Male Excel vs Evolve Telemedicine comes down to state access, price visibility and commitment. Male Excel lists 39 states and an injection-plus-thyroid bundle from a $219 monthly equivalent before extras; Evolve lists broader access but requires a quote for annual care. Neither is the first step for an unconfirmed diagnosis or near-term fertility plans.
That's the short answer. The longer one has two surprises in it. One is a single line on Male Excel's homepage that every first-time buyer should read before booking. The other is a clause in Evolve's terms that decides whether you ever see your care-plan money again. We'll get to both.

- Consider Male Excel if you live in one of its 39 listed states, your low testosterone has been properly evaluated, and you want published core fees with a monthly membership. Shipping, any extra testing, and your exact prescription still need a quote.
- Consider Evolve if you live in Alabama, Alaska, Arkansas, Connecticut, Hawaii, Idaho, Louisiana, Minnesota, Mississippi, New Hampshire or Rhode Island, subject to confirmation that it offers TRT where you will be. Also consider it if you prefer a lab blood draw and can accept an annual plan once you've seen its price and payment schedule in writing.
- Pick neither yet if your low testosterone has not been properly evaluated, you want children soon, or you need insurance to pay.

Evolve's homepage, men's TRT page, treatments list, testosterone and enclomiphene pages, FAQ, Commitment to Patients page and Terms & Conditions. Both companies' Trustpilot profiles. The DEA/HHS telemedicine rule, the Endocrine Society's testosterone guideline, the AUA/ASRM male-infertility guideline, and FDA guidance on compounding, thyroid medicines and testosterone labeling.
Confirmed as published: Male Excel's listed price components and separate membership/medication billing. Both service-area lists. The lab collection methods each describes. Both refund and arbitration clauses. Each company's statements about compounded testosterone. Male Excel's FAQ names two pharmacies. These are document checks, not proof that each service is delivered as described.
What the companies say but we didn't test: Male Excel's 60-day check-ins, 6-month labs, 54-marker member panel and "one provider" promise. Evolve's "regular monitoring" and its description of its pharmacies as "FDA-inspected."
What we could not confirm: Evolve's annual plan price, payment schedule, complete required-lab price or patient-specific medication prices. Male Excel's shipping amount and full first-year bill. Whether two separate morning diagnostic tests and a baseline red blood cell check (hematocrit) are included for a new patient at either company. The pharmacy and exact product for an individual prescription, including Male Excel's injection.
What we didn't do: Sign up, buy treatment, obtain a personal quote, test support or cancellation, or check an assigned clinician's or pharmacy's license. This is a public-source comparison, not a hands-on review.

Some readers should stop comparing brands and start with a clinician. If your low testosterone has not been properly evaluated, you want children soon, need insurance, or have certain heart, prostate, blood or sleep problems, a different first step belongs ahead of a paid program.

Start somewhere else if…

- You have prostate or breast cancer history, a high red blood cell count, untreated severe sleep apnea, or a heart attack or stroke in the past six months. These need a clinician who can examine you. That's based on the Endocrine Society's guideline.
The right TRT provider is not the same for every person — it depends on whether low testosterone has been properly evaluated, your symptoms and health history, your fertility plans, your state, your insurance or cash-pay preference, your treatment-format preference, your budget, and whether online or in-person care is the better starting point. Some situations belong with primary care, urology, endocrinology, reproductive urology, or urgent or emergency care when the symptoms warrant it. Does Male Excel or Evolve Telemedicine serve your state?
Both companies' published service lists include the same 39 states. Evolve also lists 11 states that Male Excel excludes: Alabama, Alaska, Arkansas, Connecticut, Hawaii, Idaho, Louisiana, Minnesota, Mississippi, New Hampshire and Rhode Island. Its list also includes DC, Puerto Rico and Guam, but does not guarantee every treatment in every location.
No state on Male Excel's published list is absent from Evolve's general service list.
We built this by lining up Male Excel's published exclusion list against the service list in Evolve's Terms, section 6, which Evolve dates as effective January 26, 2026. Then we added Evolve's lab rules for four states.

