What changes your number:
- Vial size. The 1 mL, 200 mg/mL listing cost 5.7 times more per labeled milligram than the 10 mL, 200 mg/mL listing. That is package-price math, not a reason to change your prescription or reuse a single-dose vial.
- Insurance. Coverage depends on your exact product, plan rules, and any required prior approval.
- What's in the vial. FDA-approved generics and compounded testosterone are different categories. A compounded preparation is not FDA-approved.
- Brand-name products. Xyosted advertises a $199 cash program. Aveed’s roughly $2,233 price reference is for the drug alone, before clinic charges.
- Your state. Online programs don't serve every state.
- Your fertility plans. Testosterone can lower sperm production, so discuss plans for children before choosing treatment.

How much do testosterone injections cost?
Testosterone injections come with three separate costs: the medicine, the supplies to inject it, and the medical care to prescribe and monitor it. A generic vial can list below $50, but it is not a quote for a month of treatment. Visits, tests, and the number and timing of purchases determine the larger bill. Price example.
Think of it as three bills that sometimes arrive together and sometimes don't.

That's why prices online look wildly different. A "$25" pharmacy quote may cover only the medicine. A "$99 a month" price might be a membership without the medicine, or the medicine without the blood tests. A "$149 a month, all inclusive" price is a bundle — and you still need to ask what "all" means.

How much is a vial of testosterone cypionate?
GoodRx’s displayed price table listed one 1 mL vial of 200 mg/mL generic cypionate at $27.92 and one 10 mL vial at the same concentration at $48.79. That makes the smaller listing 5.7 times more expensive per labeled milligram. These are public reference prices, not confirmed offers from a pharmacy in your ZIP code. Price table.
Testosterone cypionate is available as an FDA-approved generic injection and as the brand Depo-Testosterone. Check the exact manufacturer and package on your prescription; a compounded preparation containing testosterone cypionate is not that approved generic. Generic label · FDA generic-drug facts.
Here's something we noticed right away: the headline price and the package-price table on a coupon site need not describe the same purchase. We used the labeled package rows below and divided each price by the amount of medicine listed. We could not reproduce the older second set of prices consistently, so we did not combine those figures into a current price range.

Per-100 mg figures are our math: price ÷ total labeled milligrams × 100. This compares listed package contents, not the amount you can safely use before a vial must be discarded. It is not a dosing calculation.
What this tells you:
- The 1 mL listing cost 5.7 times more per labeled milligram than the 10 mL, 200 mg/mL listing: $13.96 ÷ $2.4395, using unrounded numbers.
- The coupon can make a large difference. The same table displayed an average retail price of $185.59 for the 10 mL, 200 mg/mL vial, versus its $48.79 GoodRx price. Neither figure is a verified local cash quote.
- Match the pharmacy and discount plan. Enter the same ZIP code, concentration and quantity, then confirm the pharmacy’s price and whether a paid discount membership is needed. The table above does not establish a pharmacy-specific price or a free-versus-paid-plan match. Check the exact listing.
Is the 10 mL vial always the better deal?
Not automatically. The inspected approved generic label identifies its 1 mL vial as single-dose and its 10 mL vial as multiple-dose. Your prescriber and pharmacist decide which package fits your prescription and how long it can be used after opening. If you're on 1 mL vials and the price stings, ask whether a different approved package could fit your prescription without creating waste. Never reuse a single-dose vial. How long does a 10 mL vial of testosterone last?
Your prescription and the vial’s storage and discard instructions both matter. A 10 mL vial of 200 mg/mL contains 2,000 labeled milligrams, but dividing that total by a weekly amount does not establish safe days of use after opening. Is generic testosterone cypionate the same as Depo-Testosterone?
An FDA-approved generic testosterone cypionate product meets the FDA’s standards for its reference brand, Depo-Testosterone. That "same medicine" logic applies only to FDA-approved generics. It does not apply to compounded testosterone, and your actual copay may not follow the brand-versus-generic cash-price pattern. FDA generic-drug facts.
Why isn't a monthly price the same as a vial price?
Pharmacies price testosterone per vial or prescription fill. Online programs price it per month, per 60-day supply, per billing period or per minimum purchase. Before you compare any two prices, make sure they describe the same unit of time and the same things included.
Comparing those numbers directly is like comparing a gallon of gas to a car payment.
This is where most people get tripped up — and where the cheapest-looking number can turn into the most expensive year. Here's how each company we checked actually prices injections:

