hCG vs Enclomiphene: Which One Fits Your Labs, Fertility Plans and Budget?
hCG vs enclomiphene comes down to where your low testosterone starts, not pill vs. shot. Enclomiphene works through brain and pituitary signals. hCG acts on the testicles. Only hCG has FDA-licensed products. Neither one is TRT, and neither guarantees fertility. If you're trying to conceive now, start with a reproductive urologist.
For: adult men comparing treatment for confirmed low testosterone and fertility concerns. Not for: choosing a prescription from one test, bodybuilding or nonmedical use.
Here's the short version:
- Your pituitary works, kids are a "someday" goal, and you'd rather skip needles? Enclomiphene is one option some clinicians consider for selected men with secondary hypogonadism. Know that it has never been FDA-approved; compounded enclomiphene is not FDA-approved either. 2 21 23
- You have a pituitary problem, you're already on TRT, or enclomiphene didn't raise your numbers? Ask your clinician to reassess the cause and fertility plan before choosing hCG or switching treatment. 1 21
- Trying to conceive now? Start with a reproductive urologist or fertility team. 21
The pill isn't automatically cheaper. Strut's starting price adds up to $948 for twelve 30-day supplies before labs; hCG is priced by the vial, with care fees and unused medicine to count too. We'll show you the published prices, what each subtotal covers, and which numbers you need to ask for before you pay. See the cost table ↓
TRT Provider Guide is the independent decision resource for testosterone replacement therapy — helping U.S. adults understand how low testosterone is evaluated, compare online and local care models and providers, and choose the next step that fits their health needs, fertility plans, budget, state, and care preferences, with every material claim verified and dated.
Educational information, not medical advice. Both medicines need a prescription. A qualified clinician must assess your diagnosis and decide what fits you.
This comparison is for you if:
- A test showed low testosterone and you want to protect your fertility.
- A clinic offered you one of these two drugs and you want to understand the evidence and questions to ask.
- You're deciding between a pill and a shot.
This page isn't for you if:
- You want either drug for bodybuilding, a "cycle" or "PCT." We don't cover that.
- Your testosterone hasn't been properly tested yet. Start with how low testosterone is diagnosed.
- You're asking about hCG for ovulation or weight loss. Those are different decisions. (FDA says no hCG product is approved for weight loss.) 33
hCG vs enclomiphene at a glance
Enclomiphene and hCG act at different points in the hormone system, and neither is testosterone itself. The main differences are the diagnosis each may fit, FDA status, fertility evidence and the full cost of care. 1 2 21 23

In the next few minutes you'll learn which of these rows matter for you. The diagnosis and fertility goal determine which rows matter most.

What's the difference between hCG and enclomiphene?
Enclomiphene tells your brain to send more of its own testosterone signals. hCG acts like the LH signal, directly on your testicles. That difference helps explain why neither is testosterone replacement therapy (TRT), and why your diagnosis matters more than the pill-or-shot question. 1 21 23
How your body makes testosterone, in three steps
- Your brain (the hypothalamus) tells your pituitary gland to get to work.
- Your pituitary sends two hormones into your blood:
- LH (luteinizing hormone) tells the testicles to make testosterone.
- FSH (follicle-stimulating hormone) helps the testicles make sperm.
- Your testicles answer by making testosterone and sperm.
Testosterone and estradiol help regulate the brain and pituitary signals through feedback. 21 23
How enclomiphene works
Enclomiphene blocks estrogen's "slow down" message in your brain. Your brain thinks hormone levels are low, so the pituitary sends out more LH and FSH. If the hormone system and testicles can respond, this can increase your own testosterone production.
In two 16-week trials of overweight men aged 18 to 60 with this upstream kind of low testosterone (256 men in the main analysis), LH and FSH went up on enclomiphene. They went down on testosterone gel. Sperm concentration held steady on enclomiphene and fell on gel. 23
Enclomiphene is one of the two geometric isomers in clomiphene (brand name Clomid); the other is zuclomiphene. These are forms of a molecule with a different arrangement around a bond, not mirror images. They are not the same product, and a study of one does not establish the same benefits or risks for the other. 25
How hCG works
hCG (human chorionic gonadotropin) is a hormone that looks like LH to your testicles. Its LH-like action does not need the pituitary to release LH; it tells the testicles to make testosterone. The FDA label for Pregnyl describes exactly that: it stimulates the testosterone-making cells of the testicle. 1
hCG does not supply FSH. Some men with hypogonadotropic hypogonadism need additional FSH treatment for sperm production. 21 22
Is either one TRT?