The counts: Male Excel excludes 11 of the 50 states, leaving 39. Evolve's general list names all 50 states plus three other jurisdictions. DC, Puerto Rico and Guam are not included in the 50-state count.
What "listed" means. A listed state means the company says it offers telehealth there. It doesn't promise a clinician will prescribe testosterone to you, and neither list is a license audit. The four states where Evolve's lab rules get confusing
In New York, New Jersey, Connecticut and Rhode Island, Evolve's own pages disagree.
- Evolve's Terms (section 33b) say patients in those states must order and pay for lab work directly through Labcorp OnDemand, then upload results.
- Evolve's Commitment to Patients page says Labcorp services aren't available in those states and tells patients not to pay Labcorp directly.
Both can't be the whole story. If you live in one of these four states, ask Evolve which ordering process applies to you and what it costs before you buy any test. This is a conflict in Evolve's instructions, not proof that Labcorp has no facilities in those states. Connecticut and Rhode Island are also Male Excel-excluded states. How do Male Excel and Evolve work, step by step?
Male Excel lists a $99 consultation and a finger-prick blood kit you mail back. Evolve offers a free intake call, with physician care under a paid annual plan and clinician-ordered labs. Confirm the appointment order and payment dates: a free coordinator call is not the same as a free physician visit.
From far away they look alike. Up close, who you talk to first and when your money moves are different.

Who you're really dealing with
Neither company is itself a medical practice. Both run a platform, and separate clinicians give the care.
- Male Excel (legal name Male Excel Inc., doing business as Excel Medical) says care is provided by physician-owned practices it names: Male Excel Medical, P.A., plus related practices in California, Delaware, Kansas and New Jersey. Its testing runs through an affiliated lab. Male Excel Terms, section 1
- Evolve (E-Volve Company, Inc., doing business as Evolve Telemed) says its clinicians are "independent Providers" who are not its employees. It also says it "does not supervise or control" their medical decisions. Evolve Terms, sections 1 and 3
Why this matters to you: at either company, ask for your clinician's full name and state license before paying. Use the Federation of State Medical Boards' state-board directory to find the physician licensing authority. For a nurse practitioner or physician assistant, use the relevant state licensing board. A named medical leader is not necessarily the clinician who will treat you.
Male Excel vs Evolve Telemedicine cost: which is cheaper?
We can't say which is cheaper, because Evolve doesn't publish an itemized TRT price. Male Excel's advertised injection-plus-thyroid bundle gives a $2,727 annualized subtotal from disclosed components, not a complete first-year bill. Evolve's annual care plan, lab work and medication prices all need a written quote before a fair comparison is possible.
Male Excel's published cost components, line by line
Male Excel posts the core pieces: a $99 consultation with an at-home test, a required $99 monthly membership, and medication billed separately. Its homepage says the membership includes provider access, 60-day check-ins and blood testing every six months. The prices below are for the advertised bundles, not a recommendation to add thyroid medicine to testosterone.

Both prices appear on Male Excel's pricing page; its Triclozene page displays $95. This is a pricing conflict, not a verified dose-based range. Triclozene isn't testosterone; more on that below.
Four things the table can't show:
- "Starting at" means starting at. Male Excel's FAQ says a dose change changes the price.
- Medication shipping is extra. Male Excel doesn't post the amount. Unknown isn't zero. Its home-test page says return postage for the kit is included; that does not establish free medication delivery.
- Your card gets charged on two schedules. The membership bills monthly. Medication is advertised in 60-day blocks and billed every other month. That creates smaller and bigger billing months, but the exact dates need confirmation. Six 60-day supplies cover 360 days, not 365; do not treat a monthly equivalent as proof of a full year's medicine.
- The advertised testosterone bundles include thyroid medicine. You're being quoted for two drugs, not one. Male Excel's homepage says thyroid is used when clinically appropriate, so the bundle is not proof every patient needs it. Ask the clinician what diagnosis supports the thyroid medicine, whether a testosterone-only plan is available, and its price. Male Excel homepage · Pricing
The thyroid warning is bigger than an equivalence question. Male Excel describes desiccated thyroid, made from animal glands. FDA says animal-derived thyroid medicines are not FDA-approved and are not eligible for compounding because they are regulated as biological products. FDA is applying a risk-based enforcement approach; this does not establish that a particular Male Excel prescription or pharmacy has been found in violation. Ask for the exact thyroid product, manufacturer and approval status, and discuss any thyroid treatment with your clinician. FDA's current thyroid statement
A refill-date example, not a quote: if seven 60-day medication orders are charged inside your first treatment year, the same disclosed prices would produce $2,967 for the injection bundle ($99 + $1,188 + 7 × $240) or $3,135 for the cream bundle ($99 + $1,188 + 7 × $264), before the same excluded costs. Seven fills cover 420 days, so some supply extends beyond that year. Whether a seventh payment actually falls inside your year depends on the first fill and billing dates; ask for the schedule rather than choosing whichever subtotal looks cheaper.
That billing rhythm is one complaint worth planning for.

