What we actually verified — September 28, 2026: We checked public provider pages, counted the location listings, reviewed selected public review displays, and checked official medical and regulatory sources. Clinic operations, PPO acceptance, and treatment inclusions remain provider-stated. We did not obtain a clinic quote, attend a visit, test cancellation, or independently verify every clinician and pharmacy license. The exact medication, repeat-test process, full monitoring plan, and patient financial terms still need a clinic-specific answer.

Three things change that answer:

  • No PPO? No cash price appears in the Ageless FAQ or sign-up page reviewed for this guide. Get a written quote before your first visit. Provider sources: FAQ and sign-up page.
  • Trying to have kids soon? Start with a reproductive urologist, not a general TRT enrollment offer. Clinical guidance.
  • Live in New Jersey or Mississippi? Ageless says it doesn't offer telemedicine there. Provider disclosure.

Ageless is worth a look if you…

  • have PPO insurance, or can obtain a self-pay quote that fits your budget

  • you need home treatment and the clinic cannot confirm a suitable take-home plan
  • you need a written price and the clinic will not provide one before payment
  • you cannot make the visits your clinic requires
  • you are trying to conceive now or soon and have not had fertility-focused advice
  • you want a gel or oral medicine rather than the injections described in Ageless's FAQ

Is Ageless Men's Health legit? The 2015 settlement, explained

Ageless Men's Health operates a public clinic network and dates its history to 2007. In 2015 it agreed to pay $1.6 million to settle federal allegations that it billed Medicare and TRICARE for medically unnecessary office visits alongside testosterone injections during 2009–2013. What it was about: billing. Specifically, charging government health programs for an office visit alongside each weekly injection. Official announcement.

What it did not establish: that Ageless broke the law, harmed patients, or does anything wrong today. It covered Medicare and TRICARE claims from more than a decade ago. We could not confirm when the integrity agreement ended. This historical record is not a current legal clearance, and we do not claim to have completed an exhaustive enforcement search. Why it still matters to you: a weekly clinic plan creates repeated billing events. So ask one plain question: "How is each weekly visit billed to my insurance, and what will I owe per visit?" Get the answer in writing.

Other trust signals we checked:

  • Ageless publishes a California clinician roster. That is a useful starting point for a license check, not proof that every listing is current.
  • Its site says patients must sign an informed-consent form and an anabolic steroid agreement before treatment. That is a provider-stated process, not a clinical-quality certification. Testosterone is a Schedule III controlled substance and requires a valid prescription. Ageless disclosure · DEA · Prescribing information.
  • Ageless's own appointment form says the site is not HIPAA compliant and asks you not to type health details into it. That's honest. Take them up on it — save symptoms for the visit or a clinic-confirmed secure channel. Appointment form.
  • The Better Business Bureau lists the inspected Chicago and Atlanta profiles without accreditation. The Chicago profile shows an A+; the Atlanta profile shows no rating. BBB ratings do not establish medical quality.

How does TRT at Ageless work? Weekly shots, in the clinic

Ageless describes weekly testosterone cypionate injections given by clinic staff, with Monday–Friday hours. That would mean 52 visits in a 52-week illustration if every weekly treatment requires a visit. It offers regular in-person contact — and regular drive time. Provider FAQ · Sign-up page.

Here's how Ageless describes the routine on its own pages:

  • The drug: injectable testosterone cypionate.
  • Treatment format: a clinic-administered injection, according to Ageless's public description; this is not a self-injection guide.
  • How often: every 7 days in the advertised clinic model.
  • Who gives it: clinic medical staff. The sign-up page says treatments are supervised and given in the clinic.
  • How long: Ageless markets visits as short as three minutes. It describes the first visit as about 30 minutes. These are provider claims, not timings we measured.

It's the same question men were asking years ago. One Reddit user put it simply: his main concern was "whether I'll have to come in every week for my shots or if they'll allow me to self-administer." The post was displayed as about three years old when checked. Original discussion.

Here's the honest answer.

Ageless does not publish a complete take-home treatment policy on the pages we reviewed. If home treatment is your priority, ask the clinic to confirm it before paying. Its advertised in-clinic model does give you regular face-to-face visits and a chance to have staff administer treatment; Ageless says it accepts most PPO plans. Those are reasons to investigate the clinic, not proof that every visit or prescription will be covered. Clinic model · Telemedicine disclosure.

For a lot of men, that's the whole point. If you've never given yourself a shot, if you want someone to notice when something's off, or if accountability keeps you on track, a weekly check-in is a feature, not a flaw.