No. TRT puts testosterone into your body from outside. Your brain notices and turns down LH and FSH, which can shrink sperm production. Both of these medicines work the other way: they push your body to make its own testosterone.
That's why men who want to protect fertility look at them in the first place. For the full TRT-versus-pill decision, see enclomiphene vs TRT.
Which one can work for your type of low testosterone?
It depends on where the problem starts: enclomiphene needs a pituitary response, while hCG acts directly on the testicles. If your testicles aren't responding, either medicine may have a limited effect. LH and FSH help your clinician find the cause; they don't select a drug on their own. 20 21
Most comparisons skip this part. It's the part that matters most.
The lab-pattern map
This shows how the drugs work. It isn't a diagnosis. Your clinician reads your results.
Current TRT, hCG and other medicines can affect hormone results. Some causes of secondary hypogonadism need their own treatment before choosing a hormone medicine. 1 21
Two steps before choosing treatment
Before anyone can interpret that pattern, two things should happen:
- Confirm the low testosterone. The Endocrine Society calls for symptoms or signs of low testosterone, plus testosterone that is low more than once. Use a reliable lab test and repeat an early-morning, fasting total-testosterone measurement to confirm. Free testosterone may also be needed in some cases. One low number from a home kit isn't a diagnosis. 20
- Find the cause. LH and FSH help a clinician tell a testicle problem from a brain or pituitary problem. Depending on what they show, your clinician may order more tests. If kids matter to you, a semen analysis (a lab check of sperm count, movement and shape) belongs on the list too.
See what blood tests are required before TRT for the full list.
Not every clinic checks both
We checked the lab panels and testing descriptions published by these clinics on October 23, 2026. A test listed as possible monitoring is not necessarily included in the starting price.
If your clinic doesn't test LH and FSH, ask the evaluating clinician whether these or other tests are needed before a treatment decision. Confirm separately whether any needed second early-morning testosterone test is included in the quote. Our Labcorp and Quest guides explain the lab services; the clinician interprets the results. 20
When a specialist should come first
See an endocrinologist or urologist before choosing either drug if:
- You have new headaches or vision changes, especially with low testosterone or other hormone abnormalities.
- Your LH and FSH come back high.
- You've had testicular surgery, chemotherapy or radiation.
- Your results are far outside the normal range.
These can point to conditions that need their own workup. Sudden severe headache or sudden loss of vision needs emergency assessment. 32
The right TRT provider is not the same for every person — it depends on whether low testosterone has been properly evaluated, your symptoms and health history, your fertility plans, your state, your insurance or cash-pay preference, your treatment-format preference, your budget, and whether online or in-person care is the better starting point. Some situations belong with primary care, urology, endocrinology, reproductive urology, or urgent or emergency care when the symptoms warrant it. Because a general answer cannot resolve those for you, use TRT Provider Guide's Find My TRT Path tool to explore online TRT programs, and take the questions below to your clinician before you pay.
hCG or enclomiphene: which should you choose?
Enclomiphene raised LH, FSH and testosterone and kept sperm concentration steady in trials of selected men with secondary hypogonadism. hCG may fit a diagnosis such as selected hypogonadotropic hypogonadism due to pituitary deficiency, but being on TRT or not responding to enclomiphene calls for reassessment, not an automatic switch. If you're trying to conceive now, see a specialist first. 1 21 23

If enclomiphene fits your clinical plan: what Hone includes
Affiliate disclosure: TRT Provider Guide has commercial relationships with Male Excel and Taurus Meds. The site also discloses affiliate offers for Hone Health's TRT and ED services. We may earn a commission through labeled affiliate links; the links here lead to public information or our own guides. Compensation does not decide our conclusions. How affiliate links work.
We did not verify a current hCG offer on the Hone Health, Taurus Meds or Male Excel pages checked for this article. The hCG access table shows the offers we did find.