The 360-day catch
Male Excel shows its injection price as "$120/month" for a 60-day supply, billed every other month. Six 60-day supplies cover 360 days, not 365. In a hypothetical schedule with a first charge on day 0 and another every 60 days, a seventh $240 charge falls on day 360. That is $240 more cash paid than a six-charge illustration, and the last purchase extends beyond the first year. Published price and billing wording.
We're not saying Male Excel bills that way. We're saying ask for the exact billing dates in writing — for any program that bills in blocks of days instead of calendar months.
What do testosterone injections cost for the first year?
Published injection-program prices support budget illustrations, not one guaranteed first-year total. For example, 12 payments at Henry’s $129 starting price equal $1,548, while Male Excel’s stated components produce a $2,727 starting subtotal before shipping under a six-shipment assumption. Your actual first-year payments depend on required care, your prescription, upfront purchases and the dates you are charged. Henry · Male Excel.

These calculations assume continuing care, unchanged starting prices, no optional add-ons, and the payment counts stated. Taxes, extra clinically required tests and any fee not confirmed are not treated as zero. “Annualized” means a monthly equivalent multiplied by 12; it is not necessarily the cash charged in the first 365 days.
How we did the math:
- TRT Nation: 12 × $99.99 = $1,199.88 annualized. Three $129 panels add $387, giving $1,586.88. Outside labs have their own cost; accepted outside results are not automatically free. If five minimum 2.5-month purchases are needed, about $1,250 is paid for 12.5 months before labs, or about $1,637 with three $129 panels. - Henry Meds: 12 × $129 = $1,548. That's Henry's starting price for compounded treatment, not a quote for every patient or plan. Henry’s inclusions and terms.
- Hone Plus: $45 + $45 + (12 × $135) = $1,710. With the $50 confirmation fee listed on another Hone page, it's $1,715. With the $80 at-home option, it's $1,745. Medication is extra. Plus pricing · Testing schedule.
- Taurus: $49 + (12 × $149) = $1,837. If a $17.99 fee were charged monthly on top, it would add $215.88, for $2,052.88. The terms say per billing period, not necessarily monthly; the offer says no membership fee. This extra-fee calculation is a conditional example, not a verified quote or proven hidden charge. - Male Excel: $99 + (12 × $99) + (6 × $240) = $2,727. A seventh $240 payment makes $2,967. Shipping and any other applicable charges are extra; the seventh payment is a billing illustration, not an assertion about Male Excel’s actual invoice dates. Price components.
- Low T Center: $25 + $135 + (12 × $105.50) = $1,426, before follow-up charges and other extras. The page lists a follow-up visit and injection at $135, initially around every five weeks and later every three months as treatment stabilizes. It does not give one fixed count for every patient. - Xyosted: 12 × $199 = $2,388, plus care and possible shipping. The label describes cartons of four weekly autoinjectors. If each $199 purchase supplied four pens, 13 purchases would cost $2,587 for 52 weeks. Remember the "cheapest monthly price" trap?
Here it is. TRT Nation’s advertised monthly equivalent is $99.99. Henry Meds starts at $129 — $29.01 more. But adding three $129 TRT Nation lab panels to 12 monthly equivalents gives $1,586.88, versus $1,548 for 12 payments at Henry’s starting price with provider-stated labs included. That is a $38.88 difference in this model, before adjusting TRT Nation’s minimum-purchase cash schedule. TRT Nation prices and labs · Henry.
Accepted outside starting labs remove one TRT Nation panel charge, not every later test or the cost of obtaining those outside results. The lesson isn't "pick Henry" or "pick TRT Nation." It's this: the monthly price is only one line on the bill.