The 52-visit math. Say your clinic is a 20-minute round trip and each visit takes 10 minutes. That's 30 minutes a week, or about 26 hours over 52 visits. At a 40-minute round trip, it's about 43.3 hours. These are hypothetical travel and visit times, not an Ageless average or a prescribed schedule. Put your own numbers in before you decide.

Can you ever take supplies home? One March 2020 patient report says yes, at one clinic. Ageless's current disclosures also advertise telemedicine, but do not explain whether it includes shipped medication or self-administration for your situation. Ask your clinic directly. How much does Ageless Men's Health TRT cost?

No cash price appears in the reviewed Ageless FAQ or sign-up page. With PPO insurance, Ageless describes copays and any unmet deductible; without insurance, it describes a self-pay rate covering visits, exams, labs, and testosterone. That's the frustrating part. You can't budget from the published package description. So we pulled together the dated price reports below and did the math — without treating them as today's prices.

These are not Ageless's prices. They're what individual people reported, at one clinic, at one point in time. Your clinic may charge more or less. The only number that counts is the one Ageless puts in writing for you. The annual figures are arithmetic illustrations, not verified all-in first-year costs.

What the self-pay rate says it includes

Ageless's FAQ says the self-pay rate covers office visits, exams, lab tests, and testosterone treatment. Its sign-up page adds "up to 4 injections per month." FAQ · Sign-up terms.

That wording matters. Twelve allowances of up to four injections cover at most 48. A 52-week schedule has 52 weekly visits. Ask how visits beyond the monthly allowance are billed; we did not verify an extra-visit charge.

Ageless's treatment page also mentions two add-on medicines (more on those below). Ask whether they're in the self-pay rate or billed separately. Provider treatment description.

Get the real number: the Ageless quote script

Copy this, send it to your clinic, and ask for the answer in writing:

"Before my first visit, please send me in writing: your self-pay price per week or per month; exactly what it includes (visits, which blood tests and how often, testosterone, any other medicines, supplies); anything billed extra; whether there's a contract, minimum term, or automatic billing; how I cancel; and how I get copies of my lab results and records."

Does Ageless Men's Health take insurance or Medicare?

Ageless says it accepts most PPO insurance plans and checks your benefits before your first appointment. The reviewed pages do not establish whether your branch accepts an HMO plan, Medicare, Medicaid, or TRICARE. Copays or coinsurance may still apply after your deductible is met. PPO plans. This is a practical difference from a cash-pay membership. Ageless may be the cheaper route, but only your plan-specific quote can show that. Just remember that PPO acceptance doesn't guarantee your specific treatment is covered. Ask your insurer about the branch, clinician, lab, pharmacy, prior authorization, and the testing records it requires. Ageless FAQ · Coverage basics.

Watch the weekly copay. Here's a simple example, not an Ageless price. If your plan charges a $30 copay per visit and the clinic bills 52 weekly visits, that's $1,560 in copays alone. Ask whether deductible, lab, or medication charges also apply, and avoid counting any covered or bundled item twice.

Medicare and TRICARE. The reviewed Ageless pages do not establish current acceptance. Ask. These were the programs involved in the 2015 settlement; that history does not prove present-day coverage. Historical record.

What happens at your first Ageless visit — and is one blood test enough?

Ageless says your first visit takes about 30 minutes: paperwork, a medical history, a brief exam, and a blood draw for total testosterone and PSA. Its FAQ describes results during the visit or within 24 hours, while its clinical-experience page says 48 hours. Diagnosis needs symptoms or signs plus consistently low results confirmed with appropriate repeat early-morning testing, not one test alone. That's the question we said we'd ask first. Here's why.

Your testosterone level isn't one fixed number. It's usually highest in the morning and changes from day to day. Illness, sleep disruption, food intake, and collection timing can affect a result. The Endocrine Society recommends at least two early-morning fasting measurements with reliable tests, interpreted alongside symptoms and medical history. Clinical guideline · July 2026 statement.

A few terms, in plain English:

  • Total testosterone: all the testosterone in your blood.
  • LH (luteinizing hormone): a hormone from the pituitary gland that signals the testicles to make testosterone. It helps show why testosterone is low.
  • FSH (follicle-stimulating hormone): a pituitary hormone that helps support sperm production.
  • Hematocrit: the share of your blood made up of red blood cells. Testosterone can raise it.
  • PSA (prostate-specific antigen): a protein made by prostate tissue; a PSA blood test can help guide a discussion about prostate health.