Hone Health is not the cheapest advertised enclomiphene plan here. Strut's starting price is $948 for twelve 30-day supplies before labs. Maximus lists a $1,199.88 twelve-month plan subtotal before labs and promotions. Hims's $990 upfront price covers ten months, with a separately listed $94 starting kit (full price table below).
So what does the extra cost buy? Hone's membership is built around testing:
- Its published Plus starting panel includes LH, which helps the clinician investigate the cause of low testosterone.
- Its Premium membership lists regular lab testing and physician consults.
- Its enclomiphene is labeled plainly as compounded and not FDA-approved.
- It also offers clomiphene, so you can ask how those different products would be considered.
- Its product page advertises cancellation at any time. Membership and medication orders must be canceled separately; canceling one does not cancel the other. 35
For the man who wants testing and follow-up in the same membership, that's the trade to compare.
Hone Plus's published starting panel doesn't list FSH. AlphaMD mentions LH, FSH and semen testing as possible monitoring, without promising all three from day one. Maximus lists LH and FSH as required enclomiphene markers and bills labs separately. The starting-lab table above shows the difference.
Does that sound like your situation? Read Hone Health's published enclomiphene offer and $65 test details →
Hone lists enclomiphene from $42/month plus membership. Its biomarker test and physician consult are listed at $65 (marked down from $165). Its Premium help page starts medication at $43/month, and Plus has a different entry fee and membership. Confirm the applicable plan and medication quote. It's a compounded drug and not FDA-approved. Prices are Hone's own, checked October 23, 2026. A prescription isn't guaranteed.
Is hCG or enclomiphene better for fertility?
Enclomiphene maintained sperm concentration compared with testosterone gel in selected trials; hCG fertility care and small TRT add-on studies address different questions. We did not identify a direct randomized comparison, and neither drug promises a pregnancy. If you're trying to conceive now, guidelines point you to a fertility workup before choosing a medicine. 21 22 23 27
What the research actually shows
Here are selected relevant studies, what they compared, and what they can't tell you:
The cited enclomiphene trials studied it instead of testosterone gel. The cited hCG add-on study looked at men receiving TRT. Those designs do not rank the medicines for every man.
What the guidelines say
- AUA/ASRM male infertility guideline (amended 2024):
- Clinicians should not prescribe testosterone to a man interested in current or future fertility.
- For infertile men with low testosterone, they may use hCG, SERMs (the drug class enclomiphene belongs to) or aromatase inhibitors. That recommendation is conditional, with Grade C evidence; it does not establish isolated enclomiphene as the preferred drug. 21
- Endocrine Society (2018): recommends against testosterone therapy in men planning fertility in the near term. 20
In plain words: a clinician may consider these medication classes in selected patients. That is not a blanket recommendation for either product, and neither is a guarantee.
Why hCG alone may not be enough for sperm
hCG covers the LH job. It doesn't supply FSH. Men whose pituitary problem started before puberty often need FSH as well to make sperm. Some men with later-onset hypogonadotropic hypogonadism also need FSH. That's specialist territory. 22
What neither drug can promise
Neither can promise a sperm count, a pregnancy or a timeline. A normal semen result doesn't prove fertility either. Pregnancy depends on both partners. Semen analyses help track sperm findings; a full fertility workup considers both partners and still cannot guarantee pregnancy. 21 22
Trying to conceive now? Do this first
- See a reproductive urologist, a urologist who specializes in male fertility.
- Ask about a semen analysis and any other evaluation needed.
- Bring your partner's fertility picture into the plan too.
- Ask the specialist directly about hCG, enclomiphene and FSH.
Read the Urology Care Foundation's guidance on finding a urologist → When you call, ask whether the doctor treats male infertility and what a new-patient visit costs.

Which raises testosterone more: hCG or enclomiphene?
We found no direct randomized trial that answers this. In the closest study, a randomized trial of nearly 300 men, hCG and clomiphene (enclomiphene's parent drug) both restored testosterone. What raises your number more depends on the underlying diagnosis, how your hormone system responds and the treatment plan. 26
If your pituitary works, both can raise testosterone. If it cannot respond, enclomiphene may be ineffective; hCG still needs responsive testicles and an appropriate diagnosis. 1 21
Do better numbers mean you'll feel better?