How do you get your real testosterone injection cost before you pay?
Ask for an itemized written quote that names the exact medicine, the package, every required test and visit, the billing dates and the cancellation terms. Compare two quotes only when they cover the same things over the same period of time. Treat any unknown charge as unknown, not as zero.
This is the most useful five minutes you'll spend on this whole decision. Save this section, or copy it into your notes.
The 10 costs people miss most
- The confirmation blood test. Hone’s published options are $45 or $50 at a lab and $80 at home. TRT Nation charges $129 per panel unless it accepts outside results. Ask how a program confirms low testosterone on a separate early-morning sample, and whether that test is included.
- Follow-up blood tests. Monitoring is part of medical care, not a fee to skip. Hone publishes checks at 3, 6, 9 and 12 months; TRT Nation lists 10 weeks, six months after that, and then yearly. Your clinician may need a different schedule or extra tests.
- Prescription renewals. Testosterone is a Schedule III controlled substance. Federal rules limit a prescription to five refills within six months; further fills need a new prescription. That rule does not require two paid visits each year. Ask about the program’s actual review schedule and renewal fees.
- Supplies. Syringes, needles, alcohol pads and a sharps container. Male Excel’s listed package includes 60 insulin-type syringes per 60-day supply. Ask anyone else what is included.
- Shipping. Male Excel lists it as extra. Hone and Henry say it's included.
- Membership tiers. Hone’s $25 Basic plan is not its TRT plan — Plus ($135) or Premium ($155) is. Make sure the plan offered at intake matches the price you used.
- Bundled medicines. Male Excel's shown injection price includes thyroid tablets. Ask why any additional medicine is indicated for you and whether it changes the price; thyroid treatment is not a routine requirement of testosterone replacement.
- Prescription changes. Male Excel says dose changes affect price; Hone says medication prices vary. Ask for an updated cost when the clinician changes your prescription, rather than changing treatment to fit a price.
- Minimum purchases and prepaid plans. TRT Nation requires at least 2.5 months at a time. Henry says multi-month plans can leave a balance if you cancel.
- Cancellation windows and refunds. Taurus requires at least 72 hours’ notice before billing; Male Excel describes a 48-hour response window after a renewal reminder, before billing and shipping. A cancellation request does not automatically refund tests or medication already ordered.

"For the testosterone product and package my doctor prescribed, is prior authorization required? Is it covered under my pharmacy benefit or my medical benefit? What should you check before paying for testosterone injections?
Major medical guidelines say low testosterone should be diagnosed only when symptoms or signs occur with consistently low levels, confirmed by appropriate repeat early-morning blood tests. And testosterone injections can lower sperm production, so plans for children should be discussed before treatment. Settling both first can prevent paying for the wrong care. The cheapest testosterone injection is the one you didn't need.
Check 1: Has low testosterone been properly confirmed?
Here's what the U.S. guidance says:
- The American Urological Association’s diagnostic standard, summarized by the FDA in 2022, uses a total testosterone level below 300 ng/dL as a reasonable cut-off in support of a diagnosis. It says the diagnosis should be made only after two total testosterone tests taken on separate occasions, both in the early morning — and only when there are also symptoms or signs. (AUA guideline; also summarized in the FDA’s 2022 clinical presentation, slide 22).
- The Endocrine Society recommends diagnosing only in men with symptoms and signs plus unequivocally and consistently low levels, confirmed by repeating a fasting morning test. Free testosterone testing is used when indicated; it is not a required extra test for every patient. A few quick definitions: Total testosterone is all the testosterone in your blood. Free testosterone is the small share that isn't attached to proteins. Hypogonadism is the medical name for when your body doesn't make enough testosterone.
Why this matters for your wallet: the Endocrine Society’s 2018 explanation of its guideline reported that about 30% of men with a first result in the low range had a normal result when retested. That is a population finding, not a prediction about your result. Paying for proper confirmation can avoid paying for treatment that is not indicated. Endocrine Society’s explanation.
One number is never a diagnosis. Not from an ad, not from a quiz, and not from us.
Check 2: Are you planning to have a baby?
- The Endocrine Society recommends against starting testosterone in men planning fertility in the near term. Clinical guideline.
- Testosterone can suppress sperm production; the Azmiro prescribing information warns about azoospermia, meaning no sperm in semen. That possibility does not mean infertility is certain or that recovery can be promised. Official label, section 8.3.
Nobody can promise how your fertility will respond — for better or worse. If a baby is in your plans, see a urologist or reproductive urologist before choosing treatment. Medicines like clomiphene, enclomiphene and hCG are not testosterone replacement therapy, and deserve their own comparison — not an assumption that they make TRT fertility-safe.
Their approval status differs: clomiphene is approved for certain ovulation problems in women, so use in men is off-label. Enclomiphene alone is not FDA-approved. Certain hCG products, such as Pregnyl, are approved for selected cases of hypogonadotropic hypogonadism in males — a specific hormone-signaling disorder, not every case of low testosterone or infertility. See a doctor in person first if…
- You have prostate cancer or breast cancer, a cancer history, or an unresolved prostate finding. The testosterone labels inspected still contain relevant contraindications and warnings. In June 2026, the FDA requested labeling changes concerning prostate cancer and enlarged-prostate language; that announcement is not proof that every product’s label has changed. - You already have a high red blood cell count, untreated severe sleep apnea, uncontrolled high blood pressure, or significant urinary problems. These call for clinical assessment rather than a price-led treatment choice. Guideline · Label warnings.
- You've had a heart attack or stroke within the past six months, a pituitary or testicle problem, or very abnormal lab results. The Endocrine Society advises against starting testosterone within six months of a heart attack or stroke and recommends evaluation of the cause of low testosterone. Clinical guideline.