Evolve's price: three different numbers, no quote
Evolve doesn't publish an itemized TRT price. Worse, its own pages give three different ranges:

None of these is an itemized Evolve TRT quote or explains whether the annual care plan, required labs and medication are included. Evolve's terms say your total includes your provider's fee plus Evolve's platform fees. They also say its medication list and ingredient costs change with market conditions. Some Evolve patients say the numbers they got were clear.

The trade-off we'd want someone to tell us
Male Excel's $99 membership is not a $99 all-in TRT plan, and it won't send your prescription to an outside pharmacy. For the man who hates surprise bills more than he hates a higher sticker price, the trade can still make sense. Male Excel gives you more of the core price before intake, not a guarantee against extra charges. The membership is monthly. Medication billing is separate, and stopping future charges requires cancelling before the next charge or order is processed. Evolve describes annual care without publishing the plan price or payment schedule. Get the missing numbers from either company before committing.

What are you committing to, and can you get your money back?
Male Excel bills its membership monthly, bills medication separately, and says you can cancel anytime. Its terms say all sales are final, with a separate conditional refund of your first three months' membership. Evolve sells an annual care plan that becomes non-refundable after your first doctor visit, and its compounded medications can't be refunded or returned.
This is where the two companies differ most. We read both sets of terms clause by clause.

Male Excel's guarantee, in plain words
The headline says, "You will see results, or we will refund your membership fees." The fine print is narrower. Here's what it takes:
- You're a new member who follows the protocol for 90 days in a row.
- You attend scheduled appointments and follow your provider's plan. Male Excel may ask for proof.
- "Improvement" is measured by Male Excel's own assessment and what you report to your provider.
- You ask for the refund within 30 days after day 90.
- It covers membership fees only, meaning 3 × $99 = $297 at most. It doesn't cover the $99 startup, medication or shipping.
- Male Excel can change or end the guarantee at any time, but not for members already enrolled under the old terms.
It's a published refund condition, which we did not find for Evolve's care plan after the first physician visit. It's just not "all your money back." Full guarantee terms
Evolve's gap: what if the doctor says no?
Evolve's FAQ says it can't guarantee treatment. Its terms say the annual plan becomes non-refundable after your first physician consultation. We found no exception for the case where that physician decides TRT isn't right for you. The terms also don't say what happens if you cancel before that visit. Evolve reviewer Carly Wiltse reported account credit rather than a refund after an order was cancelled because a medication became unavailable; her review was posted February 3, 2026. Evolve replied on February 9 inviting her to discuss a resolution. That's one person's report about a medication order, not proof of the TRT care-plan policy, but it's worth asking about. Cancelling isn't the same as a refund
At either company, ask two separate questions:
- "What will you stop charging me?"
- "What, if anything, will you give back?"

