TRT and GLP-1: Can You Take Testosterone With Semaglutide or Tirzepatide?
Yes — TRT and GLP-1 medicines such as semaglutide and tirzepatide can be prescribed to the same man when each has its own clinical reason. The bigger question is order: weight loss can raise testosterone in some men with obesity-related low T, while confirmed pathologic hypogonadism still needs its own evaluation. Combination treatment is not proven to be better by default.
Here's what changes that answer:
- Your low T isn't confirmed yet. One low result isn't a diagnosis. Current endocrine guidance calls for symptoms or signs plus consistently low testosterone confirmed with repeat early-morning, fasting testing.
- You want a baby in the near term. Testosterone therapy can suppress sperm production, and the Endocrine Society recommends against starting TRT when near-term fertility is planned. If weight treatment is otherwise appropriate, discuss that separately with a fertility-aware clinician.
- You take insulin or a sulfonylurea. Androgens can lower blood glucose and insulin requirements, while GLP-1 medicines can increase hypoglycemia risk when combined with insulin or an insulin secretagogue. Coordinate a glucose-monitoring plan with your prescriber.
One number to keep in mind: in a retrospective 1,629-man study presented in 2026, median total testosterone rose from 320 to 419 ng/dL after semaglutide or tirzepatide use — with no TRT in the analysis. The study cannot prove that the drugs caused the increase.
That doesn't mean everyone should skip TRT. It means the order you do things in can decide whether you need one prescription or two. That's what the rest of this page sorts out.