What are you actually buying from an online testosterone program?
Online programs bundle care with medicine, and that medicine may be FDA-approved or compounded. The FDA does not approve compounded drugs or review their safety, effectiveness and quality before marketing. The specific product description matters more than a broad “FDA-approved medications” statement elsewhere on a company’s site. FDA compounding Q&A.
This is the question almost every "TRT cost" page skips. It shouldn't be skipped.
- FDA-approved: An approved product has gone through the FDA’s review for safety, effectiveness and quality for its labeled use. An approved generic must meet the FDA’s generic-drug standards. FDA generic-drug facts.
- Compounded: A pharmacy or other permitted compounder prepares a formulation under the applicable compounding rules. According to the FDA, compounded drugs are not FDA-approved, the FDA doesn't verify their safety, effectiveness or quality before they're marketed, and a compounded drug is not the same as a generic.
Compounded doesn't automatically mean bad. But you deserve to know which one you're paying for.

Our take: When an FDA-approved product is your priority, ask your clinician and pharmacist to identify that exact commercial product before you pay. A pharmacy’s name alone does not prove approval because pharmacies can also dispense compounded preparations. If you choose an online program, ask the question in writing and get the pharmacy's name.
Which online testosterone injection program fits your budget?
Your budget fit depends on what is included, how you pay, and whether the program can provide the care and exact medicine you need. Henry’s starting plan bundles costs that Hone separates, Taurus advertises a $49 first step, and Male Excel combines required membership with a separate medication charge. Those differences support a shortlist, not a claim that one program is medically best or cheapest for everyone.