Not necessarily, and this is the gap clinic blogs skip. When Europe's drug regulator turned enclomiphene down in 2018, one reason was that its studies didn't look at whether symptoms improved. They measured blood levels and sperm counts. 6
hCG's symptom data in men is limited too.
A higher number on a lab report and feeling better are two different results. Ask your clinic how it will track both.
What about estrogen?
Many clinic pages claim enclomiphene "doesn't raise estrogen." The original 2014 trial says otherwise: estradiol (the main form of estrogen) rose on enclomiphene. hCG can raise it too; testosterone can turn into estradiol in the testicles and other tissues. 24 34 If estrogen side effects worry you, ask how your levels will be checked. See TRT and estradiol levels.
The parent drug, clomiphene, is the one tested against hCG in the trial above. 26
Is enclomiphene FDA approved? Is hCG?
Pregnyl is FDA-licensed for selected cases of male hypogonadotropic hypogonadism due to pituitary deficiency. Enclomiphene has no FDA-approved standalone product. Compounded enclomiphene is not FDA-approved, and its legal status depends on the applicable compounding requirements. 1 2 3
This is the biggest single difference between the two drugs. Here's the record:
So is compounded enclomiphene legal?
Category 1 is not blanket legal approval. The FDA's interim policy describes conditions under which it does not intend to take action against certain compounding. A pharmacy and prescription still have to meet applicable requirements. 3 10
Compounded drugs aren't reviewed by the FDA for safety, effectiveness or quality before they are marketed. The status can also change. Check the date on the linked FDA list. 3 11
Trials of enclomiphene do not verify every compounded formula sold today. Ask for the full ingredient list, the dispensing pharmacy and the evidence for that exact treatment.
Is "compounded hCG" a thing?
It does not use the same compounding exemptions as enclomiphene. Since March 23, 2020, hCG has been regulated as a biologic; the FDA separately addresses some mixing, dilution and repackaging of licensed biologics. Ask for the exact licensed hCG product and dispensing pharmacy. 9
Before you pay anyone for hCG, ask one question:
"Which product will you send me — Pregnyl, Novarel, or generic chorionic gonadotropin?"
A clear product name is a good answer. A vague one is a reason to walk away. Skip "research use only" vials, overseas sites and any seller that doesn't need a prescription. More in hCG online for men.
"Approved drug used off-label" vs. "not approved"
These sound alike but they're different:
- Off-label means an FDA-approved drug used for something its label doesn't list. hCG added to TRT is off-label. So is clomiphene in men, since its approved use is certain ovulation problems in women. 1 25 44
- Not approved means there is no FDA-approved standalone product. That's enclomiphene. A prescription does not change that status. 2 11
How much do hCG and enclomiphene cost?
Published prices vary by product, payment term and what care is included. The tables below separate advertised medication or plan subtotals from a complete first-year quote. For hCG, a yearly budget also needs the exact vial product, whole-vial purchases and its storage and discard instructions.
We show the arithmetic for each disclosed price and the period it covers. A full first-year quote also needs all required visits, repeat labs, medicines, supplies, shipping and the actual payment schedule. Where a clinic didn't publish a number, we say so instead of guessing.
Enclomiphene: advertised prices and comparable subtotals
Provider-stated prices checked October 23, 2026. The arithmetic below shows the stated term or 12-payment subtotal; it is a complete first-year quote only if every required cost and billing date is known. Twelve 30-day supplies cover 360 days, and thirteen four-week periods cover 364 days. Extra refills, labs or prepaid renewals may therefore change cash due during a calendar year. Promotional prices, prescribed formulation and state can also change the quote.
About Hone's numbers. The Plus help page lists a $45 initial test, $135/month membership and enclomiphene from $42/month: $45 + 12 × ($135 + $42) = $2,169. The main enclomiphene page shows a $65 test, Premium at $155/month and medication from $42: $2,429. The Premium help page instead starts enclomiphene at $43/month, giving $2,441. Both help pages describe follow-up labs and consults as included. Confirm the applicable plan, formulation and final medication price before paying.
What to compare: which physician visits and repeat labs are included, which cost extra, and how often they are required. Ask every clinic: "What will follow-up labs cost, and how often?"
hCG: published vial prices and costs still to confirm
hCG is commonly quoted by the vial, but a full-year budget needs the exact product, whole vials dispensed, refill dates and the pharmacy's instructions for storage and disposal. The provider supply estimates below are not discard dates. We have not converted them into annual costs where the product and usable life are unresolved.