For Male Excel, name both the membership and medication renewal in your written cancellation request. Its FAQ describes a 48-hour action window after a refill reminder in its instructions about changing treatment formats; that is not a complete cancellation procedure. Ask support to confirm the effective cancellation date, any pending order, and that no further membership or medication charges will be made. For records, ask which treating medical practice holds them and how to request a copy or transfer. Evolve's terms state a right to access health information but do not set out a complete request workflow. We did not verify either company's response time for a full chart request. HHS says patients generally have a right to inspect and receive records held by HIPAA-covered providers; unpaid treatment bills are not a reason to withhold that copy. How does each one test you before prescribing testosterone?
Male Excel describes a five-marker finger-prick kit and says its providers may prescribe before lab results arrive when clinically appropriate. Evolve describes clinician-ordered Labcorp blood draws, but a blood-draw format does not prove repeat testing is included. For a first diagnosis, the Endocrine Society requires symptoms or signs plus consistently low testosterone, confirmed with repeat fasting morning testing; it also calls for a baseline hematocrit check before therapy.
This is the section we'd read twice.
What the guidelines ask for
The Endocrine Society says a clinician should diagnose low testosterone only when both of these are true:
- You have symptoms or signs that fit, like low sex drive or low energy.
- Your testosterone is consistently low, with a low morning total-testosterone result confirmed on a separate morning test. The Endocrine Society specifies fasting samples and an accurate, reliable test method. Total testosterone means all the testosterone in your blood.
A number such as 300 ng/dL is not a universal diagnosis. Your symptoms, repeated results, test method and clinical context matter; some cases also need an accurate free-testosterone measurement. Before treatment, the clinician should assess hematocrit, usually reported in a complete blood count alongside hemoglobin. Hematocrit is the share of your blood made of red blood cells. Testosterone can raise it, which is why clinicians check it before and during treatment. A few more terms you'll see:
- PSA (prostate-specific antigen) is a prostate blood test used in screening.
- LH and FSH (luteinizing hormone and follicle-stimulating hormone) are signals from your brain to your testicles. They help a clinician figure out why testosterone is low.
PSA testing depends on age, risk and the clinician's screening discussion; a five-marker panel is not a complete safety assessment for every man. These are clinician-led decisions, not instructions to diagnose yourself from a lab report. Endocrine Society guideline
Male Excel's test, described fairly
Male Excel's $99 starter kit uses blood spots from a finger prick. You mail it to Excel Medical Labs, which Male Excel says is its own CLIA-certified lab in North Carolina. The kit measures five things: total testosterone, estradiol, DHEA-S, free T3 and PSA. Male Excel says results come about five business days after the lab gets the kit. Male Excel home test · FAQ
Male Excel also advertises follow-up blood tests every six months. Its Excel Advantage page lists testosterone, estradiol, DHEA, free T3 and PSA; the starter page specifically names DHEA-S. It offers a "360" panel of 54 biomarkers to members only, but it doesn't say whether that panel is required before a first prescription. Excel Advantage · Male Excel homepage
Two things stand out:
- Hematocrit and LH aren't in the five-marker list, for either the starter kit or the published six-month panel. Male Excel may check them another way. We couldn't confirm it, so ask.
- Here's the homepage line we mentioned at the top. Male Excel says its providers "can prescribe based on your symptoms and health history before lab results arrive," when clinically appropriate. Its separate, non-TRT Triclozene page also advertises "immediate approval" after the online consultation; that is not a testosterone-prescribing policy or a guarantee of clinical eligibility. What to do with that: for a first diagnosis, ask how Male Excel confirms consistently low morning testosterone and checks treatment risks before prescribing. The homepage line does not tell us whether prior results are available in a given case. Repeat confirmation is not an optional marketing preference. Evolve's test, described fairly
Evolve's homepage says step one is a virtual visit with a board-certified provider, who then orders "a comprehensive lab panel." You get blood drawn at a Labcorp location using Evolve's order, then review results with your provider. Evolve doesn't publish what's in the panel or what it costs. If you've had recent labs, Evolve says they often work, as long as they include the markers its clinicians need. Evolve homepage · FAQ