- Comparing a low advertised monthly equivalent with a lab-inclusive starting plan → price TRT Nation and Henry Meds using the same tests, payment period and product requirements.
- A $49 lab-and-consult first step → examine Taurus Meds, but settle the applicable fees and refund terms before paying.
- A published quarterly first-year lab schedule → examine Hone Health, with medication priced separately.
- A clinician-recommended daily injection format and advertised dedicated-provider model → examine Male Excel’s higher starting subtotal and restrictions.
- An FDA-approved medicine you can verify → ask for an exact approved pharmacy product or an appropriate approved brand; a provider’s general marketing claim is not enough.
These are editorial matches to the documented costs and features above, not complete clinical rankings. We did not independently test appointment access, continuity with one clinician, support speed or clinical outcomes.
Here's what each one looks like up close.
Male Excel: daily-injection program with separate membership and medication charges
The cost-and-care fit: Male Excel advertises daily injections under the skin and one dedicated provider. For a patient whose clinician recommends that treatment format, those are specific service features to compare — not proof that daily injections are better or that every visit will be with the same clinician.
What Male Excel publishes (checked September 17, 2026, from its pricing page, injection page and homepage):
- $99 online consultation with a home hormone test kit to start.
- $99/month Medical Membership, required for treatment. Male Excel says it includes unlimited e-visits with your provider, check-ins every 60 days, and comprehensive blood testing every six months.
- Testosterone cypionate injection from $120/month, sold as a 60-day supply and billed every other month. The price shown covers what Male Excel calls 10 mL of testosterone, thyroid tablets and 60 insulin-type syringes. Plus shipping.
- Daily injections just under the skin, as described by Male Excel; the clinician must select the appropriate product and treatment plan.
- Testosterone compounded in the United States, per its homepage.
- No insurance. It says FSA, HSA or HRA funds may be usable, subject to the account’s rules; its terms also restrict submitting its charges to insurance for reimbursement.
- No contract; cancellation is available. The FAQ describes a 48-hour response window after a renewal reminder, before billing and shipping. Confirm how to cancel both the membership and any pending medication order. Prescription sales are generally final; canceling is not the same as getting a refund.
- The Excel Advantage Guarantee: eligible new members whose symptoms have not improved after following the plan for 90 continuous days may request three months of membership fees back under the written terms. The request must be made within 30 days after that period, and eligibility and compliance are reviewed. Medication is not refunded under this guarantee.
- Not available in Alabama, Alaska, Arkansas, Connecticut, Hawaii, Idaho, Louisiana, Minnesota, Mississippi, New Hampshire or Rhode Island.
- Its site says DEA and state laws may require an in-person exam for your treatment plan.
- Pharmacy choice is restricted. Male Excel’s terms say it will not send a prescription to a pharmacy of your choosing. Its FAQ names Anazao Health Corp. and WellDyneRx-Fl; confirm the pharmacy assigned to your prescription. That can matter when comparing an insurance or coupon-based pharmacy option.

The honest trade-off: Male Excel does not have the lowest published starting price: its listed components come to $2,727 before shipping and other applicable charges, using six medication payments. Henry’s $1,548 starting-price illustration and TRT Nation’s lab-adjusted illustrations are lower; Hone cannot be ranked on complete cost until its medication quantity is priced. Your own doctor and an approved pharmacy prescription may also cost less, but only matched quotes can establish that.
Male Excel’s stated value case is specific: a dedicated-provider model, check-ins every 60 days, comprehensive blood testing every six months, and a limited membership-fee guarantee. Those may matter to someone whose clinician recommends the offered treatment format and who accepts compounded medication and the pharmacy restriction. Ask Male Excel before you pay:
- Is the thyroid medicine required, or only if my labs show I need it?
- What does shipping cost?
- What strength is the 10 mL of testosterone, and which pharmacy makes it?
- What are my exact billing dates for medication?
- How do you confirm the diagnosis, choose treatment goals, and monitor safety?
- Is a second early-morning testosterone test included before treatment, and what happens if extra testing or an in-person examination is needed?
- How do I cancel before the next charge and obtain my records if I transfer care?
Choose something else if you live in one of the 11 excluded states, need insurance billing or your own pharmacy, require an FDA-approved commercial product, or are trying to conceive. A preference for fewer injections is a treatment-format question for your clinician, not a reason to switch on price alone. Compare online versus local care or the full Male Excel cost breakdown.