Three things that change your real cost
- Wasted medicine. Once mixed, Pregnyl's label says to use it within 60 days, and Novarel's within 30 days (refrigerated). If you can't finish a vial in time, you pay for what you throw away. A claimed 20-week or 2.5-month supply is not the discard date of either mixed product. Ask your pharmacist how this affects your plan, and count whole vials rather than fractions. Follow the label on the vial you get. 1 7
- Follow-up labs. Some clinics publish a schedule but no lab price; others include testing or charge separately. A missing number isn't a zero.
- Commitments and refunds. PeterMD requires 6 months for enclomiphene. TRT Boss requires a 3-month prepay. TRT Nation's published policy reserves a no-refund/no-return policy. Read the terms before your card is charged. 36
Cancellation can involve more than one bill. Hone separates membership from medication cancellation, does not refund part of a membership month, and does not refund processed or shipped medication orders. AlphaMD says an unpaid balance for medicine already shipped still has to be paid after cancellation. 35 37
Does insurance cover either one?
- Enclomiphene: the figures above are cash prices, not evidence of insurance coverage. Ask your plan about the exact compounded formula and ask the clinic whether it bills insurance. Hone, for example, says it does not accept insurance or complete prior authorizations. 38
- hCG: coverage depends on the exact product, diagnosis and plan. UnitedHealthcare's commercial policy, effective September 1, 2026, lists criteria for pituitary-related hypogonadism but also excludes some hCG products from most benefits; fertility coverage depends on the member's plan. 39
- Medicare: basic Part D excludes drugs when used to promote fertility. That is not a blanket exclusion of hCG for every diagnosis. 40
Ask your plan about the exact product, diagnosis and prior authorization.
More detail: hCG cost for men and enclomiphene cost.
Taurus Meds' monthly plan: what to confirm
Say your clinician is discussing enclomiphene with you, and what you want now is one simple monthly price. Taurus Meds lists enclomiphene at $49 for labs and doctor review, then $149 a month. It describes that plan as "all inclusive" and includes a routine blood test. 46
Two things to know first:
- Taurus's page doesn't list LH or FSH in its blood test.
- Taurus's page doesn't clearly label the offer as compounded. Enclomiphene has no FDA-approved product, so ask for the exact formulation and dispensing pharmacy. 2 3
Confirm both before you pay, and ask how long the advertised $149 rate applies and what the renewal terms are.
If your clinician is considering enclomiphene, see our Taurus Meds review and what its quoted price includes →
Taurus Meds' prices and terms, checked October 23, 2026: "cancel or adjust at any time by contacting support." A prescription isn't guaranteed.
Where can you get hCG, enclomiphene, or both?
We found advertised hCG offerings at TRT Boss, AlphaMD, PeterMD, Marek Health and Maximus. TRT Nation also describes hCG in its own content, although current program details need confirmation. A missing listing means access is unverified, not that a clinic can never prescribe it.
Provider descriptions checked October 23, 2026. An advertised service does not guarantee eligibility, a prescription, availability in your state or a specific pharmacy product.
Best fit by situation (our take)
- Want to ask about hCG without TRT: TRT Boss, AlphaMD and Marek describe standalone use. TRT Boss says it uses commercial FDA-approved hCG; Marek names Pregnyl. Confirm the exact product and eligibility.
- Want fertility testing (LH, FSH and semen analysis): ask which tests are needed, available and included in the quote. The lab comparison is above.
- Want a provider that names its hCG product: Marek Health's catalog names Pregnyl.
- Want enclomiphene with LH testing and consults built in: Hone Plus lists LH and includes follow-up labs and consults; confirm FSH and fertility-specific testing separately (above).
- Have your own doctor who'll write an hCG prescription: ask a retail pharmacy for the exact prescription's price, then add your doctor's visits and labs before comparing.
We list these clinics because they publish hCG offerings. The table does not rank their clinical care.
What we found on Male Excel's Triclozene page
Male Excel sells an oral product called Triclozene. Its pages describe it two different ways:
- The page title calls it "Clomid for Men … Clomiphene Citrate."