At a glance: TRT vs. GLP-1 — what does each one actually treat?
TRT and GLP-1s treat different problems. Testosterone therapy replaces a hormone your body isn't making enough of. GLP-1 medicines act on appetite, digestion, and blood sugar. A man can need one, the other, both, or neither — and the right mix depends on why his testosterone is low.
Quick definition: Hypogonadism means the body doesn't make enough testosterone. Schedule III is a federal drug category for medicines with some misuse risk, which is why testosterone has stricter prescribing rules than most drugs. Ozempic, Wegovy, Mounjaro, Zepbound: same thing?
No. People use these names like they're interchangeable. They aren't, and the difference matters for insurance, price, and which prescription you actually get.
Tirzepatide works on two gut-hormone receptors (GLP-1 and GIP). Semaglutide works on one (GLP-1). Both get called "GLP-1s," and so does this page.
Can you take TRT and a GLP-1 at the same time?
Yes, a clinician can prescribe testosterone and a GLP-1 to the same patient when each has its own indication. The main coordination issues include blood sugar in people using diabetes medicines, TRT monitoring as body weight changes, and fertility because exogenous testosterone can suppress sperm production. What is not proven is that the combination works better than treating each condition on its own.
Plenty of men end up on both. A man with confirmed hypogonadism from a pituitary problem can also have obesity or type 2 diabetes. Each condition can justify its own treatment.
The problems start when the combo gets sold as a "stack." That pitch skips the questions that actually matter. Here's what the research does and doesn't support today:
Not established (as of October 2026)
- That adding TRT to a GLP-1 reliably prevents muscle loss.
- That TRT makes semaglutide or tirzepatide work faster or better.
- That a GLP-1 can replace TRT for men whose low testosterone has a cause other than weight.
- That taking both is safer or more effective than treating each problem separately.
We looked for a large randomized trial testing TRT plus a GLP-1 together. We didn't find one.
Our honest read: the evidence is better at showing who might not need two medicines than at proving the two together are better. That's not bad news. It means the first step is sorting out which problem you actually have — not buying a bundle.
Do semaglutide and tirzepatide raise testosterone?
In studies of men with obesity, type 2 diabetes, or obesity-related functional hypogonadism, testosterone generally rose during GLP-1–based treatment. Weight loss and improved metabolic health appear to explain much of the effect, though a 2026 retrospective study found the rise was not fully explained by BMI change. In healthy eugonadal men, available evidence has not shown the same endocrine benefit.
We pulled every useful study we could find into one table. Look at the "Limits" column before you get excited. Most of this is early.
The GLP-1 → testosterone evidence table
Two terms: Total testosterone is all the testosterone in your blood. Free testosterone is the small share that isn't bound up and can act on your body right away.
Why weight loss can raise testosterone
Extra body fat pushes testosterone down in a few ways. It lowers SHBG (sex hormone-binding globulin, the protein that carries testosterone in the blood). That makes total testosterone read lower on a lab test. Obesity and insulin resistance can also quiet the brain signals (LH and FSH) that tell the testicles to make testosterone.
Lose meaningful weight and some of that pressure lifts. Doctors sometimes call low T from this cause functional or obesity-related hypogonadism. A 2025 review in the Journal of Clinical Endocrinology & Metabolism put it plainly: when there's no underlying hormone disease, testosterone treatment works less well than fixing the reversible causes — weight, diabetes, sleep apnea, and the like.
The AACE president put the 2025 tirzepatide results in context: the hormone gains appear driven mainly by weight loss. Whether these drugs have a direct effect on testosterone is still unknown.
What the studies can't tell you yet
- Most are look-backs, not experiments. They show testosterone rose alongside GLP-1 use. They can't prove the drug caused it.
- Nobody tracked what happens after stopping. The ENDO 2025 lead author warned that testosterone could drop again if the weight comes back.
- A rise isn't the same as a fix. Some men start low and land in the normal range. Others start normal and just go higher. Those are different stories.
Bottom line: GLP-1–based treatment can raise testosterone in men with obesity-related or metabolic low-T patterns, but it is not a substitute for finding the cause of persistently low testosterone. If weight and metabolic illness are not the main drivers, don't count on a GLP-1 to correct the hormone problem.
Should you start the GLP-1 or TRT first?
If your low testosterone has not been confirmed and you carry extra weight, the first job is to confirm the hormone finding and address reversible causes; a GLP-1 may be part of weight treatment when independently indicated. If pathologic hypogonadism is confirmed, TRT can be evaluated on its own merits while weight or diabetes care is handled separately or alongside it.
This is the question most pages skip. Here's the decision path we built from the Endocrine Society guideline and the studies above. It's a framework for the conversation with your clinician — not a diagnosis.
Step 1: Has low testosterone actually been confirmed?
The Endocrine Society says to diagnose hypogonadism only when a man has symptoms or signs of low testosterone and unequivocally and consistently low testosterone. It also recommends confirming with a repeat morning, fasting total testosterone test. One low number — especially an afternoon test or one taken while sick — isn't enough.
- Not confirmed, and you carry extra weight? Confirm first. In obesity without pathologic hypogonadism, weight and metabolic treatment may be the more appropriate first focus, with testosterone reassessed after clinically significant weight loss has stabilized.
- Confirmed? Go to Step 2.

Step 2: Is there a cause besides weight?
The guideline also recommends finding out why testosterone is low. That includes checking LH and FSH to tell testicle problems apart from brain and pituitary problems.
- A suspected pituitary, testicular, genetic, or other organic cause? This belongs with endocrinology or urology. TRT may be appropriate after the cause and contraindications are evaluated. - Looks weight-related? Go to Step 3.
Step 3: Do you want a child in the next year or two?
- Yes: The Endocrine Society recommends against starting testosterone in men planning fertility in the near term. Talk to a fertility-aware clinician first. See the fertility section below.
- No: Go to Step 4.
Step 4: Do you take insulin or a sulfonylurea?
- Yes: Coordinate the plan with the clinician managing your diabetes. Androgens can reduce blood glucose and insulin requirements, while GLP-1 medicines can increase hypoglycemia risk when used with insulin or a sulfonylurea.
- No: If each treatment has its own indication, your clinicians can decide whether to start them together or separately and how to monitor changes over time.