Hone Health: quarterly first-year lab checks, with medication priced separately
The cost-and-care fit: Hone charges for care and medicine separately. Its published Plus plan includes a live physician video visit to start and follow-up lab checks every three months during the first year. The total stays incomplete until your medication fills are priced.
What Hone publishes (checked September 17, 2026, from its Plus membership page, membership overview, TRT lab schedule and injection page):
- $45 at-home starting test (8 hormone and liver markers), then a live video visit with a Hone physician.
- Confirmation test before testosterone: $45 at a lab on the Plus page, $50 at a lab on the lab-schedule page, or $80 at home. (Hone's pages don't match — ask which fee applies.) It checks total testosterone, prolactin, PSA and hematocrit. PSA is a prostate blood test; hematocrit measures how much of your blood is red blood cells.
- $135/month Plus membership, which Hone says includes follow-up labs and consults at no extra charge.
- Follow-up labs at 3, 6, 9 and 12 months, then at 18 months, per Hone's TRT schedule.
- Testosterone cypionate injection starts at $28 per vial — per vial, not per month. Prices vary by dose.
- Free shipping and cancel-anytime membership are provider-stated. Confirm the effective date and whether medication orders or other charges are already committed.
- State access depends on the treatment and prescription. Hone’s June 2026 comparison article says 35 states; that is not a current state-by-state verification. Confirm service for your location before buying the initial test.
Watch for:
- The right tier. The $25 Basic plan is for lab testing without hormone therapy. TRT runs through Plus ($135) or Premium ($155).
- How many vials you'll use. Without that, "$135 + $28" isn't a monthly price. Ask for the price, quantity and days supplied for your actual prescription.
- What's in the vial. The men’s injection product page explicitly describes a compounded product. A missing marker on a price list is not evidence that it is FDA-approved; confirm the exact preparation and dispensing pharmacy.
- Which offer you enter. The men’s product page also promotes a different testing and membership path. These calculations use the specifically named Plus help-center plan, not every Hone intake offer. Product page.
Choose something else if you want one all-in price, you want a confirmed FDA-approved product, or Hone doesn't treat patients in your state.

Taurus Meds: a $49 lab-and-consult offer with terms to settle first
The cost-and-care fit: Taurus advertises a $49 blood test and consult, and says treatment is $149 a month, "all inclusive." The offer and its broader terms need to be read together before paying.
What Taurus publishes (checked September 17, 2026, from its $49 offer page, terms and cancellation and refund policy):
- $49 blood test and consultation, at LabCorp or Quest. If you live more than 50 miles from one, Taurus says it sends an at-home kit at no extra charge.
- "$149/mo – all inclusive. No membership fees. No hidden fees."
- The offer describes weekly injections into the muscle, but repeats the same injection wording on gel and oral-product cards. Those cards are not a reliable source for the treatment instructions you would receive.
- A refund for medical ineligibility is advertised; check its scope against the written refund policy.
- A 90-day “Taurus Advantage Guarantee” is advertised, excluding lab and pharmacy fees. It is not proof that a refund will be available under every circumstance.
- Taurus says it is not a medical provider. Its terms name OpenLoop Health as a partner management company, and list four partner pharmacies: RedRock Pharmacy, Health Warehouse, Precision Compounding Pharmacy and Triad Rx.
- No insurance.
Confirm these things before you pay:
- The membership fee. Taurus's terms (updated June 24, 2025) describe a $17.99 membership fee per billing period "unless otherwise stated at the time of purchase." The $49 offer says no membership fees. Get the answer for your checkout in writing.
- Refunds. Taurus's refund policy (updated February 28, 2026) requires cancellation requests at least 72 hours before your billing date, and says cancellations aren't refunded except when a provider disqualifies you medically. Its terms say shipped medications and blood tests aren't refundable. Ask how that fits with the 90-day guarantee.
- What's in the vial. The specific $49 offer describes compounded hormones. A homepage statement about FDA-approved medications does not establish that this injection is approved. Ask for the exact product and assigned pharmacy.
- When money is taken. The refund policy describes treatment charges that can occur during intake. Get the actual payment sequence before submitting payment details; the six-month table is an illustration, not a verified checkout schedule.
- Who receives your health information. The terms contain a disclaimer that the services are not tailored to comply with HIPAA. Ask which separate clinical entity handles your medical information and obtain its privacy notice. That wording alone does not establish whether every partner is or is not covered by HIPAA, but it is a reason not to assume protection from a health-service label. Terms.