- The page heading calls it enclomiphene citrate, combined with "two thyroid-boosting ingredients" the page doesn't name.
- Male Excel's pricing page calls it "clomiphene citrate with thyroid," lists "Thyroid" and selenomethionine, and shows two prices, $95 and $120 a month.
We could not resolve the discrepancy from the public pages. Clomiphene and enclomiphene are different products, and any added ingredients need their own clinical reason and safety assessment. If you're considering Triclozene, ask Male Excel in writing three things before you pay:
- Which molecule is in it.
- The complete ingredient list and strengths, including what "Thyroid" means in this product.
- Which price applies to you.
Before you sign up anywhere: 3 questions that protect you
- "Is this enclomiphene or clomiphene, and is anything else in the capsule?"
- "Which hCG product will you send: Pregnyl, Novarel or generic?"
- "Will you test LH, FSH and, if I want kids, a semen analysis?"
The full list is below.
hCG vs enclomiphene side effects: which is safer?
The evidence doesn't make either one the "safer" choice for everyone. hCG's label lists headache, mood changes, fluid retention and breast growth. Enclomiphene has no FDA-approved label, and Europe's regulator flagged a risk of blood clots. 1 2 6
The clomiphene warnings come from clomiphene's label. Enclomiphene has no FDA-approved label of its own, so we don't pin them on it. Enclomiphene is one component of clomiphene, but risks and side-effect rates cannot be assumed identical. 25
Get help right away if you notice
- Chest pain or serious trouble breathing: call 911.
- Sudden loss of vision: get emergency care.
- New swelling, pain, warmth or redness in one leg: get seen promptly. These can be signs of a blood clot.
- New blurry vision: seek prompt medical assessment and contact your prescriber.
- Sudden flashes, a sudden increase in floaters, or a curtain or shadow over your vision: seek immediate eye or emergency care. 30
These symptoms have many possible causes. A comparison page can't diagnose them. Don't wait them out. 30 31 32
More on breast changes: TRT and gynecomastia.
Already on TRT: should you add hCG or switch to enclomiphene?
hCG has been studied as a TRT add-on, but the AUA/ASRM guideline says the evidence is too limited to recommend this for fertility preservation. Enclomiphene's trials do not establish what happens when it is added to ongoing TRT, and neither approach guarantees fertility. Talk to your prescriber before changing anything. 21 23
In the small 2013 study of men on TRT plus low-dose hCG, none dropped to zero sperm during follow-up, which averaged 6.2 months. A short 2005 experiment showed hCG kept testosterone levels inside the testicles up while men took testosterone. Neither study proves every man on TRT will keep his fertility. 27 28
There's no straight swap from an hCG prescription to enclomiphene. A shot and a pill aren't interchangeable, and changing one means a prescriber has to rethink the whole plan.
For the broader comparison, see enclomiphene vs TRT.
Coming off TRT to try for a baby?
A specialist may consider hCG and other treatments when assessing persistent hormone or sperm suppression after TRT. Enclomiphene's trials do not establish a standard post-TRT recovery plan. There is no single plan for everyone, and recovery time varies a lot from man to man. This belongs with a clinician who checks your hormones and semen along the way; full recovery is not guaranteed. Please don't build your own restart plan. 21 23
See can you stop TRT.
Can you take hCG and enclomiphene together?
We did not identify a clinical trial of isolated enclomiphene plus hCG in this targeted search. In the closest trial, a three-month study of clomiphene and hCG, men reported better symptom scores on the combination, while testosterone results looked about the same. Whether to combine is a prescriber's call; it isn't a do-it-yourself stack. 26
If a clinic offers you both, ask:
- What's the diagnosis-specific reason for using both?
- What result are we trying to reach?
- How will we monitor it?
- What does the second drug add to the cost?
Do state laws, telehealth rules or sports rules affect hCG or enclomiphene?