The trade-off: a venous blood draw means a trip to a lab. Evolve reviewer Ann Ostrowski reported slow lab results and refill delays in a review posted May 28, 2026. That's an individual experience, not a measured turnaround. Evolve on Trustpilot
Evolve's published process does not establish that two separate morning testosterone measurements and a baseline hematocrit check are included in the quoted price. Ask for the exact panel, repeat-test plan and turnaround before paying. Evolve FAQ · Evolve terms
Finger prick vs blood draw
A finger-prick kit needs no lab trip. Collection format alone does not establish a test's accuracy or whether it is suitable for confirming a diagnosis. What medicine would you get, and is any of it FDA-approved?
Both companies market compounded testosterone, which is not FDA-approved, but the exact product for your prescription still needs confirmation. Male Excel lists daily injections and twice-daily cream; its advertised bundles include thyroid medicine, while its homepage says thyroid is used when clinically appropriate. Male Excel names two pharmacies in its FAQ; Evolve does not name partners on the pages we checked.
What "compounded" means
Compounding means combining, mixing or altering ingredients to create a medicine for a medical need. The FDA says compounded drugs are not FDA-approved. That means the FDA doesn't review their safety, effectiveness or quality before they're sold. Compounded doesn't mean "generic," "the same" or "safer." It doesn't automatically mean worse, either. - Male Excel says it uses "bioidentical testosterone compounded in the United States." Male Excel homepage
- Evolve says it partners with "FDA-inspected compounding pharmacies to compound all treatments." An inspected pharmacy isn't the same as an approved drug. Evolve Terms, section 33(c)
Both use the word "bioidentical." That's marketing, not a quality rating. It does not establish approval, lower risk or equivalence to an approved drug. FDA on compounded drugs
FDA-approved testosterone is a separate choice
If you want an FDA-approved testosterone product, we did not verify a dispensing path for one through either program. Start with your own doctor or a urologist and ask for the exact product and pharmacy. Our list of FDA-approved testosterone products shows what exists. A familiar ingredient name such as testosterone cypionate does not establish the approval status of a particular vial.
Male Excel: daily testosterone formats and the thyroid question
Male Excel's program is built around small daily doses:
- Injection: testosterone cypionate, once daily, with short insulin-type needles under the skin.
- Cream: Lipoderm testosterone cream, twice daily.
- Thyroid: desiccated thyroid tablets in the advertised bundles; ask whether they are indicated and what exact product would be dispensed.
Male Excel says daily dosing keeps levels steadier. That's the company's view. We haven't seen head-to-head proof that daily dosing gives better results than weekly. Evolve: clinician-prescribed plans and a bigger menu
Evolve lists testosterone as injections or topical cream. It lists 33 treatments overall, including peptides, weight-loss drugs, hair and skin products, and women's hormone care. Its terms allow orders covering up to 140 days for most medications, but the prescribed quantity and product's usable life still need to match. Evolve treatments · Terms, section 33(h)
One caution from Evolve's terms. They mention that pharmacies may label compounded medicine to be discarded after 28 days, then say it "may last well beyond" the label. Follow the date and storage instructions on your pharmacy label. A compounded medicine's beyond-use date, a manufactured product's expiration date and a discard-after-opening instruction are not interchangeable. Evolve's terms also say that when an order exceeds the use date, the pharmacy sends the usable quantity and holds the rest as a refill. The pills aren't TRT
These are non-testosterone options, not alternative formats of TRT.
Both companies sell a pill that tries to get your own body to make more testosterone. Neither pill is testosterone replacement.
- Male Excel's Triclozene is a blend with thyroid ingredients. Its pricing page and page title call the main ingredient clomiphene citrate. The same product's headline and description call it enclomiphene citrate. Those are different drugs. Clomiphene is a mix of two forms, and enclomiphene is one of them. - Evolve's enclomiphene page calls it a "Capsule" in the header and says "tablet form" in its FAQ. It says the drug helps "improve fertility." Evolve enclomiphene page
There is no FDA-approved enclomiphene product. FDA-approved clomiphene is approved for women's ovulation problems, so using an approved clomiphene product for men's low testosterone is off-label. That means a doctor is using it outside what the FDA approved it for. A compounded clomiphene or thyroid combination does not inherit the approved clomiphene product's FDA approval. Safety at home
Ask for the pharmacy name, the exact medicine and formulation, its approval status, and product-specific instructions before the first shipment. Here is what the companies disclose publicly:

