7, 12

That's the buying trap. If you're testing because you suspect your level is low, the assay's performance at low concentrations matters more than the cheapest listing.

So our editorial call: compare 15983 when your clinician wants a total-only test for suspected low testosterone, and confirm the current price difference. If you're on TRT and your prescriber accepts 873 for routine checks, use the requested assay. One more catch: Ulta says 873 reports values above 3,000 ng/dL as greater than 3,000, rather than an exact number. The MS option is not literally uncapped: Quest lists a reportable range of 1–10,000 ng/dL. 6–8, 12

Quick picker:

  • Curious, no symptoms: You may not need a test at all. The Endocrine Society recommends against routine population screening; ask whether your health history gives a reason to test. 15
  • Symptoms, first check: Arrange a clinical evaluation and a clinician-selected early-morning test; ask whether 15983 matches the request.
  • Your first result was borderline: Ask whether free testosterone is needed and which method to use; 36170 and 37073 are dialysis-based options.
  • Confirming a low result: Use another early-morning measurement on a different day, with the assay your clinician selects. 13, 15
  • Already on TRT: The exact panel your prescriber requested. See Section 5.

The right TRT provider is not the same for every person — it depends on whether low testosterone has been properly evaluated, your symptoms and health history, your fertility plans, your state, your insurance or cash-pay preference, your treatment-format preference, your budget, and whether online or in-person care is the better starting point. Some situations belong with primary care, urology, endocrinology, reproductive urology, or urgent or emergency care when the symptoms warrant it. Because a general answer cannot resolve those for you, use TRT Provider Guide's Find My TRT Path tool to explore online-program options, then discuss the right care route and questions with a clinician. The tool is educational and non-diagnostic.

How much does an Ulta testosterone test really cost?

The Ulta examples below come to $42.90–$65.90 per total-testosterone collection, using older listing prices and the published $12.95 standard fee. Two separate morning measurements double those examples to $85.80–$131.80, but that is not a complete clinical evaluation or a current quote. Ulta's terms say its charges may not be submitted for insurance reimbursement. 2, 4, 6

The draw fee is per collection visit

Ulta's retrieved FAQ says the standard draw fee is $12.95, with higher fees at some sites. Its location pages describe a prepaid fee for each collection visit. 44 Several compatible tests collected at one visit may share that fee; two separate visits need two fees. Confirm the selected site's charge before paying. 2

That matters because confirming low testosterone needs two separate mornings. You can't bundle the confirming sample into the first visit. Two tubes drawn at one visit do not provide two separate morning measurements. 13, 14

What two separate testosterone measurements cost

Here is the cost of two separate purchases at the amounts shown. These are testing illustrations, with different methods and result-support services; they are not equivalent care packages.

Ulta Lab Tests testosterone assays, costs and clinical follow-up table 4: What two separate testosterone measurements cost
Where you buy it Test One collection Two separate mornings at that price What result support is described?
Ulta Lab Tests 873 (immunoassay) $42.90 example $85.80 example Physician authorization and critical-result notifications; routine diagnostic visit not established. 2, 45
Ulta Lab Tests 15983 (mass spec) $65.90 example $131.80 example Same limits as above. 2
Ulta Lab Tests 36170 (free + total) $67.90 example $135.80 example Same limits as above. 2
Quest Health (Quest's own store) Testosterone Test, retail product 15983M $75.00 $150.00 Optional discussion with an independent healthcare provider included. 24
Labcorp OnDemand Total Testosterone $69.00 regular price $138.00 regular-price example Ordering oversight; one requested educational consult after an abnormal or critical result, at no added fee; no ongoing treatment. 25, 46, 47

Ulta: older listing illustrations, as described in the decoder above. Quest Health: $69 plus a $6 physician-service fee for an in-person draw. Labcorp: its page showed a $51.75 promotion against the $69 regular price on October 23, 2026; the same promotion is not guaranteed for a later order. The two-collection column assumes the displayed regular or example price applies twice. These are testing totals, not the full cost of an evaluation or treatment. 2, 6, 24, 25

Here's the comparison trap. Quest Health's retail code 15983M does not, by itself, prove that its offer uses the same assay as Ulta's Quest code 15983. The retail page we checked did not establish that mapping. Compare the method, the full current price and the result support before calling one the better buy. 24

Insurance, HSA and FSA

Ulta's patient terms are blunt. Ulta doesn't accept insurance or file claims, and its charges may not be submitted for insurance reimbursement. 4

So if you have decent insurance and symptoms, check that route first. With Original Medicare, a covered, medically necessary lab test your doctor orders usually costs you nothing, though an office visit may have a separate cost. 17 If you plan to use an HSA or FSA card, check your own plan's rules. Card acceptance at checkout doesn't prove the purchase is an eligible expense.