Henry Meds: a bundled starting price, with plan-specific commitments
Henry says its compounded testosterone treatment starts at $129 a month and that the price covers provider visits, lab work, medication and supplies, shipping and ongoing support. It ships injectables every 45 days. Henry's eligibility FAQ lists men ages 35 to 65. If you choose a multi-month plan, Henry says you may have to pay the rest of your balance if you cancel, unless a provider finds you can't continue for medical reasons. (Henry Meds TRT page)
Worth comparing if you’re within Henry’s stated age range and want a starting plan with labs bundled in. Not a fit if you require a confirmed FDA-approved commercial injection or fall outside the program’s eligibility rules. “Starting at” is not a guaranteed final quote.
TRT Nation: lower monthly equivalent, separate labs and advance purchases
TRT Nation advertises testosterone therapy at $99.99 a month with a 2.5-month minimum purchase, and notes California pricing may vary. Labs are not included: you can upload recent results or pay $129 per panel. It requires blood work 10 weeks after starting, six months after that, and then yearly, and it doesn't accept at-home test kits. (TRT Nation testosterone page, FAQ)
Promotions come and go, so don't build a yearly budget around one. TRT Nation’s FAQ requires blood work, but a test alone is not a diagnosis. Ask how it confirms symptoms or signs plus consistently low results with appropriate repeat early-morning testing, and what any additional tests cost. TRT Nation FAQ · Endocrine Society guideline.
The FAQ lists patients 24 and older, accepts specified outside labs up to six months old, and includes an estrogen-blocking medicine in its plan description. Ask whether any added medicine is indicated for you and what it costs; inclusion in a bundle does not establish that you need it. Provider’s eligibility and plan details.
Worth comparing if you already have results the clinician will accept and can budget for advance purchases and follow-up labs. Not a fit if you need a lab-inclusive quote or cannot get the exact medication and dispensing pharmacy confirmed before committing. Accepted outside labs do not remove the need for proper diagnosis or ongoing monitoring.
Will insurance or Medicare pay for testosterone injections?
Insurance may cover a prescribed testosterone injection when the product and treatment meet your plan’s rules. Medicare Part D generally handles covered outpatient prescriptions not covered by Part B; Part B covers only certain drugs and services under its own requirements. An injection given in an office is not automatically covered by Part B. Medicare coverage rules.