The right TRT provider is not the same for every person — it depends on whether low testosterone has been properly evaluated, your symptoms and health history, your fertility plans, your state, your insurance or cash-pay preference, your treatment-format preference, your budget, and whether online or in-person care is the better starting point. Will TRT stop muscle loss on Ozempic or Zepbound?
It is not proven. Across 2026 randomized-trial evidence, lean mass made up about 25% to 39% of weight lost with incretin-based drugs. A separate 100-man diet trial found testosterone attenuated lean-mass loss by 3.4 kg in men with obesity and low testosterone, but it was not a GLP-1 trial and the body-composition advantage was not sustained after testosterone stopped.
This is the fear behind a lot of these searches: "Am I going to lose all my muscle?" Here's what's actually known.
How much lean mass do GLP-1s take?
A 2026 meta-analysis of randomized trials estimated that lean mass accounted for about 35% of weight lost with semaglutide and about 25% with tirzepatide. In the SURMOUNT-1 DXA substudy specifically, about 26% of the weight lost with tirzepatide was lean mass. These figures are not the same as pure skeletal-muscle loss.
"Lean mass" is not the same as muscle. It also counts water, organs, and other non-fat tissue. The scans used in these trials can't separate them cleanly. So the real muscle loss is smaller than those percentages sound — though it's still worth taking seriously.
What testosterone did in the best related trial
The strongest evidence comes from a diet trial, not a GLP-1 trial. Ng Tang Fui and colleagues (BMC Medicine, 2016) studied 100 men with obesity and low testosterone on a very-low-calorie diet. Half got testosterone injections and half got a placebo.
- Both groups lost similar lean mass during the strict diet phase.
- The testosterone group regained more lean mass afterward. By the end, they had 3.4 kg less lean-mass loss than the placebo group, and their weight loss was almost all fat.
- The catch: a follow-up found the body-composition benefit was gone about 82 weeks after testosterone stopped.
What we'd tell a friend
- If you have confirmed pathologic hypogonadism, TRT may be appropriate during weight loss for that diagnosis. The diet trial supports a body-composition effect in men with obesity and low testosterone, but it does not prove that TRT should be added to a GLP-1 to protect muscle.
- If your testosterone is normal, there's no good evidence that adding TRT protects muscle on a GLP-1. And testosterone without a real need brings real risks.
- No large trial has tested TRT plus a GLP-1 for muscle. If someone quotes a precise "percent of muscle saved" for the combo, ask them for the study.
- Whatever you decide, resistance training and enough protein matter. Ask your clinician what makes sense for your body.
What labs should be checked on TRT and a GLP-1?
TRT monitoring still matters when a GLP-1 is added: testosterone and hematocrit are central, prostate monitoring may be appropriate based on age and risk, and blood pressure remains part of routine care. If you have diabetes, glucose and A1c monitoring also matter. The exact schedule should come from the clinician managing each condition.

Two questions to ask before you start:
- Who orders my testosterone and hematocrit — the TRT clinic or the weight clinic?
- If my weight and testosterone change during treatment, who is responsible for reassessing the TRT plan?
Never change a dose on your own.