The PSA discussion depends on age and risk. The Endocrine Society describes shared decisions about prostate monitoring for men ages 55–69 with a life expectancy over 10 years. It also recommends offering a monitoring discussion to men ages 40–69 at increased prostate-cancer risk. This is not one screening rule for every patient. Clinical guidance.

"Not described" doesn't mean "not done." Ageless's public pages are short. Your clinic may already do all of this. The point is to know before your first shot, not after.

A few more things to know:

  • 300 ng/dL is not a diagnosis by itself. It is a commonly used threshold, but the result must be interpreted with symptoms, repeat testing, the laboratory method, and the clinical situation. It's not a line that decides for you. Endocrine Society, July 2026.
  • Ask about outside labs. Bring existing results and ask whether your clinician can use them or needs new testing. We did not verify a current network-wide outside-lab acceptance policy.
  • Is there an age limit? Ageless's FAQ says no when answering whether someone is too old for treatment. That does not establish that it treats minors, or that aging alone is a diagnosis. This guide is for adults. Provider FAQ.
  • An evaluation isn't a guarantee. No clinic, quiz, or website can promise you a prescription. Diagnostic standard.

What does Ageless give you besides testosterone?

Ageless's treatment page describes an unnamed medicine to manage estrogen and hCG to support natural testosterone production. Those are provider statements, not proof that every patient currently receives those medicines or that they achieve the claimed result. Ask for the exact name, purpose, and price of every proposed medicine. Provider treatment page.

This is worth checking before you agree to a treatment plan. Ageless's "Low T Treatments" page describes three parts to its approach:

  1. Testosterone cypionate, injected weekly.
  2. An estrogen-management medicine. Ageless says the body turns some testosterone into estrogen and that it provides "another medicine along with your shot" to manage that. It doesn't name the drug.
  3. hCG (human chorionic gonadotropin). Ageless says it uses hCG to protect your natural testosterone production and maintain testicular size.

Other medicines are not TRT

What that means for you:

  • Ask what the estrogen medicine is. Ageless does not name it on this page, so we cannot verify the exact drug or its approved use. Ask why you'd need it, what evidence supports that use, whether it is off-label, and what it costs. Off-label means a prescribed use outside that product's FDA-approved labeling.
  • hCG is its own drug, not TRT itself. Some hCG products, such as Pregnyl, are FDA-approved for selected males with hypogonadotropic hypogonadism caused by pituitary deficiency — low testosterone caused by too little signaling from the pituitary gland. Using hCG alongside TRT to try to preserve fertility is not one of Pregnyl's labeled indications. No clinic can promise that it will preserve fertility.   • Get the price for each piece. Patrick A.'s May 2023 report of about $75 a week was for testosterone and hCG together. That historical price does not establish your current package. Original review.

  • FDA-approved means the FDA reviewed that specific drug product and approved it for the uses in its labeling. Approval does not make it risk-free.
  • Compounded medicines are prepared to meet particular patient needs. The FDA does not review compounded drugs for safety, effectiveness, or quality before they are marketed. Compounded testosterone is not the same thing as an FDA-approved generic.

One more marketing claim to read with care: an Ageless page says its approach leaves "no risk of over-or-under use." Supervision does not remove medication risks. Another Ageless page suggests clinic injections are safer than gel without supplying a supporting comparison. Different treatment formats carry different risks — gels can transfer testosterone to other people, for example — but that is not proof that injections are safer overall. Who are the clinicians at Ageless Men's Health?

Ageless's California roster lists six physician assistants and four nurse practitioners. That is not a nationwide staffing audit, and a company list is not proof of an active license. Ask who will evaluate you, who will administer treatment, and who handles problems between visits. Published California roster.

Here is what the California roster showed when checked September 28, 2026. One listed license-expiration date was August 31, 2026, already past. That does not prove an expired license — the clinician may have renewed — but it is a reason to check the official board record rather than rely on the roster. Provider roster.