Testosterone applied to the skin can transfer to a partner or child. Male Excel publishes a boxed-warning section about this and describes covering and washing the application area. Follow your clinician's and dispensing pharmacist's instructions for your exact product; this warning does not make a compounded cream FDA-approved. Male Excel's terms also say its pharmacies don't use child-resistant packaging, so store medicine where kids can't reach it. Who answers your questions and manages your follow-up care?
Male Excel advertises unlimited e-visits, one dedicated provider and a check-in every 60 days; it does not define every e-visit as a live video appointment. Evolve's annual plan covers a first consultation, an annual visit and follow-ups your provider requests, plus unlimited access to a care coordinator. Unlimited coordinator access isn't unlimited doctor access.
We didn't test either company's response times.

Customer reports raise practical questions about those structures. A Male Excel reviewer said a staff member "took the time to answer all of my questions." Ron, Trustpilot, experience dated September 18, 2026. Another said messages sent through the portal went unread. On Evolve's side, reviewer Rachel Peddle liked her doctor but said the call felt rushed, and noted she gets two calls a year. Ann Ostrowski described allowing 20 to 25 days, not two weeks, for refills in her May 28, 2026 review. These profiles include services other than men's TRT. Our read: if you want to talk to a clinician often, Male Excel's structure promises more of it. If you mainly want someone to handle the logistics, Evolve's coordinator model is built for that. Ask either company how fast a clinician answers a medical question.
Do not treat a refill reminder as a delivery guarantee. Male Excel describes a 48-hour action window after its reminder in the FAQ about changing treatment formats. Evolve asks you to contact your coordinator at least two weeks before running out, while its terms allow about 10 business days after payment just for tracking. Shipping comes after processing. Ask who handles a delay, whether the clinician can arrange a safe continuity plan, and which costs apply; do not change treatment on your own to stretch a supply. Where do Male Excel's and Evolve's own pages disagree?
Both companies publish pages that contradict their other pages, and both have gaps in what they disclose. These don't prove anything about the care. They do tell you which answers to get in writing, and they show how carefully each site is kept up.

Our take: Male Excel publishes more core price details and more visible safety material on the checked pages. Evolve publishes a dated, fuller service-area list. Neither tells you how good your clinician will be; a conversation, license check and review of the actual care plan are still needed.
Current safety information matters more than a long warning block. In February 2025, FDA required testosterone-product labeling changes that removed the boxed-warning language about increased cardiovascular outcomes and added or strengthened blood-pressure warnings. That does not erase other risks, remove all boxed warnings, or validate compounded preparations. What do customers say about Male Excel and Evolve?
Male Excel shows 4.5 stars from 6,577 Trustpilot reviews and invites customers to leave them. Evolve shows 4.1 stars from 54 reviews and hasn't invited reviews recently, according to Trustpilot. Reviews describe service experiences, not medical results, and the two pools were collected differently.

Praise in the reviews read: Male Excel reviewers mention an easy process and responsive staff. Evolve reviewers mention attentive coordinators and thorough doctors.
Complaints in the reviews read: Male Excel reviewers mention the two-schedule billing, the price, and the member portal. Evolve reviewers mention price increases, slow labs and refills, and shipping delays.
Two cautions. Male Excel actively asks customers for reviews, so the two review pools are not collected in the same way; that alone does not establish how much either score changes. Several of Evolve's five-star reviews shown on the profile were posted during December 8–16, 2025. These are company-wide reviews, including services other than men's TRT, not a representative clinical-outcomes sample. Who should choose neither Male Excel nor Evolve?
If you want children soon, if your low testosterone isn't confirmed, or if you have a prostate, blood-count, sleep-apnea or recent heart problem, start with a clinician who can examine you. A brand choice can wait. Those questions can't.
If you want children in the next year or two
Testosterone from outside your body can lower sperm production. That's called suppressing spermatogenesis, the process your body uses to make sperm. The Endocrine Society's guideline recommends against starting testosterone in men planning a family soon. The AUA/ASRM male-infertility guideline says testosterone alone should not be prescribed to a man interested in current or future fertility. This is not a risk limited to nonmedical high-dose use. No one can promise you'll become infertile. No one can promise you won't, or that fertility will come back on a schedule. See a urologist or reproductive urologist first. Pills like Triclozene or enclomiphene come up in these talks, but they aren't TRT and don't come with a fertility guarantee. If you've only had one test
For a first diagnosis, ask a clinician to assess your symptoms and arrange repeat morning confirmation of a low result. Tiredness and low drive can have other causes, including poor sleep, thyroid problems, anemia and depression. A clinician should look for those before treating a single number. If you have one of these health flags
The Endocrine Society recommends against starting testosterone in men with several conditions, including:
- a high red blood cell count (hematocrit),
- untreated severe obstructive sleep apnea,
- uncontrolled heart failure,
- a heart attack or stroke in the past six months,
- breast or prostate cancer, or certain unexplained prostate findings needing urologic evaluation,
- thrombophilia, a condition that increases the tendency to form blood clots.
These are not boxes an intake quiz can clear. Some findings need treatment or specialist evaluation before testosterone is considered; an online brand choice cannot resolve that. Endocrine Society guideline
If you want insurance or an FDA-approved product