Is TRT plus a GLP-1 risky if you have diabetes?
The main added concern is hypoglycemia if you also use insulin or a sulfonylurea. Testosterone labeling notes that androgens may lower blood glucose and insulin requirements in people with diabetes, while GLP-1 labels warn of increased hypoglycemia risk with insulin or insulin secretagogues. Medication changes belong with the prescribing clinician.
The two drugs don't fight each other. They can pile on in the same direction:
- Testosterone labels: in men with diabetes, androgens "may decrease blood glucose" and insulin needs.
- Zepbound label: using it with insulin or an insulin secretagogue (a drug like a sulfonylurea that pushes the pancreas to release insulin) can raise the risk of low blood sugar, including severe hypoglycemia. The label says reducing the insulin or secretagogue dose may be necessary.
There's one more point people miss. The Endocrine Society recommends against using testosterone as a way to improve blood sugar control in men with type 2 diabetes. TRT is for confirmed low testosterone, not for your A1c.
Know the signs of low blood sugar: shaking, sweating, fast heartbeat, hunger, confusion, or feeling faint. Treat it the way your clinician taught you. Get urgent help if it's severe or you can't stay alert.
What if fertility matters to you?
Testosterone therapy can suppress sperm production, and the Endocrine Society recommends against starting it in men planning fertility in the near term. In one 25-man trial, semaglutide improved sperm morphology while sperm concentration and total sperm number fell significantly in the TRT group. If pregnancy is in your plans, see a fertility-aware clinician before starting TRT.
Why does TRT hurt sperm? When testosterone comes from outside, your brain senses "enough" and turns down LH and FSH. Those are the same signals your testicles need to make sperm. This sperm-making process is called spermatogenesis.
The semaglutide-vs-TRT trial (25 men with type 2 diabetes and obesity) showed the difference clearly:
That's one small trial in diabetic men. It doesn't promise anything for you. But it is one reason a man with obesity-related low testosterone who wants children soon should discuss weight/metabolic treatment and fertility-preserving options before starting exogenous testosterone.
There are also non-testosterone options sometimes used by specialists to stimulate endogenous hormone signaling, such as clomiphene, enclomiphene, or hCG. These are not TRT. Brand-name vs. compounded GLP-1: what changed in 2026?
The FDA declared the tirzepatide shortage resolved in December 2024 and the semaglutide shortage resolved in February 2025, which narrowed the shortage-based pathway for routine copies. Compounded GLP-1 products are not FDA-approved. As of May 31, 2026, FDA had received 990 adverse-event reports associated with compounded semaglutide and more than 730 associated with compounded tirzepatide.
This matters on a TRT + GLP-1 page because a lot of "TRT + weight loss" packages use compounded GLP-1s.
A quick definition: an FDA-approved drug has been reviewed by the FDA for safety, effectiveness, and manufacturing quality. A compounded drug is mixed by a pharmacy for an individual patient. It has not gone through that review. Compounded is not the same as generic.
What happened, in order:
- December 2024: the FDA declared the tirzepatide shortage over.
- February 2025: the FDA declared the semaglutide shortage over.
- 2025–2026: FDA enforcement included warning letters over false or misleading marketing that blurred compounded semaglutide or tirzepatide with FDA-approved products.
- As of May 31, 2026: the FDA had received 990 adverse-event reports tied to compounded semaglutide and more than 730 tied to compounded tirzepatide. A report doesn't prove the drug caused the problem, and the FDA says these events are likely underreported.
How to spot a compounded GLP-1:
- A price far below current manufacturer self-pay pricing can be a reason to verify the exact product and dispensing pharmacy, but price alone does not prove that a drug is compounded.
- Extra ingredients like "B12" or "glycine." No FDA-approved semaglutide or tirzepatide product contains them.
- Phrases like "same active ingredient as Ozempic" or "generic Zepbound." Neither drug had an FDA-approved generic as of October 2026.

Can one online clinic prescribe both TRT and a GLP-1?
Some can, but read the fine print. We checked three online TRT clinics on October 8, 2026. Hone publicly lists compounded liraglutide and also lists Zepbound as a Premium-tier weight-management option prescribed through Hone and purchased through LillyDirect. Male Excel says it offers GLP-1 metabolic therapies but does not name the GLP-1 or publish its price. Taurus does not publicly list a GLP-1 treatment program on the pages we verified.
This is the part most "TRT and GLP-1" articles don't tell you, often because they're written by the clinic selling the package.

One detail worth knowing: if a GLP-1 product has B12 mixed into the same formulation, it is not one of the FDA-approved semaglutide or tirzepatide products. Ask for the exact drug name, formulation, and dispensing pharmacy.

Best fit: Your GLP-1 is already handled by your own doctor or a manufacturer program, your low T is confirmed, and you want close TRT follow-up while your weight is changing.