Refunds: the 60-day rule

Ulta's refund policy turns on two things: how long ago you bought, and whether your blood has been drawn. The deadlines run from the purchase date. 3

Ulta Lab Tests testosterone assays, costs and clinical follow-up table 5: Refunds: the 60-day rule
When you cancel What you get
Within 60 days, blood not yet drawn Refund to the original card when possible, or account credit; an invalid card means credit
Day 61 to 18 months, blood not yet drawn Account credit only
After 18 months Nothing. Unused orders and credits expire.
After your blood is drawn Nothing. The order is final.

Source: Ulta's Return & Refund Policy, retrieved October 23, 2026. Any test-kit logistics fee is excluded from both refunds and account credit. 3

Don't buy until you know which week you'll go. And if you've waited months, ask Ulta whether your requisition is still valid. An older version of Ulta's ordering page says requisitions last five months from approval, which is shorter than the credit window. Ask whether yours needs reissuing. Requisition validity and refund eligibility are separate questions. 3, 5

What can one Ulta testosterone result tell you?

One result tells you your testosterone level at that collection time, by that method; it can't diagnose low testosterone. The American Urological Association uses under 300 ng/dL as a reasonable supporting cutoff, with two early-morning measurements on separate occasions plus symptoms or signs. Buying an Ulta test does not establish a diagnosis or a treatment plan. 13

The two-test rule

Both major U.S. guidelines say the same thing in slightly different words:

  • American Urological Association (AUA): Use a total testosterone below 300 ng/dL as a reasonable cutoff in support of a diagnosis. Make the diagnosis only after two total testosterone measurements, each taken early in the morning, on separate occasions. A clinical diagnosis needs low levels and symptoms or signs. 13
  • Endocrine Society: Diagnose low testosterone (hypogonadism, or testosterone deficiency) only in men with consistent symptoms and signs and unequivocally, consistently low levels. Measure fasting, morning total testosterone with an accurate assay, then confirm with a repeat. 14

Quest's notes for the adult-male immunoassay put diagnostic timing more concretely: two morning specimens, 8:00 to 10:00 a.m., on different days. Shift work and treatment monitoring need a clinician's timing plan. 7

Why so strict? Testosterone swings during the day and from day to day. Sleep, illness and some medicines can affect testosterone. Some supplements can also distort certain assay results. The Endocrine Society's July 2026 statement adds a third problem: tests that aren't standardized can read the same blood sample differently. That's another reason to ask your clinician which laboratory and method to use for a comparable repeat. 7, 15

The number that can quietly mislead you

Here's the catch we promised at the top.

Your Quest report prints its own "normal" range next to your number. Quest says its adult-male range for the mass-spec total testosterone test starts at 250 ng/dL, the lower limit of its adult-male reference interval. The AUA's supporting cutoff is 300 ng/dL.

Those two numbers don't match. A result from 250 to less than 300 can show up within that test's reference interval, with no low flag, while still sitting below the guideline's supporting cutoff. 8, 12, 13 The reverse happens too. A red "low" flag isn't a diagnosis or a reason to start treatment.

We can't read your result for you, and neither can a reference range. The takeaway is simpler: don't let a green checkmark end the conversation if you still feel off, and don't let a red flag start treatment on its own. Take the full report to a clinician.

Prep that protects your result

  • Book early for diagnostic testing. Before 10 a.m. for a typical daytime schedule. If you work nights or already take testosterone, ask your clinician how to time the draw. 7, 14, 42
  • Fasting: follow the instructions printed on your Ulta requisition. The Endocrine Society recommends a fasting morning sample for diagnosis. If you have diabetes or another reason fasting could be unsafe, ask first.
  • Biotin: Tell the laboratory and your clinician about biotin supplements, and follow the preparation instructions for your exact assay. Ask before pausing anything prescribed. 7
  • Recent illness: say so. Being sick can lower testosterone for a while.
  • Don't change any prescribed medicine to "fix" a number.

What Ulta doesn't do

An authorized lab order and a result report are not the same as a clinical assessment for testosterone deficiency or ongoing TRT care. Ulta's FAQ describes critical-result notifications. That does not establish a routine diagnostic visit for every standalone purchase. Confirm whether your exact purchase includes a consultation and arrange clinical follow-up. 2, 4

A separate lab purchase can still make sense when you already have a reviewing clinician and the full quote works for you. For a clinician-requested first measurement, a confirming test, or ongoing TRT bloodwork, that's the point: buy the required order without paying for a duplicate care package.