These credentials identify clinician types, not the quality of a particular care plan. Ask about the named prescriber, their current authorization in your state, access to a physician or specialist when needed, and who reviews laboratory results. When a physician specialist is the better start: a suspected pituitary or testicular disorder, an abnormal prostate evaluation, an elevated blood count, or fertility concerns may need urology, reproductive urology, or endocrinology. A very low result needs clinician interpretation, not an automatic treatment decision. New severe headache or sudden vision changes need prompt medical assessment rather than a routine TRT intake. California has the most (10), then Arizona (9), Texas (8), Tennessee (6), and Utah (6); these are counts of published listings, not independent operating-status checks. Good to know if you're in Idaho or Mississippi: Male Excel lists both among its exclusions. Its full current list has 11 states: Alabama, Alaska, Arkansas, Connecticut, Hawaii, Idaho, Louisiana, Minnesota, Mississippi, New Hampshire, and Rhode Island. A note on remote prescribing: testosterone is a Schedule III controlled substance and requires a valid prescription. The current federal temporary extension runs through December 31, 2026 and permits qualifying telemedicine prescribing without a prior in-person examination under its conditions. What if you want kids? TRT, fertility, and Ageless

Testosterone treatment can lower sperm production, and Ageless's own FAQ lists a decrease in sperm count as a possible side effect. This is the one area where we'd tell you to slow down, whatever clinic you're considering.

Here's why. When you take testosterone from outside the body, your brain senses there's plenty and turns down its own signals — including the ones that tell your testicles to make sperm. That can lower your sperm count, sometimes a lot. AUA/ASRM fertility guidance.

What that does not mean:

  • It doesn't mean TRT is birth control. It isn't reliable birth control.
  • It doesn't mean you'll become permanently infertile.
  • It doesn't mean your sperm count will bounce back on a set schedule if you stop. Fertility guidance · Clinical guideline.

Ageless says it uses hCG to protect natural production. That may be part of a plan a clinician discusses with you. But no clinic can promise hCG will keep you fertile. Clomiphene and enclomiphene — pills you may see advertised — aren't testosterone at all, and they aren't TRT. Clomiphene use in men is off-label; enclomiphene has no FDA-approved drug product. How does Ageless monitor safety on TRT?

One reviewed patient account describes blood-count monitoring at Ageless, but a testimonial does not verify a clinic's safety process. Clinical guidance calls for a monitoring plan that covers testosterone, hematocrit, treatment response, and appropriate prostate follow-up; current testosterone labeling also addresses blood pressure. The reviewed Ageless pages do not provide a complete monitoring schedule. The Endocrine Society describes testosterone and hematocrit checks at 3–6 months, at 12 months, and then annually, with the plan adjusted to the person and treatment.   • Hematocrit. Testosterone can raise the share of red blood cells in your blood. Your clinician should check it and act if it runs high. Don't try to manage it yourself. Clinical guidance.
  • PSA. Ask about prostate monitoring that fits your age, risk, and preferences. It is not the same testing schedule for every man. Clinical guidance.
  • Blood pressure. In February 2025, the FDA announced testosterone-label changes incorporating heart-safety study findings, removing boxed-warning language about increased major cardiovascular risk, and adding or updating blood-pressure warnings. In June 2026, it requested further labeling changes. A requested change does not mean every label has already changed; check the current label for the exact product you're given.   • Sleep apnea. Ageless's FAQ lists possible worsening of existing sleep apnea. The Endocrine Society recommends against starting testosterone in men with untreated severe obstructive sleep apnea.   • Other side effects Ageless lists include acne, fluid retention, breast enlargement, and smaller testicles. Review the exact medication's risks with your clinician rather than relying on this short list. Ageless FAQ · Testosterone cypionate label.

Call the clinic promptly about new side effects or a change in your health. Call 911 for chest pain, sudden trouble breathing, fainting, or stroke signs. Before you switch, ask Ageless for copies of every lab result, your treatment history, and your visit notes, so the next clinician can decide what else is needed. Ask how to stop recurring billing and whether anything is prepaid; the website-use terms do not establish your patient cancellation or refund rights. Men leave clinics for all kinds of ordinary reasons: a move, a new job with weekday hours, or wanting to self-inject. Here's how to do it cleanly.

  1. Get your records first. Ask for all lab results, your current dose and schedule, every medicine you've been given (including hCG or an estrogen medicine), and your visit notes. Ask if there's a fee and how long it takes.
  2. Ask about billing. Ageless's website terms cover website use, not treatment contracts. Ask whether you signed a patient financial agreement, how much notice they need, and whether any balance is owed.
  3. Plan the handoff with your clinicians. Ask how treatment and monitoring will be handled during the change. A billing cancellation is not an instruction to stop, continue, or change a medicine.
  4. Moving states? Check the clinic table above. Ageless may have a clinic where you're headed. Ask both clinics about record access, clinician authorization, and whether an in-person visit is required.

Get the exit details in writing. Ask about any minimum term, automatic renewal, cancellation deadline and method, unused prepaid services, medication charges, refunds, record-copy fees, and the date billing will end.