Neither drug is federally scheduled, but state prescribing and pharmacy rules still matter. Some states classify hCG as a controlled substance, and a clinician must have authority to treat you where you are. Under WADA rules, hCG is prohibited in men and enclomiphene falls within the prohibited SERM class. 12 13 14 15 16
PeterMD's published hCG exclusions are AK, AL, AR, CA, DC, HI, IA, ID, IN, KY, LA, MA, MD, MI, MS, NC, ND, NE, NH, NJ, NV, SC, TN, VA and WA. These are provider limits, not a list of states that ban hCG. Confirm access for your location before paying. 41
If you compete under anti-doping rules, check the exact medicine with your sports body and ask whether a therapeutic use exemption is required. A prescription alone does not grant one. 18
If you're in the military, discuss either drug with your military healthcare provider and make sure prescribed medicines are recorded in your military health record. 19
What if fertility isn't a priority for you?
If kids aren't in your plans and your low testosterone is confirmed, these two drugs may not be your most direct option. Testosterone replacement therapy is an established treatment after the cause, contraindications and monitoring needs are assessed. It can lower sperm production, so fertility plans remain part of that assessment. 20
Ask yourself honestly: are you looking at hCG or enclomiphene because you want to protect fertility, or because someone told you they're "safer" than TRT? The research above doesn't support "safer." If fertility isn't a factor, it's worth comparing TRT on its own terms.
Male Excel is a TRT program we work with. Its published pricing separates a $99 monthly membership from medication and shipping. Its FAQ says repeat blood tests may be needed at six months and one year; confirm your monitoring schedule and lab charges. It's not the place to start if you want kids soon. 42 43
Compounded testosterone is not FDA-approved. Ask Male Excel which exact product its quote covers. 11 42
If that's you, review Male Excel's current membership and TRT medication costs →
TRT can lower sperm production. Don't choose it if you want children in the near future. Testosterone is a Schedule III controlled substance and needs a valid prescription.
What should you ask before paying for hCG or enclomiphene?
Before you pay for either, get four things in writing: the diagnosis being treated, the exact product, the result you're aiming for, and the full first-year cost. The questions and worksheet below help you collect those answers from a clinician and pharmacy. They don't pick a medicine for you.
Monitoring and timing depend on the diagnosis and exact medicine; there is no FDA-approved enclomiphene monitoring schedule. 1 2
10 questions to take to your appointment
- What's causing my low testosterone? Is it my testicles, my brain or pituitary, or something else?
- Do I need a repeat morning test, LH and FSH, or other tests first?
- How do my plans for kids change my options?
- Should I get a semen analysis, or see a reproductive urologist?
- Exactly which product will I get? Is it enclomiphene or clomiphene, and is anything else in it? For hCG: Pregnyl, Novarel or generic?
- Which study supports this drug for someone with my diagnosis?
- What will we track: symptoms, blood levels, semen results, or all three?
- What side effects should I watch for, and who do I call?
- What happens if it doesn't work, or if my plans for kids change?
- What's the full first-year cost, including follow-up labs? And how do refills, cancellations and refunds work?

Subtract only confirmed credits, and don't count a bundled service twice. Then list what's still missing. A quote with blanks isn't cheaper. It's unfinished.
Keep the payment dates too. A prepaid renewal can cover time beyond year one, so the cash charged during the year may differ from the cost of that year's care.

How we compared hCG and enclomiphene
We checked regulator records, drug labels, guideline recommendations and study reports or abstracts. Then we checked each clinic's own pricing pages on October 23, 2026. We kept four things separate: what's been verified, what clinics say about themselves, what studies measured, and our own editorial judgment.
We follow our documented method, How We Review TRT Providers. It separates verified facts from provider claims, shows disclosed costs and identifies what is still missing from a complete comparison. This comparison does not assign numeric scores, and listing a clinic here isn't an endorsement.
What we actually verified
Date checked: October 23, 2026
Sources checked:
- FDA records: the list of products moved to biologic status (March 23, 2020), the compounding categories list (updated May 14, 2026), and the June 8, 2022 advisory committee transcript
- Europe's EnCyzix refusal decision
- The Pregnyl, Novarel and clomiphene labels
- Relevant recommendations in the full AUA/ASRM male infertility guideline (2024 amendment), the Endocrine Society's 2018 guideline and current EAU male-infertility guidance
- The original Kim (2016) report and published reports or abstracts for Wiehle (2014), Habous (2018), Hsieh (2013) and Coviello (2005); the primary record for the Sorokin (2016) conference abstract
- Federal and selected state controlled-substance rules, the federal telehealth extension, and the 2026 WADA list
- A UnitedHealthcare hCG coverage policy
- Each clinic's own pages: Hone Health, Taurus Meds, Male Excel, TRT Boss, AlphaMD, PeterMD, Marek Health, Maximus, Strut, Hims, TRT Nation and Blokes
- SingleCare and Drugs.com price listings used in the tables
Confirmed from primary sources: FDA and EMA decisions, hCG's labeled uses and storage windows, the cited guideline statements and study findings, federal scheduling, the named state examples, and WADA status.