If you'd rather have someone hand you an answer instead of a number, skip ahead to Section 6.

When to get clinician guidance before ordering more tests

Arrange a clinical evaluation before ordering more tests yourself if any of these apply:

  • You want to have children in the next year or two
  • A result came back very low
  • You've noticed breast swelling or tenderness, or your testicles seem small or soft
  • You have ongoing headaches or vision changes
  • You have a known pituitary problem, a testicular injury, or past chemotherapy or radiation
  • You have a history of prostate or breast cancer

A urologist handles testosterone and sexual health. A reproductive urologist is the right first stop when fertility is the priority. An endocrinologist fits when a hormone or pituitary cause is suspected. These are care-route suggestions, not a diagnosis. 13, 14

Got a number but no plan? Explore an educational next step with TRT Provider Guide's Find My TRT Path tool →

The tool presents five questions and describes an online-program recommendation. Our privacy policy says individual answers are processed in your browser and are not sent to our database. It doesn't read lab results, diagnose anything, or promise a prescription. 39, 40

Can you use Ulta Lab Tests for TRT bloodwork?

Yes, Ulta can supply a clinician-requested order for men paying cash for TRT monitoring. Before you order, ask your prescriber which exact tests they want and whether they accept outside Quest results, because a panel called "TRT" isn't automatically the panel your clinician needs. 20–22

What TRT monitoring usually covers

Your prescriber sets your monitoring plan. The guidelines name a few core markers:

  • Testosterone level, to check that treatment is reaching its target. The AUA recommends testosterone checks every 6 to 12 months during ongoing therapy, with earlier checks after starting. 13
  • Hematocrit, the share of your blood made of red blood cells. Testosterone can raise hematocrit, which is monitored, often as part of a complete blood count (CBC). See TRT and high hematocrit. 14
  • PSA (prostate-specific antigen), a prostate protein. For men who choose prostate monitoring after an age- and risk-based discussion, the Endocrine Society recommends assessment before treatment and 3 to 12 months after it starts. See TRT and PSA levels. 14

Timing matters on TRT. When to draw depends on your treatment format and schedule. That's your prescriber's call, not something to copy from a forum. 13, 14

TRT Plus Panel CBC, comprehensive metabolic panel, estradiol, PSA free and total, and total/free testosterone plus SHBG (37073) Check current listing — Only if your prescriber asks for these. 22

Panel contents are seller-stated in the official product text retrieved October 23, 2026. The base-panel dollar amount is an older listing illustration under the same price limits as the decoder above; no current panel quote was verified. The ultrasensitive Package II is a different listing from ordinary Package II. 20–22

The base panel doesn't list estradiol, SHBG, LH, FSH or a metabolic panel. 20 That's fine if your prescriber didn't ask for them. A bigger panel isn't a better panel. More tests can mean a bigger bill without answering your clinician's question.

Ask your prescriber before you buy

Copy this and send it:

"Will you accept outside Quest results from Ulta Lab Tests? Which test codes do you want, and when should I have the blood drawn relative to my dose? Who will review the report?"

One more thing. Ulta lists a panel named after a TRT clinic, the "Male Excel TRT Panel." The listing does not establish whether Male Excel requires or accepts it, or whether clinic care is included. 23 If you're a patient of any clinic, ask the clinic before buying a panel with its name on it.

Should you order from Ulta or start with a TRT program that includes labs?

If you already have a prescriber, an Ulta order may fit the tests they request. If what you really want is to know whether testosterone treatment makes sense for you, a starter test that includes a clinician visit can get you there with fewer steps. Neither route guarantees a diagnosis or a prescription.

The next step differs depending on whether you already have a prescriber or are seeking your first evaluation. If you already have a prescriber, compare the exact requested tests. If you're buying a test because you're thinking about TRT, the result still needs clinical assessment; a low result needs confirmation before a diagnosis. Some online programs bundle a starter test and visit. 13, 14

Before any of this: fertility. Testosterone therapy can lower sperm production. The AUA advises against prescribing it to men who are trying to conceive. If kids are in your plans soon, start with a urologist or reproductive urologist, not a TRT program. See TRT and fertility. 13, 14