Male Excel also lists a $99 initial lab/consult step before treatment. Medication is billed separately. The tradeoff: Male Excel does not publish which GLP-1 it prescribes or what it costs. If you want a clinic that publicly names a weight-management drug pathway, Hone is more transparent because it lists compounded liraglutide and access to Zepbound through LillyDirect. If you want brand-name Zepbound or Wegovy at a posted price, the manufacturer programs below are clearer. But Male Excel builds its program around testosterone — the same provider, a review every 60 days, labs every six months. That fits a man whose GLP-1 is already sorted, because his testosterone response can shift as the weight comes off.
Know before you start:
- Male Excel says its hormone treatments are U.S.-compounded and pharmacy-dispensed. Compounded testosterone preparations are not FDA-approved.
- Male Excel's current TRT page says care starts with an at-home lab test and provider consultation. Regardless of clinic workflow, a diagnosis of hypogonadism should not be based on symptoms alone or a single low result.
- Thyroid medicine comes in its starting TRT plans. Thyroid hormone is not a weight-loss drug — its own boxed warning says so. Ask whether you actually need it.
- It doesn't serve AL, AK, AR, CT, HI, ID, LA, MN, MS, NH, or RI.

Know before you start: Hone's own compounded GLP-1 offering is liraglutide, a daily injection that is not FDA-approved as compounded. Hone's current help center also lists Zepbound (tirzepatide) as a Premium option prescribed through Hone but purchased directly through LillyDirect. It does not currently list semaglutide as a men's weight-management medication in the treatment list we verified.

Best fit: You want a TRT-focused clinic, and your weight medication comes from your own doctor or a manufacturer program.
What you get: TRT through licensed clinicians, plus erectile dysfunction treatment and peptide programs. We did not find a public Taurus GLP-1 treatment program on the pages checked. Manufacturer self-pay programs — for brand-name GLP-1s at the lowest posted cash price
We don't earn anything from these manufacturer programs. LillyDirect lists self-pay Zepbound, and NovoCare Pharmacy lists self-pay Wegovy. A valid prescription and program eligibility are required. The programs set their own restrictions and can change or end pricing at any time.
How much do TRT and a GLP-1 cost together?
At current manufacturer self-pay prices, a year of Wegovy tablets can start around $3,338 under one example titration path, while a 13-fill year of Zepbound can approach about $5,637 before supplies. Online TRT adds roughly $2,300 to $2,800 at the starting prices modeled below. The lowest-cost medically appropriate route depends on which diagnoses are actually confirmed.

Your real cost depends on how fast your dose goes up and whether you refill on time. Add a prescriber's fee if your own doctor isn't writing the prescription.

What this shows: Cost can differ by thousands of dollars, but price does not decide the medical sequence. Route A models weight treatment without TRT; Route B uses one platform but a compounded liraglutide option; Routes C and D separate TRT care from an FDA-approved manufacturer GLP-1 program. The right route still depends on diagnosis, fertility, insurance, state access, and the exact medication prescribed.
Two notes:
- Insurance: coverage for GLP-1s varies a lot by plan and by diagnosis. Diabetes uses are covered more often than weight loss alone. Online TRT rules to know before you prepay (late 2026)
Testosterone is a Schedule III controlled substance. Under the current federal temporary extension, DEA-registered clinicians can prescribe Schedule II–V controlled medications through qualifying telemedicine without a prior in-person evaluation through December 31, 2026, as long as all other federal and state requirements are met. GLP-1 medicines are not controlled substances.
This is a real date, not a sales tactic. The DEA and HHS extended the telemedicine flexibilities for a fourth time, through December 31, 2026. What happens after that depends on future rulemaking.
Before you prepay months of TRT, ask the clinic: "If the federal rule changes after December 31, 2026, could I need an in-person visit, and how would that work in my state?" State law and clinic policy can be stricter than the temporary federal rule.

Who should not combine TRT and a GLP-1?
Wegovy and Zepbound are contraindicated in people with a personal or family history of medullary thyroid carcinoma or MEN 2. The Endocrine Society recommends against starting TRT in men planning near-term fertility and in several higher-risk situations, including breast or prostate cancer, elevated hematocrit, untreated severe sleep apnea, uncontrolled heart failure, recent myocardial infarction or stroke, or thrombophilia.
Get urgent care for: severe belly pain that won't go away (a possible sign of pancreatitis), chest pain, trouble breathing, one-sided weakness, a swollen and painful leg, or signs of severe low blood sugar.