What clinics say about themselves (not independently tested): prices, memberships, vial-life estimates, what's in each lab panel, and refund and cancellation terms.
What we could not confirm:
- Each person's required testing and any charges not included in the published plan
- Which Hone plan and medication price apply to an individual; public pages show different scenarios
- How long Taurus Meds' advertised rate applies, its renewal terms, and whether its blood test includes LH or FSH
- The exact contents of Male Excel's Triclozene
- How many hCG vials any individual prescription uses
- Whether an individual plan includes every needed repeat morning test, semen analysis or extra visit
- Assigned clinician and pharmacy licenses, exact dispensing arrangements, and patient-specific refill, shipping or medical-record release details
What we did not do: take either drug, complete a clinic intake, test cancellation, or contact clinics or pharmacies. This page wasn't reviewed by a clinician.
Not covered: dosing, injection technique, restart protocols, or any non-medical use.
Prices and rules change. Use the verification date above and get the current terms in writing before you pay. See our editorial standards, privacy policy and corrections process.
Frequently asked questions about hCG vs enclomiphene
Neither medicine is best for everyone, and a prescription does not guarantee fertility or symptom relief. These answers cover the differences that affect the next care decision.
Is hCG better than enclomiphene?
Neither is better for everyone. If your pituitary works, enclomiphene can work, and it's a pill. If the pituitary response is impaired or you're on TRT, the diagnosis and fertility plan need review; neither situation automatically selects hCG. FDA-licensed hCG products are available; enclomiphene has no FDA-approved product. 1 21 23
Is enclomiphene FDA approved?
No. The FDA declined to approve it in 2015, and Europe refused it in 2018. Compounded enclomiphene is not FDA-approved; its Category 1 listing is part of a conditional enforcement policy, not blanket legal approval. 3 4 6 10
Is enclomiphene the same as Clomid?
No. Clomid is clomiphene, a mix of two forms of the molecule. Enclomiphene is one of those forms, sold separately. Clomiphene is FDA-approved for certain ovulation problems in women; use in men is off-label. 25
Can enclomiphene replace hCG?
Not automatically. It only works if your pituitary can respond, and it isn't established alongside TRT. Any switch is a prescriber's decision, not a dose conversion.
Does enclomiphene raise estrogen?
It can. In the original 2014 trial, estradiol rose on enclomiphene. Pages that say it "doesn't raise estrogen" aren't matching that study.
Does enclomiphene shrink your testicles like TRT can?
Enclomiphene increased LH and FSH in selected trials. A 2016 conference abstract also examined testicular volume, but that limited evidence does not guarantee that an individual's testicular size will be preserved. 23 29
Does hCG replace FSH?
No. hCG covers the LH job. Some men with hypogonadotropic hypogonadism need additional FSH treatment from a specialist for sperm production. 21 22
Which is cheaper, hCG or enclomiphene?
Neither is automatically cheaper. Enclomiphene plans differ in membership, lab fees and prepaid terms. For hCG, compare the exact product's whole-vial refill cost, storage and discard timing, visits and labs. The published subtotals above help you start; a written complete-care quote makes the comparison.
Is hCG the same as TRT?
No. hCG makes your testicles produce testosterone. TRT supplies testosterone from outside, which turns your own production down.
Will either one make me fertile?
Neither can promise that. Sperm results, your underlying condition and your partner's health all matter. Semen testing and a fertility workup help assess the situation, but cannot guarantee pregnancy. 21 22
Do I need a prescription?
Yes, for both. Be wary of any seller that skips the prescription.
Are hCG and enclomiphene banned in sports?
Under WADA rules, hCG is prohibited in men and SERMs, the class enclomiphene belongs to, are prohibited at all times. Check your sports body's rules and therapeutic-use exemption process. 16 18