For a home starter test with an online consultation: Male Excel

Male Excel's $99 starter is a finger-prick kit you do at your kitchen table. It lists testosterone, estradiol, DHEA-S, free T3 and PSA, and includes an online consultation. If a vein-drawn sample from day one matters to you, Hone's at-lab option or Taurus's Labcorp/Quest draw is the better fit. But skipping the lab trip is the point for a lot of men who've put this off for a year. Before paying, ask how Male Excel handles a confirming second test and what it costs, because the starter page doesn't price it. 31, 32

For an at-lab starter with a physician video visit: Hone Health

Hone's Premium option is a $65 at-lab test with a physician video consult. Its Premium plan page lists a $25 at-lab confirmation test; Plus lists $45 at a lab or $80 at home. But a separate Hone help article says clinically needed retesting is free. Ask which applies to you. 33–35

Start with an in-network clinician instead if you want to use insurance, and confirm your plan's lab rules. If you already have a clinician, ask whether an Ulta cash-pay order fits the plan. Fertility plans or a complex health history should guide the choice of clinician before a treatment subscription. That's a perfectly good plan. 4, 13, 14

What do Ulta Lab Tests testosterone reviews actually say?

The review platforms below show favorable company-wide ratings, alongside individual reports of ordering, refund and collection problems. Reviews can tell you how ordering went. They can't tell you whether a test is accurate or whether your clinician will accept it.

The collection site cannot find the order An individual customer reported an order-transmission problem; the cause was not independently verified Bring a printed requisition and your order number, and confirm the site before you go. 30
A collection visit fails or is delayed Individual reviews describe location and scheduling problems; they do not establish a failure rate Confirm that the site accepts the order and book an appointment. 29
Results take longer than expected Completion estimates differ by the exact assay; a general quick-results message is not a guarantee Read the estimate for every component and ask when its clock starts. 2, 6

How to order an Ulta testosterone test without wasting the draw

Pick the test before you pick the time. Confirm your state and accepting collection site, match your clinician's requested test code, and follow the requisition's prep notes. Diagnostic testing usually needs early-morning collection and a repeat on another morning; TRT monitoring follows your prescriber's timing plan. 5, 13, 14

  1. Check your state. Ulta lists NY, NJ, RI and HI as unavailable for its direct testing. 1
  2. Pick the test by Quest code using the decoder above. Match the clinical request, including the method.
  3. Confirm the collection site and full price before paying. Ulta publishes a standard $12.95 collection fee, with higher fees at some sites; confirm the charge for your visit. 2
  4. Get your requisition. Print it. Note the fasting and supplement instructions.
  5. Book the appointment at the agreed time. For diagnosis, that usually means early morning; for TRT monitoring or shift work, follow your clinician's plan. Bring photo ID, the requisition and your order number. 5, 7, 14, 42
  6. Watch for the results email. Use the estimate for your exact assay; the MS and dialysis tests can take longer than the basic immunoassay. Download the full PDF with units and the reference range. 5, 6
  7. Take it to a clinician. If a diagnostic result is low, discuss a confirming early-morning measurement on a different day and which laboratory and assay to use. 13, 14

What we actually verified

We compared Ulta's published test listings with Quest Diagnostics' own test descriptions, Ulta's policies, a state regulator's notice, and the current clinical guidelines. We did not buy a test, give a sample, or contact Ulta's support. This is editorial research, not a hands-on test of the service.

Sources checked: October 23, 2026

Ulta source access: We read indexed copies of Ulta's official FAQ, terms, refund policy, ordering guide and product listings. Fresh direct access to those pages was blocked. The available catalog copies were about four months old; current checkout prices and promotions were not verified.

Checked directly: Quest's test descriptions for codes 873, 15983, 36170, 37073 and 14966; its testosterone clinical FAQ and assay-method table; Hawaii's January 6, 2026 notice; AUA's own published guideline recommendations and current guideline-status page; the Endocrine Society's 2018 guideline and July 2026 statement; the linked competitor and program sources.

Confirmed in primary sources: the Quest assay methods, the diagnostic and monitoring guidance, testosterone's Schedule III status, and what Hawaii's notice ordered and assessed. The notice's later outcome was not established.

Provider-stated, not service-tested: Ulta's authorization, collection fees, state restrictions, insurance and refund policies, turnaround estimates, results workflow and panel contents. Male Excel, Hone Health and Taurus Meds offers and terms were checked on October 23, 2026; they are advertised terms, not verified patient invoices or service delivery.

Customer-experience signals: the displayed review-platform scores and the two attributed customer accounts. These are company-wide experiences, not evidence of testosterone-test accuracy or a measured failure rate.