Will you need an in-person visit for online TRT in 2027?
An in-person visit may be required, including before 2027 if your state, clinician or clinical situation requires one. The verified federal temporary rule permits qualifying controlled-substance prescribing by audio-video telemedicine through December 31, 2026, subject to its conditions and other applicable rules. That end date alone does not tell you what every patient will need in 2027.
Here's the background:
- The default rule. A federal law called the Ryan Haight Act generally requires an in-person exam before a controlled substance, like testosterone, is prescribed online.
- The current exception. The DEA and HHS extended a temporary exception for the fourth time. It lets DEA-registered clinicians prescribe by audio-video telehealth without an in-person exam, if other federal and state rules are met. It runs from January 1 through December 31, 2026. - What's next. Do not treat a proposal or a report that a rule is under review as a new prescribing entitlement. This comparison relies on the published temporary rule, not an assumed 2027 policy. Male Excel already warns that "your testosterone treatment plan may require an in-person medical exam." The FAQ on Evolve's homepage says you can get a prescription without visiting a clinic in person. That is not a blanket rule for every patient, state or prescription; the clinician must meet the applicable requirements. Male Excel · Evolve homepage
Before you pay for anything that runs past December, ask either company: "If an in-person exam becomes required in 2027, how will you handle it, and who pays?" This matters for Evolve's annual plan even though its payment schedule is not publicly specified: the post-consultation refund limit could still affect your options. The DEA's current extension announcement gives the verified federal end date.
Male Excel vs Evolve Telemedicine: which fits your priorities?
In the 39 states on both lists, Male Excel is easier to compare on core fees and monthly membership terms, not proven cheaper or clinically better. Evolve is worth checking in the 11 states Male Excel skips and for readers who prefer venous lab testing and coordinator support, once state access, required testing and the annual-care quote are confirmed. Neither is the right first step for near-term fertility plans or an unconfirmed diagnosis.

Male Excel is one connected system: affiliated clinicians and a lab, plus designated pharmacies. You give up pharmacy flexibility. You get published core fees and a monthly membership, but medication billing is separate and some charges remain unpriced. The useful advantage is a clearer starting budget and a shorter membership commitment, not a promise of no surprises or a refund for pending orders.
Evolve emphasizes coordinator help and lists more places. But its annual care price and payment schedule need a quote, and the care plan isn't refundable once you've had the first physician consultation.
Neither answer is right for everyone. That's why the verdict changes with the buyer.
What should you ask before paying either one?
Get the exact medicine, the total you'll pay before a doctor decides, and the refund terms in writing. The two questions below do most of the work for either company. The rest fill in the gaps.
The two that matter most:
- "What will I pay before a clinician decides whether I'm a candidate, and what do I get back if they say no?"
- "Will you check my hematocrit and repeat a morning testosterone test before I start, and what does that cost?"
Then ask these:
- Which pharmacy will fill my prescription, and where is it licensed?
- Is my testosterone compounded or a commercial FDA-approved product?
- (Male Excel) Why is thyroid medicine in my plan, and can I take testosterone without it?
- (Male Excel) What is the shipping charge, and on what dates will each charge hit my card?
- (Evolve) What does the annual plan cost, when does it renew, and can I cancel before my first physician visit?
- How do I cancel, how do I confirm billing has stopped, and how do I get my records?

Count the cash charged during the first 365 days, then check what that cash actually covers. Add the entry assessment, care fees, required labs, medication orders, shipping and other required charges. Do not count an included item twice. Record the last covered day of medication and the next renewal charge separately; this prevents a 360-day medication allowance from masquerading as a full-year supply.

How did we compare Male Excel and Evolve Telemedicine?
We read both companies' public pages and terms on September 28, 2026, lined up their service-area lists, and did the cost math from posted prices only. We didn't sign up or buy treatment, and we don't give providers numeric scores. Where a fact was missing, we say so instead of guessing.
We followed How We Review TRT Providers, our published method. It keeps four kinds of information apart:
- Verified facts: authoritative medical or regulatory statements checked against their issuers, document text we directly read, and math you can repeat. Confirming that a provider published a claim does not independently confirm its service performance.
- Company says: claims we can quote but didn't test, like response times or monitoring schedules.
- Customer experience: individual reviews. They can raise a question. They can't prove a policy, a safety record or a medical result.
- Our judgment: conclusions like the verdict table, built from the facts above.
Why we don't call this a review: we didn't go through either program as patients. We didn't get a personal quote, meet a clinician, receive medication or test a cancellation.