What happens to testosterone if you stop the GLP-1?
It may fall again if weight and metabolic disease return, but current studies do not establish a predictable post-GLP-1 testosterone trajectory for every man. Weight regain after stopping GLP-1 therapy is common in obesity trials, while testosterone changes after stopping have been studied much less directly. Do not change either prescription without the clinician managing it.
The testosterone side has a similar pattern. In the diet trial above, the lean-mass benefit from testosterone was gone about 82 weeks after it stopped. Neither drug is a one-and-done fix.
If treatment changes materially, ask the clinician managing your hormone evaluation when repeat testosterone testing would be meaningful; timing can matter, especially during active weight loss. Can you stop TRT?

We pulled the published and presented studies on GLP-1s and testosterone, and we checked each clinic's and manufacturer's live pricing pages on October 8, 2026. Frequently asked questions
Can I take Ozempic while on TRT?
A clinician can prescribe both when each has its own indication. Make sure each prescriber knows about the other medication. TRT monitoring still applies, and people also using insulin or a sulfonylurea need a coordinated plan for hypoglycemia risk.
Can you take TRT and tirzepatide (Mounjaro or Zepbound) together?
Yes, with the same cautions as semaglutide. Research on combining them is early. The best tirzepatide study is an 8-week pilot comparing it with testosterone gel, not a combination trial.
Does semaglutide increase testosterone?
In men with obesity or type 2 diabetes, studies mostly show increases, largely tied to weight loss. In healthy-weight men, a 2025 review found no change. Semaglutide is not FDA-approved to treat low testosterone.
Does tirzepatide raise testosterone more than TRT?
One 83-man, two-month controlled pilot found higher total, free, and bioavailable testosterone in the tirzepatide group than in untreated and transdermal-testosterone comparison groups. Baseline differences, short follow-up, and the pilot design mean it should not be read as proof that tirzepatide is superior to TRT for confirmed pathologic hypogonadism.
Should I lose weight before starting TRT?
If your low testosterone isn't confirmed and extra weight may be the cause, weight treatment first is often reasonable. If low T is confirmed or has another cause, there's no rule that you must lose weight first.
Will TRT make me lose weight faster on a GLP-1?
There is no good evidence that adding TRT makes GLP-1 weight loss faster. A separate diet trial in men with obesity and low testosterone found a body-composition benefit from testosterone, but it was not a GLP-1 trial and does not justify adding TRT solely to change weight-loss composition.
Does TRT prevent muscle loss on Ozempic or Zepbound?
It isn't proven. A 100-man diet trial showed testosterone reduced lean-mass loss by 3.4 kg in men with low T, but no large trial has tested it with GLP-1s, and the benefit faded after testosterone stopped.
Can a GLP-1 replace TRT?
Not automatically. It depends on why your testosterone is low. If weight is the main cause, a GLP-1 may raise testosterone enough to change the plan. If the cause is pituitary or testicle disease, it won't.
Is GLP-1 or TRT better if I want kids?
TRT can lower sperm production, and the Endocrine Society advises against starting it when fertility is a near-term goal. In one small trial, semaglutide didn't lower sperm concentration. See a fertility-aware clinician before deciding.
Is compounded semaglutide with B12 the same as Ozempic?
No. Compounded GLP-1s aren't FDA-approved, and no FDA-approved semaglutide contains B12. The FDA has warned companies for marketing compounded versions as if they were the same as brand-name drugs.
Can one online clinic prescribe both TRT and a GLP-1?
Some can. In our October 2026 check, Hone publicly listed compounded liraglutide and also Zepbound access through LillyDirect for Premium members. Male Excel said it offers GLP-1 metabolic therapies but did not name the drug or price. Taurus did not show a verified GLP-1 treatment program on the public pages we checked.
Do I need an in-person visit for TRT in 2026?
Federal rules allow video-only prescribing of testosterone through December 31, 2026. State rules and individual clinics can still require an in-person visit. Ask before you prepay.
Does a GLP-1 affect my PSA or hematocrit?
PSA elevation and erythrocytosis are TRT monitoring issues, not established direct GLP-1 effects. If you are on testosterone, keep the monitoring plan your TRT clinician recommends even when a GLP-1 is added.