Our calculations: every "with draw fee" and "two separate mornings" total. We show the inputs. Ulta amounts are older listing illustrations; the comparisons do not establish a complete evaluation or first-year treatment budget.

Editorial conclusions: the test-code comparison, fit guidance and before-you-pay checklist come from those sources. They do not decide which test or treatment an individual needs.

Not confirmed: the identity or license of the physician who would authorize your Ulta order; an actual selected site's current fee or acceptance; clinic acceptance of outside results; your state-specific treatment access; individual program medication, shipping and repeat-test charges; complete first-year treatment totals; or the later outcome of the Hawaii case.

Not done: purchases, blood draws, support contact or medical review. No ratings or scores were invented. The tool's privacy description comes from our published policy, not a technical test of answer handling.

Ulta testosterone test FAQ

The right answer depends on your exact order and why you are testing. These answers cover the remaining purchase questions; a clinician still decides how a result fits your care.

Is Ulta Lab Tests the same as Quest?

No. Ulta Lab Tests is a separate company that sells Quest Diagnostics test orders. Quest runs the test, while Ulta handles the order and your online results. Confirm the accepting collection site before you go. 4, 5

Is this the Ulta Beauty website?

No. This guide covers Ulta Lab Tests, LLC at ultalabtests.com, the laboratory-test ordering service. Ulta Beauty's retail website is ulta.com. 4, 43

Do I need a doctor's order to use Ulta?

You don't need your own doctor's order. Ulta says a physician authorization is included with every purchase. That's an order, not a doctor visit or a diagnosis. 2, 45

What's the cheapest Ulta testosterone test?

The lowest listing example among the five decoded here was Testosterone Total Male Test (Adult Only), Quest code 873: $29.95, or $42.90 with a $12.95 fee. Those are older figures, not a current quote. It uses an immunoassay; Quest points to MS testing when low concentrations are expected. 6, 7

How long do Ulta testosterone results take?

Ulta's general estimate is 1–3 business days, with complex tests taking 7–10. Its retrieved testosterone listings were more specific: about 1–2 business days for 873 and 4–5 for the four MS-based options above. These are provider estimates, not measured delivery times; check the current estimate and when its clock starts. 2, 5, 6

Do I need to fast before an Ulta testosterone test?

Follow the instructions printed on your Ulta requisition. For diagnosing low testosterone, the Endocrine Society recommends a fasting, early-morning sample. 14

What time should I get my testosterone tested?

For diagnosis, early morning. Quest's adult-male immunoassay notes point to 8:00–10:00 a.m. on two different days. If you work nights or already take testosterone, ask your clinician for a timing plan. 7, 14, 42

Is 300 ng/dL low testosterone?

The AUA calls under 300 ng/dL a reasonable cutoff in support of a diagnosis. A diagnosis also needs two low early-morning tests plus symptoms or signs. Quest's own adult-male range for its mass-spec test starts at 250 ng/dL, so your report's flag and the guideline cutoff may not match. 8, 12, 13

Should I order total or free testosterone?

Guidelines start with total testosterone for evaluation. A clinician may add free testosterone when total is borderline or a condition changes SHBG. Ulta's 36170 measures free testosterone by equilibrium dialysis, a method the Endocrine Society accepts. 9, 14

Can I get a refund from Ulta Lab Tests?

Yes, within 60 days of purchase if your blood hasn't been drawn. From day 61 to 18 months, you get account credit only. Once your blood is drawn, the order is final. Test-kit logistics fees are excluded from refunds and credit; if the original card is invalid, an eligible refund becomes credit. 3

Does insurance cover Ulta Lab Tests?

Ulta's terms say it doesn't accept insurance and its charges can't be submitted for reimbursement. If you're using an HSA or FSA, check your plan's rules. 4

Which states can't use Ulta Lab Tests?

Ulta lists New York, New Jersey, Rhode Island and Hawaii as unavailable for its direct testing. Hawaii's health department took enforcement action against Ulta in January 2026. 1, 16

Will my TRT clinic accept Ulta results?

Acceptance is clinic-specific and was not established in this review. Ask before you buy, and have the test code, collection date and time, units and reference range ready.

Can I use Ulta results to change my testosterone dose?

No. Dose decisions belong to your prescriber, who also decides when your blood should be drawn.

Is Ulta Lab Tests a scam?

Its published terms and Quest test-code listings support a real test-ordering service; they do not guarantee every order will go smoothly. The limits still matter: Ulta says it does not take insurance, refunds become credit-only after 60 days, four states are excluded, and a routine diagnostic visit is not established for a standalone order. 1–4, 6