The main differences are published core fees, service-area lists and the annual-versus-monthly care commitment. These frequently asked questions separate what the companies publish from what still needs a written answer.
Is Male Excel better than Evolve Telemedicine?
Neither is better for everyone. In the 39 states on both lists, Male Excel is easier to evaluate on published core fees and monthly membership; it still has unpriced costs. Evolve lists service in the 11 states Male Excel excludes and uses clinician-ordered Labcorp blood draws. Confirm actual TRT access, the repeat-test plan and the total quote before choosing. Is Male Excel or Evolve cheaper?
No one can establish that from the public prices checked, because Evolve doesn't publish an itemized TRT quote. Male Excel's advertised injection-plus-thyroid bundle gives a $2,727 annualized disclosed subtotal: $99 to start, 12 × $99 membership, and 12 × $120 medication equivalent. That is not a complete first-year bill; shipping, applicable tax, any extra required testing and actual fill dates still matter. Get Evolve's quote in writing and compare the same coverage period. How much does Evolve Telemedicine cost per month?
Evolve's pages give three ranges: around $75, $50 to $100, and $50 to $1,000 a month. None itemizes a complete TRT price or explains whether the annual care plan, labs and medication are included; the annual plan's own price isn't published. Ask for an itemized written quote. How much does Male Excel cost per month?
After the $99 startup, Male Excel's advertised injection-plus-thyroid bundle has a $219 monthly equivalent: $99 membership plus $120 medication. The cream-plus-thyroid equivalent is $231. The separate non-TRT Triclozene offer has conflicting $95 and $120 medication prices, making its listed membership-plus-medication equivalents $194 or $219, not a settled quote. Do Male Excel or Evolve take insurance?

What states does Male Excel not serve?

Does Evolve Telemedicine serve every state?
Evolve's terms list all 50 states plus DC, Puerto Rico and Guam, effective January 26, 2026. That's the company's service list, not a promise that a clinician will prescribe testosterone to you. Confirm TRT is available where you'll be during your visits. Evolve terms, section 6
Can Male Excel prescribe testosterone before my blood test results come back?
Male Excel's homepage says its providers can prescribe before lab results arrive when clinically appropriate. The Endocrine Society calls for symptoms or signs plus consistently low testosterone, confirmed by repeat fasting morning testing. For a first diagnosis, ask how that confirmation and baseline safety assessment will be completed before treatment. Do Male Excel and Evolve use compounded testosterone?
Both market compounded testosterone. Male Excel explicitly describes its cream as compounded; its broad marketing also says its testosterone is compounded in the United States, but we did not identify the exact injectable product. Evolve says its partner pharmacies compound its treatments. Compounded drugs are not FDA-approved, which means the FDA doesn't review their safety, effectiveness or quality before sale. Can I use my own pharmacy with Male Excel or Evolve?

Can I get a refund if the doctor says TRT isn't right for me?

What is Male Excel's money-back guarantee?
If you follow the protocol for 90 days and your symptoms don't improve by Male Excel's own assessment, you can request a refund of your first three months' membership fees, up to $297. Which one is better if I want to keep my fertility?
Neither company's public pill or TRT offer establishes that your fertility will be protected. Testosterone can lower sperm production, and the Endocrine Society advises against starting it when you're planning a family soon. See a urologist or reproductive urologist first, and raise future fertility goals even if you are not trying now. Neither company's pill option comes with a fertility promise. Is Triclozene the same as enclomiphene?

Is Evolve Telemedicine legit?
Evolve operates a public care platform under the name E-Volve Company, Inc., with a Bloomfield Hills, Michigan address and a displayed LegitScript seal. Its terms describe its clinicians as independent providers. A displayed seal is not an independent check of current certification, clinician licensure or pharmacy authority; we did not complete those checks for an individual treatment arrangement. Is Male Excel legit?
Male Excel is operated by Male Excel Inc., doing business as Excel Medical, in Charlotte, North Carolina. Its care comes through physician-owned practices named in its terms, and it displays a LegitScript seal. It publishes safety information and named medical leaders. Can I switch from Evolve to Male Excel, or the other way?
A transfer may be possible after the receiving clinician reviews your case and confirms state access. Male Excel says it takes patients from other TRT programs. Before switching, get your lab results, visit notes and medication list, and check what you've prepaid. Evolve's annual plan isn't refundable after your first visit. Will I need an in-person visit for online TRT in 2027?
There is no universal answer. The verified federal temporary flexibility for qualifying audio-video prescribing runs through December 31, 2026, subject to other federal and state requirements. An in-person exam may be needed now for some patients, and this end date does not establish every 2027 requirement.