Ask the insurer for the denial reason in writing. Then ask your prescriber whether an appeal or a letter of medical necessity makes sense. The brand-name prices we checked were higher than the generic vial listings, but they buy different products and may involve different care charges. Xyosted advertises a $199 monthly cash program; the Azmiro listing was $1,008.80 for four prefilled syringes, and Aveed’s price reference was about $2,233 for 3 mL of medication alone. A few things to know:
- These aren't dose-for-dose swaps. Product labels have different strengths, instructions and schedules. Compare the prescribed product’s full fill and care costs, not milligrams across brands. Azmiro prescribing information.
- Aveed requires a special safety program because of serious reactions, including oil entering blood vessels in the lungs and severe allergic reactions. Every injection requires the supervised setting and observation stated in its label. Aveed prescribing information.
- Xyosted contains sesame oil. Its label lists allergy to its ingredients as a contraindication. The generic cypionate label inspected lists cottonseed oil; do not assume all products or compounded preparations use the same ingredients. Xyosted label · Inspected generic label.
Is it legal to get testosterone injections online in 2026?
Online prescribing can be lawful when a properly authorized clinician issues a valid prescription and all applicable requirements are met. Testosterone is a Schedule III controlled substance; the federal temporary extension permits qualifying audio-video prescribing without a prior in-person examination through December 31, 2026. State law, the clinician’s authority and your clinical needs still matter. DEA’s extension · Schedule III regulation.
What Schedule III means for you. A Schedule III drug has accepted medical uses but tighter prescribing and dispensing rules. For you, that means a valid prescription is required. Under 21 CFR 1306.22, a prescription cannot be filled or refilled more than six months after it is issued, or refilled more than five times. Further fills require a new prescription; the rule does not set a universal number of paid appointments.
The December 31, 2026 date. The DEA and HHS extended the relevant temporary telemedicine flexibilities through the end of 2026. The extension does not guarantee online prescribing for everyone, override state requirements, or establish the rules after that date. What else should you know about testosterone injection costs?
A price per shot, a vial price and a monthly program fee describe different purchases. Use the exact prescription, required care and payment period to compare them; a coupon or questionnaire does not replace a valid prescription or medical evaluation.
How much is one testosterone shot?
There's no single price per shot. The 200 mg/mL generic cypionate listings checked were $27.92 for one 1 mL vial and $48.79 for one 10 mL vial, for medication only. A clinic may add administration or visit fees, and a single-dose package must not be treated as a reusable supply. Price table · Inspected label.
How much is a 10 mL vial of testosterone cypionate?
The GoodRx table checked listed one 10 mL vial of 200 mg/mL at $48.79 and an average retail figure of $185.59. Neither was a matched local pharmacy quote. Different concentrations and quantities have different prices. GoodRx package-price table.
How long does a 10 mL vial of testosterone last?
Ask your pharmacist for the prescribed days’ supply and the discard date for the exact product. A vial’s total milligrams do not establish how long an opened vial is safe to use. Do not stretch a supply or reuse a single-dose vial to lower cost. CDC injection safety · Product label.
Is the 10 mL vial always cheaper?
The 1 mL, 200 mg/mL listing cost 5.7 times more per labeled milligram than the 10 mL listing at the same concentration. That comparison does not account for unused medicine that must be discarded. Your prescriber and pharmacist decide which package fits you. Prices · Package label.
Are testosterone injections covered by insurance?
They may be, depending on the exact product, diagnosis documentation and plan rules. Ask about prior authorization, your deductible, covered pharmacies and separate visit or lab charges. A general statement that “TRT is covered” is not a quote for your treatment.
Does Medicare cover testosterone injections?
Part D plans may cover eligible outpatient prescriptions under their drug lists and rules. Part B covers certain clinician-administered drugs and services when its requirements are met; receiving an injection in an office alone does not establish coverage. Medicare’s drug-coverage explanation.
Can I use a GoodRx coupon with my insurance?
Not on the same fill. Discount-card purchases are separate from insurance claims, so ask the pharmacist to compare both prices. Ask your insurer whether the cash payment can count toward your deductible rather than assuming it will. Discount-service terms.
What's the cheapest way to get testosterone injections?
There is no verified cheapest route for every patient. Start by comparing the complete cost of your existing clinician and a prescribed approved generic with any online program, including labs, visits, supplies and advance payments. TRT Nation and Henry have lower starting-price illustrations than some other programs here, but their products, inclusions and commitments must be matched before choosing.
Why do online TRT programs cost so much more than the vial?
You're paying for clinician visits, blood tests, supplies, shipping and ongoing support — not just the medicine. Some programs also use compounded testosterone rather than an FDA-approved generic. Is compounded testosterone the same as generic testosterone cypionate?
No. The FDA says compounded drugs aren't FDA-approved and aren't the same as generics, which are FDA-approved. Ask any program which one you'd get. FDA compounding Q&A.
Are labs included in online TRT prices?
Sometimes. Henry says labs are included. Hone's Plus membership includes follow-up labs but charges for the starting and confirmation tests. TRT Nation charges $129 per panel unless it accepts outside results. Ask about extra or repeat tests required for your situation. Henry · Hone · TRT Nation.
Can I use HSA or FSA money for testosterone injections?
Prescribed treatment for a medical condition can be a qualifying medical expense, but that does not automatically qualify every membership, wellness service or bundled item. Confirm with your plan administrator and keep itemized receipts. IRS Publication 502.
Do testosterone injections affect fertility?
They can. Testosterone can lower sperm production, and the Endocrine Society recommends against starting it in men planning fertility in the near term. Discuss plans for children with a urologist or reproductive urologist before choosing care; no program can guarantee preserved or restored fertility. Endocrine Society guideline.
Can I buy testosterone injections without a prescription?

What happens to online TRT after December 31, 2026?
The current temporary federal extension runs through December 31, 2026. It does not establish the rules after that date or guarantee that all patients can receive prescriptions remotely. How much does Xyosted cost without insurance?
Xyosted’s manufacturer advertises $199 per month through its cash program for eligible patients with a valid prescription. Shipping may be extra, and the purchase cannot be reimbursed through insurance or combined with other offers. What should I do before canceling or switching programs?
Ask for the cancellation deadline, any remaining balance, the status of medication already ordered and copies of your medical records. Arrange continuity with a treating clinician rather than changing or stopping medication based on a billing decision.
